[Heterozygous beta thalassemia and gastrointestinal pathology].

A 29 month personal experience with patients clinically followed-up at the divisions of internal medicine and surgery of Comacchio Hospital, located in an area of high incidence of heterozygote Beta thalassemic carrier is presented. A greater predisposition of these heterozygote carriers to manifest duodenal ulcers and gastric neoplasms is noticed.

The surgical treatment of bronchial carcinoid adenoma: a personal experience.

Clinical and pathological observations on 7 resected cases of bronchial carcinoid have been reviewed. Bronchoscopy was the most definitive diagnostic procedure. Bronchoplastic procedures were the treatment of choice for these low-grade potentially malignant lesions. Indications for extensive lung resection were distal suppuration and tumor extention into the parenchyma. There were no operative deaths or major complications. All the patients are still alive (mean follow-up 39.4 months) with no recurrence of tumor.

Sexuality, personality, and eating disorders.

To assess sexuality, personality, and eating pathology in women with eating disorders (EDs), we asked a random sample of 234 clinicians to describe an ED patient (age 16-65). Restricting AN patients tended to be childlike and prim/proper, while BN patients tended to be flirtatious and promiscuous. A constricted/overcontrolled personality predicted a childlike sexuality independent of AN diagnosis, and an undercontrolled, emotionally dysregulated personality predicted impulsive sexuality above and beyond BN diagnosis. Constraint and impulsivity are not isolated to the domain of eating in women with EDs. Personality provides a link between eating and sexuality in at least a subset of women with EDs.

[Pericystic pathology and surgery of hepatic hydatidosis. Studies and personal experiences].

In the general study of the consequences of hydatid cysts on the parenchyma of the organ involved, particular interest was given to the conditions of the pericyst. Thus a genuine pericystic pathology was systematized for most evolved pericysts. The signs vary according to the various conditions, sometimes including simple fibrosis, sometimes calcifications with specific and different appearances, almost always chronic inflammation. Fissuration was rare and, more importantly, exogenous vesiculation, a frequent cause of recurrence, was more often noted. The treatment is chosen on these bases, and determined by the local situation, hepatobiliary complications and the general condition of the patient. The author demonstrates the surgical evolution he followed in relation with the progress made in liver surgery, from plain drainage methods to radical pericystectomy and hepatic resection procedures. The conclusions he has drawn as a Symposium moderator during the World Congress of Hydatology held in Madrid in 1985, which will be summed up in the report, still seem to be perfectly valid.

Personality and biographic data that characterize men who abuse their wives.

Personality and background data were presented on 250 men enrolled in group treatment for wife abuse. Follow-up data were available for 44 of these men. Standard measures available included 16PF, PSI, and MAST for personality and alcoholism. Background data included self-reports of level of violence, income, education, occupation, and violence in family of origin. Prior findings that link alcoholism and violence in family of origin to current family violence were confirmed. Level of violence was found to be related to more general pathologies, to alcoholism, and to prior family violence. Otherwise, no typology of violent men emerged in terms of pathology or general personality.

[Effects of sporting activities on the psyche of elderly people (author's transl)].

During a 1 year long investigation, 93 50-70 year old male persons took part in a special sports program. The comparison between the first and the second investigation showed a success of the training for several psychological dimensions in direction of less depression and less aggression, as well as a greater emotional stability, although results of the first investigation showed no pathologic deviation of personality structure of the participants. Comparing three groups; (sportsmen, former sportsmen, and people who never did sports before) it could be demonstrated that members of all three groups showed positive results concerning their personal traits, so that a positive coherence between sporting activities and less depression and more emotional stability became statistically significant.

Coping strategies, personality, and voice quality in patients with vocal fold nodules and polyps.

Emotional state affects the physiological mechanism involved in phonation. Differences in acoustical parameters of the voice under stress have been attributed to the coping mechanism used, which is based on the individual's perception of the situation. This study examines the relationship between coping strategies, personality, and voice in female subjects, ranging in age from 19.3-55.7 years, diagnosed with vocal nodules or polyps. The differences between coping strategies and personality are examined and compared with another group with no history of voice pathology. The relationship of personality and coping strategies to voice quality variables is reported. Results show that patients use emotional coping strategies more and cognitive coping strategies less than the comparison group. Type of voice pathology was found to be related to dominance, and a number of coping and personality variables were found to correlate significantly with voice quality.

[The value of child development psychological models for child and adolescent psychiatry].

The literature on personality research, developmental psychology and psychopathology has consistently revealed two types of personality. According to different theoretical and methodological approaches these types are considered as prototypes of a personality dimension, or as behavioral patterns in psychopathology. In the research on adult personality, they are known as extraversion and introversion. Child psychiatrists refer to these prototypes as externalizing and internalizing disorders. However, there are few empirical studies on the congruency between adult personality types and childhood disorders. Longitudinal studies from the USA and New Zealand give evidence that adult personality types can predict coping styles of school age children in stressful and challenging situations. Research on infant attachment to caregivers, revealing consistently different qualities of interaction patterns, may also be predictive for different coping styles in children of preschool age. Interdisciplinary discussions will focus on the question, whether early behavior patterns should be seen as predictors for adult traits (homotypic continuity) or as developmentally determined indicators for underlying functions (heterotypic continuity). Possibly, in early developmental stages behavior patterns have adaptive functions and in later stages become chronic strategies over time. Another topic concerns the relation between personality traits and pathologic behavior patterns and whether chronic strain may be influential.

[The most significative parameters for the diagnosis of focal neuropathy of the cubital nerve in the elbow].

OBJECTIVE: Determine the diagnostic validity of the most usual neurophysiological procedures in the examination of compressive neuropathies and/or ulnar nerve entrapment at the elbow. PATIENTS AND METHODS: A prospective observational study was carried out over a period of one year which included 19 patients with clinical features compatible with entrapment/compression of the ulnar nerve at the elbow, their neurophysiological diagnosis and surgical confirmation. We also studied 49 patients who were attended in our clinic for other reasons not related to disorders of the ulnar nerve. The variables studied were: maximum rate of motor conduction above the elbow; the amplitudes between spikes and the area between the first negative deflection and the last positive spike of the responses at the wrist and above the elbow. RESULTS: In the group of persons without disorders of the ulnar nerve the average rate of motor conduction was 58.07 m/s, with a confidence interval of 53-65. In the group of patients with specific clinical features the average rate of motor conduction was 47.26 m/s, with a confidence interval of 43.74-50.77. No relation was found between age and pathology, although rate of motor conduction was related to the presence of pathology. CONCLUSIONS: It was seen on logistical regression that the main variables for prediction of the presence or absence of pathology in a person were firstly motor conduction and secondly the amplitude index.

[Oral glucose tolerance tests--with or without estimation of serum insulin? (author's transl)].

Blood glucose and serum insulin values were determined after oral glucose tolerance tests in 528 persons with suspected pathological or disturbed glucose tolerance. Results were evaluated depending on sex, age, overweight, and familial diabetes. There were no differences of serum insulin secretion and behaviour of blood sugar between males and females. With increasing age there was a marked decrease of insulin secretion. In contrast there was increased hyperinsulinaemia with increasing overweight. This was particularly evident in persons with comparably reduced glucose tolerance but with different body weight. "Classical" changes of insulin secretion, as described for prediabetes and manifest diabetes in contrast to persons with normal metabolism, were only seen in selected groups of probands with normal weight. Serum insulin shows greater variability than blood glucose. Estimations of insulin are not necessary when assessing disturbances of glucose tolerance.

Pathology versus statistical infrequency: potential sources of gender bias in personality disorder criteria.

The antisocial, narcissistic, dependent, histrionic, and borderline personality disorders often obtain differential sex prevalence rates. One explanation has been that the diagnostic criteria for these personality disorders have different gender implications for maladaptivity (e.g., perhaps the dependent personality disorder diagnostic criteria are considered by clinicians to be more pathological for women than for men). This hypothesis was explored in two studies that obtained judgments by professional clinicians of the maladaptivity and statistical infrequency of personality disorder diagnostic criteria. Significant differences across gender were obtained for the frequency of diagnostic criteria but not for their maladaptivity. The personality disorder diagnostic criteria appear to be gender neutral with respect to their implications for maladaptivity.
