Regulation of gene expression in post-meiotic male germ cells: CREM-signalling pathways and male fertility.

Spermatogenesis is a remarkably complex process in which diploid spermatogonial stem cells undergo a series of mitotic and meiotic cell divisions to give rise to haploid round spermatids. These haploid cells then go through a dramatic morphological remodelling involving extensive chromatin condensation, reduction in nuclear and cytoplasmic volume, formation of an acrosome system and tail, all of which contribute to the formation of a mature spermatozoon fully capable of fertilizing the oocyte and passing along its genetic information to the next generation. To accomplish such a complex program, an intricate and efficient mechanism is required to finely tune the levels of expression of specific genes necessary for this process. Accordingly, the regulation of gene expression in post-meiotic male germ cells is governed by specific mechanisms unique to these cells. The cyclic adenosine monophosphate (cAMP) response element modulator (CREM) is an essential component of this program, and its activity is regulated through interactions with a germ cell-specific, CREM phosphorylation-independent transcriptional co-activator, activator of CREM in testis (ACT). In turn, the ability of ACT to regulate CREM activity is controlled by a germ cell-specific kinesin, Kif17b, which regulates the subcellular distribution of ACT. Further, the mRNA from CREM target genes interacts with several germ cell-specific RNA-binding proteins, which function to transport and stabilize these mRNAs. This sophisticated and complex regulation of gene expression in post-meiotic germ cells is governed by unique mechanisms specific to these cells and is fundamental to male fertility.

[Muscle efficiency in total shoulder prosthesis implantation: dependence on position of the humeral head and rotator cuff function].

Modern shoulder prostheses permit an anatomic reconstruction of the joint, although the biomechanical advantages are not proven. The goal of this study was to investigate the relationship between position of the humeral head and function of the shoulder prosthesis (muscle efficiency). Shoulder elevation-motion and rotator cuff defects were simulated in vitro in a robot-assisted shoulder simulator. The EPOCA Custom Offset shoulder prosthesis (Argomedical AG, Cham, CH) was implanted in seven normal shoulders (77 +/- 20 kg, 55 +/- 14 years). Active elevation was simulated by hydraulic cylinders, and scapulothoratic motion by a specially programmed industrial robot. Muscle efficiency (elevation-angle/muscle-force of the deltoid muscle) was measured in anatomic (ANA), medialised (MED) and lateralised (LAT) positions of the humeral head, with or without rotator cuff muscle deficiency. Medialisation increased efficiency by 0.03 +/- 0.04 deg/N (p = 0.022), lateralisation decreased it by 0.04 +/- 0.06 deg/N (p = 0.009). Supraspinatus muscle deficiency increased the deltoid force required to elevate the arm, and thus decreased efficiency (ANA p = 0.091, MED p = 0.018, LAT p = 0.028). The data confirm that the position of the humeral head affects the mechanics of total shoulder arthroplasty. Medialisation increases efficiency of the deltoid muscle and may prove useful in compensating isolated supraspinatus muscle deficiency. Lateralisation, in contrast, leads to an unfavorable situation.

[A back rehabilitation school for patients with vertebral fractures].

This paper describes the conception and the evaluation of a back school for patients with vertebral fractures. The school curriculum includes detailed information for the patient as well as physiotherapy, training in the proper way of performing the ordinary activities of the everyday-life, systematic self observation and relaxation training. From October 1987 until July 1989, 136 patients with vertebral fractures have completed the back school program. The effectiveness of the program was evaluated by a knowledge test and by an obstacle course. Both in the pre-post-confrontation as well as in the experimental group-control group comparison we obtained significant differences in the goals. In conclusion, the present results imply that the described back school for patients with vertebral fractures is an efficient addition to conventional programs with physiotherapy only.

Comparison of ionization chamber efficiencies for activity measurements.

The calibration of ionization chamber measuring systems in terms of activity is described. The energy-dependent efficiency curves of three chambers at the Bureau International des Poids et Mesures, the National Physical Laboratory and the Physikalisch-Technische Bundesanstalt are determined and compared using a fitting procedure for the experimental radionuclide efficiencies by the Microsoft (MS) EXCEL Solver program. An estimation of the uncertainty of the efficiency curves and the deviations of experimental and calculated radionuclide efficiencies are given. By this fitting method, discrepancies in the efficiency determination can be detected at a level of about one percent. Systematic deviations entering into the calculations either from emission probabilities per decay or from absolute activity standardization are discussed.

In vitro embryo production in buffalo species: state of the art.

In the last several years, there has been an increasing interest in in vitro embryo production (IVEP) technologies for faster propagation of superior germplasm in buffalo because of the low efficiency of superovulation (SO) and embryo transfer (ET) programs in this species. Although the IVEP efficiency has improved, embryo yield and development to term are still very low. This paper reviews the progress made in optimizing the IVM, IVF, and IVC systems. It also highlights the importance of embryo cryopreservation, which might critically contribute to the diffusion of ET procedures in the field. The acquisition of more information on embryo physiology, metabolism, and culture requirements in this species is critical to optimize the efficiency of advanced reproductive strategies. Further studies are also needed to improve the cryopreservation of IVEP embryos. The second part of the work underlines the potential impact of ovum pick-up (OPU) technique combined with IVEP on genetic improvement of buffalos. The OPU technique is a non-invasive and repeatable procedure for recovering large numbers of meiotically competent oocytes from antral follicles of live animals. Our experience, in buffalo, has demonstrated that OPU is superior to SO because it can yield more transferable embryos (TE) per donor on a monthly basis (2 TE vs 0.6, respectively). Therefore this technology has great potential to improve the genetic progress of buffalo through the maternal lineage.

A practical simulated annealing program and its application to quantitative CBED pattern matching

A modified simulated annealing (SA) program is presented (source code available from the authors). The program was used for the refinement procedure in quantitative convergent beam diffraction (QCBED). Considering the practical situation in QCBED, three modifications have been made to the conventional SA algorithm. First, a most probable search boundary (MPB) was suggested, which guarantees the program to search mainly within the MPB, but allows it also to go out of the boundary sometimes. Secondly, the MPB was set to shrink during the minimization. This algorithm decreases the computing time significantly. Thirdly, a more meaningful stop criterion was adopted using the nominal physical precision of each refinement parameter. The feasibility and the efficiency of the modified program were tested using both calculated and experimental CBED patterns. The presented program is proved to be capable of high performance.

A computerized intensive care unit order-writing protocol.

OBJECTIVE: To present a computerized intensive care unit order-writing protocol. DESIGN: Descriptive report. SETTING: Eight-bed surgical intensive care unit, Department of Surgery, Department of Veterans Affairs Medical Center, Bronx, NY. METHODS: IBM-based, computer network program that provides user-friendly, logical, and comprehensive organ-system order sequences for patient management. RESULTS: Since July 1988, an order program that stresses (1) improved and more efficient patient care, (2) the use of program-integrated automatic safety features, (3) the substitution of computer entry for handwriting, and (4) the assurance that physicians deliver obligatory care in a logical organ-system-based progression has been implemented. CONCLUSIONS: The order protocol system presented is simple to introduce and operate, has minimal training and technical requirements, and is demonstrably reliable.

Requisites, benefits, and challenges of sustainable HIV/AIDS system-building: where theory meets practice.

This paper is the third and final of a series that has previously presented the rationale (Rier and Indyk, this volume) and major program elements (Indyk and Rier, this volume) of an approach to link community and tertiary sociomedical providers, clients/patients, sites, and systems into an integrated response to HIV/AIDS. The primary goal has been to improve sociomedical HIV/AIDS services for a hard-to-reach inner city population. The current paper first summarizes the main advantages (e.g., greater efficiency; more realistic, effective programs with greater credibility among the community; stimulation of knowledge production and dissemination amongst players rarely formally engaged in such activities; creation of a platform useful for other applications) of this work. It then examines some of the main organizational challenges in conducting the work (involving issues such as personnel, coordination, funding, turf conflicts, sustainability). From this discussion emerge organizational requisites to conducting this work (e.g., development of key boundary-spanning figures; attention to the specific interests of potential linkage partners; translation efforts to demonstrate the value of participation; a continuous quality improvement approach featuring wide distribution of feedback in user-friendly form; flexibility, tact and patience), so that others can adapt and apply the linkage approach to manage HIV/AIDS or other problems. Finally, we explain how theory and practice have driven one another in this work.

Achievement of the POSITIVE (Participation-Oriented Safety Improvement by Trade Union InitiatiVE) activities in the Philippines.

The POSITIVE (Participation-oriented safety improvement by trade union initiative) programme was introduced into the Philippines in 1995. The monitoring of activities was carried out in 2004 among core trainers who had been trained before. The results of the questionnaire survey showed that the core trainers evaluated their activities satisfactory in general, and particularly the training activities were considered excellent. Also, the union workers who had been trained by the POSITIVE programme implemented improvements at the rate of around 5 examples a year on average. It was of note that the installation ratio (the number of installations/that of plans) was higher in small- and medium-sized enterprises than in larger companies, although the numbers of plans and installations of improvements were greater in large enterprises. Together with the previous findings, the present results suggest that the POSITIVE-style participatory training program is effective and efficient for workers to take actions for the OSH in not only larger enterprises but also small enterprises.

How to develop a competency-based head nurse orientation program.

The ability to effectively and efficiently develop competent head nurses is a major challenge in today's health care environment. Memorial Hospital of South Bend, IN, a 526-bed tertiary care facility, faced this challenge. As a result, a program was developed that reduced the learning time of newly appointed head nurses. Mentoring and hands-on practical learning strategies were used. This article describes (1) the reasons for developing a competency-based head nurse orientation program, (2) the steps necessary for its implementation, (3) the benefits of such a program, and (4) the results of the program at Memorial Hospital.

Preparation of silymarin proliposome: a new way to increase oral bioavailability of silymarin in beagle dogs.

The aim of the present study was to find a method to increase oral bioavailability of silymarin, that is to say, by the preparation of silymarin proliposome and to compare the pharmacokinetic characteristics and bioavailability after oral administration of silymarin proliposome and silymarin in beagle dogs. Silymarin proliposome was prepared by the film-deposition on carriers. After the proliposome was contacted with water, the silymarin liposome suspensions formed automatically. The tests of physicochemical properties including SEM, TEM, encapsulation efficiency, dissolution studies, particle size of the reconstituted liposome and stability of the silymarin proliposome were determined by laser-particle-sizer, HPLC, etc. The concentrations of silymarin in plasma of beagle dogs and its pharmacokinetic behaviors after oral administration of silymarin liposome suspensions and silymarin were studied by RP-HPLC. The pharmacokinetic parameters were computed by software program 3p97. The encapsulation efficiency of silymarin liposome could be more than 90%, with an average particle size of about 196.4 nm and the proliposome appeared a very stability at 40 degrees C during 3 months. It was found that mean plasma concentration-time curves of silymarin after oral administration of liposome suspensions and silymarin in beagle dogs were both in accordance with open two-compartments model and first-order absorption. Pharmacokinetic parameters of silymarin proliposome and silymarin in beagle dogs were Tmax both 30 min; Cmax 472.62 and 89.78 ng mL(-1); and AUC0-infinity 2606.21 and 697 ng mL(-1)h, respectively. The high bioavailability of silymarin proliposome could be obtained by oral administration. Silymarin proliposome was stable and did enchance the gastrointestinal absorption of silymarin.

Effective/efficient mental health programs for school-age children: a synthesis of reviews.

The prevalence of mental health problems, some of which seem to be occurring among younger cohorts, leads researchers and policy-makers to search for practical solutions to reduce the burden of suffering on children and their families, and the costs to society both immediate and long term. Numerous programs are in place to reduce or alleviate problem behaviour or disorders and/or assist positive youth development. Evaluated results are dispersed throughout the literature. To assess findings and determine common elements of effective children's services, a literature search was undertaken for evidence-based evaluations of non-clinical programs for school-age children. Prescriptive comments aim to inform service-providers, policy-makers and families about best practices for effective services such as: early, long-term intervention including reinforcement, follow-up and an ecological focus with family and community sector involvement; consistent adult staffing; and interactive, non-didactic programming adapted to gender, age and cultural needs. Gaps are identified in our understanding of efficiencies that result from effective programs. Policy implications include the need to develop strategies for intersectoral interventions, including: new financing arrangements to encourage (not penalize) interagency cooperation and, to ensure services reach appropriate segments of the population; replication of best practices; and publicizing information about benefits and cost savings. In many jurisdictions legislative changes could create incentives for services to collaborate on service delivery. Joint decision-making would require intersectoral governance, pooling of some funding, and policy changes to retain savings at the local level. Savings could finance expansion of services for additional youth.

Newborn screening for PKU and congenital hypothyroidism in Latvia.

The newborn screening program in Latvia was started in 1980 as pilot study project. Mass screening for phenylketonuria (PKU) in the whole republic was started in 1987, but for congenital hypothyroidism (CH) it begun in 1996. Last two years the Latvian State Medical Genetics Center (SMGC) screened nearly 25,000 newborn dried blood specimens per year. Approximately 25% are repeat and control specimens. Efficiency of the screening program is nearly 97%. Fluorometric method is used for PKU screening, followed by amino acid analysis (HPLC) and analysis of DNA samples for mutations of the PAH gene. Most of the 51 Latvian PKU patients detected from 1980 to 1998 were selected by neonatal screening (38), while the others (13) were diagnosed during genetic counseling. The incidence of PKU in Latvia is 1:8,700 births. CH screening is based on measurement of thyroid stimulating hormone (TSH), using fluoroimmunoassay method, with cut-off value 10 mlU/l, as the primary screening test, and T3 and T4, as confirmatory tests for diagnosis. From 38,684 newborns 37,380 were screened for CH in 1996 and 1997. From these 1,438 (3.8%) newborns were recalled because of initial elevation of TSH level. From these 85 (0.2%) newborns had elevated TSH level during first two weeks of life and were referred to SMGC for repeat testing and follow-up. The diagnosis of CH has been confirmed in 6 babies. The incidence of CH in Latvia is about 1:6,450.

AQA, HQA collaborate on national quality strategy.

Coordinating measurement activities will give patients and providers a better look at quality across the entire continuum of care. Organizations say collaboration will make quality measurement more accurate and more efficient. Newly launched AQA pilot programs will benefit from formation of steering committee.

[The "quality" system in health care].

The evolution of the quality assistance care management, is contributing to create a new strategy toward the single operations responsibility. This program is based on efficiency improvements, based on a synthesis of scientific evidences translated with specifications and criteria, that represents the application of the progress of clinic methodology and of the scientific information in the clinics. The evaluation of new criteria, like the guidelines and evidence based medicine, are our goal of this work, because can be a continuous improvements of the healthy and care programs at all the levels of applications.

Ectopic expression of the CCAAT/enhancer-binding protein alpha promotes the adipogenic program in a variety of mouse fibroblastic cells.

The CCAAT/enhancer-binding protein alpha (C/EBP alpha) has been implicated in the regulation of adipoblast differentiation. In this study we investigate the potential of C/EBP alpha to promote the adipogenic program in a variety of fibroblastic cells. Transduction of the C/EBP alpha gene into eight mouse fibroblastic cell lines by retroviruses and DNA transfection generates adipocyte colonies at variable frequencies. The most dramatic results are obtained with NIH-3T3 cells, in which the percentage of G418-resistant colonies that exhibit the adipocyte morphology is reproducibly > 50% when the C/EBP alpha gene is transduced by retroviruses. The ability to promote the adipogenic program requires the potent transcriptional activation domain of C/EBP alpha and is not observed with C/EBP beta. Paradoxically, in spite of its antimitogenic effects, clonal cell lines that stably express high amounts of C/EBP alpha can readily be generated. Stable expression of C/EBP alpha in BALB/c-3T3 cells dramatically enhances their ability to terminally differentiate into adipocytes. The results demonstrate that C/EBP alpha can efficiently promote the adipogenic program in a variety of mouse fibroblastic cells, including those that have little or no spontaneous capacity to undergo adipogenesis.

Ongoing challenges of a global international patient assistance program.

The Glivec International Patient Assistance Program (GIPAP) that is supported by Novartis Oncology provides Glivec (imatinib), an innovative oral therapy for chronic myelogenous leukemia and gastrointestinal stromal tumours, at no cost to patients in developing countries who could otherwise not afford treatment. Using a novel direct-to-patient approach this program is currently successfully providing Glivec to underserved patients in 81 countries. A number of challenges were faced in the implementation of this program in countries where cancer is not a priority, and where the healthcare and other infrastructure can be very limited. By working within the existing healthcare system of each country, and with any national cancer control programme that may be in place, through careful selection of qualified institutions and physicians, while maintaining a global approach to ensure consistency and quality, GIPAP has become an efficient and sustainable access program. Novartis has made a long-term commitment to GIPAP, and is currently exploring new patient access programs for other drugs.

Collection of peripheral blood stem cells in normal donors.

Although committed progenitor cells have been harvested during the standard plateletpheresis, only few experiences were specifically carried out in normal donors. The major drawback is the very low number of circulating haemopoietic stem cells and, besides a donor manipulation, a good harvesting technology is a fundamental prerequisite. To verify the effectiveness of an automate system, twenty normal volunteer donors were randomly assigned to four protocols of simultaneous PBSC and platelet collection. A Baxter separator CS 3000 was employed and we compared the yields and the efficiency of the modified Program 1 plus isoradial chamber (Protocol A), the modified Program 1 plus granulo chamber (Protocol B), the standard Program 3 plus isoradial chamber (Protocol C) and the standard Program 3 plus granulo chamber (Protocol D). The absolute yields for WBC (x10(9)), MNC (x10(9)), platelets (x10(11)) and CFU-GM (x10(4)) were respectively: 10.4, 10.1, 6.5 and 29.5 for Protocol A; 10.1, 9.9, 5.2 and 14.7 for Protocol B; 10.0, 9.9, 5.2, 14.7 for Protocol C, and 12.7, 12.4, 6.8 and 57.4 for Protocol D. The best CFU-GM efficiency was achieved with Protocol D (33%) vs. Protocol A (27.5%), Protocol B (14.5%) and Protocol C (27%). The above differences, although remarkable, were not statistically significant. Though preliminary, this study demonstrates the feasibility of a combined platelet and PBSC collection in normal donors.
