[Improved sternotomy closure with polyester tapes and conventional wires].

In the purpose of reducing the occurrence of postoperative sternal dehiscence, improved technique of closure with supplemental use of polyester tapes was performed. To add to the conventional closure with wires, two polyester tapes were used around the body of sternum through the intercostal spaces just before approximation of sternum. The sternum was firmly fixed at the body and the tapes will prevent cut sternum by the wires and disruption of the wires themselves. In our series of recent 155 cases with this technique of closure, no sternal dehiscence occurred.

[A case of chylomediastinum following open heart surgery via median sternotomy].

The case was a 17-year-old woman on whom open heart surgery was performed for ASD. Anterior mediastinal drainage alone revealed chyloid liquid, leading to diagnosis as chylomediastium. Ramification of thoracic duct and direct injuries of thymus and lymphoduct were presumed to be responsible for the occurrence of this complication. Operative treatment was positively performed because of its unimprovement by conservative treatment. Re-operation led to its healing in a comparatively short period of time. Idea of disease, cause of occurrence, and plan of treatment of this complication were examined for reporting below.

Coronary steal through anomalous internal mammary artery graft. Treated by ligation without sternotomy.

A patient who had undergone adequate coronary revascularization with a left internal mammary artery graft to the left anterior descending coronary artery and with saphenous vein grafts to the right coronary artery and to the 1st and 2nd obtuse marginal branches presented with early-onset recurrent angina. A repeat angiogram showed an abnormally large branch arising from the very proximal segment of the left internal mammary artery and supplying the whole lateral chest wall via many intercostal tributaries. Relief of symptoms was achieved by ligation of this branch, and the patient remains symptom free more than 6 years after the procedure.

[New method for closure of median sternotomy: usefulness of barbed staple].

In this report, 15 cases using straight staple (group A) and 35 cases using barbed staple (group B) were compared for the purpose of investigating efficacy for median sternal closure. The staples were placed by a Stapilizer powered metaphyseal staple system following partial transsternal fixation with two wires. As a means of assessing the status of back out, which is a major cause of poor fixation, the back out rate (BOR) was measured on lateral sternal radiographies. The average BOR was found to be 33.8% in group A and 21.2% in group B (p < 0.001). Barbed staples seemed to be more useful for sternal fixation than straight staples. This method of applying barbed staples had the advantages of speed, ease of insertion and noninvasion of the retrosternal region. It should be recommended in cases with severe adhesion of the retrosternal region after coronary operation.

[Surgical treatment for chronic constrictive pericarditis through a combined median sternotomy and left anterolateral thoracotomy].

A 61-year-old and a 64-year-old male patients with chronic calcified constrictive pericarditis underwent pericardiectomy through a combined midsternotomy and anterolateral thoracotomy. X-ray films and CT scans of the chests demonstrated prominent calcification of the pericardium. At cardiac catheterization, the right atrial and ventricular enddiastolic pressures were elevated and the typical dip and plateau pressure curves were found in the right ventricle. A combined midsternotomy and left anterolateral thoracotomy gave a good operative view for the surgery. The post-operative course was uneventful in both patients. This combined approach has greater ease of decorticating the lateral to posterior surfaces of the left ventricle than a simple conventional approach.
