Sexual assault history in a community mental health center clinical population.

One hundred and twenty-two adult outpatients, 81 women and 41 men, in a community mental health center responded to a survey of history of sexual assault and abuse. Overall, 48% reported they had been sexually assaulted or abused, 59% of women and 24% of men. Forty-two percent of women and 17% of men reported they had been raped, 27% of women and 7% of men reported attempted rape, and 31% of women and 10% of men reported molestation. Characteristics of assaults and assailants were examined. Implications for training clinicians in the treatment of victims of sexual assault and abuse are discussed.

An indigenous community mental health service on the Tohono O'odham (Papago) Indian Reservation: seventeen years later.

The status of a fully indigenous mental health program serviced and controlled by the Tohono O'odham (Papago) Indian tribe is reviewed from the perspective of its 17-year history. The program functions in large measure in a crisis intervention model, with suicidal or acutely disturbed cases being most frequent. However, a whole range of disorders and ages are seen. Traditional Medicine Men and Women are often used as consultants, as are some professionals. In recent years child sex abuse and abuse of drugs among youth are prominent problems. The program experienced problems of obtaining services off reservations for patients in need, and in establishing credibility of the Indian Mental Health workers with the outside service providers.

Pedophilia: an update on theory and practice.

The number of sexual attacks on children is staggering, yet it is extremely difficult to understand what effective treatments are available for the pedophile by reviewing the literature. Therefore, the authors present an update of the changing conceptualizations regarding the offenders and their victims. An overview of the currently used modalities to treat the pedophile is presented with emphasis on the promising cognitive-behavioral approach. A brief description of this approach is given with two case illustrations.

Assessment and treatment of Spanish-speaking sex offenders: special considerations.

While there is an expanding body of literature pertaining to the special considerations related to the treatment of Hispanic patients whose primary language is Spanish, there is virtually no information reported to aid in the assessment and treatment of Hispanic and primarily Spanish-speaking sex offenders. Beyond the obvious problems posed by differences in language, there are a host of transcultural factors that may impact on the evaluation and engagement of this patient population when they are also largely psychologically unsophisticated and from a low socioeconomic level. The experiences in modifying and implementing a special program to accommodate the needs of this patient group is presented. Reticence to discuss sexuality in a group setting, the relationship between therapists and patients as effected by rules of social etiquette, and the reluctance to criticize other members in the group were prominent features attributed to religious and cultural factors. The authors consider it a necessity to be sensitive to these subtle nuances in order to provide adequate treatment.

DNA investigations on fetal material from paternity cases.

Three paternity cases have been investigated where DNA was extracted from fetuses (age: 8-10 weeks old) after interruption of pregnancy. In each case it was possible to clearly identify the putative father using 5 or 6 single locus probes (SLP's). Fetal bands (SLP) could be clearly identified from mixtures of placental and fetal DNA by comparison with the maternal and paternal bands. However, it was very difficult to resolve the fragment patterns of tissue mixtures with one multi locus probe (MLP), because of band overlap. Another advantage of using SLP's was that biostatistical calculations could be carried out and very informative Essen-Möller values for the probability of paternity were obtained.

Folie á deux in a child.

Folie a deux is the transference of delusional ideas and behaviors from one person to another with whom there has been a close association. Its occurence in children has been rarely described in the literature, which was reviewed for case reports, diagnostic criteria, and etiological theories. A detailed case history of a 10-year-old girl who had delusions of special powers and delusions of persecution as well as hypochondriacal and hysterical symptoms is presented as an example of folie a deux between child and parent. This girl had developed a symbiotic incestuous relationship with her stepfather, a paranoid schizophrenic patient. Six criteria for making the diagnosis of folie a deux in a child were applied to the case studied. General and specific aspects of treating this disorder were discussed.

The Clarke SHQ: a clinical sex history questionnaire for males.

The development of a 225-item sexual history questionnaire is reported. The frequency, desire for, and disgust for a wide range of sexual behaviors, including anomalous behavior, were examined. Twenty-four scales were derived from factor analysis of the items. The scales were represented by modest unitary factors but were relatively free of bias from age, education, intelligence, social desirability, and defensiveness. The scales generally discriminated clinically relevant groups from control subjects and therefore meet a clinical need in the assessment of anomalous sexual behavior. The instrument also appears to be useful in examining hypotheses for research. Among these observations was the continuity between exhibitionism and heterosexual pedophilia and the apparent bisexuality of some homosexual males. The scale allows for the assessment of the preferences of sexually inexperienced males as well.

Grooming and consort partner selection in a troop of Japanese monkeys (Macaca fuscata).

The Arashiyama West troop of Macaca fuscata consists of 150 monkeys transported from Kyoto, Japan, in February 1972 to their present location near Laredo, Texas. At this site the animals range over 108 acres and during the study period were only minimally provisioned and disturbed. In the report of a 3-year study of consort partner selection it was suggested that yearlong social bonds within the troop appeared to be distinct from consort bonds formed during mating season. Like Sade, we consider grooming to be a good measure of yearround affinitive bonding in macaques. This study compares the pairs formed for consorting with those formed for yearlong grooming activities, by the 94 individuals who formed consort relationships during the mating season of 1973--1974. It was found that yearlong grooming involved pairs of monkeys which were significantly different from those for consorting. Grooming was relatively frequent between related monkeys, while consort partners were seldom related. In addition, yearlong grooming patterns were not disrupted during mating season. These results suggest that consort and grooming activities are behavioral expressions of two important social networks, involving mutually exclusive social bonds.

The sex offenses of blacks and whites.

Little is currently known about differences in the sex offenses of blacks and whites ii the 1nited states. Many major studies of sexual behavior and of sex offenders have purposely excluded data on blacks. This paper presents comparative data on 47 black and 47 white sex offenders in terms of the nature of the offense and victim characteristics. It was found that black offenders weremore likely than white offenders to engage invaginal intercourse with their victims, showed a trend to use force in their offenses, and were more likely to select adult female victims. These differences, however, appear to be due primarily to class rather than racial differences. Limitations of these dataare discussed.

Black female transsexuals and schizophrenia: a serendipitous finding?

While increasing numbers of individuals have identified themselves as transsexuals and requested sex reassignment surgery, the number of black female applicants is grossly underrepresented. Indeed, only 3% of the patients applying to the Case Western Reserve University (CWRU) Gender Identity Clinic (5 of 174 patients) were black females; and in one survey of nine gender identity clinics, only 1.1% of the applicants were black females. In this study, all five black female applicants who applied for transsexual evaluation to the CWRU Gender Identity Clinic served as subjects. The patients' characteristics, their psychological test results, clinical interview material, and psychological and psychiatric diagnoses are presented. All patients had severe psychopathology; three were schizophrenic, one was a schizophrenic character, and the last diagnosed as either a psychotic character or borderline personality. Among the hypotheses, it was suggested that black women may be "inoculated" against severe gender identity pathology and only exhibit such pathology as a consequence of a schizophrenic illness or severe borderline schizophrenic state. The data also suggest that more attention should be placed on investigating the family and cultural dynamics related to transsexualism. The implications of these findings for developing a comprehensive theory of transsexualism are presented.

Legal, social, and biological definitions of pedophilia.

Although there is substantial evidence in the historical and anthropological record of the sexual use of children by adults, surprisingly little is known about the etiology of pedophilia or its relation to other forms of sexual aggression. After briefly reviewing the research on pedophilia, we argue that one major difficulty in conducting or interpreting such research lies in the different definitions "pedophilia" has received. Most important, much of the research has accepted a legal definition of pedophilia, treating all offenders convicted of "child molestation" as pedophiles, regardless of the age or appearance of the victim. We argue that a distinction should be made between biological children and sociolegal children. Laws governing child molestation reflect sociolegal childhood, regardless of its discrepancy with biological childhood. "True" pedophiles should be identified by their preference for biological children. By using legal classifications, researchers may well be confusing two distinct types of offenders, child molesters and rapists, and confounding attempts to understand pedophilia.

The relation between sexual activity among children during preadolescence and/or early adolescence and sexual behavior and sexual adjustment in young adulthood.

This study examined whether current sexual behavior and sexual adjustment in a sample of 433 college students differed as a function of having engaged in sexual activity with other children during preadolescence (prior to age 13) and/or early adolescence (ages 13-15). Four groups were compared: subjects who only had an early adolescent sexual experience; subjects who had both preadolescent and early adolescent experiences; subjects who had no sexual experience with another child during either preadolescence or early adolescence. There were no significant differences between these groups on measures of incidence of premarital intercourse, age at first intercourse, number of different intercourse partners, sexual satisfaction, sexual arousal, or sexual dysfunctions. It was concluded that in general the simple occurrence or nonoccurrence of sexual activity among children during these developmental periods has little impact, either positive or negative, on later sexual adjustment during young adulthood. Some expections, depending on the specific types of childhood sexual activity involved, were noted.

Long-term effects of sexual experiences with siblings and nonsiblings during childhood.

Long-term differences in young adults' sexual behavior and adjustment following preadolescent sibling and nonsibling peer childhood sexual experiences were investigated. Questionnaire data were collected from 526 undergraduate college students. Of this sample, 25 (5%) reported having had only a sibling sexual encounter, 61 (12%) reported having had both a sibling and a nonsibling childhood sexual experience, 236 (45%) reported having had only a nonsibling sexual experience, and 204 (39%) reported no sexual experience with another child prior to age 13. No differences were found between the sibling, nonsibling, and no-experience groups on a variety of adult sexual behavior and sexual adjustment measures, including incidence of premarital intercourse, age at first intercourse, number of intercourse partners, sexual satisfaction, sexual arousal, and sexual dysfunctions. It was concluded that adult sexual adjustment is not negatively or positively influenced by typical early childhood sexual experiences among similar-aged siblings.

Sexual molestation of men by women.

The belief that it is impossible for males to respond sexually when subjected to sexual molestation by women is contradicted. Previous research indicating that male sex response can occur in a variety of emotional states, including anger and terror, are corroborated. Eleven cases of male sexual molestation by females are classified and described. A post-trauma reaction occurs in which sexual function and psychological state are affected. The men were all personally interviewed. Recognition of this phenomenon should lead to increased identification of male victims as well as to better medical, psychological, and legal services for them.

Buspirone hydrochloride in the treatment of an atypical paraphilia.

A case report involving buspirone hydrochloride in the successful treatment of a patient with an atypical paraphilia and transvestic fetishism is presented. Treatment outcome was assessed by the patient's self-report as well as by retrospective examination of detailed notes about paraphilia fantasies which unknown to the therapists, had been kept by the patient. Preliminary evidence indicates that buspirone appears to effectively treat some paraphilias.
