Randomized, open-labeled, non-inferiority study between ciclopiroxolamine 1% cream and ketoconazole 2% foaming gel in mild to moderate facial seborrheic dermatitis.

BACKGROUND: Topical ketoconazole (KC) is considered a standard treatment for seborrheic dermatitis. In a placebo-controlled, double-blind clinical study, we demonstrated that antifungal ciclopiroxolamine (CIC) 1% cream was effective in mild to moderate facial seborrheic dermatitis. OBJECTIVES: We report here the results of a randomized, open-labeled clinical study comparing CIC 1% cream and KC 2% foaming gel in patients with mild to moderate facial seborrheic dermatitis, using a non-inferiority trial design. METHODS: Three hundred and three patients were enrolled, 154 patients in the CIC group and 149 patients in the KC group, and comprised the study population for intent-to-treat (ITT) analysis. The per protocol (PP) population comprised a total of 282 patients, 147 in the CIC group and 135 in the KC group. Patients were randomly allocated to apply either the CIC 1% cream twice a day for 28 days maximum (initial phase), followed by once a day for another 28 days (maintenance phase); or the KC 2% foaming gel twice a week at the initial phase, followed by once a week during the maintenance phase. Test lesions were defined as lesions localized to the nasolabial folds, alae nasi, and/or the eyebrows. The main efficacy parameter (endpoint) was the proportion of patients who presented a complete disappearance of both erythema and scaling on test lesions and pruritus on all lesions at the end of the initial phase (28 days or less). RESULTS: At baseline, both treatment groups were comparable in terms of demographic data and lesional status. At the end of the initial phase, responders were found to be non-inferior with CIC treatment compared with KC treatment in both study populations (ITT population: 37% CIC responders and 34% KC responders; in the PP population: 39 and 36% responders, respectively). The 95% confidence interval limit for differences were -7.99-13.56 in the ITT population, and -8.06-14.5 in the PP population. At the end of the maintenance phase, treatment response to CIC was greater than to KC in both ITT and PP populations (57 and 44% in both populations, respectively, p = 0.03). Local tolerance as well as global acceptability was better with CIC than with KC (p = 0.001, intergroup analysis). CONCLUSION: CIC 1% administered as a cream demonstrated to be non-inferior to KC 2% foaming gel in mild to moderate facial seborrheic dermatitis.

CCR4 Expression by atypical T cells in systemic pilotropic lymphoma: its behavior under treatment with interferon gamma, topical PUVA and systemic retinoid.

We describe an 88-year-old Japanese patient with pilotropic T cell lymphoma involving the peripheral blood as well as lymph nodes. This patient presented with multiple red follicular papules, confluent, infiltrated erythematous plaques and nodules. Moreover, he was conspicuous for the presence of total alopecia of the scalp and eyebrows. Histopathologically, the lesional skin showed dense follicular and perifollicular infiltrates of atypical lymphocytes. The flow cytometry disclosed the presence of weakly CD4+ CCR4+ cell populations that would not be detected in the peripheral blood from healthy controls. The patient responded well to topical PUVA and systemic etretinate (retinoid-PUVA) and intravenous IFN-gamma. Parallel with the decrease in atypical cells in the peripheral blood, the percentage of weakly CD4+ CCR4+ T cells declined. However, about 1 week after we discontinued this treatment because of the side effects, the lymph node swelling became prominent, and, 4 weeks later, the patient died before restarting any specific chemotherapy.

Hereditary hypotrichosis simplex of the scalp.

We report on a female aged 13 years, whose scalp hair began to disappear at the age of 9 years, leaving only sparse wispy hairs in the parietal-occipital region. Eyelashes, eyebrows and body hair were unaffected. There were no signs of ectodermal dysplasia on the skin, nails, teeth and eyes nor other congenital abnormalities. The family pedigree showed 15 relatives similarly affected according to an autosomal dominant mode of transmission. Clinical, genetic, histological and ultrastructural aspects led to a diagnosis of hereditary hypotrichosis simplex of the scalp (Toribio-Quiñones type).

Atrichia and papular lesions: report of a case.

We report a case of atrichia with papular lesions in a 4-year-old girl. The scalp was completely hairless since birth, except for dark, shiny, coarse hair on the frontoparietal region. Eyelashes and eyebrows were sparse. Numerous papular lesions developed on the hairless scalp, cheeks and neck during the second year of life. Teeth and sweating function were normal. The family history was negative. Histologic examination of a papular lesion showed the presence of a keratin-filled cyst in contact with the overlying epidermis. The pathology of the bald scalp showed the presence of tubular epithelial structures devoid of hair bulbs extending from the epidermis to the deep dermis and the superficial hypodermis. Sebaceous and outer root sheath differentiation was evident in most of the tubular structures that also frequently contained small ducts surrounded by two or three layers of flat epithelial cells. The superficial dermis contained horny cysts, similar to those present on the cheeks.

Progressive generalized perforating folliculitis.

A case is presented of a 52-year-old female exhibiting a severe progressive generalized perforating folliculitis accompanied by widespread erythroderma, follicular hyperkeratosis, keratoderma of palms and soles, alopecia of scalp and eyebrows, and nail changes. The disease seemed completely resistant to steroids, adrenocorticotropin, PUVA, cytostatic drugs, and aromatic retinoids. This type of skin reaction does not seem to have been described in the dermatologic literature.

Ocular involvement in tuberculoid leprosy--a case report.

Leprosy is rarely diagnosed in our part of the world. In our 26-year-old patient, borderline lepromatous leprosy was first diagnosed in 1992 and was treated with Rifoldin, Lemprene and Dapson according to the standard WHO scheme of treatment. Ophthalmic examination showed minor epithelial lesions of the cornea in both eyes, a reduced corneal reflex in the left eye and a scleral leproma nasally and close to the limbus, also in the left eye. In 1995, the patient was examined again and was treated as above for lepromata of the left eyebrow nasally and of both lower legs. Ophthalmic examination revealed conjunctival irritation, anterior uveitis with leprosy pearls on the pupillary margin and secondary glaucoma. The glaucoma was treated with hypotonics, the uveitis was treated with topical cortisone. The intra-ocular pressure normalised and the uveitis improved. Of the two main types of leprosy, lepromatous leprosy and tuberculoid leprosy, our patient had the second, milder form.

Eccrine porocarcinoma: Report of a case with fine needle aspiration cytology, histopathology and immunohistochemistry.

BACKGROUND: Although relatively rare, eccrine porocarcinoma (EP) is widely recognized in the literature as the most common of the sweat gland adenocarcinoma types. EP is an adenocarcinoma of the eccrine sweat gland with a propensity to recur locally and metastasize to regional lymph nodes. This paper presents the second case of fine needle aspiration (FNA) cytology of an EP along with histopathology and immunohistochemistry. CASE: A 64-year-old Filipino woman had a history of EP of the right eyebrow and presented with a right preauricular mass. The cytopathologic features of the case included: (1) clusters and sheets of polyhedral epithelial tumor cells with abundant, cyanophilic, vacuolated cytoplasm; round to oval, hyperchromatic nuclei; and occasional prominent nucleoli; (2) multinucleated tumor cells; (3) singly dispersed and relatively large aggregates of parakeratotic squamous cells; and (4) a background of necrotic debris. CONCLUSION: EP is crucial to developing an effective (curative) surgical plan. FNA cytology potentially provides a convenient, safe and effective approach to solving a challenging differential diagnosis. The constellation of cytologic findings probably is distinctive and, in the proper clinical setting, may be diagnostic. History is important in making an accurate diagnosis.

Long-latency ERPs and recognition of facial identity.

N400 brain event-related potential (ERP) is a mismatch negativity originally found in response to semantic incongruences of a linguistic nature and is used paradigmatically to investigate memory organization in various domains of information, including that of faces. In the present study, we analyzed different mismatch negativities evoked in N400-like paradigms related to recognition of newly learned faces with or without associated verbal information. ERPs were compared in the following conditions: (1) mismatching features (eyes-eyebrows) using a facial context corresponding to the faces learned without associated verbal information ("pure" intradomain facial processing); (2) mismatching features using a facial context corresponding to the faces learned with associated occupations and proper names ("nonpure" intradomain facial processing); (3) mismatching occupations using a facial context (cross-domain processing); and (4) mismatching names using an occupation context (intradomain verbal processing). Results revealed that mismatching stimuli in the four conditions elicited a mismatch negativity analogous to N400 but with different timing and topographical patterns. The onset of the mismatch negativity occurred earliest in Conditions 1 and 2, followed by Condition 4, and latest in Condition 3. The negativity had the shortest duration in Task 1 and the longest duration in Task 3. Bilateral parietal activity was confirmed in all conditions, in addition to a predominant right posterior temporal localization in Condition 1, a predominant right frontal localization in Condition 2, an occipital localization in Condition 3, and a more widely distributed (although with posterior predominance) localization in Condition 4. These results support the existence of multiple N400, and particularly of a nonlinguistic N400 related to purely visual information, which can be evoked by facial structure processing in the absence of verbal-semantic information.

Ethological study of facial behavior in nonparanoid and paranoid schizophrenic patients.

This investigation addressed the question of affective disturbance in schizophrenia by applying quantitative measurement techniques to patients' facial behavior. The subjects were medication-free male inpatients: nine nonparanoid and six paranoid schizophrenic patients and 12 drug- or alcohol-abuse rehabilitation control patients. Two judges scored the subjects' behavior, which was recorded on videotape, according to a system that included 16 different types of facial movements. Eye blinks, eye contact, and words spoken were also scored. Compared to the control patients, the nonparanoid schizophrenic patients spoke significantly fewer words and had significantly less eye contact, while the paranoid schizophrenic patients had significantly fewer eyebrow and lower facial movements.

Bilateral otorrhagia and orbital hematoma complicating subgaleal hematoma.

A patient with bilateral otorrhagia and orbital hematoma following massive subgaleal hematoma is reported for the first time. The pathogenesis of the extensive subgaleal hematoma as well as the production of these unusual complications is discussed. This case would suggest that the weakest points in the attachment of the epicranial aponeurosis to the base of the calvarium are its attachments to the squamous part of the temporal bone and to the frontal bone under the eyebrow.

Ankyloglossia with deviation of the epiglottis and larynx.

We observed ankyloglossia to be usually accompanied by displacement of the epiglottis and larynx. Infants with this disease developed dyspnea and skin and hair abnormalities. In addition, they had other symptoms, such as a dark forehead, a frowning expression, a dark color around the lips, scanty eyebrows, swelling around the palpebrae, harsh respiratory sounds, hard crying, snoring, and frequent yawning. In spite of these abnormalities, they had been considered to be healthy by their pediatricians. Arterial oxygen percent saturation (SaO2) was measured while the infants were asleep, suckling, and awake. The results revealed that their SaO2 was unstable and slightly low. The symptoms and signs of this disease were very similar to those observed in victims of sudden infant death syndrome before their death. Correction of the ankyloglossia and deviation of the epiglottis and larynx resulted in great improvement of these signs as well as a stabilization and increase of SaO2.

Valproic acid and congenital malformations. A case report.

An infant exposed in utero to valproic acid was born with multiple congenital malformations, including duodenal atresia, lumbar vertebral fusion, thoracolumbar scoliosis, renal abnormalities, diastasis recti abdominis, depigmentation of the eyebrows, and a large hemangioma. The child had neonatal hyperbilirubinemia and postnatal growth deficiency. This case report is an addition to the literature implicating valproic acid as a human teratogen.

Palpebral fissure length. In Chinese newborn infants. Comparison with other ethnic groups.

The palpebral fissure length was measured in 60 normal Chinese term infants in the first 48 hours of life. The measurement of the palpebral fissure length was performed with a vernier caliper by placing it across the greatest horizontal axis of the eye from the medial to the lateral canthus. The head circumference was measured by placing a tape anteriorly just above the eyebrows and posteriorly at the maximum point of the occiput. There was no difference according to sex or between the measurements in the right and the left eye. The palpebral fissure length was 1.94 +/- 0.17 cm (mean +/- standard deviation (SD)) and the head circumference was 34.0 +/- 1.3 cm (mean +/- SD). These data suggest that Chinese neonates have a shorter palpebral fissure length than do black American neonates and a longer palpebral fissure length than do white American and Turkish neonates.

Perceiving prosody from the face and voice: distinguishing statements from echoic questions in English.

We examined the processing of potential auditory and visual cues that differentiate statements from echoic questions. In Experiment 1, four natural speech statement-question pairs were identified by participants, and then analyzed to determine which characteristics were ecologically valid. These characteristics were tested in subsequent experiments to determine if they were also functionally valid. In Experiment 2, the characteristics of the most discriminable utterance pair were successfully extended to the other utterance pairs. For Experiment 3, an auditory continuum (varying in F0, amplitude, duration) was crossed with a visual continuum (varying in eyebrow raise, head tilt), using synthetic speech and a computer-animated head. Participants judged five levels along each of these two speech continua between a prototypical statement and prototypical question, in an expanded factorial design. Experiments 4 and 5 were unable to appreciably enhance the weak visual effect relative to the strong auditory effect (from Experiment 3). Overall, we found that both auditory and visual cues reliably conveyed statement and question intonation, were successfully synthesized, and generalized to other utterances. However, the weak visual effect relative to the robustly strong auditory effect precluded optimal integration and conclusive examination of information processing through model-fitting.

How children and adults produce and perceive uncertainty in audiovisual speech.

We describe two experiments on signaling and detecting uncertainty in audiovisual speech by adults and children. In the first study, utterances from adult speakers and child speakers (aged 7-8) were elicited and annotated with a set of six audiovisual features. It was found that when adult speakers were uncertain they were more likely to produce fillers, delays, high intonation, eyebrow movements, and "funny faces." The basic picture for the child speakers was somewhat similar, in that the presence of certain audiovisual cues correlated with uncertainty, but the differences were relatively small and less often significant. In the second study both adult and child judges watched responses from adult and child speakers selected from the first study to find out whether they were able to correctly estimate a speakers' level of uncertainty. It was found that both child and adult judges gave more accurate scores for answers from adult speakers than from child speakers and that child judges overall provided less accurate scores than adult judges.

Interstitial deletion of 14q, 46, XY, del (14) (q24.3q32.1) associated with status nonepileptic myoclonia and delayed myelination.

A Japanese boy with interstitial deletion of the long arm of chromosome 14, including band 14q31, is described. The characteristic dysmorphic facial features, such as dolichocephaly, bushy eyebrows, horizontal narrow palpebral fissures, long philtrum, etc, and mental and motor developmental delay were observed. Other characteristic clinical manifestations were anuresis and status nonepileptic myoclonia The finding of delayed myelination of the cerebral white matter was observed on magnetic resonance examination, suggesting that an unknown factor related to myelination in the central nervous system might be localized in band 14q31.

Unilateral giant coloboma of the upper eyelid associated with other congenital anomalies (33 years follow-up of surgical repair).

PURPOSE: To describe a case of congenital unilateral giant coloboma and its successful surgical repair with 33 years of follow-up. CASE REPORT: A 6-year-old boy presented with a congenital unilateral giant coloboma of the right upper eyelid associated with madarosis of the eyebrows, microphthalmos, dystopia of the hair, and coloboma of the apex of the nose. The patient underwent surgical repair of the multiple anomalies in different steps. DISCUSSION: A multiple-step, two-layer technique for the reconstruction of the right upper eyelid was performed in a 6-year-old boy with congenital unilateral giant coloboma associated with multiple ocular and facial anomalies. After 33 years of follow-up, the cosmetic results are excellent, although it has not been possible to preserve the visual function of the right eye, which had to be enucleated.

The role of the superciliary approach in the surgical management of intracranial neoplasms.

OBJECTIVE: The authors present and evaluate the experiences with 76 frontolateral keyhole craniotomies performed for supratentorial intracranial tumors via superciliary skin incisions. METHOD: The exploration is a modification of the generally used pterional approach. Out of a series of 302 cases operated on with frontolateral keyhole craniotomies via superciliary skin incisions, 76 had various tumors at the frontal base, intra-, supra-, parasellar and intraorbital regions. The operations were carried out through a approximately 2.5 x 3 cm frontolateral miniaturized craniotomy following a skin incision at the upper edge of the eyebrow. CONCLUSION: Despite the small size of craniotomy the superciliary approach is a suitable technique for resection of skull base tumors in the anterior fossa, orbit, sellar, parasellar and suprasellar regions. The exploration allows enough room for intracranial surgical manipulation with maximal protection of normal brain, vessel and nerve structures. The presented patients did not have any craniotomy-related complication.
