Do fee descriptors influence treatment choices in general practice? A multilevel discrete choice model.

Before 1990 Australian general practitioners (GPs) were remunerated according to consultation length. This was assumed to encourage GPs to prescribe more, counsel less and provide fewer treatments than were 'appropriate'. In an attempt to change this behaviour, the remuneration system was altered to reflect the content of consultations. This paper analyses, through the use of multilevel modelling, the effect of content-based descriptors on the discrete choice behaviour of GPs while controlling for patient, GP and practice characteristics. GPs who used content-based descriptors were just as likely to prescribe, counsel and treat compared to GPs who used time-based descriptors.

Establishing fees.

Everyone seems to agree that we should be concerned with the economic security of all small businesses, and in particular, the clients whom we serve. Businesses and consumers are equally concerned and occasionally scared. Both are cautious when spending hard-earned dollars. Animal care, which is not a routine budgetary expenditure, is often difficult for a family to fund. As a servicing veterinarian with a product to sell one must be cognizant of these economic dilemmas to understand your consumer. Your product must fit your consumers' needs and ability to pay, and you should not try to mold your clients to your needs. This current recession will close some veterinary practices. Those practices with high fixed overheads, overextended borrowing, too many leases, declining income, and poor management will suffer the most.

Free services versus fee for services.

In summary, there are many benefits that can be argued, many risks that can be pointed out, in addressing the question of user charges in community health and social services and institutional services. There is no simple road map for how to make the final political decisions about what charges any government wishes to put in place. As the Ontario government approaches this, it is considering what its options are. It has asked its staff to review current charges to examine what the possibilities are and, in the context of reforming the overall long term care system, will again be looking at some of these questions that I have presented to you.

Comprehensive reimbursement model: an alternative to fee-for-service reimbursement by blood centers.

"If we could first know where we are, and whither we are tending, we could then better judge what to do and how to do it." This quote from Abraham Lincoln epitomizes where SMF and its CRM hospital customers are today. The CRM has been received by SMF's hospital customers as a step in the right direction. Currently, it seems to be the best way to partner with hospitals by sharing risk in the current health care delivery system milieu. The CRM focus on patient outcomes will provide hospitals and blood centers a common goal. Modifications to the CRM will most certainly be necessary. With input from customers and the community over time, and flexibility from SMF, a revised model will evolve that will be even more mutually beneficial.
