Clinical and bacteriological features of relapsing shunt-associated meningitis due to Acinetobacter baumannii.

A case is reported of a patient who developed Acinetobacter baumannii meningitis ten days after a ventriculoperitoneal shunt had been placed for control of elevated intracranial pressure. Intravenous antimicrobial therapy was instituted with imipenem and tobramycin after shunt removal and insertion of a ventriculostomy tube. Following the placement of a ventriculoatrial shunt, clinical and microbiological relapse occurred that was eventually cured after complete removal of the ventricular drainage system and a second course of systemic antibiotics. Relapse was confirmed by modern molecular typing techniques including plasmid DNA fingerprinting and analysis of total genomic DNA by pulsed-field gel electrophoresis.

Bacteriophages of Nocardia asteroides.

101 Nocardia strains from different species and different sources were examined with respect to their lysogeny. 18 phages from 17 Nocardia asteroides strains could be isolated, purified, and propagated. All phages were released spontaneously. Induction experiments with mitomycin C and UV-rays brought no practicale advantages. Using 14 of the 18 phages, 38 of the 101 Nocardia strains tested could be grouped into 18 phage patterns. Nearly all of the typable strains belonged to the species Nocardia asteroides.

Studies on bacteriophages of Propionibacterium acnes.

With the help of adaptation experiments, 61 phage preparations out of 36 Propionibacterium acnes bacteriophages (32 isolated by us and 4 sent from abroad) were established. On the basis of their stability, spectrum of activity, and virulence, 13 phages were selected for phagetyping. 58 well-classified P. acnes strains were grouped into 7 phage-types. 7 strains of P. granulosum, two strains of P. avidum, and one yet ungroupable microserophilic propionibacterium strain were resistant to all 61 phages, even when tested in 100 X RTD. A combination of phagetyping with biotyping resulted in data especially useful for the differentiation of P. acnes.

The correlation of susceptibility of different Propionibacterium strains to macrophage killing and antitumor activity.

The stimulation of the reticuloendothelial system and antitumor activity against sarcoma-L-1 of three different strains of Propionibacterium was investigated. The bacteria injected intraperitoneally with a single dose of 1 mg per mouse appeared to possess different effectiveness in both assays used. The same strains were examined for their susceptibility to phagocytosis and intracellular killing by murine peritoneal macrophages. The results obtained showed correlations between Propionibacterium resistance to degradation by phagocytic cells and their antitumor and stimulatory activities. It is concluded that the potency of antitumor and stimulatory activities of different strains of Propionibacterium are closely related to their cell wall structure.

Rifampicin-induced suppression of antitumor immunity.

The effect of rifampicin on transplantable tumor growth and antitumor immunity was studied. The growth of transplantable lung sarcoma was significantly enhanced in mice when rifampicin was given for 7 days or longer. Also, the antitumor effects of preimmunization of the animals with killed tumor cells were abrogated by this antibiotic. In vitro studies showed that spleen cells from tumor-bearing animals expressed significantly lower specific cytotoxicity toward syngeneic tumor cells, when the animals had been previously treated with rifampicin. Also, natural killer activity in healthy animals was diminished in mice receiving the antibiotic.

Comparative study on the immunostimulatory potency of different Propionibacterium strains.

198 strains of Propionibacterium acnes, P. avidum, and P. granulosum were tested for their immunostimulatory potency using splenomegaly as index of stimulation, the activity differed widely from strain to strain. The antitumor and antimetastatic properties of six strains with differing stimulation activities were carefully examined. A close correlation between spleen enlargement and antitumor effects could be observed. The simple spleen enlargement test is recommended for determination of the immunopotency of propionibacteria and for the standardisation of vaccine preparations.

Antibiotics and immunomodulation: effects of cefotaxime, amikacin, mezlocillin, piperacillin and clindamycin.

The effects of 7 days' chemotherapy on the humoral and cellular parameters of the host immune system are described. In Balb/c mice the effects of cefotaxime, amikacin, mezlocillin, piperacillin and clindamycin were examined. The delayed-type hypersensitivity reaction, as well as the IgM and IgG responses, were suppressed by four of the five drugs tested: cefotaxime, amikacin, mezlocillin and piperacillin. One to two weeks after completion of chemotherapy with cefotaxime and amikacin, these parameters returned to normal values, whereas the mezlocillin- or piperacillin-modified reactions were still suppressed after 20 days. The concanavalin A and lipopolysaccharide-induced proliferative activities of mouse spleen cells were suppressed, especially by mezlocillin. The possible consequences of the immunomodulating effects of antibiotics for antimicrobial chemotherapy are discussed.

A comparison of staphylococcal nasal carrier rates in Germany and Poland.

Frequencies of different staphylococcal nasal carrier classes were studied in groups of students of nursing and of laboratory personnel in Cologne, West Germany, and in Krakow, Poland, by six consecutive samplings of their nasal vestibules. The general number of persistent carriers of S. aureus appeared to be lower than in previous studies, paralleling the diminished numbers of staphylococcal infections in populations. Differences in numbers of persistent carriers between the Cologne and Krakow groups were probably related to the incidence and types of S. aureus in the environment.

Beneficial response of local immunotherapy with Propionibacterium granulosum KP-45 in combined treatment of inflammatory breast carcinoma.

Nine 44- to 67-year-old patients with inflammatory breast carcinoma were treated over 2 weeks with intratumoral injections of Propionibacterium granulosum KP-45 (KP). This period of immunotherapy was succeeded by four courses of chemoimmunotherapy (FAC: 5-fluorouracil, adriamycin, and cyclophosphamide + intratumoral KP). Inflammatory symptoms disappeared in three patients during immunotherapy and in the remaining six patients during the following chemoimmunotherapy. Finally, 3 to 4 months after starting the therapy, all nine patients were free from inflammatory symptoms and it became possible to perform radical (seven cases) or simple (two cases) surgery. Thereafter routine therapy (radiotherapy, fractionated dose of 5500 R, followed by 10 FAC courses + single injections of KP for each FAC course) was used. After 19 to 32 months observation time all patients are still in complete remission with no local recurrences. Only one patient showed distant metastases during the observation period.
