Between 1975 and 1985 from 522 patients, who had undergone caesarean section during their previous delivery or deliveries, per cent 63 have been delivered vaginally and 37 per cent with a caesarean section (52.8% primary, 47.2% secondary). The rate of spontaneous labour was higher, if patient had a spontaneous delivery before caesarean section or the first caesarean section has been performed because of a placenta praevia, a breech presentation or a fetal distress syndrome. Cephalopelvic disproportion went on in 67.2 per cent with a caesarean section. Rupture of the scare occurred in 2.9 per cent. Expectative management of delivery is justified following previous caesarean section. Oxytocin infusions are possible in cases if internal tocography will be done.
A comparison of dimensions of the pelvis in healthy children (340) and children with developmental defects of the ano-rectal area (40) has shown all the dimensions to be altered in children with the congenital pathology which illustrates different degrees of underdevelopment of the anatomical structures of this field. The greatest deviation in the pelvis dimensions values was revealed in children of the older age group and appears to be an illustration of decreased trophic functions of the tissues.

The realisation of a clinical disproportion is--still nowadays--a high demand for obstetricians. The ultrasonic examination of pelvis represents an intermediate stage between clinical and radiological examination. Description of pelvis by measuring the true conjugate and intrapartal diagnosis of abnormalities of adjustment and bearing of the fetus. It enables in many cases to set the indication of caesarean section earlier and safer.

By using symphysis-fundus measurements serially and plotting them on a curve, small-for-gestational-age babies can be detected. To determine which symphysis-fundus curve to choose for our population, the predictive values of three of the commonly used of these growth curves were compared using serial measurements obtained from 97 low-risk obstetric patients with accurate gestational ages. The curves of Calvert and Quaranta had the best sensitivities of 92.9% each compared with Belizan's (85.7%). However, the specificity of Calvert's and Quaranta's curves were poorer being 74.7% and 50.6% compared with 89.2% for Belizan. The positive predictive value for the curves were Belizan 57.1%, Calvert 38.2% and Quaranta 24.1%. The results indicate that for a Third-World urban population Belizan's curve is most suitable.
Obstetric intervention has been the subject of much recent criticism from both the medical profession and consumer groups who point to increasing caesarean section rates and question their justification. This article examines the indications for caesarean section and assesses the relative benefits and disadvantages.

Real-time ultrasound equipment has the potential for investigating the fetal neural axis and allows the diagnosis of many anomalies arising from this area. Five cases of Dandy-Walker syndrome, one of the major causes of congenital hydrocephalus, were diagnosed antenatally. In all cases ultrasound allowed specific recognition of the defect in the cerebellar vermis. This defect connected the fourth ventricle to a posterior fossa cyst in four cases and to an occipital meningocele in one case. In three cases the diagnosis was made prior to viability, and the parents elected termination of pregnancy. In two cases recognized in the third trimester, neurosurgical care was provided soon after delivery.

The value of radiopelvimetry is increasingly under question in cases of brow presentation except in cases of scarred uterus. Radiopelvimetry, in which the antero-posterior, transverse median and bi-sciatic dimensions were determined was carried out in 564 patients who gave birth during 1984. It would appear that cases of major narrowing have virtually disappeared and that most frequently encountered is transverse median narrowing. In this group of patients, the presence of a radiologically abnormal pelvis doubled the incidence of cesarians carried out during labor. Even if the biparietal diameter is included, it is very difficult to define a definite lower limit for eutocia. Radiopelvimetry is a useful indication but must leave an important place for the labor test.
291 giant babies with a birth weight over 5000 g (= 0.58 per cent) were registered among 49,976 newborns, delivered in the Central Hospital of Az Zawiyah (Libya) within 9 years. Some etiological factors have been made responsible for the origin of this intrauterine macrosomy. High multiparity combined with high age of the mothers are of evident importance. A second equivalent factor is the habit of nourishment with vegetable in this region. Diabetes mellitus and hypertensive disorders in pregnancy are not so much frequent causal factors of giant babies than in middle of europe, where the total frequency is considerably lower. The stillbirthrate of 12.7 per cent is significant increased compared to a control group, but in the same level of comparable studies.
