Self-rated health and mortality in a Lithuanian and a Dutch population.

The hypothesis that the evaluation of one's health as poor is associated with mortality, independent of the results of a standardized medical examination, was tested in the Kaunas-Rotterdam Follow-Up Study. In this study two cohorts, one consisting of 2452 Lithuanian males and one of 3365 Dutch males, aged 45-60, were screened for cardiovascular risk factors in 1973, using identical protocols, and were followed for about ten years. Self-rated health was assessed by two direct questions: 'How would you assess your own health?' and 'What do you think of your own health compared to that of other men of your age?' as well as by a Semantic Differential Test of 'My Health'. In both cohorts a negative evaluation of one's health was associated with mortality, controlling for past or present heart disease, cardiovascular risk factors, parental life span, socio-economic and marital status. Especially the data with regard to the comparative question indicate that self-rated health is associated with mortality in men living in two different socio-cultural systems. The date suggest that a weak sense of mastery may explain the association between health perception and mortality.

Multiple feature sets based categorization of laryngeal images.

This paper is concerned with an automated analysis of laryngeal images aiming to categorize the images into three decision classes, namely healthy, nodular, and diffuse. The problem is treated as an image analysis and classification task. Aiming to obtain a comprehensive description of laryngeal images, multiple feature sets exploiting information on image colour, texture, geometry, image intensity gradient direction, and frequency content are extracted. A separate support vector machine (SVM) is used to categorize features of each type into the decision classes. The final image categorization is then obtained based on the decisions provided by a committee of support vector machines. Bearing in mind a high similarity of the decision classes, the correct classification rate of over 94% obtained when testing the system on 785 laryngeal images recorded at the Department of Otolaryngology, Kaunas University of Medicine is rather promising.

Arsenic in sediments from the southeastern Baltic Sea.

Arsenic occurs as a persistent constituent in many of the chemical weapons dumped into the Baltic Sea; it can be used as an indicator of leakage and dispersal of released munitions to the marine environment. Total arsenic was analysed in sediment samples taken from the Lithuanian economic zone in the Baltic Sea, which included samples from the chemical munitions dumpsite in the Gotland Basin and national monitoring stations in the southeastern Baltic Sea. Arsenic concentrations in sediments ranged from 1.1 to 19.0 mg kg(-1), with an average of 3.4 mg kg(-1). Although there was evidence of slightly elevated arsenic content in sediments near the weapons dumpsite, arsenic concentrations were nevertheless quite low relative to other investigations in the Baltic and North Seas.

The humoral immuneresponse to Helicobacter pylori infection in children with gastrointestinal symptoms.

The prevalence of Helicobacter pylori is high in Eastern Europe. The purpose of this study was to estimate the prevalence of H. pylori in symptomatic Lithuanian children and to identify the infection by clinicopathological and serological analyses. One hundred sixteen symptomatic children (age 8-16) with gastritis and duodenal ulcer were included. Biopsies were histologically assessed according to the Sydney-System. Serum IgG antibodies against H. pylori were detected by an enzyme-linked immunosorbent assay (ELISA), using low molecular mass antigen. The western blot technique was used to detect serum antibodies against the cytotoxin-associated protein (CagA) using whole cell antigen. Histologically the prevalence of H. pylori infection was 79% and not influenced by demographic factors. Mucosal inflammation and atrophy were associated with a H. pylori infection. Intestinal metaplasia was found in eight children, suggesting early H. pylori acquisition in life. Increased levels of IgG antibodies were detected in 57% of children. The prevalence of IgG antibodies was significantly higher in patients with duodenal ulcer compared to children with gastritis. Forty-four (67%) H. pylori-seropositive children had antibodies against CagA. Low molecular weight-ELISA and whole cell-western blot results were significantly associated with histopathology, the presence of duodenal ulcer and the CagA status. A high number of false seronegative cases were due to poor immunological responses in children and poor locally validated tests. The prevalence of H. pylori infection in Lithuanian children is higher compared to Western Europe. The infection is acquired in early life. Diagnosing H. pylori infection, serology is helpful, but endoscopy/histology remains as gold standard.

Allele distribution of 15 STR loci in a population sample of the Lithuanian minority residing in the Northeastern Poland.

Allele frequencies for the 15 STRs included in the AmpFlSTR Identifiler kit were determined in a population sample of 140 unrelated individuals of the Lithuanian minority residing in the Northeastern Poland. Departure from the Hardy-Weinberg equilibrium for loci VWA, TPOX and D7S820 did not sustain after applying Bonferroni correction for multiple testing. Exact tests disequilibrium analysis revealed five departures from independence out of 105 pair-wise comparisons. The combined MP and PE for all 15 loci are 5.00 x 10(-17) and 0.9999, respectively.

Population genetics of Y-chromosome STRs in a population sample of the Lithuanian minority residing in the northeastern Poland.

Haplotype and allele frequencies for the 12 Y-STRs were determined in a population sample of 124 unrelated males--members of the Lithuanian minority residing in the northeastern Poland. Three of the haplotypes were encountered in duplicate, while 118 haplotypes were unique. The overall gene diversity was 0.9952. Analysis of molecular variance revealed that the Lithuanian minority can be distinguished from the autochthonous Poles, although these two populations are very close to each other. This database study provides an essential precondition for applying Y-chromosomal STRs estimates in forensic identification of male DNA and tracing of paternal lineages.

Mood and thyroid immunity assessed by ultrasonographic imaging in a primary health care.

BACKGROUND: Psychiatric morbidity is highly prevalent in the primary health care settings and it may be related to autoimmune thyroid disease (AITD). The aim of this study was to evaluate the impact of thyroid immunity, evident by hypo-echoic thyroid ultrasound pattern, on prevalence of depression and anxiety in a primary care setting. METHODS: In a cross-sectional design, 504 consecutive primary care patients were invited to the study and 474 patients completed the study. They were screened for depression and anxiety using the Hospital Anxiety and Depression Scale (HADS), were interviewed for affective disorders using the Mini International Neuropsychiatric Interview, and were evaluated by ultrasonographic imagining of the thyroid gland. RESULTS: Among patients with hypo-echoic thyroid (n=122) prevailed women and those patients were older than patients with normo-echoic thyroid (n=352). Women, but not men, with hypo-echoic thyroid compared to those with normo-echoic thyroid had higher scores on the anxiety subscale of the HADS (p=0.03). Among women with hypo-echoic thyroid, only those pre-menopause, but not those post-menopause, had greater prevalence of high scores on the depression subscale of the HADS (p=0.02) and a greater likelihood of using psychiatric medications (p=0.001). LIMITATIONS: Lack of cytological evaluation of the thyroid gland; lack of serum thyroid antibodies concentrations; and lack of thyroid hormone concentrations. CONCLUSIONS: Thyroid immunity is related to mood symptoms in primary care patients. These effects are gender specific and in women, they are most evident before menopause.

Biomarker responses in flounder (Platichthys flesus) and mussel (Mytilus edulis) in the Klaipeda-Būtinge area (Baltic Sea).

During the EU project BEEP a battery of biomarkers was applied in flounder (Platichthys flesus) and the blue mussel (Mytilus edulis) collected at three locations off the Lithuanian coast (Baltic Sea) in June and September 2001 and 2002. The elevated biomarker responses in specimens sampled in September 2001 were apparently related to the extensive dredging activities in the Klaipeda port area and subsequent dumping of contaminated sediments. High concentrations of organic pollutants (organochlorines and PBDEs) were also measured in the tissues of both indicator species. In addition, response levels of genotoxicity, cytotoxicity, immunotoxicity as well as concentrations of PAH metabolites in the bile of flounder showed elevations in 2002 after an oil spill in the Būtinge oil terminal in November 2001. In flounder, biomarker measurements 10 months after the spill indicated recovery processes but in mussels a high level of genotoxicity could still be observed 22 months later. The present study illustrates the usefulness of the multi-biomarker approach in the detection of biological effects of pollution in this region of the Baltic Sea.

Implementation of the patient education about cardiovascular risk factors into a daily routine of the Cardiology Unit of the hospital.

BACKGROUND: Cardiovascular disease remains the leading cause of death in developed countries. Main modifiable cardiovascular risk factors are smoking, hypertension and dyslipidemia. We sought to introduce the patient education about these risk factors into a daily routine of the Cardiology Unit of the hospital. METHODS: In November 2003, 56 patients filled out the questionnaire at discharge from the Cardiology Unit. Collected data were discussed with the physicians of this unit. In December 2003, 64 patients of this unit once again were asked to fill out the questionnaire. RESULTS: In December, less patients were not informed by physicians about smoking risk (3.1% versus 18.2%, P = 0.03) and diet (0% versus 18%, P = 0.0001), more patients quitted smoking (75% versus 50%, P = 0.03), less patients claimed unaware of their blood pressure (6.25% versus 21.4%, P = 0.01), cholesterol level (3.1% versus 60.7%, P = 0.00001) and the necessity to correct them (0% versus 22.7%, P = 0.0001, and 0% versus 64.3%, P = 0.00001). CONCLUSIONS: Implementation of the patient education about cardiovascular risk factors by physicians into the daily routine of the Cardiology Unit was successful.

Investigation of absolute metal uptake efficiency from precipitation in moss.

Bioaccumulation of metals in moss was studied by the absolute method using a specially designed 'moss bag' technique. Bulk deposition was collected in open areas along with bulk deposition collected under moss. An absolute uptake efficiency of approximately 60% was obtained, which is in good agreement with the value obtained by a relative method. The absolute uptake efficiency varied between 0 and 92%, hence uptake of metals depends very strongly on microenvironmental conditions which can even cause leaching of metals from moss. The best retained elements were Pb, Ni, Cu and V, while Cd, Cr, Fe and Zn exhibited significantly lower uptake efficiencies. Manganese in all cases showed leaching of the element from moss.
