[Alternative surgical procedure in pseudarthrosis of the scaphoid bone with a small proximal fragment].

The management of non-union of the scaphoid with a Herbert screw assumes a specific size of the proximal pole fragment. In cases of extremely small proximal pole fragments the length of the leading thread of the screw can cause blockage of the proximal pole fragment. In these instances a dorsal approach is used and the Herbert screw is inserted in a proximal-to-distal direction rather than the usual distal-to-proximal insertion. The advantages are a stable internal fixation and an abbreviated period of immobilization in plaster; however, the small number of cases reported does not permit us to draw conclusions regarding indications for use of this technique.

[Pseudarthrosis of the capitate].

A 54 years old railway engine-driver fell on his extended right hand in November 1978. In the next few hours he developed increasing pain. He reported similar slight trauma some fourteen months before, but without pain. The tomographic X-ray examination proved the presence of an isolated non-union of the capitate with some shortening. Four days after the second accident an autologous graft was performed following the technique of Matti. The non-union healed after three months in a plaster cast, and the patient returned to full work after four months. Examining him four years and nine months later, the patient complained about persisting pain when stressed. The mobility of the wrist was moderately reduced, and the healed capitate showed unchanged shortening. Isolated non-unions of the capitate can, therefore, be painless for a long time--a situation well-known with scaphoid non-unions. Considering the operative technique, a cancellous graft alone is not able to restore the function of the wrist because of associated collapse of the carpus. To obtain a better late result, the length of the capitate ought to be restored and a cortico-spongious graft inserted.

[Pseudarthrosis of the lower end of the radius due to interposition of the long extensor of the thumb].

The authors describe one case of a severely displaced Smith's fracture which was originally immobilised by external fixation. Three month's later the fracture had not united and active extension of the thumb was absent. At operation, the extensor pollicis longus tendon was found to be dislocated into the fracture site. The tendon was repaired and the non-union was treated by plating. A review of the literature discovered only one other similar case.

[Amputation below the fracture site in pseudarthrosis complicated by infection or vascular lesion].

In cases of fractures of the shaft of a bone complicated by non-union, infection and vascular and/or neurological lesions, amputation may be the ultimate possible treatment. In some cases, an amputation at the level of the fracture site would produce too short a stump for a normal prosthesis. The authors consider that, in this type of lesion, it may be wise to preserve some of the distal portion of the bone below the site of the fracture, even in cases of infected non-union. They performed this technique in seven cases, the distal fragment of the bone being covered by an osteomyoplasty, infection being treated by antibiotics. Early rehabilitation was started with immediate weight-bearing in the prosthesis. In all cases, union was obtained secondarily and in five cases infection healed as well. Two still had an intermittent sinus. The authors consider that the infected non-unions healed because of suppression of the weight of the limb. The technique allowed better rehabilitation because of the presence of a sufficiently long stump.

[Current methods of treating pseudarthrosis].

Treatment of 706 patients with 708 false joints is described. 756 operations were fulfilled. Consolidation was achieved in 632 patients (89,9%). Principles of the treatment are described.
