Single cell RNA sequencing provides novel cellular transcriptional profiles and underlying pathogenesis of presbycusis.

Age-related hearing loss (ARHL) or presbycusis is associated with irreversible progressive damage in the inner ear, where the sound is transduced into electrical signal; but the detailed mechanism remains unclear. Here, we sought to determine the potential molecular mechanism involved in the pathogeneses of ARHL with bioinformatics methods. A single-cell transcriptome sequencing study was performed on the cochlear samples from young and aged mice. Detection of identified cell type marker allowed us to screen 18 transcriptional clusters, including myeloid cells, epithelial cells, B cells, endothelial cells, fibroblasts, T cells, inner pillar cells, neurons, inner phalangeal cells, and red blood cells. Cell-cell communications were analyzed between young and aged cochlear tissue samples by using the latest integration algorithms Cellchat. A total of 56 differentially expressed genes were screened between the two groups. Functional enrichment analysis showed these genes were mainly involved in immune, oxidative stress, apoptosis, and metabolic processes. The expression levels of crucial genes in cochlear tissues were further verified by immunohistochemistry. Overall, this study provides new theoretical support for the development of clinical therapeutic drugs.

[Agreement between observations on the functional capacity of home care elderly patients].

OBJECTIVE: To assess the agreement between measures of caregiver's perception and health provider's direct observation of performance in daily living activities among home care elderly patients. METHODS: Performance observations were carried out among 40 elderly patients and their respective caregivers were interviewed. During home visits, data on caregiver's perception were collected by an independent observer using the Functional Independence Measure (FIM) instrument. Health providers evaluated elderly patients on their functional capacity in six dimensions: self-care, sphincter control, mobility, walking, communication and social cognitive skills. RESULTS: Intraclass correlation coefficient (ICC) showed an excellent agreement in all dimensions and the highest total FIM score was 0.95. Only the dimensions self-care and mobility showed a bias error between measures (signal test: p<0.001). Bias errors were not found when Bland-Altman graphic was applied. CONCLUSIONS: The agreement between health provider's and caregiver's measures demonstrated a good interaction between program team and caregivers, which is of great importance on evaluating and treating elderly patients. The results also suggest that when a valid and reliable instrument such as FIM is used, there is a high reproducibility of measures, even for different types of evaluations.

Pattern of satisfaction among neurosurgical inpatients.

OBJECTIVE: To measure satisfaction among patients receiving indoor neurosurgical care and analyse the profile of the dissatisfied patients. DESIGN: Cross-sectional study. PLACE AND DURATION: This study was undertaken at the Department of Neurosurgery, Pakistan Institute of Medical Sciences (PIMS), Islamabad from March to April 2005. PATIENTS AND METHODS: A total of 133 patients were included in the study by convenience sampling technique. All the patients, who received indoor care for a minimum of 24 hours and were discharged home, were included in the study . Patients who remained hospitalized for more than 4 weeks and those not consenting to participate were excluded. A questionnaire was used for the study that covered five fundamental areas of hospital care i.e. availability and behaviour of the staff, communication of information, residential and management issues. A five-point response scale was used to rate responses to the questions in each of these areas. The demographic profile of the patients and respondents, mode of admission, diagnosis, operation and duration of hospitalization were also recorded. The average of the responses to the questions in each of the five areas was taken as the fundamental area score (FAS) and the average of all these individual area scores was taken as the patient satisfaction score (PSS). Overall satisfaction index (OSI) was measured by calculating the average of PSS, willingness to return score and willingness to recommend score. RESULTS: Response rate was 100 %. Generally, patients were satisfied with care and rated various areas favourably. Behaviour of the staff was the highest rated area (95% score) while management was the lowest rated area (86.97% score). Dissatisfaction was more frequent among the young, the educated, the male and the relatives. The PSS was 91.32 %. Willingness to return score was 97.89% while willingness to recommend score was 95.48 %. The OSI was 94.89 %. CONCLUSION: Analysis of patients' dissatisfaction over specific aspects of health care serves to identify areas that could be improved by simple interventions, hence, patient satisfaction surveys should be conducted on regular basis in order to utilize patients' critical feedback for achieving service excellence and improved quality of care.

[Investigation of the exposure to radiation of the embryo/fetus in the course of radiographic examinations during pregnancy 1st communication: Tissue-air ratios for X-rays with tube voltages between 60 kV and 120 kV (author's transl)].

A relatively important radiation dose delivered to the embryo (fetus) is a decisive criterion, if interruption of pregnancy is considered for radiation-hygienic reasons. Several methods of assessment by calculation of the fetal exposition are discussed; in the last analysis, there are always needed the values of the relative depth doses or tissue-air ratios. The tissue-air ratios for the diagnostical X-ray qualities from 60 to 120 kV (total filtration 2.6 mm Al) were ascertained with the help of a water phantom. They are represented for layers from 0 to 20 cm by means of tables, and a comparison is made with data from the literature.

Factors that enhance or impede critical care nurses' discharge planning practices.

INTRODUCTION: Any illness that is serious enough to require admission to the critical care unit will intensify the physical and psychological effects that the patient and their significant others experience. Hence, the discharge needs of patients admitted to critical care are unquestionably complex, diverse and dynamic. METHODS: Utilising an exploratory descriptive approach 502 critical care nurses, identified from the Australian College of Critical Care Nursing (ACCCN) (Victoria) database were invited to participate in this study. A 31-item questionnaire was developed and distributed. A total of 218 eligible participants completed the survey. One-to-one semi-structured interviews with 13 Victorian critical care nurses were also conducted. RESULTS: Participants reported that a lack of time was a barrier to discharge planning. Communication however, could enhance or impede the discharge planning process in critical care. Participants considered that the critical pathway, used in the care of cardiothoracic patients, did assist with communication of discharge planning processes, hence enhancing the process. CONCLUSIONS: While these findings provide some understanding of the factors that enhanced or impeded critical care nurses' discharge planning practices further research is indicated. The findings reported here may, however, provide a starting point for improving the discharge planning process in critical care.

Are alexithymia, depression, and anxiety distinct constructs in affective disorders?

OBJECTIVE: the present study was undertaken to gain a better insight into the relationship between alexithymia, anxiety, and depression. Two hypotheses were tested: (1) whether a depressive or anxiety disorder is associated with an elevation of one or more dimensions of alexithymia; and (2) whether alexithymia is an independent construct from depression and anxiety in patients with depressive or anxiety disorders. METHOD: a total of 113 patients with depressive or anxiety disorders (DSM-IV) and 113 control subjects completed the 20-item version of the Toronto alexithymia scale (TAS-20) and the hospital anxiety and depression scale (HADS). RESULTS: the TAS-20 total score was higher in depressed and anxious patients than in controls. This finding mainly depended on an increased score for "difficulty identifying feelings"(DIF), and (only in depressed patients) on an increased score for "difficulty communicating feelings" (DCF). The factor analysis of the TAS-20 and HADS items showed that depression is a construct different from alexithymia, whereas some overlap exists between anxiety and DIF dimension. CONCLUSION: our results suggest that in depressive and anxiety disorders, alexithymia and depression are separate constructs that may be closely related; in contrast, there are some overlaps between the DIF dimension and anxiety.

[Diagnostic Observation Scale for Autistic Disorders: initial results of reliability and validity].

OBJECTIVE: To examine the psychometric properties of the German version of the Autism Diagnostic Observation Schedule (ADOS). METHODS: Interrater and retest reliability, internal consistency, convergent and diagnostic validity were determined in a total sample of 137 subjects with autism, 23 with atypical autism or pervasive developmental disorder not otherwise specified, 16 with Asperger-syndrome and 13 with other psychiatric disorders. RESULTS: Interrater and retest reliability on the level of diagnosis (kappa w = 1.00 and .62) and raw-scores (ru = .84 and .79) were good. Likewise, the internal consistency of the algorithm scale communication and social interaction of modules 1 to 4 was fair (ru = .78 to .89). The categorical convergence for autism between the ADOS and the Autism Diagnostic Interview-Revised (ADI-R) reached 79% (kappa = .23), with their corresponding subscales correlating moderately (rtc = .31 to .45). The concordance of the ADOS judgment and the clinical consensus diagnosis was 77% (kappa w = .37), with a sensitivity of the ADOS of 90.4% and a specificity of 48.1% regarding the discrimination of autism and other autistic disorders. CONCLUSIONS: The ADOS is a reliable and sufficiently sensitive diagnostic tool in the assessment of autistic disorders. For ICD-10/DSM-IV classification and to ensure a high specificity of diagnosis additional information concerning repetitive, stereotyped behavior and early development (e.g. taken from the ADI-R) has to be collected.

On the relationship between the efficiency and the quality of the consultation. A validity study.

OBJECTIVES: The aim of this study was to determine how the medical performance of physicians during consultations is related to doctor-patient communication and satisfaction of patients, taking into account the actual length of the consultations. In addition, we studied the validity of the 'efficiency-per-time score' as a measure of competence. METHODS: General practice trainees participated in a test situation in which they were confronted with six consultations with standardized (simulated) patients (SPs). All consultations were videotaped and evaluated by multiple observers, using national guidelines on medical content and on communication. The SPs scored satisfaction with the consultation using a satisfaction checklist. Forty GP-trainees were invited, of whom 34 participated. The main outcome measures were the number of obligatory actions undertaken by the GP-trainees, total number of actions undertaken, consultation time, efficiency-per-time score, patient satisfaction and quality of communication score, and the Pearson correlations between these measures. RESULTS: There was a negative correlation between the 'efficiency-per-time score' of the GP-trainees and the satisfaction of the SPs in five of the six consultations [Pearson r from -0.29 (P < 0.05) to -0.58 (P < 0.001)] and between the 'efficiency-per-time score' and the quality of the communication in three of the six consultations [Pearson r from -0.34 (P <.05) to -0.51 (P < 0.001)]. CONCLUSIONS: Short consultations with high technical medical efficiency seem to be related to bad communication and dissatisfied patients, thus questioning the validity of the 'efficiency-per-time score' as a measure of competence.

Mother-infant interaction: achieving synchrony.

BACKGROUND: Interventions that promote positive mother-infant interactions may reduce the risk of poor developmental outcomes for the child. OBJECTIVE: To examine the effect of infant communication education presented prenatally to first-time mothers on the quality of interaction that occurs between the mother-infant dyad in the first 24 hours following birth. METHOD: Twenty-nine first-time mothers were randomly assigned to either an intervention or control group. The intervention group received education on infant behaviors, states, and communication cues. A specific mother-infant interaction was videotaped and scored using the Nursing Child Assessment Teaching Scale (NCATS). The scores between groups were compared to determine the effect of education on the interaction that occurred between the dyads. RESULTS: Significant intervention effect was found in the overall totals (t(27)= 1.69; p = .05) as well as the contingency scores related to sensitivity to cues (t(27)= 1.93; p = .05) and social-emotional growth-fostering behaviors (t(27)= 1.93; p = .05). CONCLUSION: A videotaped educational intervention on infant communication implemented prenatally resulted in significant differences between the intervention and control groups on NCATS scores (totals, sensitivity to cues, and social-emotional growth-fostering behaviors). The use of videotaped educational information facilitates very early mother-infant interaction.

Inappropriate use of the title 'chiropractor' and term 'chiropractic manipulation' in the peer-reviewed biomedical literature.

BACKGROUND: The misuse of the title 'chiropractor' and term 'chiropractic manipulation', in relation to injury associated with cervical spine manipulation, have previously been reported in the peer-reviewed literature. The objectives of this study were to--1) Prospectively monitor the peer-reviewed literature for papers reporting an association between chiropractic, or chiropractic manipulation, and injury; 2) Contact lead authors of papers that report such an association in order to determine the basis upon which the title 'chiropractor' and/or term 'chiropractic manipulation' was used; 3) Document the outcome of submission of letters to the editors of journals wherein the title 'chiropractor', and/or term 'chiropractic manipulation', had been misused and resulted in the over-reporting of chiropractic induced injury. METHODS: One electronic database (PubMed) was monitored prospectively, via monthly PubMed searches, during a 12 month period (June 2003 to May 2004). Once relevant papers were located, they were reviewed. If the qualifications and/or profession of the care provider/s were not apparent, an attempt was made to confirm them via direct e-mail communication with the principal researcher of each respective paper. A letter was then sent to the editor of each involved journal. RESULTS: A total of twenty four different cases, spread across six separate publications, were located via the monthly PubMed searches. All twenty four cases took place in one of two European countries. The six publications consisted of four case reports, each containing one patient, one case series, involving twenty relevant cases, and a secondary report that pertained to one of the four case reports. In each of the six publications the authors suggest the care provider was a chiropractor and that each patient received chiropractic manipulation of the cervical spine prior to developing symptoms suggestive of traumatic injury. In two of the four case reports contact with the principal researcher revealed that the care provider was not a chiropractor, as defined by the World Federation of Chiropractic. The authors of the other two case reports did not respond to my communications. In the case series, which involved twenty relevant cases, the principal researcher conceded that the term chiropractor had been inappropriately used and that his case series did not relate to chiropractors who had undergone appropriate formal training. The author of the secondary report, a British Medical Journal editor, conceded that he had misused the title chiropractor. Letters to editors were accepted and published by all four journals to which they were sent. To date one of the four journals has published a correction. CONCLUSION: The results of this year-long prospective review suggests that the words 'chiropractor' and 'chiropractic manipulation' are often used inappropriately by European biomedical researchers when reporting apparent associations between cervical spine manipulation and symptoms suggestive of traumatic injury. Furthermore, in those cases reported here, the spurious use of terminology seems to have passed through the peer-review process without correction. Additionally, these findings provide further preliminary evidence, beyond that already provided by Terrett, that the inappropriate use of the title 'chiropractor' and term 'chiropractic manipulation' may be a significant source of over-reporting of the link between the care provided by chiropractors and injury. Finally, editors of peer-reviewed journals were amenable to publishing 'letters to editors', and to a lesser extent 'corrections', when authors had inappropriately used the title 'chiropractor' and/or term 'chiropractic manipulation'.

[Factors influencing headache in children].

OBJECTIVES: This descriptive study was conducted to examine the degree of stress, coping styles, communication with the mother and depression between headache-suffering children and headache-free children and to explore predicted factors for headache occurrence in children. METHOD: The subjects of this study consisted of 196 headache-free children and 107 headache-suffering children. They were 4th-6th graders of an elementary school in T city. The instruments in this study were David's stressor of children, Lazarus & Folkman's Stress Coping Style, PACI (Parent-Adolescent Communication Inventory) by Barnes & Olsen and Kovac's CDI (Children's depression inventory). Data were collected from May 2 to July 16, 2004. RESULTS: Total stress (t=-3.76, p=.035), school stress (t=-3.02, p=.001), mass media stress (t=-1.39, p=.029) and depression (t=7.62, p=.001) in headache-suffering children were significantly higher than those of headache-free children. Problem-oriented coping skills (t=1.23, p=.023), and the score of communication with the mother (t=2.32, p=.012) in headache-suffering children were lower than those of headache-free children. Logistic regression analysis (stepwise) showed that the most powerful predictor was stressors in school, followed by depression, stressors in mass media and communication with the mother. CONCLUSIONS: This study revealed that important factors such as the degree of school stress, depression, the degree of mass media stress, communication with the mother and problem-oriented coping skills should be controlled for reducing of headaches in children.

The ADL ability and use of technical aids in persons with late effects of polio.

OBJECTIVE: The purpose of this study was to describe functional performance in activities of daily living (ADL) and the use of technical aids among persons with late effects of polio. METHOD: Abilities in ADL of 150 participants 20 to 82 years of age were assessed with the Sunnaas Index of ADL, and the participants' use of technical aids was recorded. RESULTS: The activities in which most participants were independent were eating, daily hygiene, and communication. Many needed technical aids, adaptation of their homes, or both to perform mobility-related activities and to dress or undress, take a bath or shower, cook, or manage toilet visits. In total, 86 (57%) used mobility aids such as canes, crutches, and walkers. Thirty-one (21%) used wheelchairs within or outside the home. Bath and shower aids were the most commonly used technical aids other than mobility aids. The activity where most participants depended on others was housework. CONCLUSION: In spite of their disabilities, most participants performed well in many ADL, functioning independently by using technical aids and by living in an adapted environment.

Assessing physicians' continuing medical education (CME) needs in the U.S.-associated Pacific jurisdictions.

OBJECTIVE: To assess the self-perceived continuing medical education (CME) needs of physicians in American Samoa, Commonwealth of the Northern Mariana Islands, Guam, Federated States of Micronesia, Republic of the Marshall Islands, and the Republic of Palau. METHODS: Questionnaire-based survey of all physicians. RESULTS: Responses obtained from a total of 143 physicians in the region provided information on training backgrounds, previous experiences with CME, local access to regular CME sessions, perceived priority educational needs and preferred methods of CME delivery. CONCLUSIONS: Overall 64% of respondents had attended a formal CME event in 1999 or 2000, and 71% had access to local weekly or biweekly CME. However the perceived usefulness of these events varied by region. Priority learning needs were identified by physicians including non-communicable diseases such as diabetes, hypertension, cardiac disease; communicable diseases such as tuberculosis, HIV/AIDS and tropical diseases; as well as skills such as EKG and X-ray interpretation, trauma management and cardiac life support. Information on the most pressing educational needs and desired methods of delivery will be crucial in planning CME in this region.

IAIMS at Columbia-Presbyterian Medical Center: accomplishments and challenges.

The concept of "one-stop information shopping" is becoming a reality at Columbia-Presbyterian Medical Center. Our goal is to provide access from a single workstation to clinical, research, and library resources; university and hospital administrative systems; and utility functions such as word processing and mail. We have created new organizational units and installed a network of workstations that can access a variety of resources and systems on any of seventy-two different host computers/servers. In November 1991, 2,600 different individuals used the clinical information system, 700 different individuals used the library resources, and 900 different individuals used hospital administrative systems via the network. Over the past four years, our efforts have cost the equivalent of $23 million or approximately 0.5% of the total medical center budget. Even small improvements in productivity and in the quality of work of individuals who use the system could justify these expenditures. The challenges we still face include the provision of additional easy-to-use applications and development of equitable methods for financial support.

[Ultrasound guided percutaneous puncture and sclerotherapy with alcohol for peripelvic cysts].

OBJECTIVE: To summarize the experience of ultrasound guided percutaneous aspiration and sclerotherapy for peripelvic cysts and investigate the clinical effect. METHODS: A total of 169 cases of peripelvic cyst patients were evaluated, of whom 36 cases (21.3%) had hydronephrosis secondary to peripelvic cysts, 8 cases (4.7%) had renal calculus. All patients underwent ultrasound and intravenous phelography (IVP) examination, 59 patients also had CT scan. Hydatid fluid was analyzed by amine test. Percutaneous aspiration was guided by ultrasound, 95% alcohol was used to sclerosis the peripelvic cysts when amine test was negative or positive but did not have communication with pelvis through opacification. Ultrasound were done at 1, 3, 6 months and every 1 year thereafter, follow-up period were 6 months to 5 years. RESULTS: One hundred and sixty-five peripelvic cysts (97.6%) were cured by once sclerotherapy, the diameter of another 4 cases (2.4%) diminished to less than 1.5 cm by once sclerotherapy. Thirty-six cases of hydronephrosis were all resolved after sclerotherapy. Eight cases had renal calculus, of whom 4 cases underwent extracorporeal shockwave lithotripsy, 2 cases underwent percutaneous nephrolithotomy, and 2 cases did not treat the stone. Five patients had gross hematuria after aspiration, but all diminished in 3-5 d. CONCLUSION: Ultrasound guided percutaneous aspiration and sclerotherapy for peripelvic cysts had the superiority of safety, effectiveness micro-invasion and low complication.

Dental hygienists' utilization of periodontal assessment skills and perceived collaboration with dentist-employer.

PURPOSE: As cotherapists in providing total patient care, dental hygienists must foster their pivotal role in the prevention, recognition, and initial therapy of periodontal disease. Cooperation and communication between the hygienist and dentist are necessary to assure the best possible sequence of procedures for a complete periodontal assessment. This study examined the extent of periodontal assessments performed by Ohio dental hygienists treating new patients in private practice and correlated the use of these skills to perceived collaboration with their dentist-employers. METHODS: In January 1990, self-designed mail questionnaires were sent to a cohort of 1,671 licensed dental hygienists randomly selected from the State of Ohio Dental Board Roster. Data were analyzed using standard descriptive statistical methods and the non-parametric Spearman's correlation coefficient. RESULTS: Responses were received from 1,071 subjects, an 81% response rate of deliverable questionnaires and a 64% response rate of original 1,671 subjects. Results suggest that although hygienists perceive a high level of collaboration with their dentist-employer, periodontal assessments are being performed at a minimal level. There appears to be a fairly consistent, weak positive correlation between overall periodontal assessment and perceived collaboration scores. CONCLUSION: These data indicate that continued emphasis is needed on the hygienist's role in periodontal assessment. Delineation of this role may be enhanced by the establishment of specific collaborative efforts between the hygienist and dentist in defining the practice's periodontal protocol.

Validation of an EEG-derived spectral frequency index (SFx) for continuous monitoring of sleep depth in humans.

Sleep in humans is classically assessed by recording a multichannel electroencephalogram (EEG) in connection with an electrooculogram (EOG) and an electromyogram (EMG). In general, human sleep is manually staged into 6 categories (from awake through REM sleep to stage 4 reflecting deep sleep) on the basis of a visual inspection of EEG periods (epochs) of 20 - 30 s duration. This cumbersome methodology is still used in practice and for reference purposes. - The conversion of EEG-signals by means of Fast Fourier Transformation provides objective and reproducible information reflecting specific communicative features of the central nervous system. A special part of this information based on a specific algorithm is defined by the so-called spectral frequency index (SFx). This SFx-algorithm contains relationships among some particular EEG frequencies and provides objective percentage values about the state of consciousness of a person. In order to validate this new SFx-method, sleep as a physiological state of continuous alterations of consciousness and vigilance was chosen. A total of 36 nights of sleep from 18 healthy volunteers were staged manually by a scientist unaware of the protocol. The volunteers received either placebo or lormetazepam prior to commencement of the nocturnal recordings. The manually staged data were compared with the data obtained by the SFx-analysis. Both data sets SFx values and manually staged data were made comparable by averaging their values to a basic period length of 2 min duration giving 7960 pairs of data. The SFx data for sleep were found within a range from 35% to 100%. The SFx-medians of the manually staged data from "awake" to stage 4 were found in a decending order ("awake": 83% (lower and upper quartile 78% / 87%);"REM": 68% (63%/74%),"stage 1" :63% (57%/70%),"stage 2" :51% (47%/57%), "stage 3" :44% (42%/46%) and "stage 4" :42% (40%/44%). The rank correlation coefficient between the data pairs was calculated to be 0.79 indicating a substantial matching between the manually staged score and the SFx. We therefore conclude that the SFx is a suitable and objective indicator of sleep depth in humans.

Surface consolidation of Pentelic marble: criteria for the selection of methods and materials. The Acropolis case.

UNLABELLED: GOAL AND SCOPE: In order to realize surface consolidation of Pentelic marble on the Acropolis monuments, we have collected the criteria that have to be fulfilled by the methods and materials without side effects on the marbles (and other stones). When the existing methods and materials did not satisfy the requirements (criteria), we invented new ones. METHODS: The criteria were collected from the literature, by thermodynamic and kinetic viewpoints, by laboratory experiments and in situ observations on monuments, both in Greece and in several countries in which the various methods and materials were applied. Concerning our method of 'Inversion of sulfation', performed by spraying a K2CO3 solution, its concentration and its temperature were controlled in order to have a total inversion (certified by liquid crystals) of gypsum back to CaCO3. For the new material of ours, 'Reinforced Lime', in order to eliminate the disadvantages of plain lime (low carbonation rate, low mechanical properties of CaCO3 formed, lack of carbonation in the bulk of lime), measurements of the rate of carbonation were performed in plain lime and in lime with a different CaCO3 concentration beforehand as well as in a different CO2 environments, using DTG and XRD to measure the rate of carbonation. The detachment (kg/cm2) of marble specimens stuck by lime in each case was also measured. A microscope was also used to measure the dimensions of the grain in each case as well as ammonium citrate and thymolphthalein to certify the depth of carbonation in each case. Part of the work was communicated in an international congress. RESULTS: A table with the criteria of the selection of the methods and materials for the consolidation of Pentelic marble surfaces (and other stones) was established. None of the employed methods and materials fulfilled the criteria. Only the 'Inversion of gypsum' and the 'Reinforced Lime' were adopted and applied to the Acropolis monuments and to other monuments in Greece and in several other countries. RECOMMENDATION AND OUTLOOK: All criteria must be used for the selection of methods and materials for the consolidation of the monument surfaces. This is why we have already studied with the same procedures, methods and materials for restoration, surface cleaning and protection.
