Paneth cell-like metaplasia of the prostate gland.

We report two cases of Paneth cell-like metaplasia of the prostate gland, one in poorly differentiated carcinoma and the second in benign hyperplasia. By light microscopy, the Paneth-like cells were indistinguishable from Paneth cells found in the normal small intestine and ultrastructurally showed electron-dense granules typical of Paneth cells. Immunohistochemical stains were positive for prostate-specific antigen and prostatic acid phosphatase and negative for lysozyme and alpha 1-antitrypsin. The clinical significance of Paneth cell-like metaplasia is unknown and may represent an example of the multipotential metaplastic capability of actively dividing cells.

[Osseous metaplasia in a colonic adenocarcinoma].

Osseous metaplasia is rare in tumors of the gastrointestinal tract. We report the case of a 76-year-old man presenting with an adenocarcinoma of the transverse colon containing osseous tissue. Calcifications were seen on the abdominal CT-scan. Pathogenesis of the osseous metaplasia is still unknown.

Squamous metaplasia mimicking papillary carcinoma in the upper urinary tract.

We experienced a case of squamous metaplasia mimicking papillary urothelial cell carcinoma in the upper urinary tract. A 69-year-old woman, who complained of gross hematuria and intermittent left flank dull pain underwent nephrectomy with the clinical diagnosis of papillary urothelial carcinomas in the left upper urinary tract according to positive split urine cytology and tumorous filling defects of contrast media by the abdominal CT scan. Pathological diagnosis was squamous metaplasia and concomitant foreign body granuloma. Those changes were judged due to a tiny calculus in the ureter. Our presented case implies that a tiny calculus can cause the metaplastic change in the urothelial epithelium and the combination of radiographical and cytological diagnoses would not be enough to lead the correct diagnosis and the definitive surgical treatment against protruding lesions in the upper urinary tract requires more reliable diagnostic modalities.

[Columnar cell metaplasia of the esophageal epithelium in reflux disease].

Diagnosed in a clinical setting were 28 cases of histologically verified columnar cell metaplasia of the esophageal epithelium in patients with grave forms of reflux disease of the esophagus. For the specialized type metaplasia characteristic for the Barrette's esophagus, which was detected in 22 (78.6%) patients, signs of availability of its neoplasic potential included dysplasia of the epithelium, inversion of the mucus-producing process, and eosinophilia of the inflammatory infiltrate.

Extended intestinal metaplasia. A survey of 1392 gastrectomies from dwellers of the Pacific basin.

BACKGROUND: To assess the extent of gastric intestinal metaplasia (IM) in gastrectomy specimens in populations of the Pacific basin having different incidence of gastric carcinoma. MATERIALS AND METHODS: One thousand three hundred and nine-two gastrectomies were investigated: 1088 had a gastric carcinoma and 304 miscellaneous gastric diseases. Twenty-one thousand three hundred and fourteen histological sections were reviewed under low-power (4X). IM was either spotty (SIM) or extended (EIM= encompassing one or more entire low-power fields/section). Widespread IM (WIM) was regarded as EIM if present in > or =5 histological sections. RESULTS AND CONCLUSION: The percent of gastrectomies harboring a carcinoma increased significantly with increasing age more notably in those with diffuse carcinomas (DC) than in those with intestinal carcinomas (IC). The percent of gastrectomies with EIM was significantly higher in specimens with IC than with DC, particularly among elderly patients, and in specimens from countries with a high cancer incidence. The percent of gastrectomies with WIM was higher in specimens having IC than in those having DC. Migration per se did not influence the frequency of specimens with EIM in elderly Japanese patients: Japanese migrants to Hawaii had a similarly high frequency of EIM as those dwelling in Japan. Japanese patients with a gastric carcinoma showed atypical mitoses in areas with EIM far from the tumor, suggesting that cellular mutation(s) play a role in the evolution of EIM towards gastric dysplasia and carcinoma in that ethnic group. The drawback of gastric biopsies in assessing the extent of gastric intestinal metaplasia and, thereby, estimating possible cancer risk in long-term studies has been stressed.

Expression of estrogen receptor-beta isoforms in Barrett's metaplasia, dysplasia and esophageal adenocarcinoma.

We have previously shown that the majority of esophageal adenocarcinomas (EA), and its precursor Barrett's metaplasia (BM), express estrogen receptor beta (ER-B). Several isoforms of ER-B have been described and are presumed to have different functions, but their distribution in BM and EA is not known. The aim of this work was to determine which ER-B isoforms are expressed in EA and BM. Sections of formalin-fixed and paraffin-embedded esophageal tissue from 33 esophageactomy specimens, of which 27 had invasive EA, were stained for the ER-B isoforms ER-B1, ER-B2, ER-B3 and ER-B5 utilizing the immunoperoxidase method ER-B1 was detected in 23 out of 27 (85%) EA compared to 3 out of 14 (21%) Barrett's metaplasia negative for dysplasia (BMND) (p =0. 0001); ER-B2 was expressed in 22 out of 27 (81%) EA in contrast to 3 out of 14 (21%) BMND (p=0.0004); ER-B3 was positive in 27 out of 27 (100%) EA in contrast to only 1 out of 14 (7%) BMND (p<0001); ER-B5 was detected in 27 out of 27 (100%) EA compared to 9 out of 14 (62%) of BMND (p=0.0027). High- and low-grade dysplasia showed a similar ER-beta isoform expression profile to that of EA Cancers invasive through the esophageal wall had a higher percent of cells with cytoplasmic expression of ER-B1 than tumors limited to the wall (T3 vs. T1 and T2, p=0.051). We conclude that ER-B1, ER-B2, ER-B3 and ER-B5 are overexpressed in EA compared to its precursor lesion BMND, suggesting a significant biological role for steroid hormones in EA.

Tumor induction by azoxymethane (AOM) and 2-amino-1-methyl-6-phenylimidazo[4,5-b]pyridine (PhIP) in F344 rat gastric mucosa featuring intestinal metaplasia caused by X-irradiation.

Male F344 5-week-old rats were X-irradiated, and 16 weeks after the first dose, azoxymethane (AOM) was injected or 2-amino-1 -methyl-6-phenylimidazo [4,5-b]pyridine (PhIP) was given by intragastric intubation. Tumors in the pylorus of the glandular stomach were observed in 4 out of the 29 animals receiving X-rays + AOM and in 4 out of the 25 animals receiving X-rays + PhIP, 12 months after administration. No such lesions were found in the chemical or X-ray alone groups. Intestinal metaplasia and some induced tumors were positive for CDX2. It was concluded that the presence of intestinal metaplasia may increase sensitivity to the induction of gastric tumors by colon carcinogens.

Gastric heterotopia together with intestinal metaplasia in the gallbladder: case report and review of literature.

Heterotopic gastric mucosa in the gallbladder is extremely unusual. In this study, we aimed to report a case of gastric heterotopia together with intestinal metaplasia in the gallbladder of a 16-year-old male patient who experienced a sudden onset of epigastric pain with nausea. He was admitted to the hospital with a prediagnosis of mild degree obstructive jaundice. Cholecystectomy and hepaticoduodenostomy were carried out. In the microscopical examination of the gallbladder, an antral and pyloric type gastric mucosa together with intestinal metaplasia were clearly evident in the gallbladder submucosa, and the adjacent gallbladder mucosa showed typical features of chronic cholecystitis.

Lipomatous metaplasia occurring within meningiomas--two case reports.

Lipomatous meningiomas have recently been described as rare well defined variants of meningiomas characterized by meningothelial cells which undergo a metaplasia to adipocytes. Presence of intratumoral fat can sometimes cause confusion in preoperative radiological diagnosis. We report two such histologically unusual cases of meningiomas with significant adipose tissue metaplasia.

Breast carcinoma with three coexistant type of metaplasia: sarcomatoid, giant cell and squamous differantiation.

We report on a women with metaplastic carcinoma of the right breast. After diagnosis she had a simple mastectomy operation. Pathological investigations were carried out on mastectomy material. Immunohistochemical analyses were performed. Bizarre, malignant giant cells and diffuse inflammatory reactions were predominantly noted. Sarcomatous metaplasia and rare squamous metaplasia were also seen. There was not any area of classic intraductal, invasive ductal, lobular or papillary carcinoma. The patient is still alive and well for more than five years. This uncommon case is described and discussed, and the related literature is reviewed.

[Experimental study of the mechanism of chondroid metaplasia of the bilaminar zone following anterior disc displacement of temporomandibular joint].

PURPOSE: To investigate the source of the metaplasia chondrocytes in the bilaminar zone following anterior disc displacement (ADD) of temporomandibular joint (TMJ). METHODS: 12 Japanese adult rabbits were randomly divided into A and B groups. Each group contained 4 experimental and 2 control rabbits. The disc of the experimental rabbits was induced forward and fixed into the holes made in the zygomatic arch. Group A was stained by HE and examined for the expression of proliferating cell nuclear antigen (PCNA) and fibroblast growth factor receptor 3 (FGFR3) by immunohistochemical methods. Group B was observed under transmission electron microscopy (TEM). RESULTS: Chondrocytes appeared in the bilaminar zone of each experimental rabbit. Both PCNA and FGFR3 were positive in part of them. Under TEM, some cells took on the double characters of chondrocyte and fibroblast and some others appeared the characters of naive cells. CONCLUSIONS: Following ADD of TMJ, some metaplasia chondrocytes might be proliferated and differentiated from mesenchymal stem cells.

[Cytokeratin expression in squamous-cell metaplasia of the cervix uteri: an immunohistochemical study using monoclonal antibodies].

Immunohistochemical study of the exo- and endocervix in squamous metaplasia is performed using EE21-06d monoclonal antibodies revealing simultaneously five cytokeratinous polypeptides characteristic of squamous epithelium, and f-12-19h revealing simultaneously wide spectrum of cytokeratinous polypeptides characteristic of the epithelium various types. PAP method and the reaction of immunofluorescence were used. Monoclonal antibodies 12-19d label cells of all layers of normal exocervix, endocervix and metaplastic squamous epithelium, both immature and differentiated. Monoclonal antibodies EE21-06d react in the exocervix with suprabasal and higher located cells and in the endocervix with reserve cells only. Positive reaction with EE21-06d in the metaplastic foci increases in proportion with the squamous cell differentiation, thus this antibody being the marker of reserve cells and the degree of the squamous metaplasia maturity.

[Lectin-histochemical characteristics of the epithelium in intestinal metaplasia, polyps and cancer of the stomach].

Distribution of lectins (PNA, SBA, WGA, and Con A) has been studied in 53 stomach biopsies. Positive lectin reaction in the foveolar epithelium is observed, including dysplastic foci in the apical plasmalemma and in the cytoplasm around the nucleus this corresponding to the Golgi complex. It was mainly the apical plasmalemma of the columnar epithelium in the foci of intestinal metaplasia. Differences in the receptor localization were established in non-complete intestinal metaplasia, hyperplastic polyps and adenomas this depending on the cell type. Carcinoma cells not infrequently had all the plasmalemma stained with granules of the reaction product being distributed through all the cytoplasm. Lectin reactions are more pronounced in carcinomas accumulating mucus. Lectin binding by tumorous cells containing mucus was enhanced after the elimination of sialic acid as a result of the incubation with neuraminidase. Intensity of lectin reactions somewhat differs in various histological forms of stomach cancer.

[Tissue culture of human lens epithelial cells. Part II: Suppressive effect of diclofenac sodium on their proliferation and metaplasia].

We studied the suppressive effect of a non-steroidal anti-inflammatory drug (NSAID) on the proliferation and fibrous metaplasia of human lens epithelial cells (LEC) in cell culture. We cultured human LEC attached to a round piece of the central anterior capsule obtained by anterior capsulotomy during cataract surgery and added diclofenac sodium, the concentration of which varied from 0.5 microgram/ml to 30 micrograms/ml gradually. Proliferation as well as fibrous metaplasia of LEC were suppressed. Histopathological examination revealed cell degeneration and death. Besides the apparent inhibition of diclofenac sodium on the biosynthesis of prostaglandins, this suppressive effect on LEC proliferation also may play a role in anti-inflammation after intraocular lens implantation. Further, secondary cataract may be prevented by this effect.

[Precancerous conditions in gastric cancer (II): relationship between atrophic gastritis-intestinal metaplasia-gastric dysplasia-gastric carcinoma].

We studied a group of gastric precancerous lesions in order to analyze their mutual relationship as well as and their association with the development of gastric carcinoma of the intestinal type. This study was performed on 850 gastric biopsy specimens. Our results demonstrate the strong association of atrophic gastritis, intestinal metaplasia and gastric dysplasia (p less than 0.001) which should be considered as precursor lesions of gastric cancer. Moreover, the relations between these lesions and the intestinal type of gastric carcinoma suggest their active participation in the genesis of this type of malignancy. Our results also suggest that chronic atrophic gastritis and intestinal metaplasia evolve to the dysplasia stage before developing gastric cancer.

Recombinant human secretory leukocyte-protease inhibitor: in vitro properties, and amelioration of human neutrophil elastase-induced emphysema and secretory cell metaplasia in the hamster.

Studies were undertaken to evaluate the in vitro properties of recombinant human secretory leukocyte-protease inhibitor (rSLPI) that had been made in Escherichia coli in an inactive form and refolded, and to determine whether emphysema and bronchial secretory cell metaplasia, induced in hamsters by intratracheal treatment with human neutrophil elastase (HNE), could be amelio-rated by prior intratracheal instillation of rSLPI. Chromatographic studies indicated that 3H-rSLPI formed a 1:1 complex with HNE. Blockage of the active site of HNE by a covalently bound tetrapeptide chloromethyl ketone reduced complex formation with 3H-rSLPI by more than 98%. Incubation of 3H-rSLPI-HNE complex with alpha 1-protease inhibitor for 3 hours at 37 degrees C decreased the amount of complex compared with incubation in the presence of bovine serum albumin (70% vs 27% dissociated). The calculated dissociation rate constant was 1.1 x 10(-4) sec-1, indicating a 1.8 hour dissociation half-life. Dissociated 3H-rSLPI retained its ability to recombine with HNE. rSLPI was as effective at inhibiting HNE released from stimulated neutrophils as 3H-rSLPI was at inhibiting purified HNE. Intratracheal pretreatment of hamsters with 3000 micrograms of rSLPI as long as 8 hours before the intratracheal instillation of 250 micrograms of HNE, resulted in significant protection against induction of emphysema and secretory cell metaplasia. One and 4 hours after instillation of rSLPI, 59% and 44%, respectively, of the initial functional activity was recovered in lung lavage supernatant, indicating a half-life of approximately 2 hours.(ABSTRACT TRUNCATED AT 250 WORDS)

Syringosquamous metaplasia. A distinctive eruption in patients receiving chemotherapy.

Squamous metaplasia of the upper portion of the sweat ducts has been observed in four patients who received chemotherapy; two patients with acute myelogenous leukemia, one patient with testicular carcinoma, and one patient with anaplastic carcinoma of the lung. This change is different (but perhaps related) from what is described in neutrophilic eccrine hidradenitis in patients with acute myelogenous leukemia and other tumors after chemotherapy. Syringosquamous metaplasia may be confused with well differentiated squamous cell carcinoma histologically.

Spindle cell and cartilaginous metaplasia in a breast carcinoma with osteoclastlike stromal cells. A difficult fine needle aspiration diagnosis.

The cytologic picture in fine needle aspirates from a unique type of breast tumor, with stromal proliferation of osteoclastlike giant cells, cartilaginous metaplasia and metaplastic spindle-shaped carcinomatous cells, is described. In this case, an erroneous cytologic diagnosis of fat necrosis associated with a rich component of reactive fibroblasts was made. This false-negative report, which was mainly due to lack of an obvious carcinomatous component in the aspirated material, is discussed with emphasis placed on the need to exclude spindle cell metaplasia in a breast carcinoma whose aspirates are characterized by a rich component of spindle-shaped cells.
