Use of a multiple logistic regression model to determine prognosis of dairy cows with right displacement of the abomasum or abomasal volvulus.

Data at admission and at surgery were collected on 458 cows with right displacement of the abomasum or abomasal volvulus, to derive multiple logistic regression models for predicting postsurgical outcome (productive, salvaged, or terminal). The derived models contained few and easily obtained variables. The weight associated with each variable was determined objectively. Three admission variables (heart rate, base excess, and plasma chloride concentration), and 5 surgical variables (heart rate, base excess, diagnosis, method of decompression used, and appearance of abomasal serosa) were used in the final models. Predicted outcomes that used the admission and surgical models were closely related with actual outcomes. Total correct classification for satisfactory (productive) versus unsatisfactory outcome (salvaged and terminal) was 78.2% for the admission model and 82.7% for the surgical model. Combining data on cows with productive and salvaged outcomes as satisfactory outcome, and terminal as unsatisfactory outcome, total correct classification was 90.7% for the admission model and 93.2% for the surgical model. Using predicted probabilities, the market value of productive and salvaged cows, and the medical and surgical costs, one can calculate the expected economic value of each outcome. Treatment can be justified if the sum of the expected value of productive and salvaged outcome exceeds the sum of the medical and surgical costs and the expected salvaged value of the cow that was not treated surgically.

Continuous in-vitro cultivation of Plasmodium falciparum in Ibadan: solutions to scientific and logistical problems.

The technique of continuous in-vitro cultivation of Plasmodium falciparum has not been widely applied in malaria-endemic areas, due to scientific as well as purely logistical problems. Methods for solving or coping with these problems are described. They have already proved effective for over 4 years. The parasites harvested have been satisfactory, as judged by reproducible logarithmic growth curves and normal morphology in Leishman-stained smears as well as phase-contrast microscopy of wet-preparations. They have also been successfully used as antigen sources in the malarial fluorescent-antibody test and to investigate enhanced platelet aggregation in experimental malaria.

[Use of linear logistic regression in classification of observations].

In the present paper the prediction method using the logistic regression is explained, and the range of problems for its use is deliminated. The example is presented of mentioned method's application on how to predict the dog survival in a radiolobiological experiment. The obtained results are compared with the prediction of outcome using the linear discriminant function. Both models are identic in a proportion of erroneously classified subjects. This method may be diagnostically supportive in ranging individuals to one of two groups delimitated previously.

Ranking the risk factors for cesarean: logistic regression analysis of a nationwide study.

The risk factors that influenced the decision to perform cesarean were ranked in a nationwide census of deliveries conducted in Israel. The study encompassed 22,815 women who gave birth between November 1, 1983 and January 31, 1984 in the 30 maternity departments in the country; 2179 deliveries were by emergency cesarean. Multivariate stepwise logistic regression technique indicated that the most important risk factors affecting the decision were the presentation of the fetus and the presence of a uterine scar, followed in descending order by placenta previa or abruptio placentae, maternal disease, primiparity, low birth weight, twins, and advanced maternal age. The mother's ethnic background and type of hospital played an insignificant role in the decision-making process. The specific statistical method applied in this work permits listing the indicators that constitute risk factors for cesarean and provides the net effect of each factor on the decision-making process.

[Risk assessment of factors related to lactation of breast-feeding women by multichotomous logistic regression with stepwise procedure].

Based on Dubin and Pasternack's solution of parsimonious parameters, the authors proposed a procedure of stepwise selection of factors for establishing the Multichotomous logistic regression "best" model. Score statistic was used to select factors into model. Ratio of likelihood function was used to eliminate factors from model. The data of lactation from 129 breast-feeding women living in rural area have been analysed by the procedure. It showed that the first breast-feeding to the newborn of over 48 hours from her childbirth has negative effect while good appetite has positive effect on milk secretion. The epidemiological meaning of parameters estimated from the data was illustrated in detail.

Confidence bands for logistic regression with restricted predictor variables.

Confidence bands are constructed for the logistic response function when there is an interval restriction on each of the predictor variables. The construction involves application of a general fitting procedure using Scheffé's S-method, described by Casella and Strawderman (1980, Journal of the American Statistical Association 75, 862-868). Specific details are given for the case of one predictor variable, along with details for a fixed-width alternative to the S-method bands. In the one-predictor case, Monte Carlo results suggest that both bands are conservative for small sample sizes, such as N = 25. By N = 200 the S-method's coverage probabilities are seen to attain their nominal levels while the fixed-width bands remain conservative. The procedures are exemplified with data from a genetic toxicology experiment.

Overview of different logistical approaches to postmarketing surveillance.

The study designs used for postmarketing surveillance are those of epidemiology, but very large sample sizes are needed. Therefore, a number of specific logistical approaches have been developed. One can use spontaneous reports of adverse reactions, aggregate population based data, computerized collections of data from organized medical care programs, data collected for postmarketing surveillance on an ongoing basis, existing data collected as part of other ad hoc studies, or data collected de novo for the study being conducted. Each is described in more detail, along with its advantages and disadvantages.

A step-wise logistic regression analysis of papillary carcinoma of the thyroid.

Fourteen cytologic features seen in fine needle aspirates of papillary carcinoma of the thyroid were subjected to a step-wise logistic regression analysis to determine which are predictive of papillary carcinoma. The study cases included 38 histologically proven papillary carcinomas and 54 other palpable thyroid nodules. The three most important variables in making the prediction of papillary carcinoma were intranuclear inclusions, papillary structures without vessels and cells with metaplastic cytoplasm.

Risk factors for mortality after bereavement: a logistic regression analysis.

A national sample of elderly widowed people was followed up for six years. Excess mortality was found for men aged 75 years and over in the first six months of bereavement compared with men of the same age in the general population. Logistic regression analysis, controlling for age and sex together, demonstrated that the best independent predictors of mortality among the elderly widowed were: interviewer assessment of low happiness level; interviewer assessed and self-reported problems with nerves and depression; and lack of telephone contacts. The general practitioner is well placed to assess levels of depression and unhappiness among the widowed and to check that they have adequate social support.

Choosing near-linear parameters in the four-parameter logistic model for radioligand and related assays.

Ten parameters extracted from six currently used parametrizations of the four-parameter logistic model, and one new proposal, were examined for their statistical behavior in nonlinear least-squares estimation in combination with ELISA and RIA data. Those which are adequately near-linear on the basis of the Lowry-Morton lambda statistic were identified and can be recommended for use in practice.

Regressive logistic models for familial disease and other binary traits.

The simple Markovian structures of dependence, defined previously for continuous traits, are extended here to familial disease and other binary traits through the use of the logistic function. The regressive models so formulated can incorporate explanatory variables and major gene effects for segregation and linkage analyses. Thus, the goals of epidemiology and genetics in the analysis of familial disease can be accomplished in the same computational scheme.

Logistic risk model for the unique effects of inherent aerobic capacity on +Gz tolerance before and after simulated weightlessness.

Small sample size (n less than 10) and inappropriate analysis of multivariate data have hindered previous attempts to describe which physiologic and demographic variables are most important in determining how long humans can tolerate acceleration. Data from previous centrifuge studies conducted at NASA/Ames Research Center, utilizing a 7-14 d bed rest protocol to simulate weightlessness, were included in the current investigation. After review, data on 25 women and 22 men were available for analysis. Study variables included gender, age, weight, height, percent body fat, resting heart rate, mean arterial pressure, VO2max, and plasma volume. Since the dependent variable was time to greyout (failure), two contemporary biostatistical modeling procedures (proportional hazard and logistic discriminant function) were used to estimate risk, given a particular subject's profile. After adjusting for pre-bed-rest tolerance time, none of the profile variables remained in the risk equation for post-bed-rest tolerance greyout. However, prior to bed rest, risk of greyout could be predicted with 91% accuracy. All of the profile variables except weight, MAP, and those related to inherent aerobic capacity (VO2max, percent body fat, resting heart rate) entered the risk equation for pre-bed-rest greyout. A cross-validation using 24 new subjects indicated a very stable model for risk prediction, accurate within 5% of the original equation. The result for the inherent fitness variables is significant in that a consensus as to whether an increased aerobic capacity is beneficial or detrimental has not been satisfactorily established. We conclude that tolerance to +Gz acceleration before and after simulated weightlessness is independent of inherent aerobic fitness.

Clinical factors predisposing to major ulcer bleeding. A logistic regression analysis.

In a consecutive series of patients with bleeding from peptic ulcer, a logistic regression analysis was used to evaluate clinical factors as predictors of major hemorrhage. A hemodynamic assessment distinguished between self-limiting hemorrhage requiring no treatment (190 patients) and major bleeding necessitating emergency surgery (86 patients). Most data, including patient age and sex, previous bleeding, and pulse rate, blood pressure and hematocrit on admission, were poor predictors of major ulcer bleeding. A few variables--onset of hemorrhage, appearance of hematemesis and site of ulcer--were statistically of predictive importance, but clinically only in combination. When one of the selected predictor variables was present, the risk of major hemorrhage did not statistically exceed the prior probability. When combined in pairs, the probability increased, primarily when duodenal ulcer or red hematemesis was present. When the patients presented with three of the selected predictor variables, the probability of major ulcer hemorrhage was markedly increased. However, only 8% of the patients presented with three of the selected variables. Clinical factors, therefore, contribute in general little in prediction of major hemorrhage form peptic ulcer.

Are there two logistic regressions for retrospective studies?

A comparison is made between two different approaches to the linear logistic regression analysis of retrospective study data: the prospective model wherein the dependent variable is a dichotomous indicator of case/control status; and the retrospective model wherein the dependent variable is a binary or polychotomous classification of exposure. The two models yield increasingly similar estimates of the relative risk with increasing degrees of covariate adjustment. When the covariate effects are saturated with parameters, the relative risk estimates are identical. The prospective model is generally to be preferred for studies involving multiple quantitative risk factors.

[Substitutes for renal function. Technical trends and logistics of treatment].

In France about 50 new patients per million of inhabitants per year with end stage renal disease (ESRD) require treatment with dialysis and/or transplantation. For the last few years new dialysis methods as hemofiltration and continuous ambulatory peritoneal dialysis have been extended. It is too early to predict what will be the role and the future of such methods as compared to hemodialysis, the gold standard. Appropriate logistics of treatment of ESRD rely on an integrated dialysis-transplantation program. In France the percentage of some modes of therapy, including home dialysis and transplantation, should be increased.

Resistant fits for some commonly used logistic models with medical application.

Logistic regression-type models are used in many applications. Some examples include the classical dose-response experiment, prospective and retrospective studies of disease incidence (with and without matching), and the analysis of ordinal data. In most instances, the model is fitted by the method of maximum likelihood, which, like least squares, is sensitive to atypical observations. An alternative to maximum likelihood is proposed and illustrated by examples.

Logistics of conducting a chronic study with 24,192 mice.

Some of the logistical problems of conducting the ED01 study at the National Center for Toxicological Research are discussed, including problems in site preparation, animal production, support during the execution of the study, pathology support, and data analysis. In order to manage the vast amount of data that would be generated by a 24,192 mouse study, computer-assisted data collection systems and automated data storage and retrieval systems were developed. These systems, along with other procedures at NCTR, made the execution of a study of this magnitude possible and allowed timely experiment management. Preliminary data analysis during the course of the study resulted in a decision to extend the duration of one segment of the study.
