Quality of life following laparoscopic anterior 90 degrees versus Nissen fundoplication: results from a multicenter randomized trial.

BACKGROUND: The short-term clinical outcomes from a multicenter prospective randomized trial of laparoscopic Nissen versus anterior 90 degrees partial fundoplication have been reported previously. These demonstrated a high level of satisfaction with the overall outcome following anterior 90 degrees fundoplication. However, the results of postoperative objective tests and specific clinical symptoms are not always consistent with an individual patient's functional status and general well being following surgery, and quality of life (QOL) is also an important outcome to consider following surgery for reflux. Hence, QOL information was collected in this trial to investigate the hypothesis: improvements in QOL following laparoscopic antireflux surgery are greater after anterior 90 degrees partial fundoplication than after Nissen fundoplication. METHODS: Patients undergoing a laparoscopic fundoplication for gastro-esophageal reflux at one of nine university teaching hospitals in six major cities in Australia and New Zealand were randomized to undergo either laparoscopic Nissen or anterior 90 degrees partial fundoplication. Quality of life before and after surgery was assessed using validated questionnaires - the Short Form 36 general health questionnaire (SF36) and an Illness Behavior Questionnaire (IBQ). Patients were asked to complete these questionnaires preoperatively and at 3, 6, 12 and 24 months postoperatively. RESULTS: One hundred and twelve patients were randomized to undergo a Nissen fundoplication (52) or a 90 degrees anterior fundoplication (60). Patients who underwent anterior fundoplication reported significant improvements in eight of the nine SF36 scales compared to four of the nine following a Nissen fundoplication. The majority of these improvements occurred early in the postoperative period. With respect to the illness behavior data, there were no significant differences between the two procedures. Both groups had a significant improvement in disease conviction scores at all time points compared to their preoperative scores. CONCLUSIONS: Patients undergoing laparoscopic anterior 90 degrees partial fundoplication reported more QOL improvements in the early postoperative period than patients undergoing a Nissen fundoplication. However, the QOL outcome for both procedures was similar at later follow-up.

Microalgae-Mediated Synthesis of Gold Nanoparticles from Indonesian Chlorella vulgaris InaCC M205 with Potential Anticancer Properties for Biomedical Application.

Sustainable nanomaterial synthesis has emerged as a critical strategy to reduce the environmental burden associated with conventional chemical synthesis method. Microalgae-derived biomolecules offer a promising platform for the green production of metal nanoparticles due to their rich bioactive compounds capable of acting as natural reducing and stabilizing agents. Here, we report the eco-friendly synthesis of gold nanoparticles (AuNPs) using extract of Indonesian microalga Chlorella vulgaris extract. To optimize the synthesis process, the effects of precursor-to-extract ratio, temperature, and incubation time were evaluated. Optimal synthesis of C5-AuNPs was obtained at 37 °C for 20 h with precursor to extract ratio of 6:4, resulting in moderately stable C5-AuNPs characterized by a surface plasmon resonance (SPR) peak at 541 nm. Furthermore, Fourier-transmission infra-red (FT-IR) analysis revealed the involvement of functional groups of C. vulgaris extract in the interaction with Au+ during the production of C5-AuNPs. Transmission electron microscopy (TEM) demonstrated the formation of uniformly spherical nanoparticles with an average diameter of approximately 8.8 nm. Biological evaluation showed that the synthesized C5-AuNPs exerted pronounced dose-dependent cytotoxicity against MCF-7 breast cancer cells with an IC50 threshold of 21.17 ppm, while no toxicity appears in normal HEK293 cells. Mechanistically, the C5-AuNPs induced early apoptosis and inhibit cell-cycle progression at the stage of G0/G1. Collectively, these findings demonstrate that C. vulgaris-mediated AuNPs represent a promising preliminary in vitro findings for cancer therapy candidate.

Synthesis, characterization, and computational study of the trans-IO2F5(2-) anion.

The combination of CH(3)CN solutions of [N(CH(3))(4)][F] and a mixture of cis- and trans-[N(CH(3))(4)][IO(2)F(4)] produces the novel trans-IO(2)F(5)(2)(-) anion. Under the given conditions, only the trans-IO(2)F(4)(-) anion acts as a fluoride ion acceptor, thus allowing the separation of isomerically pure, soluble cis-IO(2)F(4)(-) from insoluble trans-IO(2)F(5)(2)(-). The trans-IO(2)F(5)(2)(-) and cis-IO(2)F(4)(-) anions were characterized by infrared and Raman spectroscopy and theoretical calculations at the LDFT and HF levels of theory. The trans-IO(2)F(5)(2)(-) anion has a pentagonal-bipyramidal geometry with the two oxygen atoms occupying the axial positions. It represents the first example of a heptacoordinated main group AO(2)X(5) species and completes the series of pentagonal-bipyramidal iodine fluoride and oxide fluoride species. The geometries of the pentagonal-bipyramidal series IO(2)F(5)(2)(-), IOF(5)(2)(-), IF(5)(2)(-), IOF(6)(-), IF(6)(-), and IF(7) and the corresponding octahedral series IO(2)F(4)(-), IOF(4)(-), IF(4)(-), IOF(5), IF(5), and IF(6)(+) were calculated by identical methods. It is shown how the ionic charge, the oxidation state of the iodine atom, the coordination number, and the replacement of fluorine ligands by either an oxygen ligand or a free valence electron pair influence the stuctures and bonding of these species.

Syntheses of [F5TeNH3][AsF6], [F5TeN(H)Xe][AsF6], and F5TeNF2 and characterization by multi-NMR and Raman spectroscopy and by electronic structure calculations: the X-ray crystal structures of alpha- and beta-F5TeNH2, [F5TeNH3][AsF6], and [F5TeN(H)Xe][AsF6].

The salt, [F5TeN(H)Xe][AsF6], has been synthesized in the natural abundance and 99.5% 15N-enriched forms. The F5TeN(H)Xe+ cation has been obtained as the product of the reactions of [F5TeNH3][AsF6] with XeF2 (HF and BrF5 solvents) and F5TeNH2 with [XeF][AsF6] (HF solvent) and characterized in solution by 129Xe, 19F, 125Te, 1H, and 15N NMR spectroscopy at -60 to -30 degrees C. The orange [F5TeN(H)Xe][AsF6] and colorless [F5TeNH3][AsF6] salts were crystallized as a mixture from HF solvent at -35 degrees C and were characterized by Raman spectroscopy at -165 degrees C and by X-ray crystallography. The crystal structure of the low-temperature phase, alpha-F5TeNH2, was obtained by crystallization from liquid SO2 between -50 and -70 degrees C and is fully ordered. The high-temperature phase, beta-F5TeNH2, was obtained by sublimation at room temperature and exhibits a 6-fold disorder. Decomposition of [F5TeN(H)Xe][AsF6] in the solid state was rapid above -30 degrees C. The decomposition of F5TeN(H)Xe+ in HF and BrF5 solution at -33 degrees C proceeded by fluorination at nitrogen to give F5TeNF2 and Xe gas. Electronic structure calculations at the Hartree-Fock and local density-functional theory levels were used to calculate the gas-phase geometries, charges, Mayer bond orders, and Mayer valencies of F5TeNH2, F5TeNH3+, F5TeN(H)Xe+, [F5TeN(H)Xe][AsF6], F5TeNF2, and F5TeN2- and to assign their experimental vibrational frequencies. The F5TeN(H)Xe+ and the ion pair, [F5TeN(H)Xe][AsF6], systems were also calculated at the MP2 and gradient-corrected (B3LYP) levels.

Structural and water-repellent properties of a urea/poly(dimethylsiloxane) sol-gel hybrid and its bonding to cotton fabric.

A novel diureapropyltriethoxysilane [bis(aminopropyl)-terminated-poly(dimethylsiloxane) (1000)] (PDMSU) sol-gel hybrid was synthesized and applied on cotton to make it water repellent. Surface-energy values of PDMSU deposited on an aluminum substrate were determined, and the contact angle for water was assessed for impregnated cotton fabrics. The stability of the coatings was determined by repetitive washing, and their degradation was investigated with the help of the infrared attenuated total reflection (ATR) technique. The structure of PDMSU was studied by ATR and 29Si NMR spectroscopy. The results showed the active role of the urea groups in PDMSU/cotton interface bonding, but washing led to the relaxation of the urea-urea associations, as inferred from the appearance of a new amide II band at 1541 cm(-1).

Intra-amniotic infection: diagnosis, nomenclature, clinical significance, management, and microbiologic tools used for the diagnosis.

SUMMARYIntra-amniotic infection is the main cause of spontaneous preterm birth and adverse maternal-fetal outcomes; therefore, rapid, robust, and accurate diagnosis remains a clinical priority. Conventional microbiological techniques, especially culture-based methods, are limited by long turnaround times and the inability to detect fastidious or unculturable organisms. This review summarizes the diagnosis, nomenclature, clinical significance, management, and laboratory approaches for diagnosing intra-amniotic infection. Targeted nucleic acid amplification methods, including species-specific polymerase chain reaction and broad-range 16S rRNA gene sequencing, have improved the detection of bacterial DNA and enabled the identification of organisms that evade routine culture in intra-amniotic infection. More recently, whole-genome sequencing and metagenomic next-generation sequencing have provided culture-independent strategies for comprehensive pathogen profiling, allowing simultaneous detection of bacteria, viruses, and fungi, as well as characterization of antimicrobial resistance determinants and virulence-associated genes. However, challenges remain, particularly in low-biomass samples such as amniotic fluid, where contamination, host DNA background, and data interpretation can compromise specificity. This review critically evaluates the advantages and limitations of each molecular modality and discusses pre-analytical, analytical, and bioinformatic considerations essential for reliable implementation. Integration of molecular diagnostics into clinical workflows holds promise for improving etiological diagnosis and guiding targeted therapy in intra-amniotic infection, thereby improving maternal and fetal outcomes.

Phellinus Weirii and Other Native Root Pathogens as Determinants of Forest Structure and Process in Western North America.

The population structure and ecological roles of the indigenous pathogen Phellinus weirii, cause of laminated root rot in conifer forests of western North America, are examined. This pathogen kills trees in slowly expanding mortality centers, creating gaps in the forest canopy. It is widespread, locally abundant, and very long-lived. It is among the most important disturbance agents in the long intervals between stand-replacing events such as wildfire or harvest in these ecosystems and shapes the structure and composition of both wild and managed forests. Trees are infected and killed regardless of individual vigor. Management of public lands is changing dramatically, with renewed emphasis on natural forest structures and processes but pathogens, especially root rot fungi, remain a significant challenge to "ecosystem management."

Quantitative determination of coenyzme Q10 by liquid chromatography and liquid chromatography/mass spectrometry in dairy products.

The dietary sources of CoQ10 and the evaluation of CoQ10 in dairy products were characterized. For quantitation of CoQ10 in food samples, 2 liquid chromatography (LC) methods with UV and mass spectrometry (MS) detections were developed. LC with UV detection was performed at 25 degrees C on a Hyperclone ODS 5 microm 150 x 4.6 mm column with mobile phase consisting of methanol-ethanol-2-propanol (70 + 15 + 15, v/v/v). Flow rate was 1.0 mL/min. Retention time of CoQ10 was 10.9 +/- 0.1 min. The method was sensitive [limit of detection (LOD) = 0.2 mg/kg], reproducible [relative standard deviation (RSD) = 3:0%), and linear up to 25 mg/kg (R > 0.999). LC/MS analysis was performed on a LUNA C18 3 microm, 150 x 4.6 mm column, using mobile phase consisting of ethanol-dioxane-acetic acid (9 + 1 + 0.01, v/v/v), flow rate was 0.6 mL/min, and the retention time of CoQ10 was 4.1 +/- 0.1 min. Identification and quantitation were performed with a Finnigan-LCQ mass detector in positive atmospheric pressure chemical ionization mode. Mass spectra were obtained in selected-ion monitoring mode; molecular mass (M+H)+ m/z 863.4 +/- 1 was used for quantitative determination. MS detection is more sensitive than UV detection (LOD = 0.1 mg/kg), less reproducible (RSD = 4.0%), and linear in selected range. Analytical recoveries are 75-90% and depend on the ratio between the amount of fat in the matrix and the concentration of CoQ10 in the sample. Some soybean milk products were analyzed together with different cow, goat, and sheep milk products. Concentrations obtained with LC and LC/MS were compared with a few accessible results available from the literature. Concentrations varied from 0 ppm in soybean milk to nearly 2 ppm in fresh milk from local farms.

[Immunochemical study of blastolysin].

The immunochemical properties of blastolysin (BL), a preparation obtained from the cell walls of Lactobacillus bulgaricus with immunostimulating and antitumor activity were studied. The experiments on dogs and rabbits showed that the immunogenic effect of BL was not high. However, after parenteral administration to the dogs and rabbits it induced production of the specific antibodies detected with the Ouchterlony test of direct hemagglutination and immunodiffusion. This should be taken into account in development of the schemes for its use as a pharmacological agent. With the procedures of gel filtration on the columns with Sephadex G-75 and immunodiffusion the fraction composition of BL was determined and the use of the immunochemical procedures for standardization of the BL preparations was shown to be possible.
