[Effect of physical loading on serum enzyme activity in post-myocardial infarct patients].

The study of patterns of serum AST, ALT, CPK, LDH, and glycogen phosphorylase (GP) activity following bicycle ergometry in 26 male patients 1 to 1.5 months after myocardial infarction demonstrated no increase in AST, ALT and CPK activity, whereas total LDH activity was increased, with a tendency to elevated LDH-1 and LDH-2 fractions, as compared to the baseline, in those cases where exercise was discontinued because of ST depression. Patients with favorable response to bicycle ergometry that continued until the submaximum heart rate for a given age was achieved showed a tendency to elevated LDH-5 that may be a physiological response to exercise. The demonstrated increase in total GP activity, both in patients with exercise-induced ST depression and in those with elevated ST from the leads corresponding to the site of myocardial infarction, may reflect stress-induced reversible ischemia.

[Radionuclide evaluation of the cardiovascular system in arterial hypertension].

Proceeding from a study of the nature of changes in hemodynamics during development of hypertensive disease (HD) at its different stages it was shown that hemodynamic changes in 42.1% of the patients with Stage I-IIA HD were of hypertensive type, in the patients with Stage IIB-III HD normal and hypokinetic types of the blood circulation prevailed. After bicycle ergometry exercise the reactivity of the cardiovascular system was revealed more completely. The transition of one hemodynamic type into another and its detection acquired a great importance. The definition of the types of hemodynamics at rest and of effort was very important in terms of adequate antihypertensive therapy and the prediction of a subsequent course of disease. The most complete information on function of the cardiovascular system and myocardial contractility can be obtained with the help of radio-angiocardiography and radionuclide ventriculography. However in the absence of a gamma-chamber radiocardiography can provide necessary information on function of the cardiovascular system in case it is used in one and the same patient over time using bicycle ergometry testing.

[ECG changes during the performance of a loading test in coronary atherosclerosis patients with asynergies].

The findings of coronary angiography, ventriculography and bicycle ergometry were compared in 64 patients with coronary heart disease and asynergic zones in the left ventricle. An increase in the sum of the amplitudes of R waves of the ECG during the exercise test generally combined with a displacement of the ST segment was predominantly observed in patients with asynergies of ischemic genesis. By contrast, in patients with asynergies secondary to post-infarction cardiosclerosis, an elevation of R waves during bicycle ergometry cannot be a reliable sign of coronary failure, yet it permits the identification of individuals with marked disorders of the contractile function of the left ventricle.

[Hemodynamics and coronary dynamics under ISDN long-term therapy].

Although nitrates are among the oldest drugs in cardiology the problem of tolerance is a scientific challenge of today. We examined and compared the effects of initial and chronic therapy (4 weeks) with 1 X 1 ISDN 120 mg sustained release in 9 patients with coronary heart disease and impaired left ventricular function. At intraindividually identical workloads (average 50 +/- 12 watt) there was a reduction of PCWPm from 32.5 +/- 9.5 to 19.7 +/- 9.8 mm Hg. This reduction of PCWPm during bicycle ergometry was fully achieved in long-term therapy. With the first dose of ISDN cardiac index (CI) at maximum workload increased from 6.0 +/- 1.2 to 6.8 +/- 1.3 l/min/m2; during chronic therapy cardiac index improved from 5.3 +/- 1.3 to 6.6 +/- 1.1 l/min/m2. Exercise capacity during bicycle ergometry in the sitting position increased concomitantly from 414 to 686 watt X min. In long-term therapy there was a further significant improvement to 772 watt X min. ST-segment depression, measured as sum of ST-depression in all 12 standard ECG leads decreased from 0.63 mV before medication to 0.11 mV after the first dose and to 0.16 mV in long-term therapy. The investigation of ventricular function during ventriculography and the investigation of coronary vasomotion during coronary angiography after ergonovine maleate i.v. and ISDN s.l. also demonstrated the full nitrate effect in long-term therapy.

Maximal response of wheelchair-confined subjects to four types of arm exercise.

Maximal work capacity and aerobic fitness of wheelchair-confined subjects generally have been determined by one of three modes of exercise: wheelchair ergometry, wheelchair exercise on a treadmill (TM), and arm crank ergometry. In the present study, two new types of arm ergometers, the Cybex Upper Body Exercise ergometer (CUBE) and the Schwinn Air-Dyne ergometer (SAE), have been evaluated and compared with older methods for determining aerobic capacities of wheelchair-confined subjects. Seven persons with paraplegia with lesions ranging from T-3 to L-1 and one bilateral amputee each performed four exercise tests to volitional exhaustion, on separate days, utilizing four modes of exercise. These included wheelchair exercise on a treadmill and arm exercise using a Monark arm ergometer (MAE), a CUBE, or an SAE. Peak values for oxygen consumption, heart rate, and blood lactate were similar during each type of exercise. Significant (p less than 0.05) differences were observed only in the peak values for minute ventilation (CUBE greater than MAE, SAE greater than MAE). The similarity of the maximal responses observed in this study indicates that the CUBE and SAE are comparable to TM exercise and to the MAE in assessing the aerobic capacity of wheelchair-confined subjects.

Changes in cardiovascular fitness following physical fitness classes.

A program aimed at improving cardiovascular fitness was evaluated. This program involved 298 volunteer middle-aged participants who initially had a bicycle ergometry test to calculate fitness. Other measurements included body fat (estimated by skin fold measurements) and a rating of coronary-prone personality. The participants were given their results and then offered a six-week fitness class consisting of both didactic and exercise segments. After six months, bicycle ergometry was repeated. During this period, mean calculated maximum oxygen uptake (a measure of fitness with a "normal" range of 20 mL/kg/min to 60 mL/kg/min) improved from 38.0 to 39.6 mL/kg/min (t test, P = .0001). Classifying this according to a cardiovascular fitness rating based on age and sex, with 5 being excellent fitness to 1 being very poor fitness, the mean fitness rating went from 3.08 to 3.31 (t test, P = .0001). There was no change in weight or percentage of body fat. Both those who attended the fitness classes and those who did not attend improved equally. Those who did not attend classes had higher ratings on a coronary-prone personality scale compared with those attending classes. In some groups, it appears that significant improvement in cardiovascular fitness may be initiated with a five-minute, office-based bicycle test.

[Ventilation and gas exchange in the lungs during physical exertion in patients following the correction of mitral stenosis].

During rehabilitation of 40 patients after correction of mitral stenosis, a study was made of ventilation and gas exchange in the lungs under spirometric bicycle ergometry according to the "standard protocol" until the maximal oxygen consumption (MOC) was attained. The patients were examined 20-30 days, 3 and 6 months after operation. In accordance with the magnitude of the MOC expressed in metabolic equivalents (MET), all the patients were divided into 3 functional classes which corresponded roughly to the NYHA classification. It was revealed that with the same magnitude of the MOC the patients of the worst functional class had a lower production of CO2, a greater volume of the dead space and a higher level of minute ventilation. It is supposed that the described respiratory abnormalities are connected with enhanced ventilation of the poorly perfused alveoli, apparently because of venous congestion in the pulmonary capillaries. The characteristics of gas exchange and ventilation in the lungs investigated under the conditions of the "standard protocol" of spirometric bicycle ergometry objectively mark pump function of the heart and respiratory potentialities of the cardiorespiratory system, which might be the basis for the functional classification of the patients after cardiosurgical interventions.

[Importance of combined instrumental study methods in the expertise of the work capacity of ischemic heart disease patients with stable stenocardia].

Working capacity of 87 coronary patients with stable angina was assessed on the basis of a functional classification of the results of non-invasive investigations (resting ECG, bicycle ergometry, echocardiography). Bicycle ergometry proved to be the most informative method providing indirect evidence of coronary reserve. An objective assessment of the condition of coronary patients can be made using values of exercise tolerance (threshold capacity and volume of work), heart rate and double product at the peak of exercise. A close correlation exists between the severity of atherosclerotic affection of coronary arteries and individual exercise tolerance, which can be used in the medical evaluation of the working capacity of coronary patients with stable angina pectoris.

[Multifactorial analysis of the hemodynamic reactions to a physical load in hypertension].

Multifactorial analysis of findings from 66 essentially hypertensive patients and 14 normal subjects investigated during bicycle ergometry (250 and 500 kgm/min) demonstrated a relationship between hemodynamic changes and the patient's baseline status, age and stage of the disease. Hypertensive patients showed greater AP response to exercise and smaller changes in heart productivity and vascular tone, as compared to normal subjects with similar resting hemodynamic status. Patients with high resting AP and CI values showed smaller shifts during bicycle ergometry, as compared to other patients at the same stage of the disease, while diastolic AP increase was more pronounced in elderly patients.

[Changes in myocardial metabolism during atrial pacing and catecholamine stimulation in normal subjects and patients with angina].

5 patients with angina had their threshold for angina evaluated by atrial pacing and by stimulation with catecholamine (dopamine). The authors then studied the metabolic changes induced by the two types of anginogenic load (AP and D-test) applied successively, and compared them with the post-ergometry ECG changes (lowering of the ST segment by at least 2 mm) and the findings on coronary arteriography (complete obstruction of at least 75% stenosis of a major branch vessel). The metabolic measurements were controlled against those of 5 normal subjects after standard ergometry. In the patients with angina, the AP test led to a constantly negative value for %. L. By contrast, the values of D(a-v)c and % O2 which were reduced in the normals, as indirect evidence of an increased coronary flow, remained practically static. During the dopamine infusion, although the % L was reduced, it remained essentially positive, while the D(a-v)c and % O2 were consistantly lowered.

[Effect of a single dose of anaprilin and corinfar on the results of the bicycle ergometric test in patients with stenocardia].

Bicycle ergometry was conducted in conjunction with the consecutive administration of the placebo, 80 mg anaprylin, 30 mg corinfar, and 80 mg anaprylin + 30 mg corinfar in patients with 2nd--4th class angina of effort. Both anaprylin and corinfar significantly raised patients' physical stress tolerance, increased the total amount of work done, and reduced the magnitude of St depression during exercise. Combined use of anaprylin and corinfar produced the greatest rise in physical stress tolerance in more favorable hemodynamic conditions. Bicycle ergometry in conjunction with anaprylin and corinfar can supply additional functional information on angina pectoris and its pathogenetic mechanisms.

[Comparative evaluation of the effectiveness of various physical training methods with patients who have had a myocardial infarct].

A total of 180 men were examined 4 months after myocardial infarction. The patients of the main group were randomised into subgroups according to the nature of physical training (PT). B2 subgroup did in-door therapy exercises with elements of games; B3 subgroup was assigned the same exercises, but received 5,2 mg of nitrong 1 hour prior to PT; B4 subgroup practiced bicycle ergometry with stable load comprising 50% of individual exercise tolerance. In B5 subgroup the stable load was 75% and in B6 subgroup the load was gradually increased (50-75-90% of individual exercise tolerance). All the patients participating in PT showed a considerable increase of the main indices of physical capacity (threshold power, maximal oxygen consumption, double product), which was accompanied by an improvement of their state and functional class. PT with pre-exercise nitrong, and bicycle ergometry with the load constituting 50% of the individual threshold power yielded the best results.

[Information value of the changes in the ECG elements during graded physical loading in the diagnosis of ischemic heart disease].

The authors provide the data obtained on retrospective analysis of 30 ECG recorded in 30 men with coronary heart disease (CHD), confirmed by the clinical and coronarography data, during bicycle ergometry. The amplitude of the R wave in the V5 lead and the total amplitude of the R wave in the V3-V6 leads were analyzed. The area of the QRS complex was calculated as a sum of three triangles. While drawing up a triangle according to the S wave one of the sides went over from the S wave top across the J point till crossing the isoline. The amplitude and duration of the T wave were also taken into consideration. It was established that evaluation of the area of the QRS complex, whose magnitude is an integrated index of changes in all the elements of the ventricular complex, is of the greatest value in the diagnosis of CHD during bicycle ergometry.

[Method of diagnosing ischemic heart disease by using isometric loading].

Twenty-five male patients, aged 41 to 57, with typical clinical signs of angina of effort and positive bicycle ergometric tests, were investigated to compare diagnostic possibilities of bicycle ergometry, manual dynamometry and the combination of the latter with increased venous return achieved by means of changed position of the body in coronary patients. The analysis of ECG changes showed ST displacement by more than 1 mm in 100% of cases with bicycle ergometry (in keeping with the criteria of patient selection), in 24% with manual dynamometry, and in 76% with increased venous return as part of the combination. Considering disorders of cardiac rhythm as a sign of coronary disease, static load tests were positive in 32% of cases, and the same tests combined with increased venous return gave positive results in 88%. The simplicity of the test makes it acceptable for out-patient use.

[Importance of the intravenous isoprenaline test in diagnosing ischemic heart disease].

The isoprenaline test was performed in 89 patients with CHD and cardialgias of various origins. The diagnosis was verified by coronarography and bicycle ergometry in 60 patients. Isoprenaline was administered via intravenous drip, with the initial rate of 2-3 micrograms/min eventually increased until clinical or electrocardiographic signs of myocardial ischemia emerged, or the heart rate reached 130 beats per minute. When compared with coronarographic and bicycle-ergometric results, the isoprenaline test was shown to be highly sensitive and specific (64.3 and 95.2%), as evidenced by the electrocardiographic positive-result criteria, and superior to bicycle ergometry in its diagnostic possibilities. The isoprenaline test is recommended for the diagnosis of coronary heart disease in cardiologic and therapeutic hospital units. The test can be a method of choice in cases where rationed exercise tests are impossible to achieve.

[The behavior of heart rate and blood pressure during loading in stage-I and -II hypertensives--a criterion for the differential therapy of arterial hypertension].

84 hypertensives and 40 normotensives were examined on a bicycle ergometer. In the hypertensives different reaction patterns of blood pressure and heart rate were found, which could be subdivided into 5 groups. From this subdivision conclusions to the developmental mechanism of the hypertension may be derived: hyperkinetic dysregulation or resistance high blood pressure. Furthermore, a conditioned by situation "consulting hours hypertension" may be demarcated from the disease accompanied by hypertension. A haemodynamically oriented differential therapy and a following control ergometry were performed. Previously increased values of blood pressure and heart rate were significantly lower. Finally the indications for ergometry of patients with hypertension are demonstrated.

[The diagnostic accuracy of the dipyridamole test in coronary heart disease (author's transl)].

The diagnostic accuracy of the dipyridamole test in provoking coronary insufficiency was investigated in 79 patients with chest pain and the results were compared with the findings on angiography and exercise electrocardiogram. 58 patients had documented severe coronary artery stenosis, 21 had patent coronary vessels (cardiomyopathy 8, aortic stenosis 1, ectopic origin of coronary artery 1, normal 11). Anginal pain after dipyridamole was a non-specific finding. Approximately half the subjects in whom coronary insufficiency would be expected according to the coronary angiographic and ventriculographic findings evidenced ischaemic ST-segment depression after dipyridamole, which was comparable to the number of positive exercise electrocardiograms. In 23 patients, most of whom had shown an inadequate frequency response during the initial exercise test, ergometry was repeated after the administration of dipyridamole. This resulted in an increase in ischaemic ECG response from 26 to 70%. It is concluded that a stress test combining dipyridamole and submaximum exercise increases the incidence of ischaemic ST-segment depression in comparison with ergometry alone. Anginal pain without ST-segment depression proved to be without diagnostic value.

[Method of conduction of the veloergometric test in rehabilitation of cardiosurgical patients].

Determination of the physical working capacity of patients after surgical correction of heart disease and appraisal of the functional condition of the cardiovascular and respiratory systems allow for establishing a program of physical rehabilitation and judge its efficacy during follow-up of the patient. The bicycle ergometry test in cardiosurgical patients has certain features. The submaximum and threshold loads should be distinguished. The use of step-by-step increasing interrupted test is recommended. The duration of the rehabilitation period and delayed reactions are important in appraising the reaction to the load. The use of the extrapolation method in calculating taindications for the bicycle ergometry test for cardiosurgical patients.
