[Misoprostol and induction of labour].

UNLABELLED: Misoprostol (Cytotec) is a synthetic analogue of PGE1, not yet registered for Obstetric practice. Vowing to its low cost and high efficacy there is marked interest towards this medicament. MATERIALS AND METHODS: 47 patients with induction of labour are distributed into two groups: Gr.A (n = 25), receiving Misoprostol and Gr.B (n = 22) with Oxytocin in perfusion. RESULTS: Protocol of treatment with PGE, includes 50 microg per os every 2 hrs. and in Gr. B standard solution of Oxytocin 5 IU. Before the beginning of induction Bishop Score and NST for 20 mins. is evaluated. The interval induction--delivery is much shorter in Gr.A, with a median of 479.00 +/- 105.44 mins., in comparison with 727.00 +/- 88.87 mins. in Gr.B. Uterine tachysystole is registered more frequently in Gr.A--8%, with 4.54% in Gr.B. DISCUSSION: With receivers of Misoprostol, 76% deliver within 24 hrs. of induction. And there is no significant difference in the recorded monitor data for FDS and Apgar score of the neonate as well as the method of delivery. CONCLUSION: Oral Misoprostol is an effective method of induction of delivery with low Bishop Score. But there is a lack of sufficient data regarding the optimal regime and safety and with incorrect dosing may lead to unacceptable high incidence of complications.

[Computerized three-dimensional reconstruction and morphologic measurement of adult acetabulum bone structure].

OBJECTIVE: To set up three-dimensional reconstruction of acetabulum bone structure from CT scanned image in computer with software of CAD and study quantitatively the morphologic features of the acetabulum. METHODS: Through the process of CT scanning, and edge recording of the CT image, we made use of CAD software and Unigraphics software to reconstruct the 40 normal acetabulum bones for the radius of acetabulum (R), minimum thickness of medial wall of acetabulum (L), depth of Harris fossa (D) and maximum opening rim width in cross-sectional plane (W). RESULTS: The average R was 30.48 +/- 2.05 mm. The average L was 2.35 +/- 1.13 mm. The average D was 5.71 +/- 1.21 mm. The average W was 63.06 +/- 2.05 mm. There was a linear relationship between the R and the W, but no correlation between the R, the L and the D. CONCLUSIONS: There was a significance linear relationship between the R and the W in normal adult acetabulum. However no correlation between the R, the L and the D.

[Assessment of Bishop's pelvic score in induction of labor prognosis].

Aim of the present mixed prospective and retrospective study is to determine the prognostic significance of the 5 parameters of the pelvic score of Bishop for the continuity and the final outlet of induction of labor. Based on the given results the authors conclude that the dilatation of the cervix is a good prognostic factor for the nulliparous and bad prognostic factor for multiparous. The effacement of the cervix is significant for the outlet of the induction regardless of parity. The level of the presenting part is a valuable criterion for the nulliparous when the pelvic is well assessed but for the multiparous it has no prognostic value. The firmness of the cervix has a bad prognostic value regardless of parity. The position of the is a good prognostic factor for the outlet of the induction but has no value prognosis of the continuity of the induction.

[Potentialities of intravital methods of the evaluation of morphometric pelvic characteristics in adult persons].

Comparative assessment of posthumous and intravital methods of morphometric study of the pelvis in adult persons was performed in 110 embalmed anatomical specimens and in 81 patient. It was proved statistically, that magnetic resonance tomography and spiral computer tomography (SCT), performed following definite algorithm, appear to be highly informative methods for the evaluation of pelvimetric parameters. They permit to define various osseous landmarks with high precision and to perform the measurements of linear metric pelvic parameters. On the basis of the data obtained, various pelvimetric indices that characterize bony pelvis shape, could be reliably calculated. Moreover, SCT together with shade surface reconstruction in different projections permits to demonstrate visually the 3D shape of the pelvis or its separate fragments and to evaluate the symmetry, presence or absence of bone structural deformations.

[Predicting the outcome of labor in women with contracted pelvis].

The authors suggest that the coefficient, representing the ratio between the sizes of the small pelvis section and fetal head section, be used for the prediction of labor outcome for the fetus in women with a contracted pelvis. When the coefficient values K1-K4 were lower than 1.32, 1.35, 0.68, and 0.79, more than 90% of the newborns developed signs of cerebral circulation disorders, this prompting the use of cesarean section for this group of pregnant women. Timely correction of the policy of labor management with due consideration for the anatomical features of the pelvis helped reduce the perinatal mortality in multigravidae to 5/1000, mainly at the expense of carrying out planned cesarean section and monitoring in labor.

[Cesarean section in an 11-year-old girl].

We report about a term pregnancy in an eleven year old girl (51 kgs, 152 cm). At the end of the second stage of labour, the pregnancy was terminated by caesarean section for relative disproportion. The child was a healthy boy (3080 g, 53 cm). The mother accepted the child in spite of severe problems. She left the hospital on the 13th postoperative day.

Cord blood acid-base values in breech-presenting infants born vaginally.

The purposes of our investigation were to establish normal umbilical cord blood acid-base values for breech-presenting fetuses born vaginally and to compare our findings with values from cephalic-presenting fetuses delivered vaginally (control group). Our study population consisted of 30 term fetuses with frank breech presentation who fulfilled selective criteria for trial of labor and subsequently had uncomplicated vaginal breech deliveries. The mean (+/- SD) gestational age of the study group was 38 weeks, 5 days (+/- 1 week, 4 days); the mean birth weight was 3178 +/- 321 g and the median 1- and 5-minute Apgar scores were 7 (range 2-9) and 8 (range 7-9), respectively. Thirteen infants had assisted breech deliveries and 17 had assisted breech deliveries with forceps applied to the aftercoming head. We obtained umbilical arterial and venous blood samples for acid-base analysis from both the study and control infants. The results for the study group were: umbilical venous pH 7.32, PCO2 37.88 mmHg, PO2 22.88 mmHg, HCO3 25.92 mmol/L, and base deficit -4.90; umbilical arterial pH 7.23, PCO2 51.20 mmHg, PO2 18.85 mmHg, HCO3 19.42 mmol/L, and base deficit -5.86. Comparison of the subjects' umbilical cord blood acid-base values with those of controls demonstrated a statistically lower cord arterial pH (P = .0001) and a higher arterial PCO2 (P = .028). We conclude that umbilical cord arterial blood acid-base values of the breech-presenting fetus born vaginally without complications differ from established normative umbilical cord blood gas values based on data obtained from uncomplicated cephalic-presenting fetuses delivered vaginally.

Radiographic assessment of sacral screw placement.

Sacral screw penetration of the anterior sacral cortex runs the risk of injury to neural, vascular, and visceral structures. This study examined the accuracy of the standard anteroposterior (AP) and lateral roentgenographic views as compared to a modified pelvic inlet (MPI) view in determining sacral screw penetration and angulation. Ten human cadaveric specimens were studied. Screw depth and screw angulation in the transverse plane were best evaluated with the MPI view. Screw penetration was overestimated by 0.4 mm (+/- 2.2 mm) on the MPI view, whereas the lateral view overestimated screw penetration by 2.8 mm (+/- 4.7 mm). Screw angulation in the sagittal plane could only be evaluated by the lateral view. The standard anteroposterior view provided little useful information on sacral screw orientation. To study all parameters of sacral screw placement, the radiographic series should include a modified pelvic inlet view and a lateral sacral view.

A comparison of the ability of a sonographically measured biparietal diameter and the last menstrual period to predict the spontaneous onset of labor.

To study whether the estimated date of confinement (EDC) for women with optimal menstrual histories is predicted best from the last menstrual period (LMP) or biparietal diameter (BPD), 2320 women had the EDC estimated by both LMP and BPD measured sonographically in the second trimester. Those who delivered after the spontaneous onset of labor are reported here. Compared with the EDC calculated from the LMP, the EDC calculated from the BPD was postponed more than 1 week for 406 women (17.5%) and advanced more than 1 week for 56 (2.4%). For these women, the BPD estimate of EDC turned out to be closer to the day of delivery in 264 cases, whereas the LMP estimate was closer in 125 (P less than .001). For 73 women, the estimates were equally good. For 80.1% of the women, the two estimates of EDC differed by 1 week or less; in these cases, the estimates were equally good in predicting the day of delivery. We conclude that the BPD is a better predictor of the spontaneous onset of labor than is the LMP.

[Symphysis-fundus measurements in Scandinavia].

Since the late 1970s symphysis-fundus measurements have been used in the Nordic countries to predict fetal growth deviation. Via a questionnaire, sent to Nordic university hospitals representative of health care areas, existing practices have been investigated. Symphysis-fundus measurements are almost invariably obtained and plotted onto nomograms as part of routine antenatal care. Five normal curves are in use showing some variation, but there are considerable differences in the selection of material and in the statistical treatment of the data. Revision of some of these curves may be required.
