Expert panels and ontario's wait time strategy: part 2.

The debate: myalgic encephalomyelitis and chronic fatigue syndrome.

Almost every aspect of myalgic encephalomyelitis (or encephalopathy) and chronic fatigue syndrome is the subject of disagreement and uncertainty -- something that has undoubtedly hampered recognition, understanding and research. Although the pathogenesis remains the subject of intense medical debate, a number of predisposing, precipitating and perpetuating factors are now starting to emerge. Therapeutic nihilism is no longer appropriate as there is a great deal that can be done to alleviate some of the more distressing symptoms and improve quality of life for these patients.

Guideline development for office-based pharmacotherapies for opioid dependence.

The success of opioid agonist maintenance has stimulated efforts to make this form of treatment more available. Methadone medical maintenance, coordination of methadone services from a physician's office, has been offered as an alternative to narcotic treatment programs for stable, recovered and socially rehabilitated opioid dependent patients. Despite the successful implementation of methadone medical maintenance programs, a number of important questions regarding the appropriate guidelines for the use of this model of care remain. The current paper reviews the process and outcome of the Medical Maintenance Consensus Panel, which was convened for the federal Center for Substance Abuse Treatment. We outline the process and describe the two guidelines that were produced by this process that are targeted at physicians, narcotic treatment programs, and policy makers.

Definition of medication therapy management: development of professionwide consensus.

OBJECTIVE: To describe events leading to development of a professionwide consensus definition of medication therapy management (MTM) and attendant programs and services and present the document (definition, services, and program requirements) resulting from the process. DATA SOURCES: Author's own knowledge and records of events. SUMMARY: Following the late 2003 passage of the Medicare Prescription Drug Improvement and Modernization Act, the pharmacy profession had a need to act quickly to define MTM so that a consensus definition would be available as regulations implementing the Medicare Part D benefit were being written. The American Pharmacists Association facilitated this process by convening a broad working group of members and other involved parties to draft a preliminary definition. The Pharmacy Practice Activity Classification was used to check elements of the definition for consistency with services being offered in a wide variety of settings. A professionwide stakeholders conference was then convened with representatives from each of 11 national pharmacy organizations. This group, following a daylong meeting in late May 2004 and several weeks of e-mail messages and conference calls, finalized the MTM definition, which was then approved by the chief executive officers of all 11 groups. CONCLUSION: Through the extraordinary efforts of the numerous organizations and participants, the MTM Services Definition is one that is applicable within diverse pharmacy practice segments, whose services are feasible for a majority of practitioners to implement, and whose elements are supported by a profession-wide consortium of 11 national professional pharmacy organizations. This historic achievement is the first step on a journey to find the best ways to effectively deliver MTM services to patients.

Are condoms effective in reducing the risk of sexually transmitted disease?

A panel of more than two dozen experts reviewed the scientific evidence on condom effectiveness for sexually transmitted disease (STD) prevention, and a final report was released July 20, 2001, by the Department of Health and Human Services. This panel was cosponsored and convened for a two-day workshop by four government agencies (Agency for International Development, Food and Drug Administration, Centers for Disease Control and Prevention [CDC], and the National Institutes of Health [NIH]).

COPD: the scope of the problem in Europe.

In 1995, the European Respiratory Society published a European Consensus Statement on the optimal assessment and management of COPD. In the document, several important areas for future research are identified that may help to increase knowledge of the current situation of COPD within Europe; these include pathophysiology, epidemiology, and the clinical benefits of treatment. This article reviews a selection of important data that have become available since the consensus statement was published, with a specific focus on epidemiology and treatment.
