[Puncture of the anonymous vein].

To introduce a caval catheter means especially in the hands of a beginner, a certain number of potentially dangerous complications. Due to that the way of its application is a subject of further perfectioning. The puncture of anonymous vein is an innovation, thanks to the technical development of a necessary equipment such as a catheter which is introduced by a Seldinger technique, at the typical place over the clavicula, as the catheter is long only 12 cm. (for the right side); the x-ray control is necessary to exclude event. pneumothorax. Experience over a 75 patients is presented, in 6 of whom it was not possible to puncture the anonymous vein, in another 6% there was a puncture of the artery. The catheter has been left 8 days in average for infusion therapy, transfusion and measurement of C. V. P., and for taking blood samples. On the basis of this experience it can be concluded that this treatment can have the clinical application.

[Identification of the peridural space with the infusion method in relation to the incidence of inadvertent puncture of the dura].

In 868 cases the identification of the epidural space was performed by plugging a running infusion drip. After accurate identification of the epidural space the patient received either 0.5% or 0.75% bupivacaine without adrenalin for operations on the lower extremities, at the perineum and for lower and upper abdominal surgery. Analgesia was adequate . The only disadvantage of this method as with the "loss of resistance" technique consists in the fact that, if inadvertent dural puncture occurs, it is impossible to recognize the difference between the epidural and spinal space, because the saline solution might reach in any case into the spinal space. In the last 512 cases in which we used the "infusion" technique we observed only 5 patients with an inadvertent dural puncture. Thus the frequency of this complication was below 1%. This is - even according to the latest publications - quite a satisfactory result. An unidentified accidental dural puncture and associated complication did not occur.

[Cutaneous cerebrospinal fluid fistula associated with secondary puncture of the dura caused by a peridural catheter].

Prolonged liquor fistula following puncture of the subdural space for diagnostic or anaesthetic purposes has been reported repeatedly. These among other factors are held responsible for headache as well as neurological complications in connection with lumbar puncture. In contrast the appearance of an external liquor fistula in the lumbar region is an extremely rare event. Two patients developed a cutaneous liquor fistula after lumbar laminectomy following an inadvertant dural penetration when an epidural steroid injection was performed. The same occurred in a healthy young patient who had undergone several trials to puncture the epidural space for an epidural anaesthesia in a case of emergency cesarean section Our own observation concerns a 62 years old woman who developed a cutaneous cerebrospinal fluid fistula 12 h after the removal of an epidural catheter, which had caused a secondary perforation of the dura.

[Determination of the take efficiency of lyophilized smallpox vaccine as a function of the vaccination technic (double scarification, multi-puncture and jet injection)].

Revaccination against smallpox was performed by three techniques: double scarification, multiple puncture and jet injection in 352 youths aged 16 and 17 years. The results were assessed by the number of takes, the quality of the cutaneous and humoral response. The highest proportion of major skin reactions was obtained by multiple puncture vaccination (62.4%) and the best humoral response, tested by passive haemagglutination, by jet revaccination. Multiple puncture is recommended as a method of election both in restricted and mass vaccination in view of the high proportion of major skin rections accompanied by a corresponding humoral response, the rapid and readily performed technique and small amount of vaccine needed.

[Ultrasonic echography as a diagnostic measure before supra-pubic bladder puncture].

It is reported on 40 punctures of the bladder after previous ultrasound diagnostics. In all cases the puncture could be performed successfully without failure at a filling stage of the bladder of 250 to 300 ml. By this possibility to puncture the bladder at a physiological filling stage changes of the urine tests were avoided which appear by the often recommended methods for the increase of diuresis.

[Importance of aspiration puncture of the thyroid gland and of the determination of autoantibodies to its tissue in the diagnosis of autoimmune lesions of the organ in an endemic goiter locality].

Comparative studies on the effectiveness of aspiration puncture of the thyroid gland and detection of autoantibodies to its tissue were carried out in 523 patients operated for nodose non-toxic goitre and living in the different goitre-endemic areas. The puncture results and autoantibody detection (before surgery) were compared with the histological findings. It is concluded that the aspiration puncture more adequately than autoantibody detection reflects the histological type of autoagression in endemic goitre combined with phenomena of autoimmunity to the thyroid gland tissue. Nevertheless, autoantibody detection in these patients more frequently provides positive results. Therefore it is recommended that both methods be used simultaneously.

[Clinical assessment of the activity and results of therapy in joint diseases through cytologic examination of puncture fluids (author's transl)].

Activity and results of therapy of joint diseases can be evaluated by means of cytological examinations of the synovia which is at the same time a prerequisite for differential diagnosis using the puncture fluid. Our cytological examinations of more than 700 puncture fluids stemming from different joint diseases were carried out immediately after the puncture on the fresh material, in order to prevent misinterpretation. This may occur through flaking out in stored material, which results in decrease in the cell count and a non-representative differential cell picture in the supernatant, as well as through degeneration of cells. In two thirds of our patients the local process activity, measured in terms of the cytological phase of activity, became evident in the total clinical activity. The synovia analysis furthermore permits objective assessment of the result of intra-articular injection treatment. Intractable activity, substantiated through synovia examination indicates synovectomy.

[Tumor seeding of the abdominal wall after fine needle cytologic puncture of the liver. Apropos of a case].

Tumor seeding of the abdominal wall during fine needle puncture-aspiration of the liver is a very rare complication. The authors report a case of this complication affecting a 50-year-old Senegalese woman. She underwent echo-guided fine needle cyto-puncture of a hepatocellular carcinoma arising from the left lobe, followed by left lobectomy. The subcutaneous nodule was discovered 42 months after cyto-puncture. Histology following its excision revealed infiltration of carcinomatous hepatocytes between the fibers of rectus abdominis. There was no recurrence and the patient was well 10 months after excision of the subcutaneous nodule. With a review of 10 others cases reported in the literature, the authors feel that, although rare, this complication must be avoided and suggest a system for the protection of structures through which the needle passes.

[Contemporary methods of the target puncture of the mammary gland].

New accurate methods of puncture of the mammary gland, making use of preliminary layout using roentgenograms, coordination nets, Cytoguide comprter device, and ultrasound-monitored spot biopsy replaced the traditional poorly informative methods. Sonography-monitored puncture is the most informative in cases with both palpated and unpalpated formations shaped as nodules: it permits imaging in 85% of cases. However, in 25-30% of cases unpalpated cancer presents as microcalcinates and cord-like restructuring which cannot be imaged on sonograms. Moreover, unpalpated cancer presenting as a nodule against the background of fatty involution also may be undiscernible on sonograms. In such cases puncture controlled by roentgenography with a computer device and a relevant kit of disposable instruments is advisable, giving accurate information in 61.1% of cases.

[The influence of changing the Ca2+ concentration of the point quze (PC3) on the curative effect of puncturing neiguan (PC6) in experimental arrhythmic rabbits].

The experimental arrhythmic model was made with Aconitin in rabbits. The Neiguan (PC6) point was punctured to observe the influence on recovering time of arrhythmia. After the curative effect of puncturing Neiguan(P6) was confirmed, Ca2+ in the Quze (PC3) point of the Pericardium is chelated by EDTA solution to observe the influence on curative effect of arrhythmia by puncturing Neiguan. The result shows that the curative effect doesn't exist after the Ca2+ in Quze is chelated. It is probable that Ca2+ is one of the most important factors producing acupuncture effect. As a result, Ca2+ probably involves in the activities of channels and collaterals.

[Etiological diagnosis of spondylitis. II. Vertebral puncture biopsy and surgical approach].

The authors tried to determine the ways in which vertebral puncture biopsy and the direct surgical approach can helpin the etiological diagnosis of spondylodiscitis, when the clinical radiological, and laboratory examination leave the clinician in doubt. Out of 28 vertebral puncture biopsies, proof of tuberculosis was obtained in 4 cases, and proof of a non-tuberculous cause was obtained in 1 case as a result of isolating the responsible organism. Thirty-eight cases were submitted to the direct surgical approach. When the indication for surgery was solely in order to investigate the etiology, proof of tuberculosis was obtained in half of the cases, and only exceptionally was a non-tuberculous organism discovered. In almost half the cases, the histological characteristics were non-specific and no organisms were detected. Most of these latter cases were, in fact, non-tuberculous spondylodiscitis, although subsequently some of them exhibited evidence of a tuberculous character. In the present state of knowledge, it seems that, in cases of clearly non-tuberculous spondylodiscitis, the direct surgical approach is not justified if the objective is solely to isolate the organism, so that its sensitivity to antibiotics can be tested. The following reasons are given for this conclusion : the direct surgical approach only rarely leads to isolation of the causal organism; although treatment based on knowledge of antibiotic sensitivity may help to restrict evolution of the disease, it does not reduce significantly, or only rarely, the permanent partial incapacity. If the results published by Seignon and Gougeon are confirmed, early needle puncture of the diskovertebral centre of the disease should be practised more widely.

[Analysis of three-dimensional imaging of the right hepatic vein and right portal vein branch using 2D-TOF MR angiography: evaluation of utility of MR angiography in determining puncture course on TIPS].

2D-TOF MR angiographies were performed in seven patients of liver cirrhosis with hepatocellular carcinoma to investigate three-dimensional anatomical relationships of the right hepatic vein and first order of right portal vein branch. The puncture course was set up in three-dimensional rectangular coordinates, and the bend angle and turning angle of the modified Ross needle, and puncture distance were calculated of the data measured on the A-P and L-R projection images of MR angiograms. The utility and limitation of 2 D-TOF MR angiography to determine the puncture course on TIPS were discussed in this study.

Percutaneous biopsy in diffuse liver disease: increasing diagnostic yield and decreasing complication rate by routine ultrasound assessment of puncture site.

OBJECTIVE: To evaluate the usefulness of routine ultrasound assessment of puncture site before performing percutaneous biopsy in diffuse liver disease. Seven hundred fifty-three consecutive patients were studied retrospectively. METHODS: Serial scanning of the last intercostal spaces allowed us to establish the most suitable access to the thicker liver parenchyma (assessing the most favorable angulation of the needle too), avoiding the puncture of adjacent organs; no more than 1 min was necessary for such a determination. RESULTS: In 99.4% of patients, a definitive or indicative pathological diagnosis of chronic liver disease was obtained. Only one hemorrhagic complication (0.13%) occurred, requiring no surgical treatment or blood transfusion. Three cases of vasovagal reaction occurred (0.40%): two of these recovered spontaneously, while the other one needed i.v. administration of atropine. Mortality was 0 in our series. CONCLUSIONS: Routine ultrasound of the puncture site is a quick method of assessment, allowing one to increase the diagnostic yield of percutaneous liver biopsy and to maintain low complication rates for such a procedure.

Central venous puncture versus cutdown for permanent pacemaker lead insertion: a modified double introducer technique.

OBJECTIVE: The comparative safety of central venous puncture versus cutdown technique for access to the right heart. EXPERIMENTAL DESIGN: A retrospective review of five hundred and fifty-five single and dual chamber permanent pacemaker implants performed between January of 1987 and December of 1993. SETTING: Review was performed at St. Vincent's Hospital in New York, New York and its teaching affiliate, Huntington Hospital, Huntington, New York. RESULTS: A total of 310 venous punctures were performed. The complication rate was 3.5% overall, with one procedure related death (0.3%) from major venous laceration; a stiff 14 French dilator sheath was employed in this case. There were no comparable complications in the 245 lead placements performed by various cutdown methods. CONCLUSION: Venous cutdown is a safe and effective method for placement of single and dual chamber pacemaker leads. If unsuccessful, the cephalic vein guidewire and double introducer techniques may be employed to rule out placement of dual chamber leads through a hypoplastic cephalic vein, using introducers not larger than 10 French. In properly selected patients, this technique could potentially prevent the complications associated with venous puncture.

Soft tissue and bone infections from puncture wounds in children.

We report on the prevalence of osteomyelitis, the prevalence of soft tissue infections, and the type and number of pathogens encountered in bone and soft tissue infections caused by puncture wounds in children. In addition, we seek to establish whether shoe gear plays a role in the flora in infected puncture wounds and if laboratory indices are indicative of the presence of infection. The group consisted of 44 nondiabetic children admitted to hospital for puncture wounds of the foot. Cultures were positive for osteomyelitis in 7 patients (16%), all involving the forefoot (P < .04). The most common pathogen in soft tissue infections was Staphylococcus aureus. The most common pathogen in osteomyelitis was Pseudomonas aeruginosa. There was no significant difference in the prevalence of osteomyelitis and soft tissue infection based on footwear. There were no cases of osteomyelitis encountered among barefoot children (P < .04). In 10 cases (83%), P aeruginosa infection (both soft tissue and bone) occurred while the patients were wearing tennis shoes (P < .04). In this study, the leukocyte count (normal in 29 patients [66%]), erythrocyte sedimentation rate (normal in 28 patients [64%]), and temperature (normal in 44 patients [95%]) did not have any predictive value in differentiating soft tissue infection from osteomyelitis in children.

[Percutaneous puncture in hepatic echinococcosis].

This is a report on a case of severe anaphylactic shock and hemorrhage following liver puncture with diagnostic purpose. Based on literature data and personal experience, percutaneous puncture of a parasitic expanding cyst, done with diagnostic and therapeutic purpose, is discarded, assuming the method as an error of art. Its performing using updated imaging techniques--echography and computerized tomography--by no means reduce the risk of anaphylactic shock and secondary echinococcosis. In the event of being accidentally done, emergency operation followed by chemotherapy to preclude secondary echinococcosis is recommended. In cases with unspecified diagnosis, puncture under echographic control is done on the operating table with urgent cytological assessment of the punctate. The demonstration of echinococcosis is an indication for immediate operation. The percutaneous method is used in handling nonparasitic cysts.

Effect of recirculation of 99mTc-Sn colloid in estimating mean transit time of soft-tissue vascular anomalies by direct puncture scintigraphy: a simulation analysis.

We have reported direct puncture scintigraphy, a novel method to estimate mean transit time (MTT) of soft-tissue vascular anomaly. In the examination, 99mTc-Sn colloid is infused percutaneously into the lesion, and MTT is calculated using the time-activity curve for the lesion. Recirculation of 99mTc-Sn colloid is neglected in computing MTT, and this may cause error. The aim of this study was to assess this error by simulation analysis. Data simulating direct puncture scintigraphy were produced for lesions with various blood volumes and various blood flows, considering recirculation and blood clearance of 99mTc-Sn colloid, and MTT was estimated by the same method used in the patient examination. Although MTT was overestimated in large lesions with intermediate or long MTT, the error was small except in extremely large lesions. It was suggested that recirculation of 99mTc-Sn colloid does not cause a serious problem in the clinical use of direct puncture scintigraphy.

[A differentiated approach to laser puncture in facial neuritis by using the methods of acupuncture diagnosis].

Overall 87 patients with neuritis of the facial nerve were studied by making use of methods of acupuncture diagnosis as recommended by Nakatani and Akabane. The parameters under investigation included the level of physiologic corridor, left-right asymmetry, the Yang and the Yin meridians sum ratio, maximum deviation from the physiological corridor in certain meridians. The results obtained permitted the identification of three groups of patients depending on the duration of their illness. This permitted the differentiated approach to the use of laser puncture in these patients securing good results of treatment which were confirmed by both the positive dynamics of acupuncture methods of investigation and data supplied by the electrophysiological techniques (EMG, EEG). With the purpose of performing laser puncture, the infrared laser puncture apparatus Biomed-001 (wavelength 0.89, average laser emissive power 20 mw) was employed.
