[A case of stage IV advanced lung adenocarcinoma treated effectively using paclitaxel with the fibrosing radiographically on the chest CT].

A 48-year-old man presented with dyspnea. He was diagnosed as having advanced lung adenocarcinoma of stage IV (T4N2M1). After 3 courses of paclitaxel therapy every 3 weeks intravenously, subjective symptoms, arterial blood gas and radiographical findings improved very much. In the process of the tumor reduction, chest CT showed unique radiographical findings of so-called lung fibrosis over the entire lung and reduction of lung volume. An additional 3 courses of weekly paclitaxel therapy have been given on an outpatient basis. After the change of therapy, the reduction in the tumor has continued and the tumor progression is under control without a decline in the patient's QOL from the start of the chemotherapy over 6 months to the present.

[Levels of mucin-antigen 3EG5 in the blood and bronchoalveolar swabs in patients with tuberculosis, sarcoidosis and fibrosing alveolitis].

The levels of the antigen mucin 3EG5, a marker of the activity of interstitial lung diseases, were studied in the sera and bronchoalveolar swabs of patients with active tuberculosis, sarcoidosis of the respiratory organs, and idiopathic and exogenous allergic alveolitis. Its higher blood levels were detectable in patients with alveolitis (exogenous allergic ones to a greater extent) and absent in active tuberculosis and sarcoidosis. There was a correlation of the levels of the antigen mucin 3EG5 in the blood and bronchoalveolar swabs with the degree of lymphocytosis in the endopulmonal cytogram. The fact that there is a high variability of the blood levels of the antigen mucin 3EG5 in the range of nosological entities calls the differential diagnostic informative value of this parameter to question.

Fibrosing skin condition among patients with renal disease--United States and Europe, 1997-2002.

During May 1997-November 2000, eight (3%) of 265 kidney transplant recipients at a hospital in California developed an unusual skin condition posttransplant (Figure 1). On clinical examination, the patients had fibrotic skin lesions histologically resembling scleromyxedema on their distal extremities and trunk, resulting in severe contractions and limited mobility. However, the usual IgG lambda paraprotein associated with scleromyxedema was not observed in these patients. Personnel in the dermatopathology section at the University of California, San Francisco, reviewed the biopsies and concluded that this skin disorder had not been described previously. As a result, health-care providers at the hospital where the index patient was treated asked the California Department of Health Services (CDHS) and CDC to assist in the investigation. This report summarizes preliminary findings from the investigation.

[Potential predictors of therapeutic response in patients with cryptogenic fibrosing alveolitis].

The authors present a clinical observation with the objective to test which objective indicators make it possible to predict the effect of treatment in patients with cryptogenic fibrotizing alveolitis. Retrospective analysis of the documentation of a longitudinal follow up of 34 patients with this diagnosis revealed that the presence of fibroproliferative changes on the high resolution CT image predicts significantly the absence of a therapeutic effect and conversely the absence of such changes predisposes for significant improvement of pulmonary functions during treatment. They found also a significant correlation between he ratio of lymphocytes in the bronchoalveolar lavage fluid and the dynamics of the development of pulmonary functions. The age of establishment of the diagnosis correlates also significantly with indicators of the therapeutic response. Based on these results the authors conclude that a favourable therapeutic effect can be expected in patients without fibroproliferative changes, with a significant lymphopcytosis in the bronchoalveolar lavage fluid and a younger age at the time of manifestation. On the other hand, in patients with a marked fibroproliferation and absence of alveolitis there is no point in attempting to influence the disease by pharmacological gents.

Histogenesis of leiomyomatosis peritonealis disseminata (disseminated fibrosing deciduosis).

Leiomyomatosis peritonealis disseminata (LPD) is a rare clinical entity. Widespread subperitoneal nodules of apparent smooth muscle are found in patients who are pregnant or have recently been so. This clinical appearance together with mitotic figures found in frozen sections suggests malignancy. We believe, in contrast, that LPD is a benign reparative process in which fibroblasts replace subperitoneal decidua.
