Phenotypic analysis of resident lymphoid cells in the conjunctiva and adnexal tissues of rat.

The conjunctival associated lymphoid tissue is considered to be an integral part of the mucosal immune system. Under normal circumstances immune mechanisms in mucosal associated lymphoid tissue of the gut and bronchus can selectively suppress, rather than enhance, immune responsiveness to encountered antigens, inducing a state of tolerance. It is possible that conjunctival associated lymphoid tissue can also induce a state of tolerance to encountered antigens. Such a response may be exploited to modulate immune mediated ocular disease. Enhanced tolerance may protect the host against foreign antigen. Alternatively, under certain circumstances when the normal immune system is altered or disrupted the mucosal tissue may act to induce sensitisation and trigger immune mediated disease. The rat is frequently used as an animal model of immune mediated eye disease, but the normal profile of immune cells in the rat conjunctiva has not been studied. This information is essential for meaningful interpretation in the experimental situation. In this study we examined the immunophenotype of lymphoid tissue associated with the conjunctiva, lacrimal gland and Harderian gland of the Lewis rat. CD4+, Ia+ and the monocyte/macrophage population of cells were found predominantly in the substantia propria of the conjuctiva and interstitial connective tissue of the glands. CD8+ cells were distributed mainly in relation to the conjunctival and glandular epithelium. Goblet cells stained strongly with the monoclonal antibody (MAb) MRC OX-39, which is a marker for IL-2 receptors. The overall pattern of distribution of immunocompetent cells in the rat was found to be similar to that reported in humans.

Classification of adnexal tumors by transvaginal color Doppler.

The purpose was to evaluate the validity of transvaginal color Doppler sonography for differentiating between benign and malignant ovarian tumors. Color Doppler and Duplex measurements were obtained in 212 (144 benign, 68 malignant) ovarian and tubarian tumors preoperatively. One hundred and thirty-one patients were postmenopausal and 81 premenopausal. An ATL UM9/HDI was used. The following criteria were analyzed: minimum and mean resistance index and pulsatility index, number and distribution of tumor arteries, diastolic notch, and the maximum, minimum, mean, and sum of all peak systolic, maximum enddiastolic, and time-average maximum velocities. Most criteria showed highly significant differences between benign and malignant tumors with variable overlaps. RImin gives a sensitivity of 80%, specificity of 69%, and accuracy of 75% for postmenopausal patients and 80, 59, and 67% for pre- and postmenopausal patients, respectively. PImin gives equivalent results. The number of tumor arteries and the maximum flow velocities increase the accuracy. The summation of all flow velocities gives the best result with a sensitivity of 93%, specificity of 85%, and accuracy of 87% for postmenopausal and 91, 76, and 80% for pre- and postmenopausal tumors, respectively. Flow data show no relevant differences between low malignant potential tumors and ovarian carcinomas. Serous cystadenomas and benign teratomas show higher differences than mucinous cystadenomas, functional cysts, and endometriomas in comparison to malignancies. A separate analysis of pre- and postmenopausal tumors is important. Differentiation seemed better for post- than for premenopausal tumors. The four flow criteria (RImin, number of tumor arteries, and maximum and sum of all peak systolic velocities) seemed appropriate for tumor differentiation. However, this study confirms that a single measurement is not sufficient to differentiate ovarian lesions. Measurements of flow velocities (e.g., maximum and sum of all peak systolic velocities) are superior compared with RImin and PImin.

Complement C3, coeruloplasmin and PMN-elastase in the ejaculate in chronic prostato-adnexitis and their diagnostic value.

The clinical differential of chronic prostatitis and psycho-vegetative urogenital syndrome with objective laboratory tests is very difficult. 265 ejaculates with possible chronic prostatitis were bacteriologically examined (including the search for STD agents). To verify an inflammatory process in the prostate and adnexae, we tested the C3 complement, coeruloplasmin and PMN-elastase levels in ejaculate. In addition, semiquantitative leucocyte counts in stained smears of the ejaculate were carried out. 185 of 265 patients had C3 complement below detection levels or in the normal range excluding inflammation of prostate or adnexae. 16.8% of the C3-negative ejaculates showed an elevated PMN-elastase level associated with urethritis anterior and/or posterior caused by STD agents. 80 patients showed elevated C3 levels; 38.8% with elevated coeruloplasmin and PMN-elastase levels. The semiquantitative leucocyte count in the stained smear proved the least sensitive method for verifying an inflammation. Enterococci (55.3%), Mycoplasma (18.8%) and Escherichia coli (16.5) were the dominant pathogens of chronic prostatitis present in number of 10(2) cfu/ml or greater than 10(5) cfu/ml. A correlation to the intensity of the inflammation was not found. These results show how important it is to realise a complete bacteriological examination as well as to determine the C3 complement, coeruloplasmin and PMN elastase.

Adnexal ring of ectopic pregnancy detected by contrast-enhanced CT.

We present a case of a 37-year-old woman who presented with acute right lower quadrant abdominal pain. Work up with contrast enhanced CT to rule out appendicitis revealed dramatic ring enhancing cystic structure in the right adnexa corresponding to tubal ring sign of ectopic pregnancy seen on subsequent pelvic ultrasound. Right tubal ectopic pregnancy was confirmed at surgery.

[Benign adnexal skin tumors].

Epithelial tumors of the cutaneous adnexae are considered to originate from three different structures, i.e., hair follicles, eccrine/apocrine sweat glands, and sebaceous glands. Although this derivation provides a practical basis for classification, some tumors may exhibit a mixed differentiation and thus should be categorized according to the prevailing phenotype. The overwhelming majority of hair follicle tumors are benign but the main diagnostic differential for malignant tumors is basal cell carcinoma. Despite a broad histological spectrum, benign tumors of sweat glands are readily recognized on the grounds of a well characterized morphology. However, it may be difficult to rule out malignancy as each benign entity has a malignant counterpart. True sebaceous neoplasms are rare when hyperplasia and hamartoma are disregarded and in extraocular locations most of them are benign.

Adnexal torsion: MR imaging findings of viable ovary.

We report a case of torsed ovarian cystic tumor, in which contrast-enhanced high-resolution MR images accurately contributed to the diagnosis of torsion despite the lack of symptoms and to the preoperative evaluation of viability of the edematous ovary. Accurate preoperative assessment by MR images and prompt conservative surgical approach succeeded in salvaging the involved ovary.

Testicular adnexal fibrous pseudotumors.

Fibrous pseudotumors of the testicular adnexae presenting clinically as a testicular malignancy are uncommon. Although fibrous pseudotumors are benign, they are usually diagnosed after radical orchiectomy. Radical orchiectomy occasionally may be avoided if this benign process is considered in the differential diagnosis of testicular neoplasms.

Multiple adnexal tumors and a parotid basal cell adenoma.

A 72-year-old woman had a preauricular mass and several scalp nodules. Her medical history included excisions of multiple cutaneous tumors, including cylindromas, trichoepitheliomas, basal cell epitheliomas, and a trichilemmoma. The parotid mass and a scalp nodule were excised. On light microscopy, the parotid mass and scalp nodule appeared similar. Ultrastructural features revealed by electron microscopic analysis of the tumors allowed diagnosis of a cylindroma of the scalp and a parotid basal cell adenoma. The association of trichilemmoma and membranous basal cell adenoma of the parotid has not been previously described. The pathogenesis of these multiple cutaneous tumors from a common cell of origin is considered.

The VCUAM (Vagina Cervix Uterus Adnex-associated Malformation) classification: a new classification for genital malformations.

OBJECTIVE: With an incidence of up to 5% in the general population, genital malformations are a frequent clinical occurrence. However, using the existing published classifications of malformations, difficulties arise in classifying genital malformations appropriately. The aim of the present study was to produce a simple, systematic, and reproducible classification system. DESIGN: A systematic arrangement of genital and associated malformaltions, using a structure similar to that in the TNM classification of oncological tumors, was developed and validated. SETTING: Patients with genital malformations in a university hospital. PATIENT(S): Ninty-nine premenopausal patients with genital malformations. INTERVENTION(S): Patients were diagnosed for genital malformation using laparoscopy or magnetic resonance imaging. MAIN OUTCOME MEASURE(S): A new classification (VCUAM) is presented to evaluate patients with different genital malformations. RESULT(S): The external and internal female genital organs were divided into the following subgroups in accordance with the anatomy: vagina (V), cervix (C), uterus (U), and adnexa (A). Associated malformations were assigned to a subgroup (M) relative to each specific organ. The classification was validated in a group of 99 patients with genital malformations. CONCLUSION(S): The VCUAM classification for the first time makes it possible to reflect even complex malformations in a precise and individual fashion, taking associated malformations into account. The classification makes it easier to provide appropriate clinical care for the affected patients.

Distribution of cannabinoid receptor 1 (CB1) and 2 (CB2) on sensory nerve fibers and adnexal structures in human skin.

BACKGROUND: Cannabinoid receptors mediate the psychopharmacological action of marijuana and have been localized in the central and peripheral nervous system as well as on cells of the immune system. OBJECTIVE: Up to now, two cannabinoid receptors (CB1 and CB2) have been cloned and recent studies on animal tissue gave evidence for the presence of cannabinoid receptors in the skin. METHODS: In the present immunohistochemical investigation we determined the precise localization of CB1 and CB2 in sections of human skin and in one case of mastocytosis. RESULTS: CB1 and CB2 immunoreactivity was observed in cutaneous nerve fiber bundles, mast cells, macrophages, epidermal keratinocytes, and the epithelial cells of hair follicles, sebocytes and eccrine sweat glands. In epidermal keratinocytes, hair follicle and sebaceous glands, CB1 and CB2 were distributed in a complementary fashion. Double-immunostaining with an anti-CGRP antibody suggested the presence of cannabinoid receptors on small afferent peptidergic nerves. CONCLUSION: The abundant distribution of cannabinoid receptors on skin nerve fibers and mast cells provides implications for an anti-inflammatory, anti-nociceptive action of cannabinoid receptor agonists and suggests their putatively broad therapeutic potential.

Malignant adnexal masses in pregnancy.

Ovarian tumors during pregnancy are very rare; however, a cancer diagnosis causes distress to the couple. Reassurance is paramount, and the first consideration should be given to the safety of the mother. If both mother and fetus can be preserved, treatment to minimize the risks to both should be planned accordingly. It is imperative to care for the patient with a multidisciplinary team that includes a high-risk obstetrician, a gynecologic oncologist, and a medical oncologist specialized in gynecologic cancers.
