[Materials using in the prevention of scar formation after laminectomy].

The recurrent syndrome after laminectomy is related to the scar formation overlying epidural. To solve this catastrophic problem means to prevent the fibrosis. In the present review a variety of materials used in the inhibition or prevention of that syndrome are introduced, and their effects are reviewed and discussed. Some characteristics of the materials for prevention are summarized, and in our experiments the material point of view is rational.

[The long-term effect of double embolization of bronchial artery on patients with hemoptysis].

OBJECTIVE: To investigate the long-term effect of bronchial arterial embolization with different material and procedure on patients with hemoptysis. METHOD: Seventy two cases were divided into two groups, one received the double embolization of bronchial artery, the other group used single gelfoam embolization. In double embolization group, the mixture containing gelfoam microspheres (0.5 mm x 0.5 mm), erythromycin and high osmalality sodium chloride were firstly used to embolize the terminal vessels, the gelfoam particle (1 mm x 1 mm) was then used to embolize the main trunk of bronchial artery. RESULT: After 2-4.8 years following up, it turned out that the cure rate and general effectiveness were: the double embolization group 83%, 100% respectively, the single gelfoam embolization group 61%, 95%. By comparison, there were significant difference on cure rate between these two groups(P < 0.05). CONCLUSION: The bronchial arterial double embolization will increase the long-term effect obviously on patients with hemoptysis.

Impact of recombinant human bone morphogenetic protein-2 on residual ridge resorption after tooth extraction: an experimental study in the rat.

Residual ridge resorption begins following tooth extraction and continuously reduces alveolar bone volume, potentially creating a significant problem in dental implant treatment. In this study, the role of recombinant human bone morphogenetic protein-2 (rhBMP-2) in residual ridge resorption after tooth extraction was investigated. A polylactic acid/polyglycolic acid copolymer-coated gelatin sponge carrier was implanted with or without rhBMP-2 (1 microg) in the mesial root sockets after removal of maxillary first molars in male Wistar rats. Fine structural and histomorphologic analyses were conducted 3 to 84 days after implantation. Direct bone formation was first observed after 5 days on the rhBMP-2 side, which was transformed into cortical alveolar ridge with a smooth periosteal layer by 84 days, whereas the control side displayed slower healing. Bone histomorphometry revealed greater total bone area and increased bone height after 14, 28, 56, and 84 days on the rhBMP-2 side compared to the control side, and differences were significant after 14, 28, and 56 days. Larger numbers of proliferating cells and densely populated differentiating mesenchymal cells were observed on the rhBMP-2 side than on the control side in the early stage, and chondrogenesis was not observed. The findings indicate that rhBMP-2 may stimulate proliferation and differentiation of mesenchymal cells in the rat maxillary root socket to preserve cortical bone volume in the socket without any evidence of chondrogenesis.

Experimental diminution fo splenci function by selective embolization of the splenic artery.

The splenic artery was occluded by the selective injection of Amicar-mixed clot or Gelfoam in 18 dogs. The dogs were sacrificed at 24 hours, two weeks and two months. Angiography was done before and after embolization, three hours after embolization and prior to sacrifice. White blood counts and hematocrit values were evaluated weekly in the dogs sacrificed at two months. The results of this study reveal differences in the effects of the two embolic materials. Because Amicar-mixed clot was partially lysed in the first several hours and also pushed peripherally, the depression of hematologic function seen with its use was transient and most spleens returned to a relatively normal gross and histologic appearance. With Gelfoam, although the embolic material was also pushed peripherally into the branches of the splenic artery in several of the dogs, the depression of hematologic function was prolonged, and moderate splenic parenchymal fibrosis was seen. The areas between the bands of fibrosis appeared normal. The results indicate that selective embolic occlusion of the splenic artery and its branches may offer a method of depressing splenic function in patients who are not candidates for splenectomy.

[Arterial embolization in the treatment of neoplasic tumors].

Eight cancer patients treated at the centre François-Baclesse at Caen underwent arterial embolisation using Spongel. The technique was usually used in the presence of a contraindication to operation. In certain cases the problem was that of controlling haemorrhage following tumour invasion or secondary to complications of radiotherapy, under which circumstances was most successful. In addition, it was possible to reduce pain resistant to medical treatment. The method appeared to be a useful therapeutic complement post-radiotherapy for hypervascularised lesions.

[Transarterial chemoembolization (TAE) with degradable starch microspheres (DSM) in advanced hepatocellular carcinoma].

This study was undertaken to evaluate the clinical utility of chemoembolization using degradable starch microspheres (DSM), which resolve in a short period in patients with advanced hepatocellular carcinoma (HCC). Twenty-one patients underwent DSM chemoembolization 24 times. After a mixture of iodized oil and epirubicin was injected into the hepatic arteries, the patients were embolized with DSM alone 16 times. In the other 8 times, embolization was done in one hepatic lobe with DSM and in the other hepatic lobe with gelatin sponge (GS). There was no major complication related to chemoembolization. Tumor response (complete, partial, and minor responses) was found in 46% of patients after TAE. Tumor recurrence was found in 64% of responders after a mean period of 2.0 months. The response rate was significantly higher when chemoembolization was performed using both DSM and GS than when it was done with DSM alone (63% vs 37%, p < 0.04). Although the response rate after DSM-TAE is low, its anticancer effect is reinforced when used as an adjuvant therapy of GS-TAE.

[Transcatheter arterial embolization in intractable postpartum hemorrhage].

OBJECTIVE: To asses the value of transcatheter arterial embolization treatment in intractable postpartum hemorrhage. METHODS: Fourteen patients of postpartum hemorrhage failed to the drugs treatment were performed transcatheter arterial embolization (TAE). Superselective catheterization to the bilateral uterial arteries or anterior branches of internal iliac arteries was carried out after intubation to single-side femoral artery by Seldinger's technique. When the catheter tip was proved in right place by digital substraction angiography (DSA) techniques, antibiotic solution was poured into arteries and then the arterial embolization with gelfoam particles was followed. RESULTS: All the patients were performed TAE successfully. The duration of TAE was 30-50 minutes [mean (41.8 +/- 6.4) minutes]. Bleeding was found in DSA mostly as diffuse intrauterine flow outward sign from one side or both side of uterial artery branches. After one time of TAE, the vaginal bleeding of all patients was stopped in 3-10 minutes [mean (6.1 +/- 3.6) minutes]. The follow-up showed that all patients had normal menstrual periods in 2-60 months and no serious complications were found. CONCLUSION: With the characteristic of fast homeostasis and less complications, TAE was proved to be an effective method for intractable postpartum hemorrhage.

Hepatic artery chemoembolization for advanced stage HCC: experience of 650 patients.

UNLABELLED: Hepatic artery chemotherapy using cisplatin in various protocols was examined in 650 patients. Overall objective tumor response rate (PR) was 65%. Average survival was 7.5 mo in patients with tumor progression, 18.0 mo for tumor stability and 32.0 mo for PR. 1- and 2-yr survival was 70% and 40% in responders, 20% and 0% in progressors. Prognostic factors were examined in 155 patients treated with cisplatin and gelfoam chemo-occlusion. In survival groups of > 24 mo, 4-24 mo and < 4 mo, similar numbers had cirrhosis, hepatitis B virus, hepatitis C virus and alcoholism. Decreased survival was associated with abnormal bilirubin, albumin and prothrombin time. Tumor vascularity and response to chemotherapy were associated with prolonged survival. Tumor vascularity seemed important for tumor response. Portal vein thrombosis occurred in all groups. Lesion number, bilobarity and maximum size had no correlation with response or survival. We analyzed the cause of death in 425 patients. No evidence of hepatocellular carcinoma progression, judged by absence of change in CT scan or tumor marker in the last 4 months of life, was found in 42%. A group of 57 patients were treated with cisplatin in dose range 125-200 mg/m2 alone or with gelfoam. In both groups, responders survived longer than non-responders: cisplatin alone responder mean survival, 29.0 mo, non-responder 11.1 mo, P < 0.0001. There was a strong effect of dose density on median survival for cisplatin alone, but not for cisplatin and gelfoam. CONCLUSIONS: A large experience of single-agent cisplatin chemo-occlusion is summarized. Good liver function and tumor vascularity are associated with response to chemotherapy, which in turn is associated with enhanced survival. Many deaths are due to cirrhosis and not hepatocellular carcinoma.

[Clinical analysis of 42 cases uterine myoma by uterine arterial embolization].

OBJECTIVE: To assess the clinical effects and side effects of uterine arterial embolization (UAE) on uterine myoma. METHODS: UAE was performed in 42 patients with uterine myoma. The 4 approximately 5 F cobra catheters were insered into the uterine arteries for treatment. Gelfoam particles mixed with antibiotics were used to embolize the arteries. All patients were followed upto 18 monthes post operation. RESULTS: Arteriography showed that uterine myoma was mainly supplied by uterine arteries and partly by ovarian arteries. All clinical symptoms of 42 patients were relieved. Menorrhagia was reduced 26.3% approximately 75.2%. The volume of uterus myoma decreased 77.7% and 83.0% 12 and 18 months after UAE, respectively and the volume of uterine decreased 54.9% and 62.7%. CONCLUSIONS: UAE was an effective and satisfactory method for the treatment of uterine myoma. Further effects of it should be observed continuously and periodically.

Clinical studies of new material for portal vein embolization: comparison of embolic effect with different agents.

BACKGROUND/AIMS: Study of he embolic materials is necessary to select those suitable for preoperative portal embolization which increases the safety of hepatectomy. METHODOLOGY: Twenty-three patients with biliary tract cancer and hepatic tumor underwent portal embolization of the right portal vein. Four kinds of embolic materials were used. Gelatin sponge was used in 9, cyanoacrylate in 4, fibrin glue in 2, and gelatin sponge with polidocanol in 8 patients. The embolic effect in inducing hypertrophy as well as the surgical results were compared for different embolic agents. Left lobe hypertrophy was evaluated by computed tomography volume before and 2 weeks after portal embolization. RESULTS: With gelatin sponge, the volumetric increase was 21.9%, compared to 25% with cyanoacrylate, 13.5% with fibrin glue, and 35.3% for gelatin sponge with polidocanol. The volume increase of gelatin sponge with polidocanol was superior to those of other agents. In 18 of the 21 patients, hepatectomies with/without pancreaticoduodenectomy were performed. In 11 patients using the first 3 of the above-mentioned agents, 2 operative procedures were changed to limited hepatic resection, and 3 patients died of hepatic failure due to insufficient hypertrophy. Seven patients for whom gelatin sponge with polidocanol was used underwent the proposed operations with good results. CONCLUSIONS: Gelatin sponge with polidocanol induced sufficient hypertrophy and was the most suitable agent for portal embolization.

Quantitative and qualitative in vivo angiogenesis assay.

We describe the development and optimization of an in vivo angiogenesis assay utilizing gelfoam sponges impregnated with 0.4% agarose and different proangiogenic factors, such as basic fibroblast growth factor (bFGF), vascular epidermal growth factor (VEGF), tumor growth factor-alpha (TGF-alpha), and endothelial growth factor (EGF). The sponges are implanted into the subcutis of mice and harvested after different times. The gelfoam sponges are fixed, sectioned, and stained with fluorescent antibodies against CD31. The median number of CD31+ cells is determined in 10 different 0.159-mm2 fields. Proangiogenic molecules induced significant migration and proliferation of endothelial cells. To demonstrate the utility of this assay for evaluation of an antiangiogenic agent, mice were implanted with gelfoam sponges containing different proangiogenic factors and treated orally with water or PTK 787, a novel tyrosine kinase inhibitor with specific activity against the VEGF-R. PTK 787 significantly inhibited angiogenesis in sponges containing agarose + VEGF but not other proangiogenic molecules. The data show that the implanted gelfoam sponges provide a reliable quantitative assay to study in vivo angiogenesis.

Formaldehyde-formed autogenous fascia graft tympanoplasty.

The formaldehyde-formed autogenous fascia graft represents another step toward more reliable functional restoration of the diseased tympanic membrane. It combines the advantages of the availability and high nonperforation rate of autogenous fascia with the more normal anatomic restoration experienced with homograft tympanic membrane grafts. Its primary indication is in total membrane perforations with intact malleus although the concept can be employed in conjunction with various types of ossicular reconstruction.

Long-term outcomes of transcatheter arterial chemoembolization with autologous blood clot for unresectable hepatocellular carcinoma.

Ninety-two consecutive patients with unresectable hepatocellular carcinoma (HCC) greater than 2 cm and less than 5 cm in diameter were treated by transcatheter arterial chemoembolization using autologous blood clot as an embolizing agent (Short-TAE; S-TAE). Survival, patency of hepatic arteries and side effects were retrospectively analyzed. The median follow-up interval was 42 months. The median size of the main tumor was 3.4 cm in diameter. All tumors were determined to be inoperable because of intrahepatic spread of tumors and/or poor hepatic functional reserves. S-TAE was performed by injecting a mixture of iodized oil and anticancer drugs followed by embolization of hepatic arteries with autologous blood clot. Embolization with clots maintained patency of hepatic arteries in all the patients. The overall survival rates for 92 HCC patients at 1, 3, 5, 7 and 8 years were estimated by the Kaplan-Meier method to be 100, 52, 34, 12 and 6%, respectively, which were better compared with prior records for conventional method with gelfoam. S-TAE was successfully performed in 16 patients with Childs class C liver cirrhosis presenting icterus and ascitis. The survival rates for Childs class C patients at 1, 2 and 3 years were 100, 66 and 32%, respectively. The Cox proportional hazard model was used to assess influence of each pretreatment parameter (age, gender, virus marker, stage of cirrhosis, number of tumor) on survival. The univariate analysis demonstrated that both the stage of cirrhosis and the multiplicity of tumor were significant factors predicting survival (p=0.003 and 0.046, respectively). The multivariate analysis showed the stage of cirrhosis to be the sole factor which significantly affected prognosis of HCC patients (p=0.005). In conclusion, chemoembolization with autologous blood clot is an effective and safe therapeutic option for unresectable HCC smaller than 5 cm. The hepatic functional reserves play a key role in determining prognosis of HCC patients.

Osteogenesis in a rat model: use of bone marrow cells and biodegradable gelatin matrix carrier.

We investigated the use of a biodegradable porcine gelatin matrix (Gelfoam) as a carrier for marrow cells that induce osteogenesis at ectopic sites in rats. Bone marrow cells obtained from the long bones of 6-week-old Sprague-Dawley rats were dissociated enzymatically and the cells reconstituted in rat serum. Twenty million cells in 0.1 mL of serum were then adsorbed into 1-cm3 pieces of gelatin matrix substrate and implanted into ectopic sites in live Sprague-Dawley rats. The implants were retrieved and analyzed histologically for bone and cartilage formation 3, 4, 6, and 8 weeks after implantation. Woven bone was seen as early as 3 weeks and persisted through 8 weeks. No cartilage was observed. Osteoclasts first appeared at 3 weeks, peaking in number at 4 weeks. By 6 and 8 weeks, only small islets of substrate remained, surrounded by dense, woven bone. Control implants, consisting only of serum adsorbed into the carrier, showed no bone formation. We conclude that biodegradable gelatin matrix can serve as a carrier for the osteogenic cells of bone marrow in rats in ectopic sites. Such a system may be further developed to augment bone healing.
