Methodologic and logistic issues in conducting longitudinal birth cohort studies: lessons learned from the Centers for Children's Environmental Health and Disease Prevention Research.

In anticipation of the National Children's Study, lessons can be learned from the smaller birth cohort studies conducted by five Centers for Children's Environmental Health and Disease Prevention Research funded by the National Institute of Environmental Health Sciences and the U.S. Environmental Protection Agency. The populations studied are diverse in ethnicity and social class and reside in urban and rural environments. Although almost all of the centers chose to enroll participants through medical care facilities, they had to develop independent staffs and structures because of the overburdened medical care system. Some of the lessons learned by the centers include the importance of continuous funding, building community partnerships to conduct culturally appropriate research, hiring bilingual and bicultural staff from the community, prioritizing research goals, developing biorepositories to ensure future utility of samples, instituting quality control procedures for all aspects of specimen and data collection, maintaining frequent contact with study participants, ensuring ethical conduct of the research in a changing medical-legal climate, and communicating results in a timely and appropriate manner to participants and the wider community. All centers underestimated the necessary start-up time, staff, and costs in conducting these birth cohort studies. Despite the logistical complexity and added expenses, all centers emphasize the importance of studying the impact of environmental exposures on those children most at risk, those living in minority and low-income communities. These centers present barriers encountered, solutions found, and considerations for future research, with the hope that the lessons learned can help inform the planning and conduct of the National Children's Study.

The CD-ROM experience at Loma Linda: the issues of training, logistics, and creative financing.

Since the end of 1987, the Medical Center and University libraries at Loma Linda have progressed from stand-alone CD-ROM-based workstations for access to MEDLINE to networked CD-ROM access to a variety of databases. T.O.M.M.I. (Total Online Medical Material Integration) is a CD-ROM-based wide area network linking the Loma Linda University Medical Center, Loma Linda University, and other entities in Loma Linda, California. T.O.M.M.I., a microcomputer-based system, represents the implementation of IAIMS at Loma Linda. In addition to discussing the issue of "downsizing" campus-wide information networks, this article focuses on the issues of training, handling of logistical problems, and the use of creative financing.

Clinical significance of a multiple biomarker assay in patients with lung cancer. A study with logistic regression analysis.

The relative usefulness of a combination of some tumor markers, such as CEA, AFP, ferritin and NSE for the diagnosis of lung cancer was assessed by multiple logistic analysis. Serum concentration of these markers was determined in 68 patients with lung cancer (50 with NSCLC and 18 with SCLC, in 68 patients with benign lung disease and 75 normal control subjects. Ferritin proved to be the most useful in diagnosing both NSCLC and SCLC, while NSE was found to be of some help in diagnosing SCLC only. The multiple marker panel proved to be more sensitive and specific than any single marker in discriminating lung cancer from normal control tissue, but it was of limited value in discriminating malignant from benign lung disease. The results of the present study would suggest that the panel of investigated tumor markers is not of great help for the early diagnosis of lung cancer.

Evaluation of radioimmunoassays: comparison of dose interpolation calculations by four parameter logistic and spline functions.

Two computerized methods for dose interpolation calculation were compared. Generated data sets with a known coefficient of variation as well as laboratory RIA data were analysed. The four parameter logistic method, which is based on an approximation of the mass action law, performed better than the Spline method, a procedure which makes no a priori assumptions about the data. Correct weighting of the data was important for obtaining satisfactory fits. The determination of the response error relationship proved to be the most satisfactory approach in obtaining suitable weighting factors.

Hybrid coronary artery revascularization: logistics and program development.

Planning hybrid coronary artery revascularization--a combination of cardiac surgery with percutaneous procedures--requires, at first sight, a very complex logistical setup. Technical and equipment related details should be defined as early as possible in order to have time for training of all OR personnel involved. The most challenging aspect in OR-located hybrid coronary revascularization remains a very close cooperation of cardiac surgeons and interventional cardiologists. This teamwork does include indication findings and subsequent referral of multivessel coronary artery disease patients to hybrid procedures, as well as high individual flexibility of interventionalists and surgeons. The major prerequisite for this cooperation is a mutual acceptance of different revascularization approaches and the intent to combine their most striking advantages. Intraoperative graft angiography during coronary artery bypass grafting (CABG) procedures is one important step toward simultaneous hybrid coronary revascularization procedures. We describe our experience with on table angiography using a mobile C-arm for intraoperative imaging. This fluoroscopy system can in selected cases be used for simultaneous hybrid procedures.

[A logistic regression model applied to Chagas' disease]

The focus of this paper is the application of statistical models to the study of socioeconomic conditioning factors in perinatal Chagas' disease conducted in Rosario, Argentina. A case (154) and control (158) design was applied to investigate socioeconomic and cultural differences in pregnant women in Hospital Roque Sáenz Peña as to their infection status. Logistic regression models were used to evaluate the importance of antecedents linked to the infection and socioeconomic and cultural factors for infection status. For pregnant women, the importance of antecedents linked to the infection was confirmed and the women's level of schooling stood out as the predominant socioeconomic condition associated with infection. Log-linear models were used to explore the associations between certain explanatory variables. This approach pointed up the most relevant associations between such factors and Chagas' disease and provided a better understanding of the framework of relationships among them.

Logistics in acute stroke management.

Results from the two National Institute of Neurological Disorders and Stroke (NINDS) studies indicate that administration of alteplase (recombinant tissue-type plasminogen activator; rt-PA) within 3 hours of symptom onset to appropriately selected patients with acute ischaemic stroke improves patient outcome. Several factors that delay time to treatment in patients with stroke have been identified, the most important of which is probably the failure of the patient (or family member) to recognise the signs and symptoms of stroke. Once the need for help is recognised, the initial point of access to emergency medical systems should be the local emergency number (e.g. 911 in the US) rather than the family physician. Patients with suspected stroke should be evaluated and treated by a physician as soon as possible, but this will depend to some extent on the level of expertise of the attending physicians and on available resources. The NINDS-sponsored National Symposium on the Rapid Identification and Treatment of Acute Stroke has recommended ideal time goals for all hospitals that treat patients with acute stroke. These goals include 25 minutes from arrival at an emergency department to computerised tomography scan, and 60 minutes from arrival to treatment. Recommendations for enhancing the logistics of treatment for patients with stroke may involve the following: improved education programmes for at-risk populations and their families and emergency medical system personnel, identification of acute stroke as a level one emergency similar to acute myocardial infarction or trauma, and modelling of treatment algorithms accordingly, acceptance of, and commitment to, the time guidelines recommended by the National Symposium on the Rapid Identification and Treatment of Acute Stroke. Effective and safe use of alteplase will also depend on rapid access to the highest level of neurological and radiological expertise. This may require major changes in the educational curriculum of emergency department residency and ongoing continuing education programmes, and/or more intensive radiological training for neurologists and neurologists-in-training.

[Logistical function and quality control in a hospital service].

The characteristics of Logistical Function are studied, and are applied to controlling the quality of attention in a Medical service of a Hospital. Three variables were studied: number of annual admissions, average stay and DRG coefficients. The unit of time employed was the period of one year. Within the parameters that define this Function, the value K denotes the maximum capacity of yield of the Service in relation to the studied variable. The exponential factor b is the indicator of the ascending or descending course of the variable, which is qualified by the absolute value of the same, as much in one sense as in the other. It is essential, to be able to apply this procedure, that the human and material resources of the service under study do not vary during the entire period of observation.

From ideology to logistics: the organizational aspects of syringe exchange in a period of institutional consolidation.

The initial period in the establishment of syringe exchange projects is often characterized by overt conflict: between community AIDS activists, on the one hand, and public officials and political leaders who remain ideologically opposed to the introduction of measures perceived as condoning illicit drug use. In this context, professionals concerned with legitimating the new institutions of syringe exchange may sometimes neglect aspects of their everyday logistics and social organization, obscuring the important choices which have to be made to carry these initiatives forward. In particular, the contrast between formally-constituted institutions-the "storefront" or "community-based" syringe exchange programs (SEPs)-and the model of low-threshold syringe availability through pharmacies, vending machines, and user networks, is here presented not as an either/or choice but rather as a pair of complementary strategies which respond to diverse needs and target different populations. The advantages and disadvantages of each particular approach make it likely that maximum effectiveness will be achieved through a combination of every possible form of needle distribution, each tailored to specific and cultural circumstances. The case is here examined in the light of the experience of the SEPs in New York City, from their clandestine origins in 1990 through their first years of official functioning in 1992-1996.

A logistic-type curve fits pressure-diameter relationship for relaxed and contracted dog renal arteries.

In order to describe a possible effect of smooth muscle cell (SMC) activation on arterial wall distensibility, the present study derived a mathematical equation applicable to relaxed and contracted arterial walls. Pressure(P)-diameter(D) relationship of dog renal arteries was investigated in vitro under a cyclic loading and unloading process in the pressure range of 5-180 mmHg. Smooth muscle cells were activated by 10(-5)M norepinephrine. On the basis of the P-D curves obtained with fully contracted arteries, the vessel wall compliance dD/dP was assumed to be given by a second order polynomial of D, (formula; see text) The equation, including three parameters, Dmin, Dmax, and E, is integrated to yield the solution similar to the logistic curve as follows (formula; see text) where M(O) = (Dmax - D(O]/(D(O) - Dmin), and D(O) is the diameter at the point P = O. The constant, E, has the same dimension as the modulus of elasticity. The calculated P-D relationships coincided well with the experimental data for contracted and relaxed arteries. The most significant change due to wall contraction took place in the magnitude of M. This result, therefore, suggests that the parameter M is a good index of the degree of SMC contraction.

Comprehensive intravenous admixture services: logistics and quality assurance in a university affiliated teaching institution.

The role of supportive personnel, as well as the supervision of intravenous (IV) admixture compounding by staff pharmacists, should be clearly identified by the departmental manager. In doing so, the department should strive to achieve an optimal mix of professional and technical personnel with automated technology. Close attention must be paid to quality assurance in order to maintain the highest quality parenteral admixture. The logistics of comprehensive IV admixture services are described for a university affiliated teaching institution. Emphasis is made on a three-faceted approach to quality assurance, including technician training, end-product testing, and equipment maintenance.

[Risk factor of liver disorders caused by flutamide--statistical analysis using multivariate logistic regression analysis].

The antiandrogenic drug, flutamide (Odyne), is widely used in the treatment of carcinoma of prostate. It is well known that flutamide has adverse effects of liver disorders. To ascertain the risk of liver disorders before administering this drug, past history and lifestyle preferences were resurveyed in 123 patients who had been treated with flutamide. The results obtained were assessed in relation to the occurrence of liver disorders by multivariate logistic regression analysis. The incidence of liver disorders was 26% (33/123), with 64% of the disorders occurring within 9 months. The chi-square test for dependent variables revealed that three variables, i.e., body mass index, past history of liver disorders and elevated glutamic-pyruvic transaminase levels were significantly related to the incidence of liver disorders (p > 0.05). Multivariate analysis indicated that a history of liver disorders and elevated alanine aminotransferase (ALT) levels were related to a higher incidence of liver disorders. Elevated ALT levels were associated with a higher incidence of liver disorders and smoking was related to a lower incidence of the liver disorders.

Value-added logistics: the answer to mass customization.

Value added logistics: A new concept or just new terminology? If you read Today and Tomorrow you will notice that Henry Ford used postponed manufacturing to save on transportation costs: On one railway-wagon he could ship either three T-Fords or the parts for seven T-Fords. This is one of the reasons he build several assembly plants in the United States and in Europe (Dagenham, UK; Koln, Germany; and Amsterdam). Although this concept has existed for more than 70 years, I found out that it is still quite unknown. Thanks to information technology, it is becoming more powerful. This opens large possibilities for mass customization.

[A study on affecting factors on dental care demands by logistic regression model].

OBJECTIVE: To develop probability model for dental visits based on analysis of the factors affecting people's dental services utilization in urban area of Beijing, thus providing some evidence for forecasting further demanded dental care and building the more efficient oral health care delivery system. METHODS: A cross-sectional survey was conducted amnog 1,517 subjects of all age groups in Beijing selected by stratified, clustering, random sampling. The first model of dental care demand--probability model for dental visits was established with logistic regression. RESULTS: Awareness of oral health showed the most important relationship. Different social background and economic factors also had great effects on demand. Higher demand for care were existed among those who suffered from a disease that was painful or resulted in poor oral function. CONCLUSIONS: It is suggested that dental care has different characters from general health service. Utilization of dental service is largely determined by people's awareness, income and insurance system. So oral health education should be extended in order to promote the effective demands and reduce the potential demands. Rational oral health insurance system should be set up to provide some priority to certain people, thus stimulating the supply of dental care and improving the utilization of dental service. Since raising price for care will restrain the low-incomer's basic demand, and it will neither add the profit nor improve people's oral health, the price decision should be considered carefully. At last, oral health care should be emphasized on children and adolescent as priority age groups based on the conclusion from demand model.
