[Mortality risk measure inequalities among workers in Southeast Brazil].

INTRODUCTION: The main causes of illness and death in Brazil have been migrating backwards into the younger population during the last few years, increasing especially in the more productive age groups. Given the relationship between work and health/disease process, the hypothesis to be considered is that this phenomenon is partially due to the deterioration of workplace conditions. To contribute to investigating this hypothesis, this study estimates mortality risk indicators for the population of Botucatu, in the Southeast region of Brazil, classified according to their occupation. METHODS: Standardized mortality coefficient, standardized risk ratio, and years of potential life lost were calculated for the inhabitants of Botucatu who died after their 10th birthday, between January 1997 and March 1998, and classified according to their occupation and main cause of death. Occupational and medical information was obtained by interviewing families of the deceased and their doctors, and checking medical files. RESULTS: The standardized mortality coefficient ranged from 0.6 to 39.9 deaths/1000 workers in different occupations. The years of potential life lost ranged form 33 to 334 years/1000 workers. The ranking of causes of death varied according to occupation and the mortality risk considered. CONCLUSION: The risk measures analyzed showed a high heterogeneity when associated to occupation and causes of death, which reflects the great social inequality existing in the studied population.

[Identification of the encephalitis equine virus, Parana, Brazil].

INTRODUCTION: In the period of 1996-1999 some virus associated with encephalitis have been reported in horses from different regions of Paraná State, Brazil. To identify the etiologic agent associated with this illness, mosquitoes and serum samples were collected in the endemic area. METHODS: The study area corresponded to four municipalities of Paraná State, Brazil. Mosquitoes were captured in Shannon trap and human bait. After identification, they were processed for virus isolation. Blood of equines were collected in the municipalities of Querência do Norte and Colorado. Antibodies to different Alphavirus and Flavivirus were analyzed by hemagglutination inhibition test. Specific seroneutralization reactions were performed in those sera with a positive reaction in the hemagglutination test. RESULTS: The mosquitoes genus collected were: Culex, Aedes, Mansonia, Coquillettidia, Psorophora, Sabethes, Wyeomyia, and Limatus. Even thought no virus was isolated, serologic analyses showed hemagglutinazing antibodies to Eastern equine encephalitis, Mucambo, Pixuna, Maguari, and St Luis encephalitis viruses. The neutralization test showed specific reaction to Eastern equine encephalitis virus in 12 tested sera. CONCLUSIONS: Species of mosquitoes that could be potential vectors of encephalitis, buniavirus, and other arboviruses of epidemiological importance were collected. It is believed that Eastern equine encephalitis virus affected the equines populations in the study regions because of the symptoms and antibodies for the virus in the sera detected in these equines.

[Changes in composition and appropriate nutrition of family diet in the metropolitan areas of Brazil (1988-1996)].

OBJECTIVE: A new family budget survey carried out in the mid-nineties in Brazil allows an update of the secular trends (1962-1988) of dietary patterns of Brazilian population living in metropolitan areas. METHODS: Data sources are IBGE Institute of Statistics family budget surveys carried out from March 1987 to February 1988 (13,611 households) and from October 1995 to September 1996 (16,014 households) in all metropolitan areas of Brazil. The daily food availability per capita for each household was calculated dividing the total food acquired in a month by the number of individuals living in a household and the month's number of days. Dietary patterns were characterized according to the amount of selected food groups and nutrients relative to the diet caloric input. Comparisons between the two surveys included the metropolitan area population as a whole and subgroups from less (North and Northeast) and more developed (Mid-west, Southeast and South) regions. RESULTS: It was observed an increase in consumption of meat and dairy products (except for butter) and a reduction in eggs consumption in both less and more developed regions. Beans, roots and tubers consumption showed a steady decline in the whole country while cereals consumption remained the same (higher in developed regions) or had a slight increase (in less developed regions). The proportional consumption of vegetal oils and margarine remained constant in the less developed regions but their consumption was greatly reduced in the more developed ones. CONCLUSIONS: An increase in the diet's lipid content in less developed regions and of saturated fat in the country as a whole, associated with a decrease or even no consumption of beans, vegetables, fruits and complex carbohydrates, and a further increase in the excessive sugar consumption are the negative aspects of the trend observed from 1988 to 1996. Changes that may indicate a growing awareness of the population toward a healthier diet, such as a decline in egg intake and a slight reduction in diets with a high total lipid content, were found only in more developed regions.

[Dental caries prevalence and mean dmf-t among 12 to 13-years old schoolchildren between 1971 and 1997, Southern Brazil].

INTRODUCTION: Cross-sectional surveys carried out in Brazil during the last decade have showed a significant decrease in both prevalence and severity of dental caries. However, the validity of these studies is questioned. Different methodologies as well as diagnosis criteria were used. Thus, a study was carried out with the purpose of comparing the prevalence and severity of caries among schoolchildren aged 12 and 13 years in a region of Brazil. METHODS: Two cross-sectional surveys were carried out. All 12 and 13 year old children enrolled in a primary school were examined by a single dentist in 1971 (n=202) and 1997 (n=175), following the same protocol. The original criteria for the diagnosis of caries proposed by Klein and Palmer in 1937 were used both in 1971 and 1997. A total of 202 and 175 children were examined in 1971 and 1997 respectively. RESULTS: The response rate was 100%. Intra-examiner agreement was very high. Kappa test was performed on a tooth by tooth basis and all values were above 0.86. The prevalence of caries was 98% and 93. 7% in 1971 and 1997 respectively. Comparison of the severity of caries experience expressed as changes in the mean DMF-T scores showed a reduction from 9.2 to 6.2 between 1971 and 1997 in the study group. CONCLUSIONS: There was a significant reduction in dental caries prevalence and severity between 1971 to 1997 in the study population. The observed reduction of caries was not a statistical artifact as the same methodology and diagnostic criteria were applied in both cross-sectional surveys.

[Lipid profile among school children in Campinas, Brazil].

OBJECTIVE: To describe the lipid profile and the prevalence of hypercholesterolemia among schoolchildren aged 7 to 14 years in Campinas, São Paulo State, Brazil. METHODS: Plasma cholesterol levels, fractions, ratios and triglycerides were determined according to age and gender in a total of 1,600 schoolchildren. Hypercholesterolemia was considered borderline for 170 mg/dl</= cholesterol <185 mg/dl, moderate for 185 mg/dl pound cholesterol <200 mg/dl and severe for cholesterol >/=200 mg/dl. RESULTS: Schoolchildren presented a cholesterol mean of 160 mg/dl, HDL-cholesterol mean of 49 mg/dl, LDL-cholesterol mean of 96 mg/dl, VLDL-cholesterol mean of 16 mg/dl, triglycerides mean of 79 mg/dl, cholesterol/HDL-cholesterol mean of 3.5 and LDL-cholesterol/HDL-cholesterol mean of 2.1. In general, females had higher cholesterol and triglycerides values than males. The prevalence of hypercholesterolemia was 35.0%: 15.6% was borderline high, 9.8% moderate and 9.5% severe. Females presented higher prevalence of hypercholesterolemia than males. CONCLUSIONS: The results pointed to the emergence of hypercholesterolemia as a public health problem in Brazil.

[Violence and social inequalities: mortality rates due to homicides and life conditions in Salvador, Brazil].

INTRODUCTION: Some studies have been questioning the association between poverty and violence. This study's purpose is to assess the distribution of homicide indicators associated with living conditions in Salvador, Brazil. METHODS: A cluster study for the years 1991 to 1994 was carried out including the 75 data centers of the city of Salvador, BA, Brazil. Using death certificates for the study period, yearly mortality rates and mortality ratios were estimated. The 1991 census data of monthly wages and years of education for all family providers were used to define a four-category variable related to living conditions. Mortality rates due to homicide and the relative risk regarding the lowest living condition area were calculated for each social stratum. The 95% confidence intervals were calculated using the Confidence Interval Analysis software. RESULTS: The highest mortality rates due to homicide were seen in the poorest areas. The relative risk due to homicide for the lowest and the highest living condition areas was statistically significant at 5% level and ranged from 2.9 to 5.1. CONCLUSIONS: The data show a strong association between social inequalities and homicide in this urban area, emphasizing the importance of crime reduction programs.

[Abundance and infection of Biomphalaria glabrata with Schistosoma glabrata in the state of Rio de Janeiro, Brazil].

OBJECTIVES: To investigate the spatial distribution, abundance and natural schistosomiasis infection levels in the snail Biomphalaria glabrata, the intermediate host of Schistosoma mansoni in an area of the State of Rio de Janeiro, Brazil. METHODS: In the Pamparrão area, Sumidouro county, RJ, Brazil, snail captures were carried out every other month from June 1991 to November 1995. There were 23 collecting sites along the Pamparrão stream and its three tributaries (A, B and C). Captured snails were examined in the laboratory for infection. Data analyses were performed using Spearman coefficient (0.5% significance level) and Qui-square test. RESULTS: The abundance of B. glabrata was variable in both time and space. Most of the collection sites showed a negative correlation with rainfall. The tributary B had the highest and most steady infection rates (more than 25% in some sites). There were found significantly more infected snail in the dry season (chi2 = 20.08; p=0.001). CONCLUSIONS: B. glabrata population in the Pamparrão valley was negatively influenced by rainfall, especially at the Pamparrão stream. The dry season seems to promote the infection probably due to the lower water volume, increasing the chance of interaction between the intermediate host and the parasite.

[Growth and work among elementary and high school students in São Paulo, Brazil].

OBJECTIVE: To assess risk factors for low height and students and working adolescents in cities of State of São Paulo, Brazil. METHODS: A stratified sample, consisting of 50.0% of students from 5th grade to last year of high school, of State of São Paulo, Brazil, in 1999, was drawn from two largest schools of two different cities (urban and rural). A total of 756 individuals were studied. The height/age indicator, according to the 1977-NCHS (National Center for Health Statistics) standards, defined malnutrition. Height/age distribution and multivariate analyses were carried out using the stepwise method and low-height as the dependent variable. RESULTS: Of the adolescents, 12.7% (96) fell below percentile 5; 24.4% (184) between percentiles 5 - 15; and 47.1% (356) between percentiles 15 - 50. Low height was associated with age: taking age-group 10-13 as reference, low-height was twice as likely in students aged 14-17 years (OR adj.=2.49). For those aged 17-19 years, low height was three times as likely (OR adj.=3.37). Being unemployed increases the risk for low-height (OR adj.=2.86) when compared to working adolescents. Also, low height is higher (OR adj.=1.81) among part-time workers. CONCLUSION: Economical determinants contribute to the risks for chronic malnutrition among students, since these adolescents rely on work to live on. It is worth emphasizing that underage labor legislation should be enforced in conjunction with compensation public programs.

Reproductive outcomes in an area adjacent to a petrochemical plant in southern Brazil.

OBJECTIVE: To evaluate possible adverse reproductive outcomes in an area adjacent to a petrochemical plant in southern Brazil. METHODS: A review of 17,113 birth records of the main hospital of the municipality of Montenegro, southern Brazil, from 1983 to 1998 was carried out. Three groups of cases were selected: (1) newborns with major congenital malformations; (2) newborns with low birth weight (<2,500 g); and (3) stillborns (>500 g). A control was assigned to each case. Controls were the first newborns weighing </= 12,500 g without malformations and of case-matching sex. Mother's residence during pregnancy was used as an exposure parameter. Statistical analyses were performed using Chi-square test or Fisher test, odds ratio, 0.05 significance level, and 95% confidence interval. RESULTS: For unadjusted analysis, it was found a correlation between low birth weight and geographical proximity of mother's residence to the petrochemical plant (OR = 1.66; 95% CI = 1.01 - 2.72) or residence on the way of preferential wind direction (OR = 1.62; 95% CI = 1.03 - 2.56). When other covariates were added in the conditional logistic regression (maternal smoking habits, chronic disease and age), there was no association. CONCLUSIONS: Despite final results were negative, low birth weight could be a good parameter of environmental contamination and should be closely monitored in the studied area.

[Epidemiology of dengue infection in Ribeirão Preto, SP, Brazil].

OBJECTIVE: To assess dengue transmission in a correctional facility for juvenile delinquents in Ribeirão Preto, state of São Paulo, Brazil. METHODS: A serological and virology investigation was carried out among inmates and employees of a correctional facility for juvenile delinquents in Ribeirão Preto, Brazil. The study population consisted of 105 inmates and 91 employees representing 89% of the exposed. The collected blood was stored and processed using MAC-ELISA and virus isolation. A questionnaire was applied to each subject at the time of blood collection. RESULTS: Of the total of blood samples collected (n=196), 42 (21.4%) were positive for IgM antibodies and 43 (21.9%) for IgG antibodies; of which, 15 were both IgM and IgG positive and 28 (14.3%) were IgG positive only. Serotype 1 dengue virus was isolated in 5 samples. Out of 42 IgM positive samples, 14 (33.0%) subjects did not have any physical complaints. The incidence rate was 23.8% and 18.6% among inmates and employees, respectively. The first cases in the facility were reported in 1997 and the last ones in March 1997 though results are suggestive of an earlier onset of transmission. CONCLUSIONS: The high incidence rate of dengue infection can be explained by the high population density of the facility, high Aedes aegypti infestation, high numbers of asymptomatic subjects, and a higher transmission of disease in a closed setting.

[An accident involving Tityus serrulatus and its epidemiological implications in Brazil].

This is a case report of the first accident involving a scorpion Tityus serrulatus in the city of Porto Alegre, Brazil, that occurred in March, 2001. A 21-year-old male patient, while handling vegetables proceeding from other Brazilian states, was bitten by a scorpion Tityus serrulatus, developing local and systemic reactions, requiring an anti-scorpion antidote. The authors highlight the epidemiological importance of this case in the South of Brazil.

[Trends in mortality due to respiratory diseases in elderly, Brazil, 1980 to 1998].

INTRODUCTION: The increase in life expectancy and the decline in mortality rates in Brazil have an impact on social programs to the elderly, especially related to health care. The objective of the study isto analyze the mortality trends for respiratory diseases in elderly. METHODS: An ecological time series was carried out and mortality data was obtained from the Mortality Information System of the Ministry of Health (SIM/MS-DATA-SUS). it was analyzed the time trends of standardized mortality rates according to age groups (60 to 69, 70 to 79 and 80 years old and more) and gender, using linear regression models. The proportional mortality for this disease group compared to all causes of death was also studied. RESULTS: There were increasing trends for mortality rates in both sexes, especially among males. The proportion of deaths due to respiratory diseases was higher among older ages. CONCLUSION: Respiratory diseases are one of the main causes of hospitalization and death in the elderly population. Prevention and care for elderly, as well as further etiology studies should be a priority in Brazil.

Aedes albopictus may not be vector of dengue virus in human epidemics in Brazil.

Over 60,500 dengue cases were reported in the state of Espírito Santo (ES), Brazil, between 1995 and 1998. The study's purpose was to identify whether Aedes albopictus was transmitting the dengue virus during an epidemic in the locality of Vila Beth nia (Viana County),Vitória, ES. From April 3 to 9, 1998, blood and serum samples were collected daily for virus isolation and serological testing. Four autochthonous cases were confirmed through DEN 1 virus isolation and two autochthonous cases through MAC ELISA testing. Of 37 Ae. aegypti and 200 Ae. albopictus adult mosquitoes collected and inoculated, DEN1 virus was isolated only from a pool of two Ae. aegypti female mosquitoes. The study results suggest that Ae. albopictus still cannot be considered an inter-human vector in dengue epidemics in Brazil.

[Ischemic heart disease: mortality in natives and migrants, São Paulo, Brazil, 1979-1998].

OBJECTIVE: The State of S o Paulo has been a major center of attraction for foreign and Brazilian migrants since the 19th century. The pattern of mortality due to ischemic heart disease according to place of birth is, however, unknown. Thus, the objective of the study was to identify differences in mortality due to ischemic heart disease among residents of the State of S o Paulo according to their place of birth. METHODS: Analytical time series study based on secondary data. Rates were not calculated due to lack of denominators. Instead, non-census indicators (proportional mortality and standardized mortality ratio weighted for years of potential life lost before the age of 100 years) and medians were utilized. Temporal trends were analyzed by means of simple linear regression. RESULTS: In the State of S o Paulo, about 40% of deaths due to ischemic heart disease occurred among migrants. The trend was negative for foreigners and positive for Brazilian migrants. Those born in northeastern Brazil, who presented the highest percentage of deaths in hospitals, had the worst performance: their proportional mortality due to ischemic heart disease remained unchanged throughout the study period (while this clearly declined for all other groups) and they died at younger ages. CONCLUSIONS: Migrants' share of total deaths due to ischemic heart disease in the State of S o Paulo is very high. While there is a declining trend for foreigners, it is growing among Brazilian migrants. Among the compared groups, mortality due to ischemic heart disease in the State of S o Paulo is most prominent for those born in northeastern Brazil.

[Dominance of Aedes aegypti over Aedes albopictus in the southeastern coast of Brazil].

OBJECTIVE: To assess infestation of Aedes aegypti and Aedes albopiticus and describe their association with weather conditions and container colonization in urban areas. METHODS: The town of São Sebastião in the southeastern coast of Brazil was selected. It was used data from the Dengue and Yellow Fever Vector Control Program of the State of S o Paulo, Brazil, that encompasses entomological surveillance at strategic positions, traps and foci demarcation. Strategic positions sites are fixed sites with containers in adequate conditions for larvae growth. Statistical analysis was performed using Kruskal-Wallis test, Dwass-Steel-Chritchlow-Fligne test and Mann-Whitney test. RESULTS: There was found an annual growth of positive traps and strategic positions of Ae. aegypti and a drop in Ae. albopictus population. An increase in immature Ae. aegypti and a drop of the other species were also seen. A gradual increase of positive dwellings for Ae. aegypti was noticed outgrowing Ae. albopictus population. There was a weak correlation of the Aedes species with abiotic factors. Higher numbers of immature mosquitoes of both species were found in artificial containers which could be eliminated. CONCLUSIONS: The study showed Ae. aegypti was predominant in urban areas, indicating that its growth seems to have affected the coexistence with other species. Natural selection could be in place contributing to Aedes aegypti and albopictus species separation.

[Prevalence of asthma and asthma symptoms among 13 and 14-year-old schoolchildren, Brazil].

OBJECTIVE: Before the use of the ISAAC (International Study of Asthma and Allergies in Childhood) questionnaire in epidemiologic surveys, little could be told about the comparative occurrence of asthma in the world due to differences in employed methods. In Brazil, the ISAAC questionnaire has been used in some urban regions. In this study it was applied in both, urban and rural areas, in order to estimate the prevalence of asthma among schoolchildren living in Montes Claros, Brazil. METHODS: A cross-sectional study was carried out using the ISAAC written questionnaire with four questions added on exposures of interest. The questionnaire was self-applied in 3,770 randomly selected schoolchildren aged 13 and 14 years. RESULTS: The prevalence of "wheezing in the last year" was 15.8%, and "asthma or bronchitis ever" was 23.8% with no statistically significant difference between boys and girls. There were significant differences between girls and boys regarding "wheezing ever" (37.8% and 33.6%), "sleeping disturbed by wheezing" (13.7% and 9.5%), and "nocturnal dry coughing without respiratory infection" (36.6% and 28.7%), respectively. "Wheezing in the last year" was found to be positively associated with "pet contact" (OR=1.27; 95% CI: 1.03-1.56), and "family history of asthma" (OR=1.79; 95% CI: 1.50-2.14), and negatively associated with "rural school" (OR=0.63; 95% CI: 0.44-0.91). But no association was found with sex, age, private/public school, and passive smoking. CONCLUSIONS: The prevalence of asthma in Montes Claros was high and some symptoms were seen mainly among girls. The occurrence of "wheezing in the last year" showed to be associated with family history, contact with pets and urban schools.

[Excess mortality by diarrhea simultaneous to a cholera epidemic in Northeastern Brazil].

OBJECTIVE: To evaluate excess mortality due to infectious diarrhea without etiological diagnosis, occurring simultaneously to Vibrio cholerae circulation. METHODS: Statistical modeling was applied to a time series of deaths ascribed to "poorly defined intestinal infections" (International Classification of Diseases -- ICD-9) and "presumably infectious diarrhea and gastroenteritis" (ICD-10), between 1980 and 1998 in the Northeast region of Brazil. The prediction of expected values was obtained after a discontinuity point. Excess mortality was calculated as the difference between observed figures and those estimated by the model. RESULTS: Between 1992 and 1994, only 19.3% of deaths by cholera were actually notified. In 1993, the year in which the epidemic reached its peak in the area, our modeling estimated underregistration at 82.2%. CONCLUSIONS: Results indicate substantial underregistration of cholera mortality during the epidemic in Northeastern Brazil.

[Evaluation of hospital resources for perinatal assistance in Brazil].

OBJECTIVE: To verify the adequacy of hospital resources for perinatal care in Belo Horizonte, MG, Brazil in 1996. METHODS: This is a cross-sectional study conducted encompassing all the 28 maternity-hospitals in the city of Belo Horizonte, capital of the State of Minas Gerais, Brazil. A model of evaluation denominated "Níveis de Complexidade e Segurança em Potencial de Unidades Perinatais de Hospitais-Maternidade" [Levels of Complexity and Potential Safety in Maternity-Hospitals]. This model is based on the evaluation of three major areas in the hospital: general structure, perinatal clinical infra-structure and resources available in perinatal units. Two scores involving these three areas were created. When evaluating low risk perinatal care, the score may sum to a total of 1,000 points. On the other hand, when high to medium perinatal risk care is considered, the score may sum to 2,000 points. Hosptals that scored more than 500 points were classified in two levels: I-A and I-B (for low risk); and six levels I-A through III-B (for medium/high risk). RESULTS: Five hospitals were classified as level 0 according to the low risk score, that is, they were not considered adequate to assist any delivery. When the medium/high risk score was employed, a total of seven hospitals were considered inadequate to assist any delivery. CONCLUSIONS: This evaluation model was capable of classifying hospitals according to available infra-structure for perinatal care.
