Sexual health promotion in adolescents with chronic health conditions.

Sexual health promotion is essential to the health and well-being of adolescents, including those with chronic health conditions. The purposes of this article are to define sexual health and to describe the physical, cognitive, psychosocial, and spiritual domains of adolescent development, with special focus on the sexual health needs of adolescents with various chronic health conditions. The article concludes with several suggestions for healthcare professionals, parents, and teachers in addressing sexual health promotion in this vulnerable population.

The diagnosis and care of transsexual children and adolescents: a pediatric endocrinologists' perspective.

The normally developed child whose gender identity and anatomic sex disagree is referred to as a transgendered child, or as used subsequently in this text, a transsexual. The ramifications of this disagreement include a high risk of psychiatric conflict and maladjustment, for both the individuals themselves and their families. Despite the efforts of researchers to systematically study this group of children, many fundamental questions remain. In many respects, those lingering questions are shared by patients with physical intersex who have been cared for by pediatric endocrinologists. In intersex and transsexual patients, the medical community, although sincerely interested, remains wary to intervene in ways that may lead to further inconsistency between anatomic sex and adult gender identity. A perspective on the problems of differentiating permanent from transient gender identity, some thoughts on the most appropriate management of the transsexual child/adolescent as well as remaining questions are discussed. Both the flexible and therefore potentially misleading gender identity in children and the medical communities' pledge to first do no harm (primum non nocere) have regrettably fostered disharmony between gender disordered patients, their families, and the practitioners who want to help them.

Diagnostic play interview. Its role in comprehensive psychiatric evaluation.

The purpose of this article is to describe the importance of including a diagnostic play session as a major component of a comprehensive psychiatric assessment of young children. It is through observing a young child's play that a clinician can gather a wealth of information about a child's developmental level in the cognitive, social, language, and motor domains as well as some insight into the development of a child's personality and inner world. This data combined with the results of developmental testing, school observations, and parent interviews can be important in making formulations and diagnoses and planning strategies for interventions.

[The psychodynamics of violence against children (author's transl)].

It was attempted to describe some important aspects of the complicated psychodynamics of violence against children. The empirical research has still the task to solve many essential problems concerning this subject. Nevertheless we can conclude from our knowledge we gained up to now, that great attention has to be paid to the psychodynamics in all therapeutic, preventive and legislative initiatives, which concern the problem of battered child syndrom and physical punishment. This is necessary in order to focus on a symptom only, that is the flogging of children, but to influence the psychodynamic conditions as well.

Management of agenesis of the vagina.

The management of 92 women with vaginal agenesis is presented. All patients were managed by a modified McIndoe procedure. Four women (4 per cent) had intraoperative or postoperative complications, but only one woman had a complication that resulted in failure of vaginal function. We advocate the McIndoe vaginoplasty as the primary technique of management for women with vaginal agenesis.

Thoughts on psychoanalytic views of female psychology 1927-1977.

Stimulated by the recent volume on female psychology edited by H. Blum (1977), the author reviews the beginnings of her own interest in the subject more than fifty years ago, particularly her early studies of the preoedipal development of the little girl. Dr. Lampl-de Groot's experience has led her to emphasize the common characteristics in the early development of boys and girls: they both experience curiosity, envy, competition, and jealousy. Only in the phallic-oedipal phase, when rivalries center on the parents, do penis envy and a specific sense of female identity develop in the girl, while boys must resolve their envy of the baby as a prized "product" only the female can produce. Recent literature is criticized, particularly writings by some women authors who attempt to reverse unconscious feelings of inferiority by using recently discovered "facts" of biology for polemical purposes.
