The Oregon lesson: results of postelection research. Bureau of Economic and Behavioral Research.

Definite issues are involved in the denturism controversy, such as consumerism/economics, sympathy for the elderly population, freedom of choice, and acceptance of paraprofessionals. The dental profession must deal with these issues. Its concern for oral health and quality of training can only be presented (and accepted) if it is placed within the context of the issues as the public sees them.

Selection of restorative materials in general dental practice in Sweden.

The aim of this study was to record any differences in the selection of materials in private dental practices in Sweden during 1993-95, after a ban on amalgam had been proposed by politicians, compared with those obtained in a similar survey in 1978-79. A total of 177 clinicians participated in the present survey, which recorded the total number of restorations they placed and the number of composite, amalgam, and glass ionomers inserted in the treatment of primary caries and for replacement of restorations. Furthermore, the number of intact amalgam restorations replaced by composite and the number and types of inlays and laminates cemented were accounted for. Marked changes in the selection of direct restorative materials were noted in the present study compared with the survey in 1978-79. Resin-based composite materials are taking over the routine posterior restorative material, also in stress-bearing areas. Glass ionomer restorative materials are used preferentially in the treatment of primary caries. The use of amalgam restorations has decreased in relative and absolute numbers. Inlays are infrequently used but, when used, usually include three or more surfaces, and ceramic materials predominate.

Some thoughts on dental malpractice.

Malpractice litigation is not new nor is it limited to the professions of medicine and dentistry. The number of dental malpractice claims is increasing in many countries though the percentage of cases in which the patient succeeds remains constant at about ten to twelve per cent. In successful cases however the monetary compensation awarded has increased substantially in the past few years. The increasing use of multi-chair dental offices and the employment of more auxiliary workers in dental practice may have contributed to the increase in litigation as has the widespread dissemination of knowledge of patients' rights by newspapers and television. It is unfair to blame the legal profession for this increase. There is evidence that the 'contingent fee' arrangement between plantiff and lawyer contributes to the number of cases brought. It has been suggested that the existence of malpractice insurance may make the dentist less careful to prevent untoward effects of dental treatment. In many states the law will not permit punitive damages to be paid by the insurance agency nor are fines imposed for criminal activities covered. It is important that the dentist should be fully informed about the details of the cover which his insurance does in fact provide.

[Forensic dentistry as an important part of legal medicine].

Forensic dentistry is divided into two main subjects: 1. Dental jurisprudence and ethics and 2. Dental criminology. Dental jurisprudence and ethics include the legal duties of the dentist. They concern foremost the contractual relations between him and the patient and the duties which result from his actual dental activities, especially concerning his therapeutic measures. In major disasters successful identification is possible if a careful register is taken of dental states by all dentists.
