Haemophilus influenzae type b polysaccharide-tetanus protein conjugate vaccine does not depress serologic responses to diphtheria, tetanus or pertussis antigens when coadministered in the same syringe with diphtheria-tetanus-pertussis vaccine at two, four and six months of age.

The safety and immunogenicity of a vaccine against Haemophilus influenzae type b consisting of purified polyribosylribitol phosphate conjugated to tetanus toxoid (PRP-T) were evaluated in 277 Chilean infants who were randomly assigned to one of three treatment groups: Group A, PRP-T mixed with diphtheria-tetanus-pertussis (DTP) vaccine in a single syringe and given as a single inoculation in one arm and placebo in the other arm; Group B, PRP-T given in one arm and DTP in the other arm; Group C, DTP given in one arm and placebo in the other. Infants were immunized at 2, 4 and 6 months of age and examined daily for 4 days after each immunization. Serum PRP antibodies; tetanus, diphtheria and pertussis antitoxin; pertussis agglutinins; and antibodies to Bordetella pertussis filamentous hemagglutinin were measured at baseline and 2 months after each dose. PRP-T was well-tolerated. After three doses of PRP-T vaccine 100% of infants attained PRP antibody concentrations > or = 0.15 micrograms/ml and 96 to 99% achieved high anti-PRP concentrations (> or = 1.0 micrograms/ml). The post-third dose anti-PRP geometric mean titer was high (6.94 micrograms/ml) in infants who were given PRP-T combined with DTP, although it was somewhat lower than the geometric mean titer of the group who received PRP-T in a separate arm (9.93 micrograms/ml) (P not significant).(ABSTRACT TRUNCATED AT 250 WORDS)

Safety and immunogenicity in young infants of Haemophilus b-tetanus protein conjugate vaccine, mixed in the same syringe with diphtheria-tetanus-pertussis-enhanced inactivated poliovirus vaccine.

Because inactivated poliovirus vaccine (IPV) and Haemophilus influenzae b vaccine are advised in many programs and may be incorporated further in other programs, we undertook a study to determine whether the administration of a tetravalent preparation of diphtheria-tetanus-pertussis-IPV mixed in one syringe with tetanus-conjugate H. influenzae b vaccine (DTP-IPV-PRPT) is associated with increased reactogenicity or interference with immunogenicity of individual vaccine components. In a placebo-controlled, double blind study, a total of 161 infants were enrolled (80 DTP-IPV-PRPT and 81 DTP-IPV-placebo). Vaccine was administered at 2, 4 and 6 months of age. Oral poliovirus vaccine was added at 7 months of age and a booster of oral poliovirus vaccine and DTP-IPV was also administered at 12 months of age, according to the policy in Israel. Local and systemic side effects were similar in both groups except for irritability after the second dose and use of acetaminophen which we observed slightly but significantly more often in the DTP-IPV-PRPT recipients. After the third dose the geometric mean titers of anti-polyribosyl-ribitol phosphate antibodies were 3.7 and 0.05 micrograms/ml in the PRPT and placebo groups, respectively (P < 0.001). Higher tetanus antitoxin titers were observed among recipients of DPT-IPV-placebo (1.1 IU/ml vs. 0.7 IU/ml, P = 0.003). A similar trend was found for pertussis agglutinin titers (93.4 vs. 65.4, P = 0.054). No difference was observed for anti-diphtheria toxoid and poliovirus 1, 2, and 3.(ABSTRACT TRUNCATED AT 250 WORDS)

HIV incidence among New Haven needle exchange participants: updated estimates from syringe tracking and testing data.

This article provides updated estimates of the rate of new HIV infections among participants in New Haven's legal needle exchange program from syringe tracking and testing data. We previously reported that based on data collected from November 1990 through May 1992, the maximum likelihood incidence rate was zero with a 95% confidence interval of 0-10.2 new infections per 100 drug injectors per year. Expanding this data set through August 1993, the same statistical methods yield a maximum likelihood estimate of 1.63 new infections per 100 drug injectors per year with a 95% confidence interval of 0-7.2. Given these data, the null hypothesis of no new infections cannot be rejected, providing further support for the efficacy of New Haven's needle exchange program.

Drug injecting and syringe use in the HIV risk environment of Russian penitentiary institutions: Qualitative study.

BACKGROUND: Evidence highlights the prison as a high risk environment in relation to human immunodeficiency virus (HIV) and hepatitis C virus (HCV) transmission associated with injecting drug use. METHODS: We undertook qualitative studies among 209 injecting drug users (IDUs) in three Russian cities: Moscow (n = 56), Volgograd (n = 83) and Barnaul in western Siberia (n = 70). RESULTS: Over three-quarters (77%) reported experience of police arrest related to their drug use, and 35% (55% of men) a history of imprisonment or detention. Findings emphasize the critical role that penitentiary institutions may play as a structural factor in the diffusion of HIV associated with drug injection in the Russian Federation. While drugs were perceived to be generally available in penitentiary institutions, sterile injection equipment was scarce and as a consequence routinely shared, including within large groups. Attempts to clean borrowed needles or syringes were inadequate, and risk reduction was severely constrained by a combination of lack of injecting equipment availability and punishment for its possession. Perceptions of relative safety were also found to be associated with assumptions of HIV negativity, resulting from a perception that all prisoners are HIV tested upon entry with those found HIV positive segregated. CONCLUSION: This study shows an urgent need for HIV prevention interventions in the Russian penitentiary system.

Administration of growth hormone by disposable preloaded syringes.

The convenience of Genotropin KabiQuick, a novel single-use disposable injection system for growth hormone (GH) administration, was studied in 20 GH-treated children aged 8-16 years. The convenience of the device and tolerance to it were assessed by means of a questionnaire, which was completed by the patients and their families after a treatment period of 2 weeks. The families were asked to compare the new device with the conventional system of GH administration, involving the use of standard ampoules and ordinary syringes. The majority of patients considered the new device superior to the conventional system; 85% of patients found reconstitution easier and 70% found dosing easier. Sixty-five per cent of patients expressed a preference for using Genotropin KabiQuick for future therapy. It is concluded that Genotropin KabiQuick is well tolerated and that the majority of patients prefer it to conventional injection systems for GH administration.

Bilateral syringeal interaction in vocal production of an oscine bird sound.

The vocal organ, or syrinx, of oscine birds has two parts, each of which has generally been presumed to operate independently of the other. A significant counter-example is now demonstrated in the production of a common vocalization by the black-capped chickadee (Parus atricapillus), in which the two acoustic sources interact in a nonlinear fashion. This bird produces a sound with multiple frequency components that are heterodyne products resulting from cross-modulation between two signals, thus providing evidence that avian phonation can involve cooperative coupling between the two syringeal sources.

Multiple syringe inoculator for agar plates.

The design and operation of a manually operated multiple syringe inoculator was described. Either 9 or 21 inoculations of constant volume could be made simultaneously. Up to 100 plates could be inoculated in 15 min with excellent reproducibility. No contact occurred between the inoculating needles and the agar surface. Construction was simple and inexpensive, with minimal maintenance.

Investigation by syringe method of effect of tampons on production in vitro of toxic shock syndrome toxin 1 by Staphylococcus aureus.

A syringe method was designed to test the effect of tampons on the growth of three toxic shock syndrome-associated strains of Staphylococcus aureus and their in vitro production of toxic shock syndrome toxin 1 (TSST-1) under different conditions. Various amounts of TSST-1 were recovered from different tampons inoculated with these strains. Generally, the addition of 10% porcine blood to the growth medium, incubation in the presence of 5% CO2, or the combination of these two factors resulted in the stimulation of TSST-1 production.

Gowers' syringal haemorrhage.

The second case of Gowers' syringal haemorrhage, successfully treated by surgery, and the first such case in a child, is described. It is suggested that the very slow development of haematomyelia within an existing hydrosyringomyelia may originate from a torn intraspinal vein, deprived of its normal neural and glial support.

Use of a portable syringe pump for glucagon administration in abdominal computed tomography.

A portable syringe pump was used to administer glucagon by intravenous infusion in order to achieve bowel paresis during abdominal computed tomography. This technique produced a reduction in glucagon dosage and an improvement in scan quality when compared with intermittent intravenous injection. A smaller reduction in dosage with little change in scan quality was achieved with intramuscular infusion.

Fine-needle aspiration biopsy for cytopathologic analysis: utility of syringe handles, automated guns, and the nonsuction method.

The performances of seven techniques and devices used with 22-gauge needles to obtain biopsy specimens for cytologic analysis were compared by means of single-blinded evaluation with an objective, previously published grading scheme. A total of 420 specimens were obtained from 10 fresh human cadavers (42 specimens per cadaver), including 30 hepatic, 20 renal, and 10 pancreatic specimens per technique or device. No statistical differences existed in the liver, kidney, or pancreas or in the combined data in the performance of the aspirator gun, syringe holders, vacuum needle, and end-cut gun versus the manual aspiration biopsy technique performed with a 22-gauge Chiba needle. However, nonaspiration, fine-needle capillary biopsy (FNCB) performed statistically significantly worse than any other technique or device in the kidney and pancreas and in comparison with the overall combined data. In the liver, no statistically significant difference existed in the overall performance of FNCB versus conventional aspiration biopsy, but the amount of cellular material obtained with FNCB was statistically significantly less.

Computerized determination of pneumotachometer characteristics using a calibrated syringe.

A computerized method has been developed to determine the conductance characteristics of pneumotachometers. Conductance values of the flowmeter, which correspond to all pressure values, can be determined by a weighted averaging technique, when multiple strokes of a precision 3-liter calibrated syringe are used. The conductance values then allow the measured differential pressures to be converted, point by point, into flows. The accuracy of measured volumes is within +/- 0.5% for a 100-stroke calibration process, and calibration with 50 strokes provides +/- 1% accuracy. The method improves the accuracy of the on-line measurements of ventilation. O2 consumption, and CO2 production during exercise.

Performance of the Atom 235 syringe infusion pump under hyperbaric conditions.

Inotrope-dependent patients may become haemodynamically unstable during treatment in the hyperbaric chamber. One possible cause is altered performance of infusion devices. We tested six Atom 235 syringe infusion pumps under hyperbaric conditions of 2.8 atmospheres absolute in a multiplace chamber using a gravimetric method. There was a small but significant reduction in infused volume during a thirty-minute period including compression time. We concluded that altered inotrope delivery may explain at least some of the haemodynamic instability sometimes seen, although a physiologic effect of hyperbaric treatment cannot be excluded.

Needle and syringe sharing practices of injecting drug users participating in an outreach HIV prevention program in Tehran, Iran: a cross-sectional study.

HIV infection rates have reached epidemic proportions amongst injecting drug users (IDUs) in Iran. Although a number of community-based interventions have being implemented in the country, there is little information on the risk behaviors of IDU participants in these programs. This cross-sectional report aimed to compare the risk behaviors of injecting drug users with differential exposure rates to an HIV outreach program in Tehran, Iran. Results indicated that shared use of needle/syringe in the past month was significantly lower among IDUs who received estimated > or = 7 syringes per week than those who did not [adjusted odds ratio (OR) = 14.36, 95% confidence interval (CI) 2.30-89.56]. While the effectiveness of this outreach program needs further evaluation through a longitudinal investigation, our preliminary findings suggest that the outreach program in Tehran may have been beneficial in reducing direct sharing among those who received more than several needles/syringes from the program.

[Development of syringe-type off line pre-column and simultaneous quantitation of four xanthine derivatives (caffeine, theobromine, theophylline and paraxanthine) in serum].

A new syringe-type minicolumn, called Extrashot-Silica (EXS-Silica), containing diatomaceous earth granules was described. The EXS-Silica differs from the conventional pretreatment column. Using the EXS-Silica we can execute the simultaneous extraction-injection to HPLC, column. Therefore, an analysis using the EXS-Silica is an easier and faster method than the general HPLC analysis method. In this study, we carried out the simultaneous determination of four xanthine derivatives, such as caffeine, theobromine, theophylline and paraxanthine, in serum specimens. We used dichloromethane containing 4% ethanol (v/v) for the extraction-injection and water-acetic acid-ethanol-dichloromethane (0.2:0.2:4:95.6, v/v) for the mobile phase of HPLC. The eluent was monitored with a UV detector at 275 nm. A linear relationship between the amount of drug and the peak height was confirmed in the range of 1-40 micrograms/ml for the above-mentioned four xanthine derivatives in the serum. When a 5 microliters aliquot of the serum was subjected to this method, the observed detection limits of the drug were far less than therapeutic concentrations. The analytical accuracy of our method was finally confirmed by comparing the obtained analytical data by the new method with those obtained using the fluorescense polarization immunoassay method. Serum concentrations of theophylline obtained by these two methods correlate satisfactorily. Except for minor modifications in the injector, the existing liquid-chromatographic equipment can be used.
