Clinical signs of the pigment dispersion syndrome in blacks.

BACKGROUND: The pigment dispersion syndrome (PDS) is considered rare in blacks, and minimal literature exists concerning the condition in this patient population. The diagnosis of PDS in blacks may present unique challenges because some of the typical clinical signs that are present in whites, including iris transillumination defects, posterior iris bowing, and noticeable anterior iris stromal pigment dusting, may not occur as commonly. Diagnosis can be particularly difficult when neither these signs nor significant corneal endothelial pigmentation exists. Although zonular and peripheral lens pigment has been found to be consistently present in whites with PDS, attention has not been given to this as a potentially important diagnostic sign in blacks. METHODS: From among a primary care population, we identified and studied 7 patients (13 eyes) who exhibited moderate to heavy trabecular meshwork (TM) pigmentation, as well as zonule and/or peripheral lens pigmentation. Patients were identified during routine clinical care provided by one of the authors, as well as from notification by other practitioners. All patients received complete eye examination and other signs of PDS were looked for. RESULTS: Four males and 3 females were identified, their average age being 37 years (range = 15 to 51) at the time of their initial identification. All but one patient was myopic (average approximately -2.50 D spherical equivalent). Iris transillumination defects were present in only one eye of one patient, and no eyes showed overt posterior iris bowing, although the iris contours were usually flat and the anterior chambers appeared relatively deep. Corneal endothelial pigmentation was frequently barely detectable and could not be relied on as a predictor of trabecular meshwork or lenticular pigmentation. Glaucoma, or a suspicion of glaucoma due to increased intraocular pressure (IOP) or cupping, was common among the group. Using heavy TM pigmentation as well as any degree of zonular and/or peripheral lenticular pigmentation as a criteria for the diagnosis of PDS, we calculated the prevalence of PDS among blacks in a nonreferred primary care population (> age 7) to be at least 15 cases per 10,000. CONCLUSIONS: More investigation is needed to study the clinical presentation of PDS in blacks because it may be substantially different than in whites. Zonular and peripheral lenticular pigmentation may be a particularly useful diagnostic sign of PDS in blacks, especially in those cases where other traditional signs, including iris transillumination defects, pronounced corneal endothelial pigmentation, posterior iris bowing, and visible anterior iris stromal pigment dusting, are absent. The "classic" variety of PDS may be more common among blacks than previously recognized.

[Immunocytochemical and ultrastructural studies of human scleral spur. Characterization of a contractile cell population].

Contraction of the ciliary muscle induced by cholinergic drugs causes movement of the scleral spur and separation of the trabecular meshwork lamellae. The effect of epinephrine, however, does not seem to be mediated by ciliary muscle tone. We investigated the scleral spur in 37 human eyes (age 17-87 years). Serial tangential sections were studied with ultrastructural and immunocytochemical methods. The ciliary muscle cells do not enter the scleral spur, but their tendons, which consist of elastic fibers, are continuous with the elastic fibers in the scleral spur. Within the scleral spur is found a population of circularly oriented and spindle-shaped cells. The scleral spur cells form no bundles, but are loosely aggregated. They have long cytoplasmic processes and are connected to each other by adherence type and gap junctions. The scleral spur cells show intense staining for smooth-muscle-specific alpha-actin and myosin. Ultrastructurally, the scleral spur cells contain abundant actin filaments, but otherwise do not show the typical ultrastructure of ciliary muscle cells. The scleral spur cells do not express a complete basal lamina. They form individual tendinous connections with the elastic fibers in the scleral spur, which are continuous with the elastic fibers of the trabecular meshwork. The scleral spur cells are in close contact with nerve terminals containing small granular vesicles that are typical for adrenergic terminals. We conclude that the scleral spur cells are contractile myofibroblasts. Their contraction might be involved in the effects of epinephrine on the aqueous outflow.

Expression of dominant negative Rho-binding domain of Rho-kinase in organ cultured human eye anterior segments increases aqueous humor outflow.

PURPOSE: Based on pharmacological inhibition of activity, a role has been proposed for Rho-kinase in the modulation of aqueous outflow and intraocular pressure (IOP). This study employed a molecular approach to specifically address the role of Rho-kinase in the modulation of aqueous humor outflow. METHODS: Adenoviral vectors expressing green fluorescent protein alone (Ad-GFP) or the dominant negative Rho-binding domain of Rho-kinase and GFP (Ad-DNRK-GFP) were utilized in these experiments. Human and porcine primary trabecular meshwork (TM) cells were infected with 30 MOI (multiplicity of infection) of Ad-GFP alone or with Ad-DNRK-GFP. Changes in cell shape, actomyosin cytoskeletal integrity, and the status of myosin light chain (MLC) phosphorylation were evaluated. The aqueous outflow facility was determined in organ cultured anterior segments of human cadaver eyes infected with 10(7) pfu adenoviral vectors (Ad-GFP or Ad-DNRK-GFP) using a constant flow perfusion system. RESULTS: Expression of DNRK resulted in changes in cell shape (cell rounding, cell-cell detachment) and decreased actin stress fiber and focal adhesion staining in TM cells. These cellular changes were associated with substantially reduced myosin light chain phosphorylation. Additionally, organ cultured human eye anterior segments infected with Ad-DNRK-GFP exhibited a significant increase in the outflow facility (80%, n=9) compared to control anterior segments infected with Ad-GFP (5%). CONCLUSIONS: This study demonstrated that specific inhibition of Rho-kinase activity in trabecular meshwork cells led to alterations in cell shape and presumed contractile properties, and we hypothesize that these morphological and contractile events underlie the observed effects of dominant negative Rho-kinase on the aqueous humor outflow facility. These data provide molecular evidence for the hypothesis of Rho-kinase being a potential cellular target involved in the regulation of aqueous humor outflow resistance, with implications for novel glaucoma therapy.

Measuring triamcinolone acetonide in aqueous humor by gas chromatography-electron-capture negative-ion mass spectrometry.

Intravitreal triamcinolone acetonide (IVTA) injection has been used in the treatment of various posterior segment diseases. One of the side effects of IVTA is raised intraocular pressure, which may be secondary to triamcinolone acetonide (TAA)'s effects on the trabecular meshwork that affects aqueous outflow. In order to study the biological effects of TAA on the trabecular meshwork, we firstly need to reliably and accurately detect the concentration of TAA in tissue cells or fluids. In this study we have described a technique of using gas chromatography-electron-capture-negative-ion mass spectrometry (GC-NCI-MS) to develop a simple, sensitive, selective and validated method to detect TAA in aqueous humor (AH) of rabbits following IVTA and subconjunctival TAA injections. We derivatized TAA from extracted aqueous sample by acetic anhydride and BSTFA, respectively, and analyzed by GC-NCI-MS. The detection limit was 0.3ng/ml, linearity over 0.995 from 0 to 300ng/ml. The reproducibility ranged from 10.4 to 3.9 for concentrations from 3 to 300ng/ml, and recovery was over 95% for the concentrations 10, 60, and 200ng/ml. No interference was found from 159 aqueous samples. There was no TAA residue carried to the next injection from previously high concentration injection, 10,000ng/ml. We have provided an alternative, rapid, and robust method other than LC-MS-MS for TAA detection in AH.

Nitric oxide/guanylate cyclase pathways and flow in anterior segment perfusion.

BACKGROUND: The nitric oxide/guanylate cyclase pathway has been suggested to participate in the regulation of intraocular pressure. In the present study, the involvement of nitric oxide pathways on the outflow through the trabecular meshwork was assessed using pharmacological manipulation of the nitric oxide pathway. METHODS: Anterior segments of human donor eyes were maintained in an organ culture perfusion system, and the effects of L-NAME, an inhibitor of nitric oxide synthase, on the flow rate was determined. In a second series, the effects of consecutive application of L-arginine as substrate for nitric oxide synthase, L-NAME, and sodium nitroprusside, a nitric oxide-donor, were studied. The cyclic GMP levels in the perfusate were assessed with an ELISA immunoassay kit. RESULTS: In the first series of experiments, L-NAME caused a statistically significant decrease in flow rate of 10%, accompanied by a decrease in cGMP levels. In the second series, L-arginine did not alter flow, and the effect of L-NAME seen in the first series was prevented by the high preload of L-arginine. Nitroprusside caused a significant 10% increase of flow rate. In the perfusate, cGMP levels were not altered by L-arginine and L-NAME, but were increased after nitroprusside. CONCLUSION: Under organ culture perfusion conditions, modulation of the nitric oxide/guanylate cyclase system alters the flow rate through the trabecular meshwork within a total range of 20%; i.e. the difference between inhibition of NO synthesis and the presence of a NO-donor. These results indicate that the nitric oxide/guanylate cyclase system plays a role in aqueous humour dynamics and, therefore, in the regulation of intraocular pressure.

Effects of TGF-beta2 in perfused human eyes.

PURPOSE: TGF-beta2 is known to be present at elevated levels in the aqueous humor of patients with primary open-angle glaucoma (POAG). Studies have shown that TGF-beta2 influences cultured trabecular meshwork (TM) cells, but the effects of this cytokine on intact TM and outflow facility have not been studied. The purpose of this study was to investigate whether TGF-beta2 treatment induces changes in outflow facility and morphologic changes in the TM tissue and whether these changes are comparable to those previously recorded in glaucomatous eyes. METHODS: Baseline facility was measured in paired human eyes (n = 8 pairs), with a constant-flow anterior segment culture system. Medium perfusing experimental eyes was then supplemented with activated human recombinant TGF-beta2 (3.0 ng/mL, comparable to or slightly greater than measured aqueous humor levels in patients with POAG), and facility was measured for at least 8 days. At the conclusion of the perfusion, eyes were fixed and processed for light microscopy, transmission electron microscopy, and immunolabeling studies. RESULTS: TGF-beta2 perfusion reduced outflow facility by 27% (P = 0.03) and promoted focal accumulation of fine fibrillar extracellular material in multilayered structures under the inner wall of Schlemm's canal. In treated eyes, Schlemm's canal was 27% shorter (P = 0.02), and the length of the inner wall apparently available for fluid flow was 33% less (P = 0.001), both compared with paired control eyes. CONCLUSIONS: TGF-beta2 reduces outflow facility when perfused into cultured human anterior segments. Furthermore, TGF-beta2 affects the extracellular matrix of the trabecular meshwork in a manner that is consistent with the observed reduction in outflow facility. Although the distribution of accumulated fibrillar material was different in these perfused eyes than that in POAG, the difference could be due to variation in biomechanical environment for TM cells in cultured anterior segments compared with the living eye. Overall, these results support the hypothesis that elevated TGF-beta2 levels in the aqueous humor play a role in the pathogenesis of the ocular hypertension in POAG.

Shock-wave effect on anterior segment structures following experimental neodymium:YAG laser iridectomy.

The short-pulse laser may prove superior to the argon laser in producing noninvasive iridectomies. Little is known of the effects of the laser shock-wave on anterior segment structures adjacent to the iris. To investigate this question, iridectomies were produced in two owl monkeys using a neodymium (Nd):YAG laser and tissues were evaluated by light microscopy and scanning and transmission electron microscopy. For purposes of comparison, one eye received an iridectomy produced by an argon laser. Results indicate that while clean iris colobomas can be produced with a well-focused Nd:YAG laser, its shock-wave affects tissues in both the trabecular meshwork and corneal endothelium if the iridectomy is located within 0.8 mm of the limbus. No damage to the trabecular meshwork or corneal endothelium was evident after the argon laser iridectomy.

The development of the irido-corneal angle in the chick embryo.

The iridocorneal angle of the chick embryo was examined by scanning electron microscopy from the 7th day to the 19th day of development. The mesodermal tissue situated between the corneal endothelium and the iris differentiated into trabecular cells. These cells formed a strip on the 9th day and enlarged on the 13th day to form long cellular fibres. The trabecular meshwork was recognizable on the 15th day and reached full development only on the 19th day. The development of the trabecular meshwork was probably much earlier than is usually described. It began with differentiation of the corneal endothelial cells and these cells acquired a multipolar cell body with long cellular processes. The last stage is probably regulated to avoid an abnormal development of the angle structure. These results can be correlated with those obtained in the morphological examination of the structure of the angle in congenital glaucoma and the "cleavage syndrome" of the anterior chamber.

Current concepts in pigmentary glaucoma.

Since its initial description over 50 years ago as a rare clinical entity, pigmentary glaucoma has become recognized as one of the most common forms of secondary open-angle glaucoma. Pigmentary glaucoma affects a much younger patient population than most other forms of open-angle glaucoma, and has a predilection for Caucasian males with myopia. Hallmarks of this disease include midperipheral iris transillumination defects, Krukenberg spindles and a heavily pigmented trabecular meshwork. The mechanism of pigment dispersion appears to be a rubbing between iris pigment epithelium and packets of lens zonules, possibly associated with an inherent abnormality of the pigment epithelium, and the mechanism of aqueous outflow obstruction is believed to involve accumulation of the pigment granules in the trabecular meshwork, followed by denudation, collapse, and sclerosis of the trabecular beams. Current management includes standard antiglaucoma drugs, laser trabeculoplasty, and filtering surgery, although research suggests the possibility of earlier intervention with medication or surgery to arrest the pigment dispersion and reverse or prevent the secondary glaucoma.

Trabeculotomy combined with phacoemulsification and implantation of intraocular lens for primary open-angle glaucoma.

Retrospective study examined the surgical effects of lowering intraocular pressure of trabeculotomy combined with phacoemulsification and implantation of an intraocular lens. Included in the retrospective study were 96 eyes of 64 patients with primary open-angle glaucoma. Preoperative mean IOP was 25.6 mmHg. At final examination, the IOP was well-controlled at 21 mmHg or lower without medications in 32 of 96 eyes. In another 62 eyes, the IOP was well-controlled with antiglaucoma medications. The postoperative IOPs were in the high teens after surgery. The life table analysis using Kaplan-Meier methods showed that the success probability after phacoemulsification and implantation of intraocular lens, combined with trabeculotomy (PIT)-I and PIT-II, were 93.9% and 82.6% at 4 years, respectively. Postoperative visual acuity improved by more than two lines in 79 of the 96 eyes. In no case was the visual acuity decreased by more than two lines. Deterioration of the visual field was found in 4 eyes. There were no complications such as shallow anterior chamber, choroidal detachment, malignant glaucoma, hypotonic maculopathy, and endophthalmitis. This triple procedure should be performed in the early stages of glaucoma. Trabeculotomy is thought to relieve the resistance to aqueous outflow by mechanical cleavage of the trabecular meshwork and the inner layer of Schlemm's canal. This technique leads to aqueous outflow the from the opening of the internal trabecular meshwork to the collector channel. For this reason trabeculotomy was developed by a number of surgeons. Recently, however, trabeculotomy has not been selected for those patients with advanced stages of primary open-angle glaucoma because of the disadvantages such as transient intraocular pressure (IOP) elevation several days after surgery and somewhat higher levels (18 mmHg) of postoperative intraocular pressure (Fig. 1). To avoid the IOP spike (transient IOP elevation) after trabeculotomy, we reported previously that the new technique of trabeculotomy combined with outer sclerectomy was a useful surgical option. The results of our previous study indicated that the postoprative intraocular pressure levels after combined trabeculotomy and outer sclerectomy were significantly lower than that of trabeculotomy alone. On the other hand, trabeculotomy with mitomycin C is currently the standard filtration procedure for glaucoma. This technique, however causes severe postoperative complication such as hypotonic maculopathy, bleb leakage and late bleb infection. The major advantages of trabeculotomy preclude these severe complications resulting from creating progressive filtration of aqueous humor from the anterior chamber to the subconjunctival space. The recent advance of small-incision phacoemulsification procedure prompted phacoemulsification and implantation of intraocular lens and trabeculotomy. The theoretical advantages of smaller scleral, conjunctival incision, reduced stimuli to wound healing, and inflammation, could improve long-term IOP control in patients with glaucoma. Therefore several reports have been published on the surgical outcomes of combined trabeculotomy and modern phacoemulsification. These reports suggested that the combined trabeculotomy and a small-incision with intraocular lens implantation is effective in controlling IOP in patients with glaucoma.

Kinetics of phagocytosis in the normal canine iridocorneal angle.

Microspheres of 3 different sizes were infused separately into the eyes of dogs with normotensive and hypertensive intraocular pressures. Latex spheres (0.5 or 1.0 micron) or 3.0 micron plastic spheres were added to Ringer's solution with 6% gelatin. Initially, this mixture was injected into the anterior chamber of dogs with intraocular pressures of 20, 50, or 75 mm of Hg. After 10, 20, 30, 60, or 90 minutes had elapsed, the dogs were euthanatized and the gelatin was hardened. Tissues were subsequently studied by light and transmission electron microscopies. Phagocytosis of the 0.5 and 1.0 micron spheres by trabecular cells was first detected within 10 minutes and within 20 minutes for the 3.0 microns spheres. Migration of cells in the corneoscleral trabecular meshwork was observed after 30 minutes. Phagocytosis was less active at hypertensive pressures and had larger sphere sizes. Microspheres in the uveal trabecular meshwork were ingested mostly by macrophages and polymorphonuclear leukocytes.

Mapping of Fc gamma receptors in the human and porcine eye.

By using an immunohistochemical technique, we detected a strong reaction product for Fc gamma receptors in the keratoconjunctival epithelium, uveal trabecular meshwork, optic nerve, and muscle walls of the retinal and ciliary vessels of infant human eyes; moderate staining was seen on the corneal endothelium, corneoscleral trabecular meshwork, non-pigmented ciliary epithelium, ciliary muscle fibers, endothelial lining of ocular blood vessels, as well as the anterior region of Muller cells, the outer nuclear layer, and pigment epithelium of the retina. In aged human and porcine eyes, the same structures stained somewhat less intensely than did those in infant human tissues. Primary cultures of porcine trabecular cells demonstrated a more intense immunoreaction product than did tertiary cultures. We discuss the possible role of Fc gamma receptors in various ocular inflammatory conditions and the implications of age-dependent expression by human trabecular cells in the pathogenesis of open-angle glaucoma.

Posterior polymorphous corneal dystrophy: a disease characterized by epithelial-like endothelial cells which influence management and prognosis.

This thesis contains a clinical and laboratory summary of findings in PPMD and, for the first time, reports the results of a large series of patients who underwent keratoplasty surgery. Posterior polymorphous dystrophy is bilateral and autosomal dominantly inherited. Slit lamp findings include corneal edema in the more advanced cases, calcific and lipid degenerative changes in severe cases, band-like lesions at the level of Descemet's membrane, localized or diffuse thickenings of Descemet's membrane, posterior corneal vesicular-like lesions, islands of abnormal cells surrounded by normal-appearing endothelial cells. Iridocorneal adhesions ranged in severity from fine or broad-based adhesions seen only on gonioscopy to large iridocorneal adhesions often associated with a glass-like membrane that are seen easily by slit lamp examination. All patients with broad-based iridocorneal adhesions have elevated intraocular pressure. Some patients have elevated pressure but no adhesions. Laboratory examination of corneal, iris, and trabecular meshwork tissue from patients undergoing penetrating keratoplasty and filtering operations reveals an abnormal endothelial cell layer covering the posterior cornea and growing across the trabecular meshwork and onto the iris. Although this tissue contains a variety of cells, the most prominent type is an epithelial-like cell. Extensive laboratory studies demonstrate features in common between the epithelial-like cells and normal epithelium. These include a multilaminar pattern, desmosomal junctions, microvillous projections, cytoplasmic keratin, sparse mitochondria, and rapid growth in tissue culture. These cells appear to determine the management and prognosis of patients with PPMD undergoing surgery. Twenty-two corneal transplants were performed on 20 eyes of 13 patients with PPMD. Their ages ranged from 11 to 77 years. The follow-up time after keratoplasty averaged 4.75 years. Nine grafts (41%) failed. Two failed because of an endothelial rejection, three because of glaucoma, one because of a retrocorneal membrane, two from both a retrocorneal membrane and glaucoma, and one from both an endothelial rejection and glaucoma. Thus, glaucoma was involved in six of the nine failures (27% of all grafts). Clinically visible retrocorneal membranes formed in 4 of the 22 grafts. All four eyes had preoperative slit lamp-visible iridocorneal adhesions. The factor that most prominently influences keratoplasty prognosis is the presence of iridocorneal adhesions.(ABSTRACT TRUNCATED AT 400 WORDS)

Phagocytosis of latex microspheres by bovine meshwork cells in culture.

The ability of cultured bovine trabecular meshwork cells to phagocytose latex microspheres was studied by electron microscopy. In addition, the incorporation of 125I gamma-globulin-coated microspheres by the meshwork cells was monitored. The cultured cells, on the basis of quantitation by scanning electron microscopy, did not show significant preference between biotic particles (gamma-globulin-coated microspheres) and abiotic particles (uncoated microspheres). Surface microvilli appeared to be involved in the initial stages of phagocytosis; indeed a significant negative correlation was demonstrated between the incidence of microvilli and the commitment to phagocytosis of preconfluent cells. Two weeks postconfluent cells were shown to be significantly less phagocytic than preconfluent cells. The importance of these findings in terms of the behaviour of meshwork cells in vivo is discussed.

Visualization of hyaluronic acid in the anterior segment of rabbit and monkey eyes.

Hyaluronic acid was visualized using a new histochemical technique: the hyaluronic acid-binding region of the cartilage proteoglycan was isolated, linked with biotin and used in histological sections as a ligand for hyaluronic acid. Staining was performed using the avidin-biotin-peroxidase technique. The presence of hyaluronic acid in the anterior segments of rabbit and monkey eyes was studied in fresh-frozen, as well as in fixed paraffin sections; addition of cetyl-pyridinium chloride to the fixative was essential. In both species intense staining was seen in the stroma of the conjunctiva, the connective tissue of the ciliary processes, and on the apical membranes of the corneal endothelium. Species differences were observed in both conventional and unconventional outflow pathways. In rabbits, the tissue located directly adjacent to the aqueous plexus and the connective tissue surrounding the poorly developed ciliary muscle were stained. In monkeys, only the anterior, non-filtering part of the trabecular meshwork and the outer wall of Schlemm's canal showed intense staining in paraffin sections. In frozen sections, the inner wall (cribriform region of the trabecular meshwork) was also stained. The spaces between the anterior part of the ciliary muscle bundles remained almost unstained.

Pigment dispersion with elevated intraocular pressure after AcrySof intraocular lens implantation in the ciliary sulcus.

A 45-year-old white woman had phacoemulsification with intraocular lens (IOL) implantation. The surgery was routine except for a linear tear in the posterior capsule; there was no disruption of the anterior vitreous face. After residual soft lens matter was removed, an AcrySof IOL was placed in the ciliary sulcus. One month postoperatively, the patient presented with an intraocular pressure (IOP) of 30 mm Hg and signs of pigment dispersion with 360 degrees of heavy pigmentation of the trabecular meshwork and iris transillumination defects. Intraocular pressure was controlled with a topical beta-blocker. The optic disc appearance and visual field remained normal, but the uniocular hyperpigmentation of the trabecular meshwork was still present. We hypothesize that the sharp square edge of the AcrySof IOL increases the risk of a chafing effect on the posterior iris pigment and advocate that this IOL be placed in the capsular bag and ideally have 360 degrees of protective overlapping of the anterior capsule over the edge of the optic.

Facility changes mediated by cAMP in the bovine anterior segment in vitro.

The aim of this study was to investigate the influence of substances that increase intracellular cAMP levels on the aqueous humor outflow facility (C) of isolated bovine anterior segments. Anterior segments were perfused in vitro at a constant pressure of 10 mmHg for 270 min with a general protocol as follows: 90 min control perfusion with DMEM, 90 min of experimental perfusion with DMEM containing the test drug(s), and 90 min of postdrug-perfusion with DMEM. C was calculated as the ratio between the rate of medium inflow (microliter/min) and the perfusion pressure (mmHg). Anterior segments can be perfused in vitro for up to 5 hr without significantly modifying their C. The addition of epinephrine, forskolin, dibutyryl-cAMP or isobutylmethylxanthine to the control perfusion medium elicited a significant increase of C. If, during isobutylmethylxanthine perfusion, forskolin or epinephrine was added, C increased significantly. Finally, perfusion with indomethacin prior to addition of epinephrine prevented the increase of C induced by epinephrine. Epinephrine, the adenylate cyclase activator forskolin, the cAMP analog dibutyryl-cAMP, and the phosphodiesterase inhibitor isobutylmethylxanthine all increase aqueous facility. It seems reasonable to suspect that the cAMP system is involved in epinephrine's effects on bovine trabecular meshwork cells. Moreover, the complete inhibition by indomethacin of the outflow facility increase induced by epinephrine suggests that prostaglandins may be involved in the outflow facility mechanisms related to adrenoreceptor stimulation of trabecular meshwork cells.

Canine goniodysgenesis-related glaucoma: a morphologic review of 100 cases looking at inflammation and pigment dispersion.

PURPOSE: To investigate the role of pigment dispersion and inflammation in the pathogenesis of goniodysgenesis-related glaucoma (GDRG). PROCEDURES: Cases of GDRG were selected when the duration of the disease was specified and there was not any confounding pathology. Cases were grouped into < or = 7-day (acute), and > 7-day (chronic) durations, based on the time required to effect end-stage retinal damage. Acute cases were further divided into < 4-day and 4-7-day groups to assess peracute changes. Slides were evaluated for four individual signs of pigment dispersion: segmental loss of posterior iris pigment epithelium, clumping of posterior iris pigment epithelium, pigmented cells in the trabecular meshwork or anterior chamber and preferential settling of pigmented cells in the ventral aspect of the iridocorneal angle. Slides were also evaluated for the presence of neutrophils and/or lymphoplasmacytic cells in the trabecular meshwork (TM). Differences between groups were analyzed statistically. RESULTS: Of 100 cases evaluated, 34 were < or = 7-days (acute) (14 < 4-day and 20 4-7-day) and 66 were > 7-days (chronic) in duration. Of all globes examined, 96% had at least one sign of pigment dispersion, with no significant difference between groups. Two or more signs of pigment dispersion were present in 76% of all globes. The 4-7-day group was significantly more likely than the < 4-day group to have at least two signs. The difference was not significant between < or = 7- and > 7-day groups. Neutrophils were present in the TM of 86% of < 4-day and 50% of 4-7-day cases. Cases in the < or = 7-day group were significantly more [corrected] likely than > 7-day cases to have neutrophils in the TM, with 65% and 17% [corrected] positive cases, respectively. Lymphoplasmacytic inflammation was present in 53% of all cases, with no significant difference between groups. Cases in the < or = 7-day group were significantly more likely than > 7-day cases to have both types of inflammation. CONCLUSIONS: Our results indicate that both acute inflammation and pigment dispersion may be key factors in the pathogenesis of GDRG. Pigment dispersion is prevalent at all time points and increases during the first 7 days. The finding of iris pigment epithelial loss supports the theory that pupillary block associated with iris-lens touching may be important in the pathogenesis of GDRG.
