Influence of erdosteine, a mucolytic agent, on amoxycillin penetration into sputum in patients with an infective exacerbation of chronic bronchitis.

Twenty four patients with acute infective exacerbations of chronic bronchitis received amoxycillin alone or in combination with erdosteine (a mucolytic agent) for a week in a double blind, placebo controlled study. Clinical assessment scores, body temperature, serum and sputum amoxycillin concentrations, and sputum culture results were recorded in each group. Erdosteine significantly increased antibiotic concentrations in sputum but not in serum. The combined treatment also caused a more rapid decrease in sputum viscosity and in body temperature and faster sterilisation of the sputum. These results show that erdosteine increases amoxycillin concentration in sputum in patients with acute exacerbations of chronic bronchitis. This effect may be due to a reduction in the viscosity of the bronchial secretions produced by erdosteine.

Cellular characteristics of sputum from patients with asthma and chronic bronchitis.

The reproducibility of sputum cell counts was examined and the cell counts in patients with asthma were compared with those in patients with chronic bronchitis. Three groups of subjects were studied. Sputum from eight patients with chronic asthma and with sputum production were studied to determine the reproducibility of sputum cell counts. The findings in 10 non-smokers with asthma uncomplicated by other airway disease examined at the time of an exacerbation with sputum (group 2) were compared with those from eight smokers with chronic cough and sputum but no features of asthma (group 3). Sputum plugs were selected by microscopy to ensure their origin from the lower respiratory tract. A total cell count was performed on a trypsinised suspension, and differential and metachromatic cell counts were performed on undiluted plugs. The within specimen and test-retest reproducibility of these measurements was high (reliability coefficient, R, = 0.99 and 0.89). The sputum of the asthmatic patients was characterised by eosinophilia (69%, range 46-92%) and the presence of formaldehyde blockable metachromatic cells (1.5%, range 0.6-2.8%). In comparison, the sputum of the patients with chronic bronchitis had few eosinophils (0.5%) or metachromatic cells (0.14%); the dominant cell type was the macrophage (83%). It is concluded that sputum cell counts are reproducible in the short term, the inflammation of asthma is characterised by eosinophilia and metachromatic cells in sputum, and sputum may provide a useful source of cells for investigating the cellular characteristics of airway inflammation.

Studies on the epidemiology of bovine parasitic bronchitis.

In the West of Scotland the epidemiology of parasitic bronchitis in grazing calves was studied over a two year period with the aid of tracer calves and herbage examinations for Dictyocaulus viviparus larvae. The observations of both years emphasised the importance of overwintered lungworm larvae as a source of disease. In the first year it was shown that the ingestion and development of these overwintered larvae were, by themselves, directly responsible for severe morbidity, high faecal larval counts and deaths. In the second year it was shown that pasture ungrazed during the winter and spring and from which a hay crop was removed in mid-summer was still capable of producing clinical parasitic bronchitis in susceptible calves within three to four weeks of their introduction in later summer. In both years there was some evidence that the outbreaks appeared to be associated with the sudden availability of infective larvae on the herbage. The possibility that such larvae may have survived for many months in the soil is discussed. Despite the heavy challenge with lungworm larvae experienced by the grazing calves in the first year those vaccinated with lungworm vaccine survived, their clinical signs were mild and of short duration and their faecal larval output was greatly reduced.

Control of parasitic bronchitis and gastroenteritis in grazing cattle by strategic prophylaxis with ivermectin.

In May 1985 four groups of 10 calves, aged between four and five months, were turned out on to separate, permanent pastures of equal area which had been seeded during the previous few days with larvae of Dictyocaulus viviparus. One group acted as a control, the second was vaccinated with lungworm vaccine before turnout and treated with thiabendazole three, eight and 13 weeks after turnout, while the third and fourth groups were given ivermectin three times (three, eight and 13 weeks after turnout) and twice (three and eight weeks after turnout), respectively. A severe outbreak of parasitic bronchitis resulted in the death of three control calves within five weeks of turnout and parasitic bronchitis and gastroenteritis affected the second group of calves after approximately four months at pasture. The calves given ivermectin excreted no lungworm larvae and remained free of clinical parasitism throughout the trial.

Characterisation of an infectious bronchitis virus isolated from vaccinated broiler breeder flocks.

Four apparently serologically closely related isolates of infectious bronchitis virus were obtained from two flocks of vaccinated broiler breeders, one mile apart, which were experiencing increased mortality and decreases in egg production. The isolates were serologically distinct from isolates previously described and capable of causing characteristic infectious bronchitis-like respiratory infection in young chicks. In one experiment, the H120 vaccine strain of the virus did not protect the trachea against challenge with the new isolates 21 days later.

A survey of the presence of a new infectious bronchitis virus designated 4/91 (793B).

On the basis of virus isolation and the demonstration of specific neutralising antibody in sera, infectious bronchitis virus (IBV) 4/91 (commonly called 793B) has been shown to be present in broiler, breeder and layer flocks of chickens in many parts of western Europe and also in Thailand and Mexico. These flocks had all been vaccinated against infectious bronchitis and the need for improved methods to control this new virus, still prevalent at least four years after it was first isolated, is discussed.

Mechanism of transient nocturnal hypoxemia in hypoxic chronic bronchitis and emphysema.

In five patients with hypoxic chronic bronchitis and emphysema we measured ear O2 saturation (SaO2), chest movement, oronasal airflow, arterial and mixed venous gas tensions, and cardiac output during nine hypoxemic episodes (HE; SaO2 falls greater than 10%) in rapid-eye-movement (REM) sleep and during preceding periods of stable oxygenation in non-REM sleep. All nine HE occurred with recurrent short episodes of reduced chest movement, none with sleep apnea. The arterial PO2 (PaO2) fell by 6.0 +/- 1.9 (SD) Torr during the HE (P less than 0.01), but mean arterial PCO2 (PaCO2) rose by only 1.4 +/- 2.4 Torr (P greater than 0.4). The arteriovenous O2 content difference fell by 0.64 +/- 0.43 ml/100 ml of blood during the HE (P less than 0.05), but there was no significant change in cardiac output. Changes observed in PaO2 and PaCO2 during HE were similar to those in four normal subjects during 90 s of voluntary hypoventilation, when PaO2 fell by 12.3 +/- 5.6 Torr (P less than 0.05), but mean PaCO2 rose by only 2.8 +/- 2.1 Torr (P greater than 0.4). We suggest that the transient hypoxemia which occurs during REM sleep in patients with chronic bronchitis and emphysema could be explained by hypoventilation during REM sleep but that the importance of changes in distribution of ventilation-perfusion ratios cannot be assessed by presently available techniques.

Solutions for difficult diagnostic cases of acute exacerbations of chronic bronchitis.

Acute exacerbation of chronic bronchitis (AECB) is a very common condition, which presents with deteriorating sputum production and dyspnoea in a patient with pre-existing COPD or chronic bronchitis. As these symptoms are relatively non-specific and also the presenting feature of a wide range of other conditions, the physician should carefully consider the differential diagnosis before deciding on whether or not a patient indeed has AECB. The differential diagnosis can be summarised as pneumonia, pneumothorax, cardiac failure/cor pulmonale, bronchiectasis, asthma, tuberculosis, sinusitis and other forms of upper respiratory tract sepsis, diffuse panbronchiolitis, lung cancer, gastro-oesophageal reflux, the presence of a foreign body in the airway, melioidosis, and lung abscess. This article aims to discuss these conditions, with brief presentation of clinical cases, in the evaluation of differential diagnosis of AECB.

Influence of tuberculin on ventilatory parameters in patients with chronic bronchitis.

The influence of tuberculin administered in aerosol on the ventilatory parameters in patients suffering from chronic bronchitis has been studied. The diameter of the tuberculin skin reaction 1 day after inhalation of tuberculin was found to be related to a decrease in maximal 1-sec forced expiratory volume and maximal midexpiratory flow-rate. Significant decrease in the parameters of dynamic pulmonary ventilation occurred only in a certain proportion (24.6%) of tuberculin-positive patients suffering from chronic bronchitis of the obstructive type.

Influence of 0.2 mg ipratropium bromide on mucociliary clearance in patients with chronic bronchitis.

14 patients with chronic bronchitis were subdivided into 2 groups of 7 patients. They were given at random 10 puffs of placebo or ipratropium bromide (0.2 mg). The mucociliary clearance rate was measured immediately after ipratropium inhalation (group 1) or 24 h after ipratropium inhalation (group 2). In group 1, the mucociliary clearance after ipratropium bromide was significantly better than after placebo (p less than 0.01). In group 2 there was no difference in the mucociliary clearance rates measured after ipratropium bromide or placebo, but the clearance rate still tended in favour of ipratropium bromide. We conclude that ipratropium bromide should be inhaled in high doses (0.2 mg) by hypersecretory patients with chronic bronchitis to improve mucociliary transport.

Ambroxol for the prevention of chronic bronchitis exacerbations: long-term multicenter trial. Protective effect of ambroxol against winter semester exacerbations: a double-blind study versus placebo.

In a 6-month, double-blind multicenter trial conducted over the winter, the effects of daily administration of ambroxol retard (75 mg) were compared with those of placebo in preventing exacerbations and improving symptoms and clinical signs in chronic bronchitis patients. The trial was completed by 110 patients in the ambroxol group and by 104 in the placebo group. Initially, there were no significant differences between the groups. By the end of the 2nd month of treatment, 67.2% of the ambroxol group had had no exacerbations compared to 50.4% in the placebo group. At the end of the 6-month trial, 45.5% of the treatment group had had no exacerbations, compared to only 14.4% of the control group. These differences were statistically significant. Patients in the treatment group lost significantly fewer days through illness (442) and had fewer days when they needed antibiotic therapy (371) compared to the placebo group patients (837 and 781). Ambroxol also produced statistically significant symptomatic improvement, measured as difficulty in expectoration, coughing, presence of dyspnea and the auscultatory signs as compared to controls. Since ambroxol was well tolerated and compliance was good, it appears like a drug of choice for pharmacological prophylaxis of chronic bronchitis.

Idiopathic and symptomatic plastic bronchitis in childhood. A report of three cases and review of the literature.

During a one-year period we could observe 3 children with the rare disease of plastic bronchitis. Two of them showed a symptomatic form, one with constrictive pericarditis, the other with bronchial asthma as primary diseases. No underlying disease could be found in the third case. Bronchial casts had to be extracted during endoscopy in all children. No relapse occurred after 2-3 years. Reviewing the literature, 37 cases of plastic bronchitis in childhood could be found, most of them had been reported before 1954.

Evaluation of systemic host defense mechanisms in chronic bronchitis.

Seventy-six chronic bronchitis patients were studied in order to determine the possible presence of disorders in their systemic defense mechanisms. No significant difference in lymphocyte subsets, in serum immunoglobulin and complement component (C3 and C4) levels was found in chronic bronchitis patients compared to normal adult controls. Skin tests for delayed hypersensitivity revealed a high frequency (39%) of hypoergic patients (with 1-2 positive reactions) in comparison to normal subjects. Altered values of many functional properties of both neutrophils and monocytes were demonstrated. The percentage of patients with intermediate (between 1 and 2 SD below the mean of controls) and defective (lower than 1.96 SD) values of chemotaxis, phagocytosis index and Candida killing was about 50%. Phagocytosis frequency and nitroblue tetrazolium reduction frequency were less frequently impaired.

Chronic bronchitis: definition (or redefinition?).

Although a quite precise clinical definition was given to the term 'chronic bronchitis' in the early sixties, the terminology related to nonasthmatic, nonemphysematous obstructive airway disease remains a continuous source of confusion, as indicated by a recent international survey. Although histological studies point to the presence of widespread bronchial inflammation in this condition, preference should be given to simple and descriptive terminology in which the presence of airflow limitation is clearly and predominantly indicated, since it mostly affects prognosis. There would be no need to redefine 'chronic bronchitis', but simply replacing it by 'chronic mucus hypersecretion', to which it actually refers, would be the preferred solution.

Risk factors for chronic bronchitis and chronic obstructive lung disease.

Among the risk factors for chronic bronchitis and chronic obstructive lung disease, only smoking and alpha 1-antitrypsin deficiency effects have been well demonstrated. Other factors, as air pollution, occupational exposure and childhood lung diseases may have an influence, but on a lower scale. The prevention of chronic bronchitis and chronic obstructive lung disease remains mainly based on antismoking programs.

History of the treatment of chronic bronchitis.

Although chronic bronchitis was first named and described in 1808, the disease has been known since earliest time, and numerous drugs have been utilized in its therapy. The basic historic theories of human function have readily been applied to bronchitis; thus in Greek medicine, the disease was appreciated as one of excess phlegm. Early remedies included garlic, pepper, cinnamon, and turpentine, whereas later therapies of choice emphasized coffee, ipecac, and potassium nitrate. Most of the favored bronchodilator drugs of today are derived from the traditional folk remedies, ephedrine, atropine, and theophylline. The most interesting historical drugs, however, are those that have given rise to modern mucokinetic agents such as bromhexine and iodides.

Analysis of respiratory impedance characteristics in chronic bronchitis.

Impedance measurements of the respiratory system were performed by means of the technique of forced oscillations to study the mechanical characteristics of the respiratory system in 33 patients with chronic bronchitis without spirometric evidence of airway obstruction (FEV1 greater than or equal to 70% predicted). In 23 patients (69.7%) impedance was found to be abnormal. Inhalation of a beta 2-adrenergic drug (terbutaline) resulted in acute reversibility of these findings. In the 10 patients (30.3%) with a normal baseline respiratory impedance, isocapnic hyperventilation with cold air resulted in impedance characteristics similar to the abnormalities spontaneously occurring in the other group. These changes were found to be completely reversible after the inhalation of terbutaline. On the basis of these findings we conclude that impedance measurements of the respiratory system by means of the technique of forced oscillations allow the determination of mechanical airway abnormalities in patients with chronic bronchitis.

Structural characterization of the bronchial epithelium of subjects with chronic bronchitis and in asymptomatic smokers.

A morphometric study was carried out on biopsy specimens taken from 40 smokers (27 with chronic bronchitis and 13 asymptomatic) submitted to bronchoscopy to identify and quantify the possible structural differences between the two groups. The chronic bronchitic group had a mean age of 65.67 years and 57.04 pack-years of smoking, the asymptomatic group had a mean age of 44.69 years and 22.62 pack-years of smoking. 70 biopsy specimens (45 from chronic bronchitics and 25 from asymptomatic smokers), in which large areas of best-preserved and perpendicularly cut epithelium were present, were considered suitable for the study and examined by light and transmission electron microscopy. The mean thickness of surface epithelium (p < 0.001), the number of layers of basal cells (p < 0.001), the intercellular space of the superficial zone of the epithelium (p < 0.05) and the percentage of abnormal bronchial cilia (p < 0.05) were significantly greater in patients with chronic bronchitis than in asymptomatic smokers. No significant difference between the two groups was observed in the thickness of the lamina reticularis of the basement membrane. Goblet cell hyperplasia was more marked in chronic bronchitics than in the asymptomatic smokers (p < 0.001), whereas the frequency of epidermoid metaplasia did not show a significant difference. The morphological study of the bronchial epithelium has allowed the identification of transitional cells, which gives rise to the concept that epidermoid metaplasia may result from conversion of mucous cells. This finding suggests that the different histologic types appearing in lung tumours may originate from one undifferentiated pluripotential stem cell, which is able to differentiate into different histogenetic types.
