Measurement of single nephron filtration rate in the kidney of the river lamprey, Lampetra fluviatilis L.

1. The reliability of [3H]inulin as a reference substance for the measurement of glomerular filtration rate (GFR) in river lampreys was investigated. 2. Simultaneously measured renal clearances of [3H]inulin and [14C]-polyethylene glycol (PEG) were not significantly different. 3. Recoveries of [3H]inulin in the urine following its injection into the proximal tubule and urinary duct averaged 83.5+/-4.0% (n = 14) and 93.0+/-4.6% (n = 9) respectively. 4. No evidence was obtained to suggest penetration of the tubular wall by by [3H]inulin following its introduction into peritubular capillaries. 5. Gel-filtration of lamprey plasma and urine produced no signs of any significant degradation of the inulin to fructose. 6. [3H]inulin recoveries, though significantly below 100% (P less than 0.001), were considered adequate to justify its use after allowing for possible sources of error. 7. A mean single nephron filtration rate (SNGFR) value of 7.02+/-0.27 nl/min (n = 89) was obtained from the kidneys of anaesthetized freshwater lampreys. 8. [51 Cr]EDTA was found to be totally unsuitable for the measurement of GFR in this species.

[Application of quantitative assay for endotoxin using immobilized histidine and filtration plate to parenteral drugs].

We improved the Limulus amebocyte lysate (LAL) test for endotoxins in parenteral drugs using immobilized histidine and a filtration plate. In order to deal with many samples at the same time and to apply to a routine assay, we used a filtration plate having 96 wells instead of a filter unit with a working volume of 2 ml. LAL test-affecting substances which are contained in a parenteral drug were separated from endotoxins by adsorbing endotoxins on immobilized histidine in the well of a filtration plate. Then the absorbed endotoxins were allowed to react with LAL reagent in the same well. We defined that this method had the higher precision than conventional methods and was not influenced by the concentrations of endotoxin and parenteral drugs. Hence we examined the recovery of endotoxin spiked to 23 kinds of parenteral drugs by this method, as a result, 100 +/- 25% of recovery was obtained from 17 kinds of them.

Penetrameter cassette calibration to 400 kV and effects of extra-focal radiation when measuring tube filtration.

The penetrameter cassette, when used with suitable filters and calibrated against the same average wave form, can detect changes of less than 1 kV in the range 20-50 kV, 1 kV from 50-100 and from 1-4 Kv from 100-400 kV. Problems of calibration are discussed together with the necessity to use a suitable shutter with equipment which has a significant build-up and decay time during which the radiation quality may be varying. Extra-focal radiation does not normally present a significant problem in the measurement of kV, but when the penetrameter is used to measure inherent or total filtration extra-focal radiation can be a problem and may be similar in degree to the effects of varying target angles. When used to determine filtration, the generator used for calibration of the penetrameter and the one being measured should be adjusted to have the same target angle and similar degrees of extra-focal radiation by altering the size of the tube aperture as close to the focal spot as practicable. The results presented confirm the accuracy and convenience of the penetrameter for the measurement of kV and filtration over the extended range of energies.

Barium enema: use of increased copper filtration to optimize dose and image quality.

The purpose of this study was to determine and validate the optimum copper filtration for adult double contrast barium enema examinations. Entrance surface dose rates to polymethyl methacrylate slabs and corresponding image intensifier input kermas, were measured for various added copper filters. Image contrast was assessed using a Leeds TO.10 test object. Copper filter thickness of 0.3 mm was chosen, as this reduced entrance surface dose rate by 56%, without substantially degrading image contrast due to kV and mA saturation. 20 sets of clinical films taken with each of 0.3 mm copper, 0.1 mm copper and no copper were reviewed following randomization, by a specialist gastrointestinal radiologist. Each set of digital spot and conventional films was allocated a score for each of three regions of the bowel, on a scale of 0-3 for perceived barium coating. The Kruskal-Wallis test showed no significant difference in perceived coating between the three groups (Digital spot: sigmoid colon p=0.207, splenic flexure p=0.103, hepatic flexure p=0.894. Screen-film: left colon p=0.803, right colon p=0.487, transverse colon p=0.905). All examinations but one were classified as diagnostic. The remaining one was classified indeterminate, due to poor distension of the colon. On adding 0.3 mm copper filtration, the mean dose-area product per examination was reduced by 57%, from 17.7 Gy cm(2) to 7.6 Gy cm(2). The estimated reduction in effective dose was 11%, from 3.0 mSv to 2.7 mSv. X-ray tube loading increased by 30%, but this caused no overheating with our local examination protocol and schedule. Additional filtration of 0.3 mm copper for adult double contrast barium enemas has now been implemented in routine clinical use at our hospital.

Effects of niobium filtration and constant potential on image quality in dental radiography. 2. Objective assessment.

The effects of niobium filtration and constant potential on image quality were explored by asking observers to identify numbers of circular areas of small density differences in zones of high and low density. Successful identification was related negatively to half-value layer. This relationship was particularly strong in the case of self-rectified units without additional niobium filtration and the constant potential units (r2 = 0.994, P < 0.0005). Constant potential had no effect other than in relation to half-value layer. Additional niobium filtration detracted from successful identification, but slightly less than would be predicted by the half-value layer. Whether this difference is significant or relevant has not been established. The findings of this study, in conjunction with our previous investigations, have failed to demonstrate any clear indications for preferring constant potential supply over self-rectification or for using niobium as an additional filter material. Half-value layer has been shown to be a reliable indicator of the effects of beam quality on emulsion speed, attenuation in water, and image quality. It appears that the methods employed to change the beam quality have no specific effects.

Quantitative assessment of image quality using niobium filtration for cephalometric radiography.

The effect of added niobium filtration on image quality in cephalometric radiography was quantitatively evaluated using the Modulation Transfer Function (MTF). A conventional full-wave cephalometric X-ray generator (Quint Sectograph) and a constant potential, high frequency X-ray generator (Orthophos CD) were compared with respect to the effects on the MTF of the added filtration. There was a slight reduction in image quality with both machines when adding niobium filtration.

Optical, real-time monitoring of the glomerular filtration rate.

An easy and accurate assessment of the renal function is a critical requirement for detecting the initial functional decline of the kidney induced by acute or chronic renal disease. A method for measuring the glomerular filtration rate is developed with the accuracy of clearance techniques and the convenience of plasma creatinine. The renal function is measured in rats as the rate of clearance determined from time-resolved transcutaneous fluorescence measurements of a new fluorescent glomerular filtration agent. The agent has a large dose-safety coefficient and the same space distribution and clearance characteristics as iothalamate. This new approach is a convenient and accurate way to perform real-time measurements of the glomerular filtration rate to detect early kidney disease before the renal function becomes severely and irreversibly compromised.

Determination of the glomerular filtration rate by identification of sinistrin kinetics.

A computer-based method of system identification and estimation of parameter variance for two-compartment models matched to dynamic sinistrin concentration profiles for the determination of glomerular filtration rate is described. Thereby a procedure for the judgment of the optimal sampling time horizon is presented. Since single-injection techniques are suspected of yielding systematic overestimation of the glomerular filtration rate, a method is demonstrated confirming that such a technique employing sinistrin kinetics can be used to correctly determine the glomerular filtration rate. The validation of the system parameters gained by the single-injection method is made through prediction of the concentration contour under a constant infusion regimen in the same subject on a different occasion. This was performed in healthy controls and in patients with various degrees of renal insufficiency. Upon consideration of the dependence of the clearance estimates and their variances on the protocol duration in test subjects examined from four to ten hours, an adaptive design of the protocol length is developed.

Hyperhomocysteinemia is related to residual glomerular filtration and folate, but not to methylenetetrahydrofolate-reductase and methionine synthase polymorphisms, in supplemented end-stage renal disease patients undergoing hemodialysis.

Glomerular filtration is one of the major determinants of plasma total homocysteine (tHcy). To evaluate the respective roles of residual glomerular filtration (by measuring a specific protein marker, cystatin C), genetic polymorphisms and nutritional status in tHcy blood levels in end-stage renal disease patients (ESRD) under hemodialysis and supplemented with folate, we measured tHcy, folate, vitamin B12 (B12), creatinine, cystatin C, albumin and C-reactive protein and determined the polymorphism of methylenetetrahydrofolate reductase (MTHFR) (C677T and A1289C) and of methionine synthase (MS) (A2756G) in 114 ESRD patients before hemodialysis and 76 control subjects. All patients received a folate supplementation of 700 microg/day. Hyperhomocysteinemia was observed in all patients and exceeded the upper normal limit by 2-fold in 52.4% of the patients. Serum folate was significantly increased and the B12 level was not different from controls. Folate, Cystatin C and creatinine were significantly correlated to tHcy, while no correlation was found between tHcy, albumin and C-reactive protein. No difference in genotype frequency between ESRD patients and controls was found for MTHFR A1289C and MS A2756G. The MTHFR 677TT genotype was less frequent and was associated with a significantly higher tHcy level in patients. Folate and residual glomerular filtration estimated by cystatin C and creatinine levels were two independent determinants of tHcy in ESRD patients. These data suggest that hyperhomocysteinemia is a consequence as well as a complicating factor of renal failure.

Diagnostic efficiency of cystatin C and serum creatinine as markers of reduced glomerular filtration rate in the elderly.

This is a secondary analysis of data from a cross-sectional study to evaluate the diagnostic efficiency of cystatin C as a marker of the glomerular filtration rate in the elderly. Thirty patients (15 male, 15 female, mean age 75.4 +/- 7.1 years) attending a geriatric ward were enrolled. Exclusion criteria were previously diagnosed renal disease, dementia and heart failure (NYHA III or IV). Cystatin C in serum was determined by a particle-enhanced turbidimetric assay. Inulin clearance was assessed using a single-shot method. Also, Cockcroft-Gault formula was calculated. Twelve patients had a reduced glomerular filtration rate (<70 ml/min/ 1.73 m2). The mean values were 88.4 micromol/l (+/- 27.7) for serum creatinine, 1.57 mg/l (+/- 0.34) for cystatin C and 88.7 ml/min/1.73 m2 (+/- 34.6) for inulin clearance. Maximum efficiency was 0.73 for serum creatinine (cut-off limit 82 micromol/l), 0.67 for cystatin C (cut-off limit 1.63 mg/l) and 0.8 for Cockcroft and Gault estimation (cut-off limit 54 ml/min/1.73 m2). A receiver operating characteristics (ROC) analysis did not show any differences between the various methods. Therefore, cystatin C in serum may not improve the diagnostic efficiency in detecting a reduced glomerular filtration rate in the elderly. Furthermore, normal ranges for serum creatinine in the elderly might need to be adjusted.

Improvements in qualitative characteristics of cryopreserved human spermatozoa following recovery via the SpermPrep II filtration method.

Motile, morphologically normal human spermatozoa can be separated from semen via a disposable SpermPrep filtration method. This method was employed with great success using frozen-thawed spermatozoa. The present study examined the qualitative characteristics of filtered spermatozoa via a new SpermPrep II filtration method in conjunction with the short-term freeze preservation of these spermatozoa. The isolation procedure yielded populations of spermatozoa with very high percentage motility and progressive motility grade (0-4) and which were free of seminal debris. For 20 semen samples the prefreeze values of percentage of motility and grade for the fresh and post-filtered were 58.6% and 3.1 vs 84.1% and 3.6, respectively. The sperm freeze preservation procedure involved dilution of the spermatozoa in Test-Yolk buffer (TYB) with 7% glycerol (v/v) and freezing and thawing in a conventional manner. Post-thaw percent motility, and survival % were substantially higher (p less than 0.05) for the filtered fraction than for the parent semen. The filtered fractions yielded post-thaw mean values of motility percent and grade of 68.9 and 3.4 vs 47.6 and 3.0 for the parent semen, respectively. Survival following incubation at 37 degrees C for 3 hours yielded respective values of 75.9% and 53.2% (p less than 0.05). It was concluded that the major factor in improving post-thaw quality recovery overall was the filtration via the SpermPrep II as applied in this study. This technique could have significant clinical applications in the use of frozen-thawed specimens for non-coital reproduction purposes.

Acrosin profiles of human spermatozoa recovered from the new Sperm Prep II filtration column.

Twenty semen samples were collected and two aliquots of 1.5 ml were prepared from each sample. One sample of each pair (fraction-one) was used for evaluation of semen parameters and total acrosin activity of spermatozoa. The other sample of each pair (fraction-two) was mixed with an equal volume of Ham's F-10 medium and filtered through the Sperm Prep II. At the end of the filtration, the filtrate was pooled, sperm parameters were evaluated and total acrosin activity was assessed. Percentage of normal spermatozoa, sperm motility, and total acrosin activity of spermatozoa were significantly higher in the postfiltered fraction-two than in the fresh fraction-one. Considering the great importance of acrosin for fertilization, it is suggested that Sperm Prep II filtration method may be beneficial in preparing spermatozoa from infertile men with low acrosin profiles for assisted reproduction programs. Further studies will be necessary, however, in order to confirm this, since the present study concerned men with known fertility.

Increased renal clearance of cefsulodin due to higher glomerular filtration rate in cystic fibrosis.

The steady-state renal clearance of cefsulodin was studied in 6 patients with cystic fibrosis and 8 healthy controls. The drug was administered by constant rate infusion to obtain 2 values of plasma concentration, 2 and 30 mg/L. As an estimate of the glomerular filtration rate, the renal clearance of inulin was measured simultaneously. The results showed the figures for inulin clearance to be approximately 30% higher in cystic fibrosis patients than in healthy controls at both concentrations, and a corresponding increase in the renal clearance of cefsulodin was seen in patients over controls. The ratio between the renal clearances of the 2 substances was on average 0.9 in both groups. The correlation found between the 2 renal clearances (r = 0.75; p less than 0.001) indicates that glomerular filtration rate has considerable influence on the renal elimination of cefsulodin. This finding emphasises the importance of glomerular filtration rate for the renal clearance of drugs in cystic fibrosis.

A high filtration system with synthetic permeable media for wastewater reclamation.

A novel filtration process with synthetic permeable media was investigated for secondary effluent reclamation. Polyurethane was chosen as the filter medium among three tested media. Compressibility and up-flow velocity were changed to determine the optimum operation for the system. An equation was introduced to express the relationship between the removal efficiency and up-flow velocity. In a pilot study, the synthetic medium filtration with compression showed very stable effluent quality without clogging trouble, though the system operated with three times higher filtration rate and much longer backwashing interval than conventional systems.

Glomerular filtration rate: estimation from fractional renal accumulation of 99mTc-DTPA (stannous).

99mTc diethylenetriaminepentaacetic acid (stannous) is cleared from the blood by glomerular filtration. However, calculation of the glomerular filtration rate has required analysis of serial blood samples drawn over intervals of several hours in order to determine the radionuclide clearance rate. This lengthy procedure can be circumvented by the direct scintigraphic determination of fractional radionuclide accumulation within each kidney, occurring at a specified time interval after radionuclide administration. This value has been correlated with 24 hr creatinine clearance values in a group of hospitalized patients. From these data, a formula was derived allowing for estimation of total glomerular filtration rate, as well as individually for each kidney, without requiring blood samples. This procedure needs only 6 min of patient time.

Prevention of hemodynamic and vascular albumin filtration changes in diabetic rats by aldose reductase inhibitors.

This study investigated hemodynamic changes in diabetic rats and their relationship to changes in vascular albumin permeation and increased metabolism of glucose to sorbitol. The effects of 6 wk of streptozocin-induced diabetes and three structurally different inhibitors of aldose reductase were examined on 1) regional blood flow (assessed with 15-microns 85Sr-labeled microspheres) and vascular permeation by 125I-labeled bovine serum albumin (BSA) and 2) glomerular filtration rate (assessed by plasma clearance of 57Co-labeled EDTA) and urinary albumin excretion (determined by radial immunodiffusion assay). In diabetic rats, blood flow was significantly increased in ocular tissues (anterior uvea, posterior uvea, retina, and optic nerve), sciatic nerve, kidney, new granulation tissue, cecum, and brain. 125I-BSA permeation was increased in all of these tissues except brain. Glomerular filtration rate and 24-h urinary albumin excretion were increased 2- and 29-fold, respectively, in diabetic rats. All three aldose reductase inhibitors completely prevented or markedly reduced these hemodynamic and vascular filtration changes and increases in tissue sorbitol levels in the anterior uvea, posterior uvea, retina, sciatic nerve, and granulation tissue. These observations indicate that early diabetes-induced hemodynamic changes and increased vascular albumin permeation and urinary albumin excretion are aldose reductase-linked phenomena. Discordant effects of aldose reductase inhibitors on blood flow and vascular albumin permeation in some tissues suggest that increased vascular albumin permeation is not entirely attributable to hemodynamic changes. We hypothesize that 1) increases in blood flow may reflect impaired contractile function of smooth muscle cells in resistance arterioles and 2) increases in vascular 125I-BSA permeation and urinary albumin excretion reflect impaired vascular barrier functional integrity in addition to increased hydraulic conductance secondary to microvascular hypertension associated with decreased vascular resistance.

Determining true glomerular filtration status in newly presenting type 2 diabetic subjects using age and sex adjustment.

OBJECTIVE: To determine age- and sex-adjusted reference ranges (ASARRs) for glomerular filtration status using data from nondiabetic subjects and to apply these to newly presenting type 2 diabetic subjects. RESEARCH DESIGN AND METHODS: Glomerular filtration rate corrected for body surface area (cGFR) was determined using a radionuclide (51Cr-EDTA) method in 75 non-diabetic subjects (37 men, 38 women) and 219 type 2 diabetic subjects (157 men, 62 women). The 95% constant reference ranges (CRRs) were calculated as mean nondiabetic cGFR+/-1.96 SD. The 95% ASARRs were calculated by Altman's method from the nondiabetic cGFR versus age regression residuals for both male and female subjects. RESULTS: Using Altman's method, the intercepts, but not the gradients, of the cGFR versus age regressions were significantly different between male and female subjects (intercept difference [95% CI] 8.2 [1.3-15.1], gradient difference -0.4 [-1.1 to 0.3]). Fitting a common gradient, 95% ASARRs for normofiltration were found to be from 123.9 - (0.89 X age) to 181.7 - (0.89 x age) for male subjects, and from 116.0 - (0.89 X age) to 173.2 - (0.89 X age) for female subjects. The 95% CRR for normofiltration was 70.2-138.1 ml x min(-1) x (1.73 m)(-2). When applied to the diabetic cGFRs, the CRRs and ASARRs gave, respectively, 17% (37/219) versus 21% (46/219) hyperfiltrators and 83% (181/219) versus 79% (172/219) normofiltrators. Using the ASARRs, 14 normofiltrators (6 men, 8 women) were reclassified as hyperfiltrators (change [n/total n] [95% CI] 8% [14/181] [4-12]), and 5 hyperfiltrators (5 men, 0 women) were reclassified as normofiltrators (change 14% [5/37] [5-30]). CONCLUSIONS: We conclude that age and sex adjustment are essential to assess glomerular filtration status.

Levels of reactive oxygen species before and after sperm preparation: comparison of swim-up and L4 filtration.

In previous studies, high levels of reactive oxygen species (ROS) have been associated with an adverse impact on the fertilizing ability of sperm. The present study evaluated ROS levels in unprocessed (raw) semen specimens and in specimens processed by the traditional swim-up method and by L4 filtration, thereby assessing the potential of these techniques to damage sperm. Semen specimens from 26 men (10 patients in whom subfertility was suspected and 16 donors with normal fertility) were randomly collected, and ROS levels were measured. For all specimens combined (those from patients and those from donors), ROS levels were significantly lower after L4 filtration (6.07 +/- 1.97 mV/s 10(9) sperm-1, p < .01) and after swim-up (5.70 +/- 0.96 mV/s 10(9) sperm-1, p < .001) than in unprocessed ejaculate (12.88 +/- 2.32 mV/s 10(9) sperm-1). However, ROS levels in swim-up and L4 specimens were not statistically different from each other. It would appear that sperm processing by either swim-up or L4 filtration yields specimens with significantly lower levels of ROS than are found in unprocessed ejaculate.
