[Classification and clinical-electrocardiographic evaluation of his bundle-branch block].

Investigation covered 114 patients with ischemic heart disease exhibiting electrocardiographic signs of deranged intraventricular conduction. Major ECG changes with blocked left anterior and left posterior brances of the bundle of His and also in combined blocks of two and three branches are described. A quantitative spatial analysis of the ECG in corrected orthogonal leads after McFee with determination of the initial, terminal and median orientation of the QRS vector in the frontal plane, as well as of the azimuth and elevation was made. A classification of the His bundle-branch block is proposed. It includes single-, double- and triple-bundle branch blocks. Clinical and electrocardiographic comparisons showed that disrupted conduction in the ramification system of the bundle of His occurs with marked diffuse or focal alterations in the myocardium of the left ventricle observed in patients afflicted with different forms of ischemic heart disease.

[Monitoring human African trypanosomiasis in Central Africa in 2001 and cartography: results and perspectives].

Cases of human African trypanosomiasis are distributed in changing geographical "outbreak areas" that can be visualized over time and space. Because of these variations in distribution, cartography and spatial analysis provide powerful tools for planning surveillance and control strategies. In 1996, the WHO in collaboration with the 15 most endemic countries in Central Africa undertook a program to develop a standardized inter-regional map of trypanosomiasis. This article provides a brief overview of the value of geomatic tools in public health followed by a description of the WHO program and its preliminary results. Also presented in this article is the Trypinfo site being development on the internet to increase the surveillance response-time and improve the feedback system.

The design of a web-based decision support system for the sustainable management of an urban river system.

The effects of urbanization on the aquatic environment, and solutions to the deterioration of water quality and stream ecology in the Love River have long been the main focus of environmental management in southern Taiwan. Apart from choosing the regular strategies of installing an intercept and sewer system, coastal wastewater treatment plant, and ocean outfall pipe, a new opportunity for improving the overall managerial efficiency is to design and implement a web-based Decision Support System (DSS). This DSS must be capable of managing storm water impacts when overflow is inevitable, river water quality variations loading to influences on the ecosystem, and changing land use programs along river corridors and adjacent urban areas simultaneously. This paper presents a new design framework for building such a DSS. By using the advanced information technology in the "Internet" environment, the proposed DSS may perform normal queries and statistical analyses in a web-enabled database, spatial analysis via the use of a Geographic Information System (GIS) in the Internet environment, and essential data warehousing/data mining. Possible linkage with various analytical models is anticipated. Such a DSS must be helpful for achieving the rehabilitation of the estuarine ecosystem and satisfying the goals for sustainable management in a regional sense.

[The status of conditioned reflex activity in cats after unilateral vestibular neurotomy].

In chronic experiments on 6 cats the influence was studied of unilateral vestibular neurotomy on conditioned, oculographic and electrocardiographic reactions. In operated animals appeared sharply expressed posetonic and oculomotor disturbances, lowered general functional brain state, what was manifested in an increase of specific weight of slow spindle-shaped rhythmics and lowering of the conditioned activity level. Against the background of the lowered functional brain state interhemispheric asymmetry developed with relative predominance of the contralateral hemisphere, what was reflected in electrocorticographic manifestations and disturbance of conditioned spatial differentiations. Significance is grounded of the appearing interhemispheric asymmetry in the development of disturbances of spatial analysis in operated animals.

[Conformational aspects of the biological action of functional fragments of the p21ras oncoprotein family. 1. Fragments 1-11 and 9-16 of the polypeptide chain].

Using theoretical conformational analysis, spatial structures of the N-terminal undecapeptide, common to all p21 modifications, and of the 9-16 fragments of the protein's active and passive analogues have been investigated. The data obtained reveal an essential differences between the predominant backbone forms of the active and passive modifications of the oncoprotein.

In vivo evaluation of 111In-labeled T-lymphocyte homing in experimental colitis.

UNLABELLED: Blockade of lymphocyte recruitment to the intestinal mucosa is considered a useful therapy for inflammatory bowel disease (IBD) and anti-alpha4 antibodies have clinical benefit in patients with active Crohn's disease. The aim of this study was to evaluate a scintigraphic technique to assess lymphocyte homing to the colon in 2,4,6-trinitrobenzenesulfonic acid (TNBS)-induced experimental colitis (TNBS colitis) in vivo. METHODS: TNBS-sensitized and nonsensitized murine total lymphocytes or CD4+ lymphocytes were radiolabeled with 111In-oxinate. Cells were injected into control mice (n = 5) or mice with TNBS colitis (n = 5). Specific abdominal radioactive uptake was determined by SPECT using a dedicated pinhole system 48 h after cell transfer. Radioactive colon uptake was correlated with histology and colon weight as parameters of inflammation. RESULTS: The radioactive colon uptake was most evident in mice with TNBS colitis that received sensitized lymphocytes (uptake ratio [mean +/- SEM], 0.51 +/- 0.03 vs. 0.22 +/- 0.04; P = 0.004). The sensitized 111In-labeled lymphocytes exacerbated colitis compared with nonsensitized lymphocytes. The colon uptake correlated well with both colon weight and histologic score (R2 = 0.836 and 0.933, respectively). The use of purified 111In labeled CD4+ lymphocytes resulted in a similar scintigraphic pattern. Administration of an anti-alpha4 antibody decreased radioactivity colon uptake of the (111)In-labeled cells compared with the control antibody in mice with TNBS colitis (uptake ratio, 0.72 +/- 0.14 to 0.33 +/- 0.03; P = 0.012). CONCLUSION: Animal pinhole SPECT can be applied for temporal and spatial analysis of the lymphocyte homing process in experimental colitis. This technique makes possible the in vivo evaluation of therapeutic efficacy of new drugs that interfere with lymphocyte migration. Moreover, colon uptake of radioactivity can be used as a parameter of disease activity in experimental colitis.

GIS-based prediction of malaria risk in Egypt.

Environmental variables in a malaria geographic information system (GIS) database were analysed to discriminate between governorates at high and low risk of malaria. Only Fayoum governorate was categorized as a high risk area for malaria during the last 2 decades. Discriminant models correctly classified 96.3% of the risk categories and indicated that the most important predictor of risk is hydrogeology. Further GIS spatial analysis indicated that the high malaria risk in Fayoum is associated with a unique environmental envelope of biotic (presence of both efficient malaria vectors) and abiotic (hydrogeology and soil) variables. Recommendations for surveillance and control are discussed.

[Bacterial meningitis: geographic and referential study in the City of Havana in 1998].

The behaviour of bacterial meningitis with identified agent in the City of Havana in 1998. The implementation of Geographic Information System Mapinfo 4.1 made it possible to know the spatial distribution of the sick people. As a result, we found an area of circulation of Neisseria meningitidis located to the northeast center; an area of prevalence of Streptococcus pneumoniae to the north and northeast of the city and zones located to the center, east and south where Haemophlus influenzae prevailed. These areas were related to the main sources of environmental pollution in the province. THe importance of the spatial analysis for expanding the knowledge for an epidemiological study was evidenced.

Effectiveness evaluation of different suction systems.

Microorganisms of the patient's oral cavity and his/her blood and saliva may cause different air-borne and blood-borne infectious diseases among odontologists and their assistants who work with patients. Quantitative analysis and spatial distribution analysis of the environmental spread of oral liquid and cooling liquid mixture were performed during this study. Effectiveness of suction systems of four types was evaluated: without suction, using a small-size suction pump alone, using a small-size and large-size suction pumps, using a small-size suction pump together with an experimental extra-oral aspirator. Quantitative changes of the water aerosol, which enters the environment during the preparation of teeth, were determined in respect of the used suction systems. The small-size pump system together with an experimental extra-oral suction system eliminated best the aerosol formed during the preparation.

[Zoning planning in non-point source pollution control in Hanyang district].

It is most important for managing urban non-point source (NPS) pollution, actualizing the urban sustainable development as well, that zoning planning of urban NPS pollution control is studied. A case study on principles and methods of zoning planning in urban NPS pollution is carried out. Principles of urban sustainable development, priority of urban NPS pollution sensitivity, similarity of urban NPS control direction and region conjugate are put forward. Besides, it is for the first time that a more quantitive method is presented, in the case of Hanyang district, Wuhan city, which is based on L-THIA model and spatial analysis technique in GIS. Assessment of NPS pollution status quo, as well as analysis of NPS sensitivity, is the kernel component of the quantitive method. Hanyang might be divided into four NPS pollution control zones. It is helpful for decision-making of regional NPS pollution control.

[An epidemiological survey of equine anaplasmosis (Anaplasma phagocytophilum) in southern France].

Anaplasmosis is caused by the bacterium Anaplasma phagocytophilum and transmitted by Ixodes spp. ticks. According to some reports the disease can be introduced into disease-free zones by migrating birds. The purpose of this study was to evaluate the seroprevalence of A. phagocytophilum in horses in the Camargue. Data concerning 424 horses were gathered and the sera were tested for A. phagocytophilum and for piroplasmoses using an enzyme-linked immunosorbent assay and a complement fixation test, respectively. The seroprevalence rates were 11.3 % for A. phagocytophilum, 64.4 % for Theileria equi and 19.7% for Babesia caballi. Stallions were less likely to produce antibodies against A. phagocytophilumthan were females or geldings (odds ratio [OR] = 0.3; p = 0.021). The presence of swallows increased the risk of infections in stables (OR = 5.18; p = 0.011). Spatial analysis showed the existence of groups of infected stables along canals and rivers (p = 0.008). These results suggest an emergence of A. phagocytophilum in the Camargue.

[Mortality in the area around Massa-Carrara 10 years after ANIC-Agricoltura and Farmoplant chemical plants were shut down].

OBJECTIVE: To describe mortality of residents in the area of Massa-Carrara for the period 1995-2000 and to compare it with mortality for the years 1990-1994. DESIGN: Geographical descriptive study. SETTING: In the area of Massa-Carrara cause and gender specific standardized mortality ratios (SMR), adjusted for age and municipal deprivation index (reference: Tuscany Region), have been computed for the years 1995-2000 and compared with mortality in the period 1990-1994 calculating ratios between standardized rates by age classes with a direct method (CMF). For those causes showing a statistically significant increase a spatial analysis on a group of municipalities around Massa and Carrara has been carried out. MAIN OUTCOME: Mortality for all causes, and for 30 specific causes. RESULTS: The study results confirm, for the years 1995-2000, the previous observation (1990-1994) of statistically significant excesses for mortality from all causes (SMR 109), all cancers (SMR 112), respiratory diseases (SMR 126), cirrhosis (SMR 226), liver (SMR 161) and lung cancer (SMR 115) in males. A statistically significant excess for larynx (SMR 158) and pleural cancer (SMR 178) is observed in the years 1995-2000, which was not present in 1990-1994. A mortality excess for cirrhosis is confirmed in females (SMR 158) and a new one emerges for liver cancer (SMR 144). The comparison between mortality data for the periods 1995-2000 and 1990-1994 has shown a statistically significant decrease in mortality for all causes and for several specific causes, a significant increase in liver (CMF males 1.35; CMF females 1.78) and blood cancer mortality is also detected (CMF females 1.44). CONCLUSIONS: This study confirms the negative health impact of environmental, occupational exposures in populations living in areas where high-risk factories are located. Adverse health effects are stronger in socially and economically deprived areas, and persist over many years after closing polluting industrial plants. The results of this study point out to the need of health promoting actions aiming at modifying lifestyle risk factors, ameliorating socio-economic conditions, and maintaining a constant control on environment, also through a specific environmental epidemiological surveillance to monitor the impact of reclamation procedures completed or in progress.

[Medical supervision in rescue work--systematics and personal studies].

All institutions of the organised public medical emergency services are functions of public health welfare and risk defence, hence they are subjects to the public health department in a supervisory capacity. Legal definitions and standards are the pertaining framework. Systematic and qualified improvements of areal plans, organization and realization, material and personal equipment, and hygienic standards in the public medical emergency service are Supervisory tasks that have to be tackled by public health departments. Planning requires random tests and local spatial analysis. This may be performed by the public health department itself or by external institutions. Therefore, organized public medical emergency service is part of (communal) health reports.

[The effect of amino acid substitutions on the 3-dimensional structure of fragment 1--16 of the oncoprotein p21 ras].

Using the method of theoretical conformational analysis, spatial structure of fragment 1-16 of active [( Val12-Gly13], [Asp12-Gly13], [Gly12-Asp13]) and passive [( Gly12-Gly13] and [Pro12-Gly13]) modifications of oncoproteins family p21 ras have been investigated. The activation of these proteins has been shown to be accompanied by reorganization of three-dimensional structure of the polypeptide chain.

A microcomputer-based algorithm for digitized image simplification and information extraction.

Spatial analysis of objects often requires significant image simplification prior to information extraction and application of a decision-making algorithm. Much decision making based on images (e.g., histologic diagnoses) requires identifying patterns in complex backgrounds (image simplification) and comparison of those patterns to other patterns (decision making). Automated extraction of information from images commonly requires the extraction system to recognize edges (contours) of structures and their internal discontinuities (such as gradations in density) and to selectively suppress irrelevant data in order to conserve memory and speed computation; data from homogeneous image areas occupy memory, but are noncontributory or redundant. This paper describes the development of a microcomputer-based algorithm that deletes all homogeneous information from overlaid digitized images, generating contours in the place of nonhomogeneities. Contours corresponding to different areas or objects depend on color differences between an object and its surroundings. Any set of contours can be deleted almost instantaneously, leaving only those of interest. Contours can be highlighted by an operator-driven interactive process if desired and can be deleted and retrieved until an appropriate image is obtained. This contour-generating and image-simplification algorithm facilitates three-dimensional reconstruction of an object from serial images by reducing the number of calculations required and yielding a cleaner final image.

Infectious hepatitis in urban India.

The paper is about infectious hepatitis in urban India. The discussion is on two case studies: one of Madras, and the other of Madurai: two major cities in the State of Tamilnadu, South India. The Madras study is on the temporal and spatial analysis of the distribution and diffusion of hepatitis during 1971-1978. The Madras study also relates age to incidence, the major conclusion being that children are more susceptible to hepatitis than elders. The diffusion of hepatitis is said to occur in a wave - like form, covering newer areas and intensifying in time in core areas. The Madurai study is about the epidemic of hepatitis during January-October of 1981. Relating incidence with share of slum population, and number of borewells in city localities, the study yields a positive relation between them with a correlation coefficient value of .7424 and .7812, respectively. General lack of environmental quality and use of contaminated water for domestic purposes appear to be causes of high incidence of infectious hepatitis.

Preparation and observation by SEM of hemopoietic cells cloned in soft agar.

A method is described to prepare clones of hemopoietic cells grown in soft agar for scanning electron microscopy (SEM). A critical modification of the otherwise quite standard SEM processing procedure for biological samples involved the use of silver micropore disks as an adherent substrate to support the highly labile, deformable agar slabs. This support allows maintenance of the normal flat pancake shape of the specimen through the thiocarbohydrazide osmium ligand binding steps, dehydration, and critical point drying. With this support and careful dissection of the surface agar with a fine steel needle using a stereomicroscope, selected areas and depths within the colony can be exposed and examined by SEM. Surface topography of cloned cells can be correlated with intracellular cytological features by excising areas of interest and directly embedding them in plastic for thin-section preparation and viewing by transmission electron microscopy (TEM). The dried-specimen-teasing method appears useful, because of the ease of preparation of the specimens, its reproducibility, and the degree of visibility and preservation of cell surface structures and intraclonal relationships. Our initial observations, using combined EM techniques, indicate that clonal cell topography is highly variable and that this variability appears to be related both to the relative age and proliferative status of the colony. Based on work to date, we suggest that topographical and spatial analysis, in vitro of cloned, agar-embedded hemopoietic stem cells is possible with simple modifications of conventional SEM preparative techniques.
