Methanol enhances the enantioresolution of dimetindene enantiomers with carboxymethyl-beta-cyclodextrin (CMCD) as chiral selector at a concentration below its optimal value. The same effect was observed with ethanol (EtOH), although less pronounced. On the other hand, the addition of isopropanol (IP) or acetonitrile (ACN) decreases the enantioseparation. To elucidate the underlying mechanisms of these observed effects, other neutral (beta-CD, hydroxypropyl-beta-CD, and trimethyl-beta-CD) as well as chargeable (carboxyethyl-beta-CD and succinyl-beta-CD) CD derivatives were also tested with MeOH as organic modifier. It can be concluded that the increased enantioresolution of dimetindene enantiomers was only noted with CMCD as chiral selector and a short-chain organic modifier containing an alcohol function. The slight deprotonation of CMCD at pH 3.0 was only partly responsible for the high enantioselectivity and the 'favourable' effect of MeOH or EtOH. An important feature that can be concluded from these results is that for this particular analyte approximately the same resolution can be obtained with a lower CMCD concentration and the addition of some MeOH, compared to a MeOH free buffer.

In order to establish any psychotropic effects of the angiotensin-converting enzyme inhibitor enalapril, the drug was administered in doses of 20 mg every morning for 14 days to 12 normal subjects, and compared with placebo on a battery of physiological, psychological and subjective tests, before and after the dose on the 1st and 14th days. Diastolic blood pressure was significantly reduced and heart-rate increased by enalapril as compared with placebo; one component (P1-N1) of the auditory evoked EEG response was increased and tapping rate quickened. The commonest side effect was tiredness. It was concluded that enalapril (unlike most other antihypertensive agents) did not lower mood but could enhance attention and alertness.

The basic cellular lesion in CNS of suckling rats treated with Pexid was studied by light and electron miroscopy. The most pronounced abnormality, the formation of various intracytoplasmic inclusions, was found in neurons, astrocytes, oligodendrocytes, ependymal cells, endothelial cells and fibroblasts. These abnormal inclusions were generally membrane-bound, although clearly non-membrane-bound inclusions were occasionally found. The several internal patterns of the inclusions were (1) lamellar, both concentric and parallel, (2) reticular and (3) crystalloid. These alterations were completely reversed following withdrawal of the drug. The structural characteristics of the abnormal inclusions in Pexid-treated animals were similar to those found with certain hypocholesterolemic, neuroleptic, anorectic, and antimalarial drugs. This suggests that the inclusions occurring within the cells of animals treated with any of these drugs may develop in a similar manner, and that the formation of such inclusions is likely to be a form of cellular reaction common to certain metabolic disturbances.
Lacrimal function was studied in 30 patients treated for glaucoma, with 0.25% timolol eye drops. Rose bengal and fluorescein staining disclosed punctate epithelial defects in 11 eyes after one week. During the following weeks there defects disappeared spontaneously in most eyes. Schirmer tests (I and II), tear lysozyme and pre-corneal film break-up time were significantly decreased by the treatment, while tear immunoglobulins were unimpaired. The authors conclude that topical timolol treatment decreases tear production. This effect is quantitatively limited and does not appear dangerous for normal eyes, although it may become so for eyes with an originally low lacrimal secretion.

Patients with severe hypertension and/or congestive heart failure (n = 281) who were unresponsive to other therapies and intolerant to captopril received enalapril treatment (mean dose 19.5 mg/day) under study conditions as part of a Compassionate Use Program. Many of these patients had serious concurrent disorders known to predispose them to a greater risk of adverse experiences and death. The mean duration of enalapril treatment was 29 weeks, with a range of 1 day to approximately 3.5 years. Enalapril was generally well tolerated, and the estimated long term probability of patients terminating enalapril therapy because of adverse effects was low. 20 patients had discontinued captopril treatment because of low white blood cell counts; during subsequent enalapril treatment these reactions resolved in 14 patients, persisted in 2 patients, and could not be evaluated in 4 patients. Captopril-related proteinuria improved or resolved in 9 and persisted in 2 of 15 patients, taste disturbances resolved in 35 and persisted in 2 of 38 patients; and rash resolved in all but 7 of 178 patients during enalapril treatment. 18 patients (6%) discontinued enalapril treatment because of lack of efficacy; 6 of these 18 patients died due to a progression of heart failure, and another 11 patients died for other reasons. The deaths were considered unrelated to therapy with enalapril. Adverse reactions were the reason for discontinuation of enalapril treatment in 53 patients (19%). The most common adverse experiences that resulted in discontinuation of enalapril were: impairment of renal function (5%), hypotension (2%) and rash (2%). No neutropenia, proteinuria, or new taste disturbances were recorded as reasons for discontinuation.(ABSTRACT TRUNCATED AT 250 WORDS)
