[Development of a rehydration therapy in diarrheic disease. 1980].

Intravenous rehydration is required only in patients with severe diarrhea due to V. cholerae who are in shock, with absent peripheral pulse and blood pressure; when the shock has been corrected, rehydration can be completed using an oral rehydration solution. The intravenous solution to be used is 5:4: 1 (5g of sodium chloride, 4g of sodium bicarbonate and 1g of potassium chloride per liter) or a comparable commercial alkaline solution. For oral rehydration a solution is used containing 3.5 g sodium chloride, 2.5g sodium bicarbonate, 1.5g potassium chloride and 20g of glucose (or 40g of sucrose) per liter. These fluids are administered in a volume replacing the amount lost before treatment was initiated and the fluids lost in the continuing diarrhea. With this management, a case fatality rate of 50% in the untreated falls to less than 1%. The addition of antibiotics such as tetracycline and furazolidone reduces the duration of diarrhea and the need for continuing fluid balance observation. Intravenous rehydration of severe diarrhea cases with normal saline solution or with 5% glucose solution increases the acidosis with resulting veno-constriction, which favors the pooling of blood in the heart and the pulmonary circulation leading to cardiac overload and then failure and circulatory peripheral collapse. When acidosis is corrected by the sodium bicarbonate solution and with adequate fluid replacement, normal hemodynamics are reestablished and the patient immediately recovers from the collapse. In cases of mild or moderate diarrhea, replacement entirely by oral rehydration of the estimated volume of lost fluid alone is usually sufficient. This management of diarrhea is applicable for diarrhea from any cause, including enterotoxigenic Escherichia coli, Rotavirus, Salmonella and Shigella as well as Vibrio cholerae.

[Pneumocystis carinii infection in a pediatric tuberculosis hospital].

An outbreak of pneumocytosis in a children's tuberculosis hospital was analyzed. The infection was characterized by few signs and favourable progress. Antibiotic therapy failed. To eliminate the outbreak of pneumocystosis in the hospital it was necessary to detect all the children with pneumocystosis and carriers of pneumocysts among the patients and medical staff, to use furazolidone for etiotropic treatment of the patients with pneumocystosis and to perform one-stage sanation of the carriers with antiparasitic agents.

[Enzyme-linked immunosorbent assay for the serodiagnosis of Campylobacter pylori infection].

Antibody activities to campylobacter pylori in serum were estimated by an enzyme-linked immunosorbent assay (ELISA) to crude antigens, prepared by sonication of whole organisms obtained from bacterial culture in 100 patients with chronic gastritis. Significantly raised serum IgG antibody activities to C. pylori was found in colonised patients with gastritis, especially in patients with active gastritis. High activities were also found beyond the age of 30. In 6 patients cleared of C. pylori with furazolidone and/or colloidal bismuth subcitrate (De-NoL), serial testing has shown a fall in activity to normal level by 1/2 year in 4 patients. The specificity and sensitivity of the sero-diagnostic assay was 85.3% and 97% respectively. The positive and negative predictive values were 92.8% and 93.5% respectively. The results indicate that such a serodiagnostic assay could be used to screen patients with C. pylori colonisation in epidemiological surveys.

[Drug residues in food fish].

The present paper reviews different possibilities of pharmacolegal residues in fish. Sometimes the treatment of fish with registered drugs in therapeutical dosage is provable in spite of keeping the withdrawal time. To that experiences of the persistence and values of Chloramphenicol, Chlortetracycline and Furazolidone proved after therapeutical treatment in fish correlated to the water temperature are compared with results of experiments documented in literature. Investigations on the residues of Enrofloxacin after application of different doses to rainbow trout were carried out. Sometimes the proof of drug residues in fish can be done without an advanced treatment. The recontamination of fish with drugs or their metabolites in environment is discussed as a possibility for drug residues. The carry-over-effect of drug during the animal food production can be also the reason for residues in fish. Experiments with Chloramphenicol contaminated feed in rainbow trout show residues in muscle of fish higher than 10 ppb during the feeding period. Finally the results of residues different drug used in fish--given in literature--are collected in two tables.

Cholera outbreak in Delhi--1988.

Delhi experienced an outbreak of cholera during July-August 1988 which affected residents from all walks of life. A total of 1824 laboratory confirmed cholera cases were detected in two months period at I.D. Hospital, Delhi alone. The number of cholera cases in July-August 1988 was 5-10 times that of the same period during the previous years in the Capital. The outbreak was caused by Vibrio cholerae Ogawa biotype ElTor. Majority of the laboratory confirmed cases (about 74 per cent) were seen in children under the age of 15 years. Though the cases were spread all over Delhi, almost three-fourths of total cases were reported from two specific zones (Shahdara and Civil Lines). Most of the isolates were sensitive to all antibiotics tested. The proportion of isolates resistant to furazolidone during this outbreak was substantially higher than in previous years suggesting that the outbreak may have been caused by the introduction of a new strain rather than proliferation of endemic strain. The salient features of the outbreak are discussed.

[Secondary effects caused by feed medication in swine].

Giving medical feed to weanling pigs it is possible to produce a vitamin K-deficiency as a side effect, which is caused by destruction of the intestinal flora. In this investigation the effect of different drugs in a vitamin K-deficient diet on blood coagulation of weanling pigs was examined. In a first trial clinical symptoms of a vitamin K-dependent coagulation disorder could be seen in five from six animals after feeding a combination of sulfadimidine, tylosin, furazolidone and arsanilic acid. Animals showed haemorrhages, when they were housed on flatdecks as well as on concrete floor. This indicated that coprophagy plays no role in pigs for supply of vitamin K. Feeding these drugs in other combinations or one of the drugs alone caused no clinical signs in a second trial. Significant differences to untreated control pigs were found only in one group (sulfadimidine/arsanilic acid) for activities of coagulation factors. Other groups demonstrated only prolongation of clotting times in single animals. Pigs, which received sulfadimidine within their ration, were more often affected than other animals. In a third trial suckling pigs were treated over a period of three weeks with a drug combination used in the first trial to allow an adaptation of the intestinal flora. Same treatment after weaning was unable to produce any clinical symptoms, but led to distinct coagulation disorders in pigs treated before as well as in untreated animals. It is supposed that the development of a vitamin K-deficiency depends on faecal bacterial count and on the specific resistance of intestinal flora in individuals.(ABSTRACT TRUNCATED AT 250 WORDS)

[Metaphylaxis and therapy of the MMA syndrome of sows with Baytril].

The efficiency of antibacterial metaphylaxis and therapy on the control of the MMA-syndrome has been investigated in a large breeding-herd with a total of 828 sows in the study. For this the recently developed quinolone-derivate Baytril was used. Feed medication with furazolidone and sulfadimidine reduced the incidence of disease to 29.7% (control 41.3%), injection of Baytril on the day of parturition reduced it to 10.8% (using a dosage of 1.25 mg/kg body weight) respectively 6.8% (dosage 5 mg/kg). Treatment also resulted in a lower incidence of elevated milk-pH (greater than 6.7). The development of piglets was good in all groups of metaphylaxis. Litters of sows which had been treated with Baytril as prophylaxis or therapy had no low extremes in the weight gains of the piglets. In the treatment of MMA-affected animals Baytril was significantly superior as compared to Trimethoprim-Sulfonamide, if a dosage of 5 mg/kg body weight was used. Using a dosage of 1.25 mg/kg body weight both drugs had the same effect. Even at the end of the five month of the study there was no isolation of bacteria resistant against Baytril.

[Antibiotic resistance of Campylobacter and its epidemiologic significance].

Sensitivity of 179 strains of thermophilic Campylobacter to 21 chemotherapeutic drugs was studied. Activity of the antibacterial agents against the pathogens was estimated by the MICs. The MIC50 and MIC90 were also determined. All the Campylobacter strains were sensitive to gentamicin, chloramphenicol, neomycin, furazolidone and furagin and resistant to cefazolin, polymyxin E, rifampicin, vancomycin and bacitracin. Differences in the attitude of the Campylobacter isolates from various sources and patients of various age groups to the chemotherapeutic drugs were detected. Possible consideration of the results of comparison of R spectra of Campylobacter strains and the levels of their resistance to antimicrobial drugs as epidemiological markers is discussed.

[Diarrhea associated with Trichomonas hominis in a newborn infant].

One female newborn infant with protracted diarrhea in whom associated Trichomonas homonis trophozoites were identified, is presented. Diarrhea in the infant persisted more than 30 days while in the hospital. Another causes of diarrhea, such as metabolic disorders, and bacterial, parasitic or viral infections were not identified. When furazolidone treatment was administered, the therapeutic response was very good, diarrhea disappeared and normal weight increase rate was recovered. It is pointed out that T. hominis might be a potential intestinal pathogen on human beings, specially on immunocompromised hosts, as newborn and malnourished infants are.

[Studies on antipeptic ulcer agents: the synthesis and structure-activity relationship analysis of heterocycle aldehyde (thio) semicarbazones and acyl hydrazones].

Twenty-eight condensation products of heterocycle-alpha-carboaldehydes with N-aminooxazolidones, semicarbazides, thiosemicarbazides, aminoguanidines, aromatic hydrazides and benzoxycarbonyl hydrazide were synthesized so as to deduce the antiulcer pharmacophore or fragment of furazolidone(I), the prototype, which shows therapeutic efficacy for patients with gastric ulcer. Analysis of the SAR of the compounds indicate that the substitution of furan, thiophene, pyrrole and N-methyl pyrrole rings for the 5-nitrofuran and the cleavage of the oxazolidone ring obtain the activity to some extent. The necessary electronic density of carbonyl group of compounds is of importance. A lead structure, therefore, is derived for further optimization.

Liquid chromatographic-electrochemical detection screening procedure for six nitro-containing drugs in chicken tissues at low ppb level.

A screening procedure is described for the detection of furazolidone, nitrofurazone, aklomide, zoalene, nitromide, and sulfanitran residues in a single extract of chicken liver, breast, or thigh muscle at the low ppb level. The method includes extraction of tissue with chloroformethyl acetate-dimethyl sulfoxide (50 + 50 + 0.8), adsorption on neutral alumina, and subsequent elution of the residues with pH 6.0 phosphate buffer-methanol (1 + 1). Eluants are separated on a 25 cm, 5 microns C18 column with pH 6.0 phosphate buffer-methanol (57.5 + 42.5) as mobile phase. The drugs are detected with an electrochemical detector in the reductive mode at -0.8 V. Mean recoveries from all tissues ranged from 76.5% for nitrofurazone to 97.1% for zoalene.

Antimicrobial susceptibility of Salmonella isolates from the broiler chicken industry in Ontario.

Antibacterial drug resistance among 219 salmonella isolates recovered during 1974 from poultry and poultry environments at the various production stages of broiler chickens in three integrated Ontario companies are recorded. All isolates were susceptible to ampicillin, trimethroprim-sulfamethoxazole complex, furazolidone, cephaloridine and amoxicillin. A relative increase in resistance to tetracycline and streptomycin with an accompanying decrease in resistance to triple sulfa compound was recorded when compared to a previous investigation of avian salmonella isolates in Ontario. The percentage and patterns of antimicrobial resistance were comparable at the various stages of production. Resistance to tetracycline and streptomycin was the most common pattern found among both Salmonella typhimurium and other serotypes. A notably high prevalence of resistance was found among Salmonella enteritidis isolates including some isolates with R factors for chloramphenicol resistance. This latter finding is of particular concern because of the high prevalence of this serotype in poultry and in human salmonellosis.

[Sensitivity of Proteus hauseri bacteria to chemotherapeutic preparations depending on the cultivation conditions and on the composition of the nutrient medium].

Sensitivity of 227 Proteus strains isolated from patients was studied comparatively using the agar-diffusion method (disks) and the method of serial dilutions. Marked differences in the numbers of the strains resistant to benzylpenicillin and chloramphenicol were found with the above methods. It was shown that the ingredients of Ploskirev's medium significantly (by 2.8--13.5 times) inhibited the antibacterial activity of streptomycin, neomycin, monomycin, kanamycin, ampicillin and nalidixic acid and had practically no effect on the activity of benzylpenicillin, chloramphenicol and furazolidone. The values of the MIC of the drugs used in the experiment with liquid media correlated with those obtained with Sabouro's medium, which provided recommendation of the latter for determination of Proteus sensitivity by the method of serial dilutions in the solid medium, Cultivation of Proteus at a temperature of 40 degrees C resulted in a decrease of the resistance to most of the drugs tested by (by 3--12.4 times).

[Dynamics of drug resistance in Proteus mirabilis cultures 1970-1985].

Resistance of 669 clinical strains of Proteus mirabilis to 18 chemotherapeutic drugs was studied in dynamics within 1970-1985. An increase in the number of cultures resistant to ampicillin and carbenicillin was noted while the number of cultures resistant to cephalosporines did not change. Within the period from 1970 to 1975 there was observed a marked increase in the number of Proteus strains resistant to aminoglycoside antibiotics. After that period their number gradually lowered and in 1985 reached the level of 1970. Beginning from 1973 there were observed a decrease in the number of Proteus chloramphenicol resistant strains and simultaneous occurrence of cultures sensitive to this antibiotic. The predominating number of the tested strains preserved during the whole observation period their resistance to tetracycline, doxycycline, rifampicin, novobiocin, furazolidone and furagin. No increase in the number of Proteus strains with multiple drug resistance including those resistant to 5-7 drugs was noted in the observation periods of 1970-1975, 1980 and 1985. The most frequent were Proteus strains resistant to 2-4 drugs. Among them cultures resistant to chloramphenicol and aminoglycoside antibiotics of the first generation predominated. Grouping of the strains by the same resistance spectra provided dividing the rested cultures of Proteus mirabilis into 69 variants.

[Dynamics of the antibiotic resistance of E. coli strains isolated from cows on 2 dairy farms].

Studied was the dynamic of antibiotic resistance of Escherichia coli strains as isolated from fecal samples taken from 5-day-old calves that had not been offered antibacterial means. The resistance manifested was compared to that of E. coli organisms isolated from calves that had been given antibacterial agents in the course of 36 months as well as to the resistance of E. coli strains isolated from calves at 1, 3, and 5 months of age and from dam cows in the last 12 months of the experiment. A varying period was established for receding of the signs of resistance of the strains to antibiotics--6 months for E. coli to kanamycin and furazolidone, 9 months to tetracycline, chloramphenicol, and gentamycin, and 15 months to streptomycin. Strains isolated from newborn calves on farms with no records of infection were shown to be susceptible to all antibiotics, while those isolated from calves on farms where enteritis was recorded remained polyresistant up to 36 months. No correlation was found between the antibiotic resistance of fecal E. coli strains from the dams and their calves.

[Sensitivity of clinical strains of facultatively anaerobic bacteria to antimicrobial drugs].

Six hundred and sixty five samples of clinical materials from patients with various pyoinflammatory diseases were tested for obligatory anaerobes. Anaerobes were detected in 148 samples which amounted to 22.3 per cent of the total number of the samples and to 33.2 per cent of the samples with microbial growth. A total of 171 strains of obligatory anaerobes were isolated. Among them 58.5, 24.5, 16.4 and 0.6 per cent were nonsporulating gramnegative bacilli, grampositive cocci, grampositive bacilli and gramnegative cocci respectively. Sensitivity of the isolated anaerobes was tested with the disk diffusion method. The most active drugs against the tested strains were: nitroxoline, rifampicin, metronidasole, erythromycin, carbenicillin and cefotaxim (4.2, 4.5, 9.3, 10.6, 11.5 and 11.7 per cent of the resistant strains respectively). Gentamicin, polymyxin M, novobiocin and cefazoline were the least active drugs (94.6, 78.9, 65.4 and 50.0 per cent of the resistant strains respectively). Metronidasole, levomycetin, nitroxolin, rifampicin and furazolidone showed the highest activity against bacteroids of the fragilis group (0, 0, 0, 8 and 12.5 per cent of the resistant strains respectively) while gentamicin, polymyxin M, cefazolin, oxacillin, novobiocin and penicillin showed the lowest activity (100, 100, 100, 100, 87.0 and 66.7 per cent of the resistant strains respectively).
