The migration of 60 press-fit cups fixed with screws was evaluated by radiostereometric analysis for 2 years in 57 patients revised because of aseptic loosening. The mean proximal migration of the cup center increased continuously up to 0.36 mm (range, 0.0-1.6 mm), whereas the migration in the other directions reached a level at 0.3 to 0.4 mm after 6 months. Mean rotations of less than 1 degree were recorded. Factors influencing motions were cup size, use of bone-grafts, and position of the cup center in the vertical and horizontal directions. Compared with previous stereoradiographic studies of cemented revisions, the migration of the press-fit cup was smaller, however, still higher than previously reported for uncemented cups of the same design used in primary surgery.

Twenty-six consecutive, hydroxyapatite-coated Bi-Metric femoral stem prostheses (Biomet, Warsaw, IN) were studied in 20 patients. The follow-up period averaged 37 months (range, 24-48 months). Clinical rating (Harris score) was determined and radiography were taken at regular intervals. Digital imaging and computer-assisted videodensitometry were used to quantify the cortical remodeling. The anteroposterior radiographs of the femur were scanned and digitized, and the pictures were analyzed by a digital image processing software program. The periosteal diameter, the cortices of the femur, and the total endosteal diameter in five regions were determined. The mean preoperative Harris score was 50 and improved to 97 at the latest follow-up evaluation. After surgery, there was hypertrophy of the periprosthetic cortical bone and an increase in the periosteal diameter. The medullary canal of the operated femur did not change.

Pathologic fractures from metastatic cancer pose formidable challenges to the orthopaedic surgeon technically and in terms of surgical decision making with regards to patient quality of life. We present the case of a 47-year-old woman with simultaneous bilateral pathologic femoral neck and acetabulum fractures and severe pulmonary shunt, who was treated successfully with cementless femoral and acetabular implants.
The BiCONTACT femoral stem has been developed as a modular system which is suitable for both cemented and uncemented implantation. A highly standardized procedure using an identical set of instruments for both modes of fixation ensures bone preservation. Primary stability is achieved by special design parameters. The cementless implants are coated proximally, thus facilitating osteointegration and proximal load transfer. Cemented stems provide an identical design but a smooth surface and are anchored by a homogenous cement mantle. The BiCONTACT stem is in use in the BG Trauma Center Tuebingen since 14 years and a consecutive series of 250 cases (series A) with uncemented implantation and of 250 cases (series B) with cemented implantation has been analyzed in two different prospective follow-up studies. Long-term survival after 11 years was 97.1 % (95 % confidence limits: 93.8 %-98.7 %) for series A, the follow-up rate was 99.2 %. For series B, the follow up-rate was also 99.2 %, survival estimate after 11 years was 97.5 % (95 % CI: 94.2 %-99.0 %).
