[Use of food additives in a multimodal treatment of disseminated pulmonary tuberculosis].

Effects have been studied of multimodality treatment on the function of external breathing in patients with disseminated pulmonary tuberculosis using food additives Trace Minerals. Positive dynamics has been recordable of changes in indices for forced lung vital capacity, minute oxygen consumption, both prior and after loading, and maximum volume speed (MVS75) before loading. The obtained data are indicative of the efficacy of using vitamin-mineral additives Trace Minerals evidenced by improvement in bronchial patency and normalization of ventilation-and-perfusion relations in patients with destructive forms of pulmonary tuberculosis.

Treatment of paraphilia in young adults with leuprolide acetate: a preliminary case report series.

Some juveniles who engage in sexual offenses may have a paraphilia, a psychiatric disorder characterized by a pervasive pattern of deviant and impairing sexual fantasies, thoughts, and/or behaviors. Though there is no known cure for these conditions, paraphilias can be effectively managed using a multimodal treatment approach. This may include the use of psychotherapeutic and pharmacological treatment interventions, including antiandrogen medications. One such agent, leuprolide acetate (leuprolide), a luteinizing hormone-releasing-hormone agonist, has been shown to be effective in reducing paraphilic symptoms in adult patients. To date, however, there is no published data on its use and effectiveness in adolescent and young adult paraphilic patients. This study consists of a case report series of six young adult patients treated with leuprolide. All subjects had been diagnosed with at least one paraphilia (i.e., Pedophilia, Sexual Sadism, Frotteurism, and Paraphilia Not Otherwise Specified). All subjects had been refractory to treatment in a residential program for adolescent sex offenders prior to initiation of leuprolide. All six subjects reported a reduction in sexually deviant symptoms following treatment with leuprolide. Clinicians rated four as much improved and two as moderately improved. The treatment was well tolerated in all six subjects. This preliminary case series supports the conclusion that leuprolide deserves further examination as a potentially safe and effective component in the treatment of young adult patients with paraphilia.

[The efficacy of treatment using expanded programs of megavolt irradiation and polychemotherapy in patients with lymphogranulomatosis in stages I, II and IIIA].

The authors relate the results of multimodality treatment of 90 patients with stages I-IIIA lymphogranulomatosis. The total survival amounted to 86.4 +/- 3.8% within the time exceeding 7 years. The efficacy of the treatment of patients with lymphogranulomatosis of different stages by using multi-beam and broad-beam radiation is under comparison. The data obtained indicate that total radiation of lymph nodes in patients with stage II lymphogranulomatosis is not advisable.

[Evaluation study of the opinion on Di Bella's multimodal therapy of patients with tumors. 2].

PURPOSE: To evaluate the opinion of patients with tumors who chose spontaneously to undergo the traditional anti-cancer treatment about Di Bella's therapy after the negative results of the experimental trial. MATERIALS AND METHODS: From November 23rd, 1998 to December 10th, 1998 a questionnaire was distributed among 50 patients with tumors. The questionnaire was anonymous, self-administered and included 9 questions. The data were compared, when possible, with those obtained in a previous study. RESULTS: Overall, 40 (70%) patients accepted favourably the conventional therapy; 27 (54%) patients did not believe in the experimental trial, as it was performed; 34 (68%) patients think the experimental trial should be repeated. CONCLUSIONS: The negative results of the experimental trial caused a distrustful feeling towards the Di Bella multi-therapy; but there is much confusion. There is a strong feeling that the conventional medicine is linked to powerful interests of the international pharmaceutical firms.

Radionuclide investigations of the urinary tract in the era of multimodality imaging.

This article presents the role of nuclear medicine procedures in investigating renal and parenchymal disease, as well as upper urinary tract abnormalities. More specifically, the use of scintigraphy is described in the exploration of urinary tract dilatation and UTIs, vesicoureteric reflux, renovascular hypertension, and renal transplants. With a low radiation burden and the absence of sedation, these nuclear medicine procedures are easy to perform and can provide clinicians with valuable data on renal perfusion and the function of individual kidneys, as well as on urinary tract dynamics. However, knowledge of limitations and technical pitfalls is essential in understanding the role of scintigraphy among contemporary imaging methods and the unique information it supplies in nephrourology.

Cerebral perfusion pressure and cerebral tissue oxygen tension in a patient during cardiopulmonary resuscitation.

OBJECTIVE: To report on the effects of cardiopulmonary resuscitation (CPR) instituted immediately after a cardiac arrest on cerebral perfusion pressure (CPP) and cerebral tissue oxygen tension (PbrO(2)). DESIGN: Case report. SETTING: ICU of a university hospital. PATIENT: A head-injured 17-year-old man submitted to multimodal neurological monitoring underwent sudden cardiac arrest and successful CPR. INTERVENTIONS: External chest compression, 100% oxygen ventilation, volume expansion and standard ACLS protocols. MEASUREMENTS AND RESULTS: Heart rate, ECG, mean arterial blood pressure (MABP), ETCO(2), PaO(2), intracranial pressure (ICP), CPP and PbrO(2) were continuously monitored during CPR and data recorded at 15-s intervals by a dedicated personal computer. At the onset of the cardiac arrest, PbrO(2) decreased to zero. The institution of CPR resulted in a progressive increase of MABP, CPP and PbrO(2). Assuming, on the basis of previous experimental and clinical reports, 8 mmHg PbrO(2) as a possible ischaemic/hypoxic threshold value, during the first 6.5 min of CPR, PbrO(2) values were below this threshold (range 0-7 mmHg) and CPP values were <25 mmHg for 81.5% of the time. In the following 5.5 min, more efficient CPR generated CPP values >25 mmHg for 77.3% of the time. These values were associated with a PbrO(2) >8 mmHg (range 8-28 mmHg) at all times. CONCLUSIONS: In the clinical setting of a witnessed cardiac arrest, immediate institution of CPR can be effective in generating PbrO(2) values above a supposed ischaemic/hypoxic threshold when CPP is >25 mmHg. PbrO(2) monitoring by the Licox system is sensitive and reliable, even at low values, and can be suitable for evaluating cerebral oxygenation during experimental CPR.

The National Cancer Data Base report on carcinoma of the gallbladder, 1989-1995.

BACKGROUND: Reports utilizing data from the Commission on Cancer's National Cancer Data Base (NCDB) have previously contained evaluations of time trends for stage of disease at diagnosis, treatment, and survival for multiple tumor sites. Data collected from 1989, 1990, 1994, and 1995 for carcinoma of the gallbladder are presented herein. METHODS: The data presented in this review were collected from hospital cancer registries from across the U.S. Seven calls for data yielded a total of 5,850,000 cases for the years 1985-1995, including 2574 gallbladder carcinoma cases from 1989-1990 and 2914 cases from 1994-1995 from hospital cancer registries across the U.S. These data represent approximately 8.8% and 8.4% of the estimated cases of liver and biliary track cancers diagnosed in the U.S. during the two respective time periods. RESULTS: There were no changes in patient demographics between 1989-1990 and 1994-1995. Most gallbladder carcinoma patients were white women. The documentation of tumor stage improved noticeably between the two study periods. There was no increased frequency in the occurrence of early stage tumors between the two reporting times, an era that saw the development and widespread application of laparoscopic cholecystectomy. Treatment differed according to stage of disease. Surgery alone, particularly nonradical surgery, was performed more frequently in the initial treatment of gallbladder carcinoma. A large percentage of gallbladder carcinoma patients received no definitive therapeutic intervention because of the advanced stage of disease at presentation and the lack of effective treatments for these cancers. Multimodality treatment was utilized more often for young patients. Survival was closely related to tumor stage, with 60% 5-year survival for Stage 0 patients, 39% for Stage I patients, and 15% for Stage II patients, but only 5% for Stage III patients and 1% for Stage IV patients. Patient outcome was not demonstrably affected by more aggressive therapy, nor was an adverse effect in results seen in early stage cancers between 1989-1990 and 1994-1995. CONCLUSIONS: The NCDB data are valuable in the evaluation of trends in malignant diseases, treatments, and patient survival. No substantial differences were apparent in the diagnosis, treatment, and survival of patients during this 7-year study period. The data do not support any adverse effect on outcome results with the introduction of laparoscopic cholecystectomy in the U.S.

Three-dimensional display of cortical anatomy and vasculature: magnetic resonance angiography versus multimodality integration.

We present two methods for acquiring and viewing integrated three-dimensional (3D) images of cerebral vasculature and cortical anatomy. The aim of each technique is to provide the neurosurgeon or radiologist with a 3D image containing information which cannot ordinarily be obtained from a single imaging modality. The first approach employs recent developments in MR which is now capable of imaging flowing blood as well as static tissue. Here, true 3D data are acquired and displayed using volume or surface rendering techniques. The second approach is based on the integration of x-ray projection angiograms and tomographic image data, allowing a composite image of anatomy and vasculature to be viewed in 3D. This is accomplished by superimposing an angiographic stereo-pair onto volume rendered images of either CT or MR data created with matched viewing geometries. The two approaches are outlined and compared. Results are presented for each technique and potential clinical applications discussed.

A non-rigid registration approach for quantifying myocardial contraction in tagged MRI using generalized information measures.

We address the problem of quantitatively assessing myocardial function from tagged MRI sequences. We develop a two-step method comprising (i) a motion estimation step using a novel variational non-rigid registration technique based on generalized information measures, and (ii) a measurement step, yielding local and segmental deformation parameters over the whole myocardium. Experiments on healthy and pathological data demonstrate that this method delivers, within a reasonable computation time and in a fully unsupervised way, reliable measurements for normal subjects and quantitative pathology-specific information. Beyond cardiac MRI, this work redefines the foundations of variational non-rigid registration for information-theoretic similarity criteria with potential interest in multimodal medical imaging.

Midbrain dopamine and prefrontal function in humans: interaction and modulation by COMT genotype.

Using multimodal neuroimaging in humans, we demonstrate specific interactions between prefrontal activity and midbrain dopaminergic synthesis. A common V(108/158)M substitution in the gene for catecholamine-O-methyltransferase (COMT), an important enzyme regulating prefrontal dopamine turnover, predicted reduced dopamine synthesis in midbrain and qualitatively affected the interaction with prefrontal cortex. These data implicate a dopaminergic tuning mechanism in prefrontal cortex and suggest a systems-level mechanism for cognitive and neuropsychiatric associations with COMT.

Endovascular treatment of cerebrovascular disease.

PURPOSE OF REVIEW: This review summarizes recent advances in endovascular therapy for cerebrovascular disease. RECENT FINDINGS: For ruptured aneurysms, the only large, randomized, controlled trial comparing surgical and endovascular treatment (the International Subarachnoid Aneurysm Trial) resulted in a significant reduction in death or dependency at 1 year, compared with surgery, providing sound evidence that coiling should be the treatment of first choice. Data from the International Study of Unruptured Intracranial Aneurysms demonstrated that treatment of unruptured anterior circulation aneurysms of under 7 mm with no history of subarachnoid hemorrhage is not justified. Embolization of arteriovenous malformations, as sole therapy, is curative only in a small percentage of cases, but can be part of a multimodal approach for reducing arteriovenous malformation size prior to surgery or radiotherapy. Partial treatment of complex arteriovenous malformations may be more dangerous than no treatment. Protection devices can reduce complication rates in carotid artery stenting, but scientific evidence is still lacking. Until the results of comparative trials are available, carotid artery stenting is indicated only in selected patients. Angioplasty and stenting of intracranial arterosclerotic disease is feasible but remains a high-risk procedure, indicated only in highly selected patients. In acute stroke therapy, new thrombolytics and clot-retrieval devices may result in better recanalization rates. SUMMARY: Advances in endovascular therapy have occurred in all areas of cerebrovascular disease. To obtain maximal patient benefit, endovascular treatment should be performed as an interdisciplinary approach in high-volume centers. Importantly, long-term follow-up review is necessary to clarify the overall role of endovascular treatment in the management of cerebrovascular disease.

Noninvasive imaging of oral premalignancy and malignancy.

Early detection of cancer and its precursors remains the best way to ensure patient survival and quality of life. Our specific aim is to test a multimodality approach to noninvasive diagnostics of oral premalignancy and malignancy. In the hamster cheek pouch model (120 hamsters), in vivo optical coherence tomography (OCT) and optical Doppler tomography (ODT) map epithelial, subepithelial, and vascular change throughout carcinogenesis. In vivo multiwavelength multiphoton (MPM) and second-harmonic generated (SHG) fluorescence techniques provided parallel data on surface and subsurface tissue structure, specifically collagen presence and structure, cellular presence, and vasculature. Images are diagnosed by two blinded, prestandardized investigators using a scale from 0 to 6 for all modalities. After sacrifice, histopathology is evaluated on a scale of 0 to 6. Imaging data are reproducibly obtained with good accuracy. Carcinogenesis-related structural and vascular changes are clearly visible to tissue depths of 2 mm. Sensitivity (OCT/ODT alone, 71 to 88%; OCT+MPMSHG, 79 to 91%) and specificity (OCT alone, 62 to 83%; OCT+MPMSHG, 67 to 90%) compare well with conventional techniques. Our conclusions are that OCT/ODT and MPM/SHG are promising noninvasive in vivo diagnostic modalities for oral dysplasia and malignancy.

Considerations for the future development of virtual technology as a rehabilitation tool.

BACKGROUND: Virtual environments (VE) are a powerful tool for various forms of rehabilitation. Coupling VE with high-speed networking [Tele-Immersion] that approaches speeds of 100 Gb/sec can greatly expand its influence in rehabilitation. Accordingly, these new networks will permit various peripherals attached to computers on this network to be connected and to act as fast as if connected to a local PC. This innovation may soon allow the development of previously unheard of networked rehabilitation systems. Rapid advances in this technology need to be coupled with an understanding of how human behavior is affected when immersed in the VE. METHODS: This paper will discuss various forms of VE that are currently available for rehabilitation. The characteristic of these new networks and examine how such networks might be used for extending the rehabilitation clinic to remote areas will be explained. In addition, we will present data from an immersive dynamic virtual environment united with motion of a posture platform to record biomechanical and physiological responses to combined visual, vestibular, and proprioceptive inputs. A 6 degree-of-freedom force plate provides measurements of moments exerted on the base of support. Kinematic data from the head, trunk, and lower limb was collected using 3-D video motion analysis. RESULTS: Our data suggest that when there is a confluence of meaningful inputs, neither vision, vestibular, or proprioceptive inputs are suppressed in healthy adults; the postural response is modulated by all existing sensory signals in a non-additive fashion. Individual perception of the sensory structure appears to be a significant component of the response to these protocols and underlies much of the observed response variability. CONCLUSION: The ability to provide new technology for rehabilitation services is emerging as an important option for clinicians and patients. The use of data mining software would help analyze the incoming data to provide both the patient and the therapist with evaluation of the current treatment and modifications needed for future therapies. Quantification of individual perceptual styles in the VE will support development of individualized treatment programs. The virtual environment can be a valuable tool for therapeutic interventions that require adaptation to complex, multimodal environments.

Multimodality treatment of stage IIIA non-small-cell lung carcinoma: a critical review of the literature and strategies for future research.

PURPOSE AND DESIGN: Stage IIIA non-small-cell lung carcinoma (NSCLC) is composed of regionally advanced yet potentially resectable disease. Many trials have evaluated a variety of therapeutic strategies. Most have been single-arm phase II trials, although a number of comparative phase III trials have also been performed. This review critically evaluates the existing literature on the treatment of stage IIIA NSCLC, particularly the various multimodality approaches that have been used. RESULTS: A review of the existing literature does not establish the optimal treatment of stage IIIA NSCLC. Although radiation therapy alone remains the most commonly administered therapy for apparently unresectable disease, the status of radiation as a "standard" therapy can be seriously challenged based on existing data. Similarly, although a number of studies have established that surgical resection is clearly feasible in selected patients with stage IIIA disease, the efficacy of surgery also remains to be definitively established. Many studies have explored neoadjuvant chemotherapy, either in conjunction with radiation or surgery, with results that are best described as conflicting and controversial. CONCLUSIONS: Without question, randomized phase III trials are required at this time to define what is to be considered optimal treatment. An attempt is made to define the most important questions that should be addressed in future phase III trials. Additionally, a number of study designs are suggested to best answer the therapeutic questions posed.

A critical risk-benefit assessment argues against the use of anthracyclines in induction regimens for newly diagnosed childhood acute lymphoblastic leukemia.

Although anthracyclines are associated with significant cardiac toxicity and their benefit remains unclear, they are included in nearly all current protocols for the treatment of childhood acute lymphoblastic leukemia (ALL). Currently open trials from most major groups use anthracyclines in the induction phase for all high-risk patients and in the delayed intensification phase for all patients regardless of risk classification. Our review of published randomized studies reveals no benefit for the addition of anthracyclines to induction phase of childhood ALL regimens consisting of vincristine, prednisone, and L-asparaginase (VPL), with or without a delayed intensification phase. No randomized studies have evaluated the use of anthracyclines in the delayed intensification phase of therapy. Furthermore, studies of relapsed patients indicated no benefit for the addition anthracyclines to maintenance regimens. Recent evidence from preclinical studies suggests that a combination of VPL with an anti-CD19 immunotoxin is more effective than VPL plus anthracyclines combination. Accumulated evidence exists that anthracyclines are associated with late-onset cardiac morbidity in about 25% of childhood ALL and other cancer survivors, and about 5% develop overt heart failure, with some requiring cardiac transplantation. Anthracycline-induced cardiotoxicity in children has no safe dose threshold and all doses are likely to cause significant myocardial damage. New data suggests that a unique cardiac mitochondrial exogenous NADH dehydrogenase is responsible for the anthracycline-induced oxygen radicals damage to the heart, and that chelators currently evaluated may not prevent late-onset cardiotoxicity in children. In view of these findings we urge extreme caution in using anthracyclines as part of multimodality ALL treatment programs, and strongly recommend reevaluation of what should be considered the best induction regimen for high-risk childhood ALL.

[Effect of transthoracic galvanization on the nature and incidence of complications at the hospital stage of treating patients with acute myocardial infarct].

The impact of transcardiac galvanization on the pattern and incidence of complications occurring in the hospital period was studied by using the data of examination of 110 patients with primary uncomplicated large anterior myocardial infarction who had been admitted to the hospital no later than 6 hours of the onset. ECG monitoring, echocardiography, X-ray graphy, echocardiography, and clinical study were used. It was found that a course of transthoracic galvanization in the multimodality therapy for acute myocardial infarction contributed to a more rapid relief of the pain syndrome, to a decrease in the incidence of cardiac rhythm and conduction disturbances, in relapses of myocardial infarction, left ventricular aneurysm, early postinfarction angina pectoris in the hospital period.

Construction and psychometric properties of a novel test for body representational neglect (Vest Test).

PURPOSE: Multimodal spatial neglect manifests itself also in nonvisual modalities such as audition, touch and body representation. Yet, quantitative tests for the diagnosis of nonvisual neglect are still quite rare. The purpose of the present paper was to develop and evaluate a novel, simple and sensitive test for the assessment of body representational neglect (BRN) in patients with left or right cerebral hemispheric lesions. METHODS: The vest test covers the front part of the trunk. The blindfolded subject wears the vest and is instructed to pick up all objects from the 24 pockets of the vest (12 on each side) as quickly as possible using the ipsilesional, nonparetic hand. Two samples of healthy control subjects (each N=25) using either their left or their right hand performed the test in identical way to obtain normative data for patients searching with their left hand (i.e. left hemisphere stroke patients) versus their right hand (i.e. right hemisphere stroke patients). The test can be performed within 5~minutes, even with aphasic or apractic stroke patients. RESULTS: Psychometric evaluations in a sample of 50 patients with unilateral stroke (25 leftsided, 25 rightsided) show high objectivity, high internal consistency (Cronbach's alpha=0.96), good retest-reliability (0.79 after 1 week in neglect patients) and good validity as compared with two other measures of BRN or multimodal neglect. Patient examples show that BRN as assessed with the vest test allows the detection of qualitatively and quantitatively different patterns of BRN, and shows double dissociations from visual neglect and from apraxia in left hemisphere stroke patients. Details of the test including instructions and cut-off values are given for users in the appendix of this article. CONCLUSIONS: In conclusion, the vest test is a sensitive, quick and reliable test for BRN which complements the assessment of visuo- and audiospatial neglect and allows to measure recovery (spontaneous or treatment-induced) in patients with BRN. Furthermore, it can help to improve our knowledge about the multisensory coding of our body and the surrounding space in the human brain.

Multimodal contribution to speech perception revealed by independent component analysis: a single-sweep EEG case study.

In this single-sweep electroencephalographic case study, independent component analysis (ICA) was used to investigate multimodal processes underlying the enhancement of speech intelligibility in noise (for monosyllabic English words) by visualizing facial motion concordant with the audio speech signal. Wavelet analysis of the single-sweep IC activation waveforms revealed increased high-frequency energy for two ICs underlying the visual enhancement effect. For one IC, current source density analysis localized activity mainly to the superior temporal gyrus, consistent with principles of multimodal integration. For the other IC, activity was distributed across multiple cortical areas perhaps reflecting global mappings underlying the visual enhancement effect.
