New measurement of patellar height in the knees of children using the epiphyseal line midpoint.

The height of the patella was reevaluated by the ratio of the distance from the center of the patella to the epiphyseal line midpoint in the proximal tibia (PT) to that between the distances of the distal femur and of the proximal tibia (FT) on 59 knees of 36 normal children whose ages ranged from 3 to 18 years (average 10.6 years). The averaged ratio of PT to FT was 1.31 +/- 0.9 at full extension and gradually decreased to 0.99 +/- 0.06 at 90 degrees flexion. Changes in value were minimal within the range of 0.99-1.20 in the range of knee flexion from 30 to 90 degrees.

Effect of immunosuppression on survival and growth of cartilage produced by transplanted allogeneic epiphyseal chondrocytes.

Strong short-term immunosuppression improved survival of cartilage formed by transplanted allogeneic epiphyseal chondrocytes in mice. The agents tested were cortisone acetate (CA), cyclophosphamide (CY), procarbazine (PCH), and antithymocyte serum (ATS). Their effect on syngeneic grafts was examined morphologically and histomorphometrically. In untreated recipients, chondrocytes formed cartilage nodules that underwent endochondral ossification. Except for high repetitive doses of CY, none of the other agents interfered with normal cartilage formation. However, all agents affected endochondral ossification. In the allogeneic system, the effect of immunosuppression was examined morphologically and by evaluation of specific humoral and cellular antigraft immunity. Allogeneic chondrocytes evoked a strong immune response in untreated mice, and cartilage was gradually destroyed by infiltrating cells. Endochondral ossification did not occur in this system. Neither agent given alone exerted a marked, long-lasting protective effect upon the graft. However, combined treatment with ATS and PCH inhibited immune response and completely prevented infiltrate formation and allowed endochondral ossification similar to that in the syngeneic control. Although some weak signs of antigraft immunity were seen after six weeks, it is possible that they were due to secondary exposure of antigen-bearing chondrocytes in the course of endochondral ossification.

Effect of growth hormone on epiphyseal growth plates in zinc deficiency.

This study was carried out on 18 albino rats. The control group, consisting of six rats, was fed a normal diet and tap water. The other rats were given a zinc-deficient diet and deionized water. Two weeks later the latter group was again divided into two into smaller groups. The first of these groups was given an given an additional 100 ppm of zinc acetate. From the fifteenth day onwards, the human growth hormone (Nanormon) was injected intra-peritoneally to each group at a dose of 80 micrograms/day for two weeks. At the end of the experiment, it was found that body weights were markedly increased in the control and zinc-added groups compared with the zinc-deficient rats. In the zinc-deficient group, serum zinc, hair zinc levels, alkaline phosphatase activity and serum protein levels were lower than those of the control and zinc-added groups. The zinc-deficient group had narrower tibial epiphyseal growth plates than those of the control group. The count of hypertrophied cells was also less in the zinc-deficient group. Based on these data, our conclusion was that the growth hormone becomes ineffective under conditions of zinc deficiency. This means that zinc deficiency has multidimensional effects on growth hormone activity.

Use of cultured embryonal chick epiphyseal chondrocytes as grafts for defects in chick articular cartilage.

Full-thickness defects in articular cartilage were repaired with cultured homologous embryonic chick epiphyseal chondrocytes embedded in a biological resorbable immobilization vehicle (BRIV). This graft was successfully transplanted in mechanically induced defects in the surface of condylar articular cartilage of the tibiotarsal joint of four-month-old roosters. Healing of the defects was observed macroscopically, histologically, and histochemically and with the use of biochemical analyses for six months. Chondrocyte proliferation was seen 48 hours after implantation, and a hyaline cartilage matrix surrounding the cells was present two weeks later. Within eight weeks, the defects were completely filled with hyaline cartilage, which integrated smoothly with the neighboring cartilage without the formation of fibrous tissue at the interface. The cell content and rate of proteoglycan synthesis remained high for four months, then declined slowly to the level of the surrounding cartilage. Six months after transplantation, the cartilaginous tissue in the wound at levels below the ossification front showed penetration by vascular elements and young bone trabeculae at the margins of the reparative tissue. No signs of immunogenic rejection of the implants were observed. These results may be related to the employment of a capable source of cells, i.e., cultured chondrocytes characterized by a high mitotic rate and an early stage of development. The transplanted cells grew well and maintained their initial rate of proliferation, with definite maturation and transformation. The resulting cartilage was structurally reorganized according to the host pattern and under the influence of multitudinous environmental conditions. The articular zone preserved its cartilaginous phenotype, whereas the subchondral regions were transformed into bone.

Epiphyseal growth plate fracture: Salter and Harris type V.

One of the most easily misdiagnosed injuries, which can result in severe consequences in young patients, is a fracture involving the epiphyseal growth plate. The Salter and Harris type V fracture is a relatively uncommon injury resulting from a severe crushing force applied through the epiphysis to an area of the growth plate. It is most likely to occur in the region of the knee or ankle. The following is a case report of a comminuted compression fracture of the epiphysis of the fifth metatarsal head, with complete transverse midshaft fracture of the proximal phalanx of the fifth digit.

Epiphyseal separation of the coracoid process associated with acromioclavicular sprain. A case report and review of the literature.

A rare injury of traumatic separation of the epiphysis of the coracoid process associated with Grade I acromioclavicular sprain occurred in a nine-year-old girl. The accident occurred when she was asleep. The mechanism of injury was unknown. Tenderness and mild swelling around the coracoid process were seen. The anteroposterior radiograph of the shoulder with the arm abducted 150 degrees (abduction view) clearly demonstrated the epiphyseal separation with subtle displacement at the base of the coracoid process. She was treated by simple immobilization and recovered fully with normal shoulder arm function.

[Correction of angular deformities caused by premature closure of the distal ulnar epiphyseal plate in dogs].

Surgical correction of angular deformities caused by premature closure of the distal ulnar growth plate in dogs. Correction of angular deformities in 14 cases of premature closure of the distal ulnar growth plate is described. Angular deformities were most frequently seen in large breeds during the most active phase of growth at 3-5 months of age. Preoperative axial deviations measured on radius were from 7 to 38 degrees in valgus and 0 to 35 degrees in antecurvation compared with the contralateral leg. According to the kind of treatment, the dogs were divided into two groups: In group I which consisted of cases with sufficient remaining growth potential of the radius, segmental ostectomy of ulna with insertion of an autologous fat graft in the ostectomy diastasis was performed to allow spontaneous correction by asymmetrical epiphyseal growth. In group II reconstructive osteotomy and stable, internal fixation with a plate was accomplished. The cosmetic result was judged as excellent in 10 cases, good in 3 and less favourable in 1. Postoperative follow up time was in average 33 months. On control 43% of the dogs showed no lameness, 57% were slightly lame after exercise or rest. None of the cases turned out to be permanently lame at the end of the follow up investigation.

Epiphyseal traction to correct acquired growth deformities. An animal and clinical investigation.

In a canine model with a transepiphyseal distal femoral fracture, varus deformity was corrected and the arrested limb was lengthened using transphyseal traction (turnbuckle). Correction was possible without removing the transphyseal bone bridge operatively. Subsequent growth was not adversely affected in most animals. Based on this study, two patients with significant growth deformities secondary to an enchondroma were treated by epiphyseal traction. Although correction was achieved initially, both lost the length previously gained because of premature removal of the external fixator and/or subsequent early growth plate closure.

[Morphological study of the epiphyseal cartilage in cartilage matrix deficiency (CMD) mouse--a consideration on the roles and functions of cartilage-specific proteoglycan].

Epiphyseal cartilages in mouse with cartilage matrix deficiency due to genetic failure to synthesize cartilage-characteristic proteoglycan were examined under light and electron microscope. Chondrogenesis and proliferation of chondrocytes seemed to occur, though extracellular matrix was small in amount and chondrocytes were packed closely without consistent orientation. In diaphysis, hypertrophic change of chondrocytes and perichondral ossification were observed. In the epiphysis, there was neither zone formation nor column formation, but hypertrophic chondrocytes and calcification in matrix were observed in the area adjacent to the bone shaft. Electron microscopy showed dilatation of rough endoplasmic reticulum, swelling of mitochondria and a large amount of lipid deposition in the chondrocyte. In the cartilage matrix, it was characteristic that a large number of thick collagen fibrils was arranged in parallel and few matrix granule was seen. These findings suggested that cartilage-characteristic proteoglycan was not essential for chondrogenesis, proliferation of chondrocytes and ossification, but were important for cytodifferentiation and chondrocyte activity.

[The treatment of the distal epiphyseal fracture of the tibia (author's transl)].

The distal epiphyseal fractures cases for 10 years period have been reported. The authors state that it is always worth-while to carry out an operation in group "B" of the Müller's scheme, while in group "A" and "C" only selected cases are worth to operate. In the 17 cases being observed till now no disturbed growth has been found.

Benign epiphyseal chondroblastoma adjacent to the femoral intercondylar fossa.

The authors present three cases of epiphyseal chondroblastoma localised adjacent to the femoral intercondylar fossa which proved to be relatively common in a recent review of the case material of the G. Pini Institute of Milan (3 out of 22 cases). They were able to deduce various features of value in diagnosis and treatment which enable a single stage operation to be planned. This consisted of curettage with or without spongy bone transplants, and can be carried out even when the articular cartilage has been breached. The long term results in all three cases were satisfactory.

[Tension band wiring of canine and feline supracondylar epiphyseal femoral fractures (author's transl)].

The present investigation describes the treatment of 27 cases of canine and feline supracondylar epiphyseal--metaphyseal femoral fractures with tension band wire. The wire was passed through transversal parallell drill holes, one in each fragment. The wires intercrossed dorsally. Stable fixation followed tightening of the wires. 20 out of 25 primarily successful cases were re-examined. 19 were completely well. One case showed a slight transient lameness following hard exercise.

Stereological studies on the epiphyseal growth plate in strontium-induced rickets. With special reference to the distribution of matrix vesicles.

Applying stereological principles, we recently demonstrated a matrix-vesicle distribution between the zones of the epiphyseal growth plate that is at variance with the generally accepted distribution. In the present study of strontium-induced rickets we demonstrated a similar bimodal distribution, the highest matrix-vesicle volume fraction being found in the resting and hypertrophic zones and the lowest, in the proliferative zone. These volume-fraction differences are due to differences in the number of vesicles between zones, the variation in mean diameter being rather small. The disturbed mineralization in strontium-induced rickets is associated with a diminished reduction of the matrix-vesicle volume fraction in the lower part of the growth plate compared with normal animals. The findings may indicate that matrix-vesicle removal is a part of the mineralization process, but a causal connection cannot be proved on the basis of our results. Our findings are, however, compatible with the earlier-posed cell-debris theory for matrix-vesicle origin and distribution.

An irreducible phalangeal epiphyseal fracture-dislocation. A case report.

A 20-month-old infant sustained an irreducible epiphyseal fracture-dislocation of a proximal interphalangeal joint. The dorsal dislocation of the joint was associated with a 90 degree rotational displacement (Salter-Harris Type I fracture) of the epiphysis of the middle phalanx. This injury, which seems not to have been described previously, required open reduction through both volar and dorsal incisions because the intact collateral ligaments and extensor central slip did not permit adequate distraction of the joint.

[Negative feedback in the epiphyseal-thyroid system].

The negative feedback mechanism in the epiphyseal-thyroid system was studied. The activating thyroxin influence upon hydroxy-indole-O-methyl-transferase, being the main enzyme of melatonin biosynthesis, was found in experimental Wistar rats. The "dose-response" curves were examined at varying times after thyroxin injections. The results obtained confirm the previous data concerning the activating effect of rat chronic hyperthyroidization on hydroxy-indole-O-methyl-transferase and intensified melatonin excretion with the urine in patients with pronounced thyrotoxicosis.

Therapeutic problems in familial poly-epiphyseal dyschondroplasia.

The writers describe the clinical features of familial poly-epiphyseal dysplasia without spinal involvement affecting two sisters who both underwent surgical treatment with a view to restoring balance in load bearing joints. The satisfactory results are illustrated and "biological" osteotomy is advised in such cases, even if painful symptoms have not yet appeared.
