The authors studied the anatomy of 136 patients who underwent breast reduction surgery from 1998 to 2003 to determine the relationship of breast fat to the glandular tissue of the breast. Histological sections of freshly preserved breast tissue taken from representative patients were examined and compared to depictions of normal breast anatomy as portrayed in standard anatomical texts from the classic work of Sir Astley Cooper in 1845 to current publications such as Auberbach and Riordan's Breastfeeding and Human Lactation. Most texts portray little intermix of fat with the glandular tissue of the breast. Our studies confirm the texts that demonstrate the fat and glandular tissue to be inseparable and present in continuity with each other except in the subcutaneous plane where only fat is present. The implications of this anatomical fact as it relates to breast surgery and human lactation are discussed.
Nearly 20 years ago, in vitro experiments left no doubt about the fact that laser light can ablate atherosclerotic plaque. The initial enthusiastic results with the technology, particularly in coronary arteries, were followed by reports showing unacceptably high restenosis and complication rates. These poor results were due to the premature application of an underdeveloped technology, a lack of understanding of laser/tissue interaction, and the use of incorrect lasing techniques. Consequently, and without discrimination, all lasers were banned from the catheterization laboratories for nearly a decade. Technological enhancements of the excimer laser, combined with refined catheter lasing techniques, resulted in greater debulking of atherosclerotic material in long superficial femoral artery occlusions. These results triggered the application of the excimer laser technique as an atherectomy tool in more complex lesions below the knee. The multicenter Laser Atherectomy for Critical Ischemia study clearly demonstrated that the excimer laser technology resulted in limb salvage rates >90% in patients with critical limb ischemia (CLI). Furthermore, new clinical results indicate that the excimer laser is very effective in dissolving thrombotic obstructions, redirecting this technology to the coronary field. The results of the excimer laser in CLI validate the role of the cool laser in treating complex peripheral vascular disease. The results suggest a larger indication for this technology and support a more aggressive use of these interventional techniques in the treatment of this large patient cohort. However, all lasers are not equally effective in debulking atherosclerotic material. Only the athermic process associated with the excimer laser produces a safe and effective endovascular ablation of obstructive atherosclerotic and/or thrombotic material. The appropriate and safe utilization of the equipment and lasing techniques, combined with correct indications and patient selection, will contribute to the successful application of laser-assisted atherectomy in complex peripheral and coronary artery obstructive disease. Unfortunately, little consistent scientific data has been generated to convince the interventional community of the usefulness of excimer laser ablation.
The identification of potential partners for CCN3(NOV) sheds new light on the biological activity of this signaling protein. In particular, the physical interaction of CCN3 with the IL33 cytokine combined with previous data indicating that CCN3 expression was regulated by TNFalpha and IL1 cytokines, point to CCN3 as a potent player in a variety of inflammatory responses, including neurodegenerative disease, and arthritis. Nuclear proteins that are involved in the regulation of RNA processing and chromatin remodeling were also found to interact with CCN3. These observations reinforce the concept that routing of CCN3 to the cell nucleus where it acts as a transcription regulator, might constitute a key element in the balance between the anti- and pro-proliferative activities of CCN3 proteins.

Combination chemotherapy is considered the "norm" in cancer chemotherapy. While the development of classical combination regimens took into account factors such as single-agent activity, different toxicity profiles, and advantageous pharmacology of the component drugs, in the era of targeted therapies there are additional considerations such as new mechanisms of resistance due to modulation of pathway dependence. Since it is not feasible to test all of the possible combinations clinically, some method for preclinically identifying and prioritizing promising combinations is necessary. While in vivo animal models can be used for safety testing and some pharmacokinetics, even they can quickly become prohibitively resource intensive for the purpose of efficacy determinations. Therefore, factors such as biologic rationale, reliable in vitro results in more than one tumor type, achievable in vivo pharmacology, and selectivity of primary tumor cell activity become important in the evaluation of potential combination regimens.
Overly exuberant and exaggerated past expectations and claims of the free radical theory have been quieted by extensive randomized, double-blind, controlled human studies. A half century of data demonstrates its lack of predictability and it has not been validated by the scientific method. Widespread use of antioxidants has failed to quell the current pandemic of cancer, diabetes, and cardiovascular disease or to stop or reverse the aging process. Electronically modified oxygen derivatives contribute to the modulation of cellular redox status, which is of primary importance in disease prevention and homeostasis.
It has been argued that low TAT stability estimates are an artifact of retest instructions to be original and creative. This view was supported in a recent study by Winter and Stewart (1977) who found that test-retest reliability was higher when subjects were told on the second occasion to write the same stories than when they were told to write different stories. This study attempted to replicate Winter and Stewart's results and also to determine whether the similarity of stories did vary as hypothesized. The replication was only partially successful. More importantly, it was concluded that while instructions to write similar stories did improve reliability, the increase reflected only subjects' memory capabilities and not greater stability in motive expression. While true TAT stability may still be commonly underestimated, altering retest instructions to improve reliability does not appear to be the solution.
Determinants are perceptual features of inkblots, but movement, reflections, pairs, and subdivisions of shading, scored as determinants, actually represent associative elaborations. Such misclassification of determinants obfuscates the response process and makes it difficult to evaluate the validity of inferences. To the extent that these scores are useful, they can be improved upon by fully developed content analysis. Movement, reflections, and pairs can best be understood as manifestations of object relations content themes, as scored in systems developed by Blatt, Krohn & Mayman, and Urist. Determinant scores for reflections and pairs should be eliminated. Shading should not be subdivided into texture, vista, and diffuse shading, since these are content themes without clear psychological significance or clear empirical support.
The influences of mating on serum luteinizing hormone (LH) and follicle stimulating hormone (FSH) levels were evaluated in 4-day cyclic proestrous rats of the Wistar strain. Blood samples were collected via guillotine from mated and unmated controls at 1900-2000 h or 2100--2200 h. Serum LH levels were determined by two independent radioimmunoassays. There were no significant differences in LH measurement between assays. At 1900--2000 h, LH levels were high, but not significantly different, in both the mated and unmated control animals. By 2100-2200 h, serum LH concentration had dropped in unmated controls but was significantly elevated in mated animals. There was no significant elevation in FSH either 1900-2000 h or 2100--2200 h on the evening of proestrus. The data indicate that when LH levels are normally high, coitus cannot further potentiate LH release. However, when LH levels are declining, coitus is an effective and adequate stimulus to initiate elevations in serum LH.

Sigal and Hassett published an article about Lyme disease in the EHP Supplements (Sigal and Hassett 2002), suggesting that chronic Lyme disease is "psychogenic." I do not think that Sigal and Hassett, non-psychiatrists, are qualified to speak about psychiatric matters. I, however, actually have had the disease, which they characterize as "medically unexplained," for over 25 years and have 15 years of experience as a patient advocate and educator. I beg to differ.

In this essay, I seek to place Chen Kaige's Farewell My Concubine not solely within the Chinese matrix of its production but within the transnational matrix of its distribution and viewing; indeed, the film may well have been intended for international audiences, to which the film's numerous awards testify. Yet China, too, has been fascinated with Western ideas, with Freud one particular example. So here I use a Freudian interpretation of the film's rationalization of homosexuality in terms of beating and childhood sexuality, a rationalization that diverges from the Lilian Lee novel on which the film is based.
Clinical staff at the lower levels of agencies are more likely to initiate and implement organizational changes if they operate from a sound conception of accountability. Change agents are in danger of becoming modern day Don Quixotes unless they focus on creating accountability systems that support agency efforts to change. Designing systems capable of changing is the challenge for all levels of staff.
