We reviewed 63 patients with fractures of the distal tibial metaphysis, with or without minimally displaced extension into the ankle joint. The fractures had been caused by two distinct mechanisms, either a direct bending force or a twisting injury. This influenced the pattern of the fracture and its time to union. All fractures were managed by statically locked intramedullary nailing, with some modifications of the procedure used for diaphyseal fractures. There were few intra-operative complications. At a mean of 46 months, all but five patients had a satisfactory functional outcome. The poor outcomes were associated with either technical error or the presence of other injuries. We conclude that closed intramedullary nailing is a safe and effective method of managing these fractures.

The alternative to the anchoring of an endoprosthesis by means of cement is the biological fixation by an ingrowth of bone into the implant surface (osseointegration). We examined the implant fixation properties of titanium experimental devices with 3 different surface structures after press-fit implantation into the bony bed of 12 dogs. One third of the 48 implants had a micro-structured surface roughened by grit-blasting, one third a roughened surface with an additional macro-groove structure (combination surface), and the remaining third a porous hydroxyapatite (HA) coating. Twelve weeks after implantation the bony ingrowth was evaluated biomechanically by measuring the force required to pull out the implant from the surrounding bone and histologically by morphometric assessment of microradiographs. In the pull-out-experiment the shearing forces were significantly lower (p < 0.01) in the devices with a roughened surface in comparison to the devices with a micro- and macro-structured surface and the HA surface. Thus, there was no significant difference between the forces required to pull out the devices with the porous HA surface and those with the combination surface. The histomorphometric assessment of bone density in the immediate vicinity of the implant and the extent of the direct contact surface of the bone implant resulted in no significant difference in all 3 groups. We did not find an interposing layer of fibrous tissue at the interface. We can demonstrate that roughening of the surface in combination with a groove structure creates a better bond between implant and bone than a roughening alone.(ABSTRACT TRUNCATED AT 250 WORDS)

A 7-month-old Appaloosa foal had bilateral hind limb lameness and was unable to rise from recumbency without assistance. Lameness could be exacerbated by tarsal flexion and a firm swelling at the distomedial aspect of the tibias was associated with a marked pain response when palpated. Radiography revealed enlargement of the distal tibial metaphysis physis (compared with normal), and radiolucent fissure lines extending proximally from the physis into the metaphysis at its medial aspect. Treatment included rest and dietary adjustment. Although clinical signs initially resolved following treatment, the problem recurred when the foal was 14 months old. Further treatment included 7 months of pasture rest without forced exercise, after which the foal was reported to be clinically normal.

Distal femur imaging (A-P and lateral X-rays) after internal fixation often give only poor visualization of the exact position. A model with two anteriorly positioned cortex screws was used to show that an additional oblique X-ray, or fluoroscopy of the tip of the screw, allows much more exact determination of its length, so that it can be changed if necessary. This method is considered by the authors as a simple check of screw length measurement in "foyer fermée" procedures.
In immature long bones, radical excision of malignant tumours of the metaphysis may necessitate sacrifice of the adjacent epiphysis. To preserve the adjacent joint while allowing a safe margin of excision, we used physeal distraction before removing the tumour. From July 1984 to August 1992, we operated on 20 patients by this method. After a mean follow-up of 54 months there was no local recurrence in the epiphyseal region. Three patients had developed pulmonary metastases.
