[Empty nose syndrome].

OBJECTIVE: The aim of this article is to present the concept of empty nose syndrome(ENS) and help ENT doctors take care of regular nasal turbinate surgery. METHODS: Fourteen patients who was diagnosed as ENS in our department were reviewed retrospectively. All patients had undergone various forms of nasal surgery (all had had turbinectomy). Their age ranged from 13 to 52 years. All patients were treated conservatively, among these patients 5 subjects who had more serious symptoms received submucous and subperiostal nasal implantation with ilium. RESULTS: All patients had nasal obstruction and dryness of nasal cavity, nasopharynx and oto-pharynx in 6 months--5 years after their first nasal surgery, some presented symptoms of depression. Nasoscope examination showed all patients had a cylindrically enlarged nasal cavity. Conservative treatment was effective in most cases. The effectiveness of operative treatment was encouraging during short-term follow-up. CONCLUSION: Extensive turbinectomy may cause secondary nasal mucosal atrophy and a series of subsequent symptoms. The presentation of this concept is to remind the ENT doctors prudently performing turbinectomy to avoid the occurrence of irreversible injury to the nasal cavity.

Stability of cholesterol gall stones after 165 years of burial.

A woman who died in 1837 was exhumed for the purposes of moving the grave to another location. During the excavation, small white deposits of stone were uncovered in the right abdominal region, inferior to the rib cage and superior to the ilium blade. These stones were analyzed for cholesterol, bilirubin, and calcium following solubilization using methyl tert-butyl ether as a solvent. The results of these clinical chemistry analyses showed that these stones consisted primarily of cholesterol. Under these particular soil conditions encountered in this case, cholesterol gall stones are stable for at least 165 years.

Spinal instrumentation in the treatment of severe progressive spondylolisthesis.

By using A-frame instrumentation and direct distraction force, an exceptional correction can be obtained in patients with severe spondylolisthesis. Adolescent progressive spondylolisthesis should be considered as an entity separate from the lumbosacral anomalies such as spondylolysis and grades 0 and 1 spondylolisthesis without progression. An aplastic relationship appears to exist between the sacrum and the posterior superior wing of the ilium in the adolescent with progressive spondylolisthesis. Complications have been minimal but long-term observations of patients are necessary to evaluate the contribution of instrumentation in the treatment for severe progressive spondylolisthesis.

[Anesthetic management of therapeutic angiogenesis with autologous bone marrow cells for medically intractable angina pectoris].

We report anesthetic management of therapeutic angiogenesis with autologous marrow cells for medically intractable angina pectoris. A 64-year-old man suffering from severe angina after multiple bypass surgery was presented for this novel procedure. Under general anesthesia with propofol, about 650 ml of bone marrow was aspirated from his ilium in prone position. Hypotension was treated with volume infusion and the use of vasoactive drugs. Next, left thoracotomy was performed with the patient in right lateral position. With the aid of one lung ventilation, the apex and the posterior wall of the heart, where ischemia had been localized, was exposed. An epicardial stabilizer was used to immobilize the apex where purified marrow cells were manually injected. Ventricular premature beats occurred when marrow cells were injected, but they were self-limited. The procedure was completed without any serious complications. Prevention of perioperative myocardial ischemia, volume supplementation during bone marrow aspiration and preparation for ventricular arrhythmia during injection were suggested to be the key issues in the management of this procedure.

Pelvic osteomyelitis in children: a comparison of decades from 1980-1989 with 1990-2001.

Since 1980, 64 children with pelvic osteomyelitis (40 boys) were treated at a major pediatric referral center. The average age was 11 years and 6 months. The presenting complaints included pain in 61 children, fever in 30, and altered weight-bearing in 31. The erythrocyte sedimentation rate was elevated in 56 and there was leukocytosis in 19. The most commonly affected sites were the ilium in 21 and the acetabulum in 20, followed by the pubis in 11 and the ischium in 10. Culture results were negative in 32 and positive for Staphylococcus aureus in 26. Treatment consisted of intravenous antibiotics in 62 and irrigation and debridement in five children, all of whom presented since 1990. The infection resolved in 62 children and persisted in two. Three developed complications, including fusion of the sacroiliac joint in one, deformity of the acetabulum in one, and one child with an underlying protein C deficiency developed thromboembolism. Recent patients have demonstrated increased severity, likely due to increased virulence of the organism.

[Fat at sacroiliac joints: MR imaging findings (comparison and analysis of 18 volunteers and 52 patients with ankylosing spondylitis)].

OBJECTIVE: In aid of MR imaging diagnosis of ankylosing spondylitis (AS), MRI findings of sacroiliac (SI) joints in volunteers were compared with those in patients with AS, with special reference to the fat findings. METHODS: 18 normal volunteers and 52 AS patients underwent the MRI examinations with the same protocols, i.e., SE T1WI, FSE T2WI, GE T2*WI sequences were taken using a 0.3T imager. All the images were observed and analyzed by two radiologists. RESULTS: Symmetric or patchy distribution fat component either in sacrum and/or ilium was detected in 52.8% of SI joints of volunteers. Thin zone distribution of fat was seen in 35.6% of SI joints of AS patients, which was adjacent to the erosion and sclerosis. In another 25.0% of SI joints of AS patients, the distribution of fat component was found similar to that of the normal volunteers. Cartilage changes were detected in 85.6% of the SI joints of AS patients. In addition, bone erosion, sclerosis and subchondral bone marrow edema were also detected. CONCLUSION: Subchondral fat infiltration associated with erosion and sclerosis should be considered as sacroiliitis. The knowledge of the characteristics of fat component could be helpful for evaluating sacroiliac joints.

[The inquinal-flap-anatomical studies of the axial vessels and their course].

In plastic surgery of the hand the groin-flap is preferred to other axial pedicled flaps for practical and cosmetic reasons. This groin-flap described by McGREGOR and JACKSON 1972 is based on the superficial circumflex iliac artery (Arteria circumflexa ilium superficialis = "A. cis."). Details concerning the origin and course of the A. cis. may best be described with reference to the two osseous prominences that can be palpated through the skin of the inguinal region; the anterior superior iliac spine (S) and the pubic tubercle (T) (Fig. 1).

Effect of nano-hydroxyapatite/collagen composite and bone morphogenetic protein-2 on lumbar intertransverse fusion in rabbits.

OBJECTIVE: To investigate the effect of nano-hydroxyapatite/collagen (nHA/collagen) composite as a graft extender and enhancer when combined with recombinant human bone morphogenetic protein-2 (rhBMP-2) on lumbar intertransverse fusion in rabbits. METHODS: Sixty-four adult female New Zealand white rabbits, aged 1 year and weighing 3.5-4.5 kg, underwent similar posterolateral intertransverse process arthrodesis and were randomly divided into 4 groups based on different grafts: autogenous cancellous bone alone (ACB group), nHA/collagen alone (HAC group), half autogenous cancellous bone and half nHA/collagen (ACB+HAC group) and nHA/collagen combined with rhBMP-2 (HAC+BMP group). The fusion masses were analyzed by manual palpation, radiography, biomechanical testing and histological examination. RESULTS: Fusion was observed in 4 cases in the 6th week and in 5 cases in the 10th week after surgery in ACB group. No case showed fusion in HAC group. In ACB+HAC group, there was fusion in 3 cases in the 6th week and in 4 cases in the 10th week after surgery. In HAC+BMP group, fusion in 1 case was found in the 4th week, in 5 cases in the 6th week and in 6 cases in the 10th week after surgery. It suggested that ACB, ACB+HAC and HAC+BMP groups showed similar fusion ratio and mechanical strength in the 6th and 10th week after surgery. According to the microstructure analysis of the samples, nHA/collagen had no negative effect when implanted together with ilium autograft. In HAC+BMP group, new bone-like tissue was observed in the 2nd week postoperatively, and nearly all of the implanted composites were replaced by mature bone matrix and new bones in 10th week postoperatively. CONCLUSIONS: The nHA/collagen, especially combined with rhBMP-2, is a promising bone substitute, for it has quick biodegradation, fine bone-bending ability, and high osteoconductivity on posterolateral spinal fusion in rabbits.

Selective arterial embolization in the treatment of an aneurysmal bone cyst of the pelvis.

This case report deals with an aggressive aneurysmal bone cyst of the pelvis in a 20-year-old man causing considerable destruction of the left ilium and the anterior column of the acetabulum, with extension into the pelvis. The treatment of large pelvic aneurysmal bone cysts is challenging because of local destruction of adjacent structures and because of the risk of severe intraoperative bleeding. Therefore selective preoperative embolization may be a valuable tool in the management of aneurysmal bone cysts.

[Diagnosis, classification and indications for surgical treatment of pelvic ring fractures].

In cases of high-energy trauma, it is well known that there is a high incidence of pelvic fractures. The mechanism of injury, inspection and physical examination of the victim at the accident site direct attention to a pelvic fracture. In most cases, the first radiological examination (A.P. X-ray of the pelvis, oblique view of the obturator and oblique view of the ilium) shows the extent of the bony lesion. The diagnosis and therapy of lesions of the urinary tract, of intra-abdominal organs and blood vessels are vitally important. For the definitive operation of unstable pelvic ring fractures, additional diagnostic means, i.e., CT scans to distinguish posterior instability, can be necessary. Osteosynthesis can only be successful in the pelvis if one has a biochmechanical understanding of the physiological flux of force from the neck of the femur via the acetabular fossa to the sacroiliac joint. The sacroiliac ligaments have a particularly important support function. For assessing stability and classifying the traumatic patterns, it is helpful to use Pennal's classification, which takes the direction of the action of force into account. Three basic forms can be distinguished: anteroposterior compression, lateral compression and vertical avulsion. Depending on the extent of the traumatic pattern, one can distinguish three subtypes: type 1 is treated conservatively while types 2 and 3 require surgical treatment. The biochmechanics, traumatic patterns, diagnostics and treatment techniques applied are described clearly and with good illustrations.

[Value of integrated positron-emission tomography and computed tomography in gross tumor volume delineation for radiotherapy for bone metastasis].

OBJECTIVE: To evaluate the role of integrated positron-emission tomography and computed tomography (PET/CT) in gross tumor volume (GTV) delineation for three-dimensional conformal radiotherapy for bone metastasis. METHODS: Three patients with multiple bone metastases were enrolled in this study, whose primary tumors were pathologically identified as lung cancer, liver cancer and renal cell carcinoma. In each case, 6 sites of bone metastases were selected, labeled as BM1 to BM6 which located in the lumbar, thoracic vertebra, rib, ilium, scapula and thighbone respectively. The GTVs of the 6 bone metastases were delineated on the CT scans for treatment planning by 8 radiation oncologists, and the results were compared with the delineation obtained on the basis of integrated CT and PET/CT for analyzing the interobserver variability. RESULTS: For BM1-5 with obvious bone cortex lesions by the carcinoma, the GTV delineated on the basis of CT was greater than that derived from PET/CT (P<0.05), whereas for BM6 with intact bone cortex, the contrary result was observed (P<0.05). For any of these bone metastasis, the interobserver variability in GTV delineation with PET/CT was less than that with CT. CONCLUSION: PET/CT is useful for improving the accuracy of GTV delineation for the protection of the normal tissues during radiotherapy.

[Treatment of tibial plateau fracture with reconstructed bone xenograft plus buttress plate internal fixation].

OBJECTIVE: To investigate the clinical effect of reconstructed bone xenograft plus buttress plate (T or L type) fixation in treating tibial plateau fracture. METHODS: From June 2001 to March 2003, 32 cases of tibial plateau fractures were treated by means of open reduction, reconstructed bone xenograft plus buttress plate (T or L type) fixation. There were 23 cases of bumper fracture, 5 cases of falling injury and 4 cases of crush injury by a weight; 20 males and 12 females, aged from 18 to 69 years with an average of 38 years. All patients had close fracture. RESULTS: All the patients were followed up for 9 months to 23 months, tibial plateau fracture healed satisfactorily without sunken articular surface. According to Pasmussen criterion, the results were excellent in 16 knees, good in 12 knees and moderate in 3 knees. The satisfactory rate was 87.5%. CONCLUSION: Reconstructed bone xenograft plus buttress plate internal fixation has good effect in treating tibial plateau fractures because it can avoid the complication of transplantation of ilium.

[A case of a patient with solitary prostate metastasis of the colorectal carcinoma three years after abdominoperineal amputation of the rectum].

AIM: The aim of this study is to present a case-review of a patient with an extremely rare finding of a solitary metastasis of the colorectal carcinoma in his prostate. METHODOLOGY: A Case-Review. RESULTS: The study describes a case of a 42-year-old patient who underwent abdominoperineal amputation for the rectal carcinoma pT3 N1 M0 with complementary actinotherapy and chemotherapy. Three years after his rectum amputation, a CT scan disclosed a hypodense focus in his prostate of unknown origin. Elevation of the CEA was found upon laboratory examination, other tumor markers including the PSA were not abnormal. The positron emission tomography (PET) did not disclose other distant foci. The patient was administered 4 courses of chemotherapy with, basically, no signs of regression in the prostate foci. Biopsy from the suspicious focus was conducted transperitoneally, followed by its histological examination which confirmed cribriform adenocarcinoma. Our team decided on salvage cystoprostatectomy, securing urine derivation by the ilium conduit. 6 months later, the CT examination showed a massive local relapse and two metastatic foci in the liver, further oncological therapy was not indicated. The patient died 7 moths after the radical cystoprostatectomy. CONCLUSION: This case-review points out a possibility of metastatic prostate affections connected to the colorectal carcinoma disorder, as well as its treatment procedures.

[Metastatic intrapelvic tumor from esophageal cancer: a case report].

We report a case of a metastatic intrapelvic tumor arising from esophageal cancer. The patient, a 74-year-old man, visited our hospital with the chief complaint of gross hematuria. Magnetic resonance imaging and cystoscopy revealed a huge intrapelvic tumor which invaded the bladder, rectum, sigmoid colon and left ilium. The patient underwent total pelvic evisceration with ileal conduit and colostomy. Pathologic diagnosis of the intrapelvic tumor was moderately differentiated squamous cell carcinoma. Preoperatively, gastrointestinal fiberscopy revealed an esophageal tumor, and biopsy showed moderately differentiated squamous cell carcinoma. Finally, the intrapelvic tumor was diagnosed as metastatic tumor from esophageal cancer.

[The application of reconstruction with autograft implantation in total hip replacement with regional acetabular deficiency].

OBJECTIVE: To evaluate the effect of reconstruction with autograft implantation in total hip arthroplasty(THA) with regional acetabular deficiency. METHODS: From 1991 to 2000, 39 cases of THA with acetabular deficiency were conducted. Autogenous bone implantation was used to reconstruct the deficient acetabulum. Of the 39 patients, 25 were males and 14 were females. The age ranged from 34 to 62 (45.2 on average). There were 21 cases of developmental dysplasia resulted deficiency, 14 cases of fracture of femoral neck complicated with head necrosis (10 hips) and fracture of acetabulum(4 hips). The resected femoral heads or autologous ilium were made the wedge-shaped graft and implanted into the deficient acetabulum, which included 12 cases with cement THA and 27 with cementless THA. Of all the cases, 24 were followed up 2 to 10 years (6.7 years on average). Harris scores before operation were 18 to 50 (38.1 on average). RESULTS: The limbs were lengthened by 2.4 cm on average. No serious complications were observed in these patients. Compared with the scores before the operation, the average Harris scores after the operation were 92.1 (P < 0.01)and 86.3 (P < 0.05) in the one-year and the latest follow-up respectively. The rates for the good were 91.7% and 83.3% in the one-year and the latest follow-up respectively. CONCLUSION: The acetabular reconstruction with autograft in THA will bring better stability in those patients with acetabular deficiency.

[The sacroiliac joint dysfunction: clinical manifestations, diagnostics and manual therapy].

Sacroiliac joint dysfunction is one of the proved causes of sacroiliac joint syndrome. We are talking about the restricted mobility of sacrum opposite to ilium the type of "reversible blockage of movement". Main characteristics of dysfunction are as follows: restricted "joint play", referred pain, normal radiological finding, normal lab results and disappearance of clinical symptoms after deblocking of articular bodies. Pain from a blocked joint can be referred to lower back, buttocks, hip, groin, thigh, calf and lower part of abdomen. Dispersion of painful regions is a consequence of a complex and variable innervation of articular capsule. Blocked position of articular bodies and protracted tension of articular capsule causes a stimulus of nociceptors by which a capsule is protected. Nociceptive activity is manifested with referred pains in innervational region of stimulated sensitive nerves. In the article, besides the clinical manifestations, there is described a diagnostics and manual therapy of dysfunction. Springing tests by means of which a passive mobility ("joint play") is being tested, are most valuable in dysfunction diagnostics. Manual therapy (mobilization/manipulation) is indicated and efficacious with the patients suffering from dysfunction.

[Clinical research of correlation between osteonecrosis and steroid].

OBJECTIVE: To explore the correlation between the dosage of corticosteroid, time of onset and incidence of osteonecrosis (ON) in patients with SARS. METHODS: From July 2003 to January 2004, general survey carried out for ON in 551 patients with SARS. Five hundred and fifty-one patients except 12 were administrated by corticosteroid from 80 mg to 30 000 mg. The age of patients was (33 +/- 9) years old ranging from 19 to 59 years old. One hundred and thirty-one were male, and four hundred and twenty were female. MRI and X-ray film were taken in all patients including both hips, knees, shoulders, ankles and wrists. CT scan was taken in partial patients. Common classification system were used for staging of hip (ARCO), knee (Lotka) and shoulder (Cruess). Independent test, rank-sum test and multiple factor logistic regression analysis were used for statistical analysis. RESULTS: No osteonecrosis was detected in 12 patients without corticosteroid. Osteonecrosis was detected in 176 patients (32.7 percent) among 539 patients. There were ON of femoral head in 130 cases (210 hips), ON of knee in 98 cases (130 knees), ON of humeral head in 21 cases (36 shoulders), ON of talus and calcaneus in 16 cases (26 ankles), ON of scaphoid and lunate in 11 cases (17 wrists), ON of patella in 3 cases (4 patella), ON of ilium in 1 case and bone infarction (femur, tibia) in 18 cases. One hundred and nineteen cases (195 hips) with ONFH were in stage I (IA 45 hips, IB 77 hips, IC 73 hips). Eleven cases (15 hips) were in stage II. All osteonecrosis of the knee and humoral head was stage I. Thirty-four patients with ON had one joint affected, 45 patients had 2 joints, 93 patients had more than 3 joints. The dosage of corticosteroid was (5842 +/- 4988) mg in ON group and (2719 +/- 2571) mg in non-ON group (P < 0.0001). The duration of steroid was (38 +/- 17) d in ON group and (27 +/- 15) d in non-ON group (P < 0.01). The dosage of pulse treatment was (340 +/- 207) mg/d in ON group and (211 +/- 160) mg/d in non-ON group (P < 0.01). The duration of pulse treatment was (28 +/- 13) d in ON group and (18 +/- 11) d in non-ON group (P < 0.01). All patients with ON were detected within 6 months from administration. CONCLUSION: About one-third patients with SARS who were treated with a high dose of corticosteroid occurred osteonecrosis. ON is frequently multiple focuses. The actual time of onset of ON is early of steroid used. MRI is golden standard for early diagnosis of ON. The patients who were treated with a high dose of corticosteroid should be inspected initially by MRI.
