Evaluation of ammonium lactate in the treatment of seborrheic keratoses.

A double-blind, paired comparison study was used to evaluate treatment effects of 12% ammonium lactate lotion (Lac-Hydrin) against its vehicle on seborrheic keratoses. Fifty-eight volunteer patients, 37 to 82 years of age, were studied for 16 weeks. The patients had a minimum of two seborrheic keratoses at least 10 cm apart. They applied the medication twice daily. The lesions were evaluated for height, surface characteristics, color, and length with the use of 7X calibrated loupe, a template, skin replicas, and scanning electron microscopy. Lac-Hydrin 12% lotion significantly reduced the height (elevation) of seborrheic keratoses, and two seborrheic keratoses cleared completely; however, there was no statistically significant difference in the length, color, and surface characteristics between the study group and the control group. Skin replicas and scanning electron microscopy can be used to evaluate lesion surface characteristics, dimensions, and therapeutic effects.

Solcoderm in the treatment of solar and seborrheic keratoses.

Solcoderm treatment was applied to 46 patients with solar keratosis and 40 with seborrheic keratosis. Evaluation at regular follow-up over a 10-week period showed disappearance of the lesions without any remnant in most cases. Treatment was found to be effective, convenient, and with essentially no side effects.

The sign of Leser-Trélat associated with carcinoma of the stomach.

The sign of Leser-Trélat, which refers to the sudden appearance and rapid increase in size and number of seborrheic keratosis associated with a visceral malignant tumor, is very uncommon. This case concerns a 54-yr-old woman presenting with Leser-Trélat's sign associated with advanced gastric cancer. Seborrheic keratosis faded after resection of the tumor, but returned when the cancer recurred. At that time, 24-h urinary excretion of epidermal growth factor determined by radioreceptor assay was slightly elevated. Epidermal growth factor may play a part in the development of Leser-Trélat's sign.

Scanning electron microscopy of yeasts in seborrheic keratoses.

Lesions of seborrheic keratoses were studied by scanning electrom microscopy. Numerous yeasts were present on the karatinized cells and hairs associated with the lesions. Surfaces of some yeasts were smooth and others were ruffled. The ruffles suggested the presence of a glycocalyx. The yeasts exhibited the morphological features of Pityrosporum ovale.

The seborrheic disease complex. An approach to underlying causes and therapies.

Seborrhea is a multifaceted disease, and it is a common presentation in the small animal practice setting. This article addresses the considerations to be aware of in approaching the diagnosis of this complex entity. Emphasis is placed on recent advances of diseases that cause seborrhea and on appropriate therapy. A simplified practical approach to topical antiseborrheic shampoo therapy is presented.

New treatment for seborrheic alopecia: the ligature of the arteries of the scalp.

Bilateral ligature of the superficial temporal arteries and of the posterior auricular arteries is proposed as a treatment for seborrheic alopecia. The arterial circulatory dynamics are, thus, replaced by capillary circulatory dynamics. Hypoxia is produced which inhibits enzymatic systems and lessens nocuous action of androgen and lipid factors on the pilosebaceous effectors. The histologic study shows that the production of sebum is greatly reduced and the condition of the hair follicle is strikingly improved.

A case of hepatocellular carcinoma with the sign of Leser-Trelat: a possible role of a cutaneous marker for internal malignancy.

A rare case of hepatocellular carcinoma who developed the complication of the sign of Leser-Trelat is reported. The patient, a 57-year-old male, visited our hospital with complaints of generalized malaise and anorexia. A diagnosis of hepatocellular carcinoma was made based on elevated alpha-fetoprotein measurement, ultrasonography, and hepatic arteriography findings. Chest x-ray film suggested pulmonary metastases of hepatocellular carcinoma. Thereafter, complications of the seborrheic keratosis developed in the trunk and the skin lesion was diagnosed as the sign of Leser-Trelat associated with hepatocellular carcinoma. The patient died of pneumonia 9 months after development of the sign of Leser-Trelat.

Florid cutaneous papillomatosis.

The abrupt onset of acanthosis nigricans (AN) or of multiple seborrheic keratoses in middle-aged or older people is often associated with underlying malignant neoplasms. We describe a 64-year-old woman who developed a large number of verrucous papillomas on the trunk and extremities, followed within a few months by the sudden appearance of both the lesions of AN and those of multiple seborrheic keratoses. Laparotomy disclosed a gastric adenocarcinoma. A number of similar cases have been described in the literature. It is emphasized that patients with suddenly appearing verrucous papillomas should be examined carefully for an underlying cancer. To call attention to this eruption, which may be mistaken for generalized warts, we suggest the term "florid cutaneous papillomatosis".

[Therapy of seborrheic eczema with an antifungal agent with an antiphlogistic effect].

Numerous AIDS patients show the typical seborrheic eczema in a very prominent way. For this is an inflammatory disease, combination preparations were taken frequently which contain antimycotics and corticosteroids. We investigated 7 antimycotic compounds in 3 inflammatory models: amorolfin, ciclopiroxolamine (cic), fluconazole, ketoconazole, miconazole, naftifine, and rilopirox. In an in vitro model the inflammatory activity towards the 5-lipoxygenase was investigated. 1,000 mumol naftifine, 100 mumol ketoconazole, 50 mumol cic, and 10 mumol rilopirox inhibited 5-HETE by 90%. In a cell culture model only cic had a significant activity towards cyclo-oxygenase. In this model the inhibition of the prostaglandin E2 liberation by 1 mumol cic was 40%. In an in vivo model the anti-inflammatory activity on a mouse ear was investigated (arachidonic acid induced). In this model only cic showed a significant inhibition of inflammation (50%) at 1 mg/ear. These investigations show, that cic has a strong antiphlogistic activity. In an open clinical trial with 20 patients suffering from seborrheic eczema after 4 weeks on cic cream a strong inhibition of infiltration and flakiness had been observed. The antimycotic compound cic offers a possibility to treat inflammatory mycoses without using corticosteroid combinations. In a double blind clinical trial (tinea) where cic was compared with a cic/hydrocortisone combination no statistical differences were found.

Terminal notching of the QRS complex in V1 and V2 in seborrheic patients.

Widening of the QRS complex by terminal notching in leads V1 and V2 was observed in 6 seborrheic patients with adequate personal hygiene. This anomaly disappeared when the presternal skin was degreased thoroughly with cationic detergent before placing the conductive paste and electrodes. We attribute these electrocardiographic alterations to variations in the apocrine secretions in the presternal region, which modified the resistance of the skin to electrical potentials.

Increased epidermal growth factor receptors in seborrheic keratoses and acrochordons of patients with the dysplastic nevus syndrome.

Seborrheic keratoses and acrochordons may change in biologic behavior with pregnancy or during treatment with sex steroids. Because epidermal growth factor receptor localization may change in hyperproliferative skin diseases, we compared epidermal growth factor receptor immunolocalization in seborrheic keratoses and acrochordons from women who were or were not pregnant or taking oral sex steroid hormones. Epidermal growth factor receptor was further investigated in growing and quiescent seborrheic keratoses and acrochordons from women with and without the dysplastic nevus syndrome. The epidermal growth factor receptor concentration was strikingly elevated in suprabasilar keratinocytes of growing seborrheic keratoses and acrochordons from patients with the dysplastic nevus syndrome who were pregnant or taking sex steroid hormones and less elevated in growing seborrheic keratoses from other patients. In contrast, the epidermal growth factor receptor distribution pattern in acrochordons did not correlate as well with the history of growth of these lesions in normal patients. These data suggest sex steroids may affect epidermal growth factor receptor metabolism in benign epidermal hyperproliferative lesions, particularly in patients with the dysplastic nevus syndrome.

[Experimental studies on seborrheic alopecia. II. Determination of the structural differences between testosterone transported cytosol protein of the bald and hairy areas].

By previous isolation of the testosterone transport cytosol protein in bald and hairy areas in patients with male pattern baldness, the author studies the differences concerning the protein citosol-testosterone complex, after fixation by means of labelled testosterone and electrophoresis, measuring the corresponding radioactivity curves. The results show that there are important structural differences for the cytosol protein between the hairy and bald regions.

[Facultative and obligate pathogenic moulds in skin affections].

Molds are vegetable microorganisms, which differ from dermatophytes sensitive to griseofulvin, and from yeasts, which do not form aerial mycelium. Most of the molds, phytopathogenic or which live from dead organic substances, are apathogenic to humans. Only a couple of dozen species can parasitize on the skin, usually together with dermatophytes or yeasts. Onychomycoses with molds appear mostly in elderly people, and fungus affections of external auditory passage in seborrheic eczema of the ear. The hair can be infected by Piedraia hortae, resulting in hard black nodules. After the identification of molds on the skin, criticism is necessary, since in more than 95% of the cases they are accidental germs. Several cultures and microscopic tests are necessary to assure the diagnosis. Broad-spectrum antimycotics is the predominant choice for treatment, but also amphotericin B, nystatin and pimaricin.

[Seborrheic pemphigoid (Schnyder)].

Seborrheic pemphigoid first described by Schnyder is considered to be a clinical variant of bullous pemphigoid which resembles pemphigus erythematosus. Ruptured bullae and erosions covered with crusts involving predominantly the median parts of the trunc characterize the clinical picture. Histologic and immunofluorescence findings substantiate the diagnosis of pemphigoid. The clinical, histopathologic and immunofluorescence features as well as the course of the disease in a 74 year old woman are described.

Pityrosporum ovale/orbiculare in papillomatous nevus cell nevi.

62% of 200 papillomatous nevus cell nevi compared to 45% of 40 seborrheic keratoses showed the presence of Pityrosporum ovale/orbiculare (Malassezia furfur) on their surface, supposedly representing a saprophytic state of the microorganisms in vivo. The papillomatous nevus cell nevi were of the compound or dermal type. Significant inflammation, dysplasia or major changes in color were absent. Electron microscopy revealed the characteristic, sometimes budding fungal cells. A relationship between the presence of Pityrosporum organisms and papillomatosis is discussed.

Long-term treatment with the combination ethinylestradiol and cyproterone acetate in polycystic ovary syndrome.

This study evaluates the effect of therapy over a long period of time (36 cycles without interruption) with the monophasic combination containing 0.035 mg of ethinylestradiol and 2 mg of cyproterone acetate (EE35-CPA) on hormonal and clinical parameters of 66 patients with polycystic ovary syndrome (PCOS). During the administration of the pill a significant decrease in the LH/FSH ratio and in adrenal and ovarian androgens has been observed, as well as a significant increase of the Sex Hormone Binding Globulin (SHBG). The progressive decrease of the total androgenic activity explains the clinical results that have been obtained: at the 36th cycle of therapy acne disappeared in 100% of the cases, seborrea in 76.4% and hirsutism in 75%. Our results underline the need for a continuous administration without interruption of the pill with CPA in patients with clinical hyperandrogenic symptoms.
