[Femoro-patellar joint. Advantages of axial projection in external rotation of the knee (author'r transl)].

The classical technic for radiological examination of femoro-patellar joint cannot visualize a lot of real chondromalacia. Their diagnosis, and, therefore, their treatment, usually surgical, rely on doubtful results of clinical examination. Axial projections combined with external rotation of the knee is not perfect, but it is a good improvement. It reveals many femoro-patellar syndroms without any radiological expression until this time.

Teleconsulting: a practical account of pitfalls, problems and promise. Experience from the TEAM project group.

In Wales the Tele Education and Medicine (TEAM) project has given clinicians, in both the primary- and secondary-care sectors, practical experience in teleconsulting. From this project we have been able to identify the potential pitfalls and problems in developing and using this new technology. These problems include: using teleconsulting simply because it is fashionable; the failure to base it on the needs of the patients and the primary-care physicians; the failure to validate accuracy, acceptability and cost; the failure to adapt to new technical improvements; and the failure to scale up from a pilot project to a wider teleconsulting service. In our experience technical support is essential and is best facilitated by multisite cooperation, rather than small free-standing projects.

[Overall design and proof-test of an integrated environmental control and life support system (ECLSS) for demonstration and verification].

Objective. To test and demonstrate embryonic form of our future space station ECLSS, which will also form an advanced research and test ground facility. Method. The following functions of the system were tested and demonstrated: integrated solid amine CO2 collection and concentration, Sabatier CO2 reduction, urine processing thermoelectric integrated membrane evaporation, solid polymer water electrolysis O2 generation, concentrated ventilation, temperature and humidity control, the measurement and control system, and other non-regenerative techniques. All of these were demonstrated in a sealed adiabatic module, and passed the proof-tests. Result. The principal technical requirements of the system and each regenerative subsystem were met. The integration of system general and each subsystem was successful, and the partial closed loop of the system's integration has been realized basically. Conclusion. The reasonableness of the project design was verified, and the major system technical requirements were satisfied. The suitability and harmonization among system general and each subsystem were good, the system operated normally, and the parameters measured were correct.

A prospective randomized comparison of single- and double-lumen needles for transvaginal follicular aspiration.

Patients undergoing ultrasound-directed transvaginal follicular aspiration in a large in vitro fertilization (IVF) program were randomized for retrieval with either a single-lumen needle (SLN; N = 22) or a double-lumen needle (DLN; N = 22) to compare recovery rates and the technical aspects of their use. Two hundred ten and two hundred two follicles were aspirated with each needle, respectively. Follicular diameters were measured ultrasonically at the time of aspiration and recorded. One or more washes were performed when using the DLN and the SLN was withdrawn each time to recover the fluid in the dead space of the needle. The distribution of follicular sizes was the same for both needles. Oocyte recovery rates (SLN = 65.7%; DLN = 63.9%) and the incidence of fractured zonae (SLN = 9.1%; DLN = 6.4%) were the same for both needles (alpha greater than 0.50; beta less than 0.01). Although there were no differences between the two needles in the number of oocytes provided for IVF, there were technical differences. The DLN needle was more flexible and frequently deviated from the projected path as observed by ultrasound. The SLN may be preferable because it is technically easier to use; however, there may remain specific indications for the use of the DLN.

Regional population projections: a multiple base-point approach.

Some alternative population projections are presented for Poland's major regions, based on a series of selected base years from 1977 to 1985. The author "interprets the corresponding observed demographic patterns as alternative model patterns on which consecutive projection runs are based." The projections, which are extended to 2050, exclude estimates for international migration.

Three-dimensional virtual environments for cybertherapy: a psychosocial approach to effective usability.

The present study is to be considered in the framework of the VEPSY UPDATED project, whose main objective is to prove the technical and clinical viability of using virtual reality therapy (VRT) in clinical psychology by means of portable and shared VR systems. This contribution illustrates the attempt to elaborate a new psychosocial model of usability evaluation analysis, allowing general definitions in the context of the project, by considering areas of VR not yet completely explored from the usability point of view. The theoretical background is based upon an ethnometodological approach, a perspective that gives evidence of how people, in specific social situations, are able to solve complex tasks producing shared meanings and achieving their goals during the interactions, in order to make their actions understandable and successful.

Technical aids as an economic alternative.

Interest in technical aids is growing. Occupational therapists are beginning to play a role in this area. Four programs in which occupational therapists are involved in Canada are described. The programs are: 1. a province-wide, private agency supported program 2. a university based research project funded by a provincial government 3. a technical aids component of the rehabilitation program at a Regional Rehabilitation Centre. 4. a private consulting business. The economic implications of each program are examined to determine whether the program can be justified economically and whether the occupational therapist can justify economically, adding involvement with technical aids to their existing role.

Student's perceptions of a virtual PBL experience.

OBJECTIVE: Our purpose is to present findings regarding student attitudes towards a virtual PBL program used to standardize their pediatric clinical experience. DESCRIPTION: With funding provided by the Fund for the Improvement of Post-Secondary Education, we developed Project LIVE (Learning through Interactive Video Education), a CD-ROM/Web hybrid program that uses digital video cases to conduct "virtual" problem-based learning groups with students doing a clinical rotation in a remote setting. Cases were progressively disclosed by videos of patient/physician encounters on a CD-ROM. Groups of five students and a faculty facilitator collaborated, teaching each other within the discussion section of the program. We conducted a multifaceted evaluation of Project LIVE to study the impact of case modality or distance on student learning and attitudes. We placed students in one of three groups (1) a face-to-face group with a paper case (FFT), (2) a face-to-face group with a video case (FFV), and (3) a virtual group (VG) with the digital video case. We then studied student attitudes about the three teaching formats. Over a six-month period three education specialists, who were not a part of the development team, conducted eight focus groups lasting one hour to assess student attitudes about Project LIVE. No one from the project team was present during these groups, and an independent evaluator analyzed the notes taken by each focus group leader. DISCUSSION: Trends across the groups included the following: (1) Authenticity (video)-Students reported that the authenticity of the case was a critical feature and that, "seeing (videos) made learning more memorable." Virtual and FFV groups reported more confidence in their ability to recognize abnormal findings in their patients. "You can't expect to teach clinical exam skills with a piece of paper." (2) Use of time-Students from all groups believed the cases were a good use of their time and improved their ability to solve clinical problems. They said it gave them an opportunity to "get away from just doing and focus on learning." However, the virtual groups complained of the lack of "a barometer for how much is too much" time. Some students reported spending an average of eight to ten hours per case over the period of a week. (In contrast, face-to-face groups met for three hours.) (3) Modeling clinical reasoning-Students believed the cases were valuable in structuring their knowledge, conceptualizing how to handle difficult situations, distinguishing abnormal from normal physical examination findings, and collaborating with their peers and their mentor to develop critical thinking. "It forced us to be logical" and ". how to think through the process-it mimics the real setting." (4) Technical support-The responsiveness of the Project LIVE staff was essential in assisting students in troubleshooting problems. (5) Distance component-Students preferred to work through the cases in face-to-face groups but agreed that the virtual experience is "good if you are in the middle of nowhere." This program was enjoyed by students and gave us an approach to standardizing experiences across multiple clinical sites.
