Levalbuterol aerosol delivery with a nonelectrostatic versus a nonconducting valved holding chamber.

BACKGROUND: Hydrofluoroalkane-propelled levalbuterol (Xopenex) aerosol is a recently approved formulation for delivery via metered-dose inhaler for the treatment or prevention of bronchospasm in adults, adolescents, and children > or = 4 years of age who have reversible obstructive airway disease. Valved holding chambers (VHCs) made from conventional polymers are susceptible to accumulation of electrostatic charge, which can be minimized by prewashing with ionic detergent, but it may be desirable to be able to use the product straight from the package, without pretreatment, especially during an exacerbation. METHODS: We studied the performance of the AeroChamber Plus and AeroChamber Max VHCs in delivering hydrofluoroalkane-propelled levalbuterol. Both VHCs were prewashed, rinsed, and drip-dried before testing. The AeroChamber Max is manufactured from charge-dissipative material and was therefore also evaluated without prewashing. Aerosol samples were collected at 28.3 L/min with an Andersen 8-stage cascade impactor, per the procedure specified in Chapter 601 of the United States Pharmacopeia. RESULTS: The mean +/- SD fine-particle mass (mass of aerosol particles < 4.7 microm aerodynamic diameter) values were 33.5 +/- 1.4 microg and 36.3 +/- 1.1 microg with the AeroChamber Max, without and with wash/rinse pretreatment, respectively, and 28.5 +/- 2.4 microg with the prewashed AeroChamber Plus. CONCLUSIONS: We think the small differences we observed are unlikely to be of clinical importance, given the inter-patient variability seen with inhaled drug delivery. The performance of the AeroChamber Max was substantially comparable whether or not it was prewashed.

Application of inorganic oxidants to the spectrophotometric determination of ribavirin in bulk and capsules.

Eight spectrophotometric methods for determination of ribavirin have been developed and validated. These methods were based on the oxidation of the drug by different inorganic oxidants: ceric ammonium sulfate, potassium permanganate, ammonium molybdate, ammonium metavanidate, chromium trioxide, potassium dichromate, potassium iodate, and potassium periodate. The oxidation reactions were performed in perchloric acid medium for ceric ammonium sulfate and in sulfuric acid medium for the other reagents. With ceric ammonium sulfate and potassium permanganate, the concentration of ribavirin in its samples was determined by measuring the decrease in the absorption intensity of the colored reagents at 315 and 525 nm, respectively. With the other reagents, the concentration of ribavirin was determined by measuring the intensity of the developed colored reaction products at the wavelengths of maximum absorbance: 675, 780, 595, 595, 475, and 475 nm for reactions with ammonium molybdate, ammonium metavanidate, chromium trioxide, potassium dichromate, potassium iodate, and potassium periodate, respectively. Different variables affecting the reaction conditions were carefully studied and optimized. Under the optimum conditions, linear relationships with good correlation coefficients (0.9984-0.9998) were found between the absorbance readings and the concentrations of ribavirin in the range of 4-1400 microg/mL. The molar absorptivities were correlated with the oxidation potential of the oxidants used. The precision of the methods were satisfactory; the values of relative standard deviation did not exceed 1.64%. The proposed methods were successfully applied to the analysis of ribavirin in pure drug material and capsules with good accuracy and precision; the recovery values were 99.2-101.2 +/- 0.48-1.30%. The results obtained using the proposed spectrophotometric methods were comparable with those obtained with the official method stated in the United States Pharmacopeia.

Microemulsions with timolol as potential eye drops.

Stable microemulsions oil-in-water (o/w) and water-in-oil (w/o) with timolol were obtained. These formulations were sterilized using two methods: by autoclaving and by sterile filtering according to the Polish Pharmacopeia V. Microemulsions were kept in the temperatures of 25 degrees C and 5 degrees C during three months. The following physical parameters were investigated: density, dynamic viscosity, refractive index, surface tension, pH and osmotic tension. All the microemulsions met the requirements for eye drops.

A validated spectrofluorometric assay for the determination of certain macrolide antibiotics in pharmaceutical formulations and spiked biological fluids.

This paper describes a simple spectrofluorometric method for the analysis of 4 macrolide antibiotics. The method is based on the condensation of 10% (w/v) malonic acid and acetic acid anhydride under the catalytic effect of tertiary amine groups of the studied macrolides. The relative fluorescence intensity of the condensation product was measured at 397/452 nm (excitation/emission) for azithromycin dihydrate and at 392/445 nm (for clarithromycin, erythromycin ethylsuccinate, and roxithromycin. All variables affecting the reaction conditions were studied. The effects of potential interference due to common excipients, such as starch, lactose, sucrose, glucose, gum acacia, and magnesium stearate, as well as trimethoprim and sulfisoxazole acetyl formulated in primomycin capsules and pediazole oral suspension, respectively, were studied. A validation study for the proposed method was carried out according to U.S. Pharmacopeia 2002. The linearity ranges were 3-80 ng/mL for all of the cited macrolides. The limit of detection range was 0.74-1.20 ng/mL, while the limit of quantitation range was 2.47-4.02 ng/mL. The method was applied for the assay of the studied macrolides in pure pharmaceutical formulations and in spiked biological fluids. Results were compared with those obtained from the reported method, where calculated t- and F-values indicated high accuracy and good precision for the proposed method.

Tool from ancient pharmacopoeia prevents vision loss.

PURPOSE: Bear bile has been used in Asia for over 3,000 years to treat visual disorders, yet its therapeutic potential remains unexplored in Western vision research. The purpose of this study was to test whether treatment of mice undergoing retinal degeneration with tauroursodeoxycholic acid (TUDCA), a primary constituent of bear bile, alters the course of degeneration. METHODS: Two retinal degeneration models were tested: the rd10 mouse, which has a point mutation in the gene encoding the beta subunit of rod phosphodiesterase, and light induced retinal damage (LIRD). For LIRD studies, albino Balb/C adult mice were subcutaneously injected with TUDCA (500 mg/kg body weight) or vehicle (0.15 M NaHCO(3)). Sixteen h later, each mouse received repeat injections. Half of each treatment group was then placed in bright light (10,000 lux) or dim light (200 lux) for seven h. At the end of exposure, animals were transferred to their regular housing. Electroretinograms (ERGs) were assessed 24 h later, mice sacrificed, eyes embedded in paraffin and sectioned, and retina sections assayed for morphology and apoptosis by TUNEL and anti-active caspase-3 immunoreactivity via fluorescent confocal microscopy. A subset of mice were sacrificed 8 and 15 days after exposure and retina sections analyzed for morphology and apoptosis. For rd10 studies, mice were injected subcutaneously with TUDCA or vehicle at postnatal (P) days 6, 9, 12, and 15. At p18, ERGs were recorded, mice were euthanized and eyes were harvested, fixed, and processed. Retinal sections were stained (toluidine blue), and retinal cell layers morphometrically analyzed by light microscopy. Consecutive sections were analyzed for apopotosis as above. RESULTS: By every measure, TUDCA greatly slowed retinal degeneration in LIRD and rd10 mice. ERG a-wave and b-wave amplitudes were greater in mice treated with TUDCA compared to those treated with vehicle. Retinas of TUDCA-treated mice had thicker outer nuclear layers, more photoreceptor cells, and more fully-developed photoreceptor outer segments. Finally, TUDCA treatments dramatically suppressed signs of apoptosis in both models. CONCLUSIONS: Systemic injection of TUDCA, a primary constituent of bear bile, profoundly suppressed apoptosis and preserved function and morphology of photoreceptor cells in two disparate mouse models of retinal degeneration. It may be that bear bile has endured so long in Asian pharmacopeias due to efficacy resulting from this anti-apoptotic and neuroprotective activity of TUDCA. These results also indicate that a systematic, clinical assessment of TUDCA may be warranted.

Sterility testing of antimicrobial-containing injectable solutions prepared in the pharmacy.

The need for sterility testing of antimicrobial-containing injectable solutions is discussed and specific testing methods are described. Despite their antimicrobial activity, antimicrobial-containing injectable drug products are not necessarily self-sterilizing and can become contaminated. In addition to practicing aseptic technique, pharmacists must perform end-product sterility testing on intravenous solutions to ensure their sterility. The United States Pharmacopeia provides guidelines for the performance and validation of two sterility test methods: membrane filtration and direct transfer to culture media. Membrane filtration is the method of choice for sterility testing of many antimicrobial-containing injectable solutions. After the test article is filtered, the membrane is rinsed with sterile fluid to remove residual antimicrobial agent, cut into two portions, and immersed in two types of culture medium. Visible turbidity of a sample within the appropriate incubation period indicates the presence of a contaminating microorganism. Closed filtration systems minimize false-positive results. In the direct transfer method, samples of the test article are directly inoculated into vessels of culture media, and antimicrobial activity is eliminated by dilution or by deactivation with chemical or enzymatic agents. Sterility testing as well as aseptic technique is needed to ensure the sterility of antimicrobial-containing injectable solutions.

Polyvinyl chloride matrix membrane electrodes for manual and flow injection analysis of chloroquine in pharmaceutical preparations.

Two types of polyvinyl chloride (PVC) matrix membrane electrodes responsive to the antimalarial drug chloroquine have been constructed, electrochemically evaluated, compared and used in pharmaceutical analysis. Type 1 is the classic PVC model with chloroquine-tetraphenylborate (TPB) sensor; Type 2 is a coated silver disk without internal filling solution. Both electrode types exhibited rapid linear potentiometric response to the logarithmic concentration of diprotonated chloroquine cation in the 10(-1) - 10(-6)M range with calibration slopes 28-30 mV/concentration decade over the pH range 1.8-6.2. These electrodes were sensitive enough to permit determination of chloroquine phosphate at concentrations as low as 5 microgram/mL with good accuracy and precision. Determination of chloroquine in various pharmaceutical preparations using direct potentiometry and potentiometric titration with NaTPB gave an average recovery of 98.8% of the nominal values (SD 0.5%). The Type 2 electrode was also assessed in a flow-through sandwich cell for flow injection analysis. Results were compared with data obtained by the U.S. Pharmacopeia method.

[The light sensitivity of corticosteroids in crystalline form. Photochemical studies. 59. (1)].

The light sensitivity of corticosteroids in the solid state have been investigated and the photodecomposition rates of the compounds have been estimated by HPLC. After 48 h UV-irradiation desoxycortone acetate, hydrocortisone, hydrocortisone acetate, prednisolone, methylprednisolone, dexamethasone, betamethasone and triamcinolone acetonide decompose between 20 and 50%. Cortisone acetate, prednisone and fluocinolone acetonide show photostability in contrast to requirements of the Pharmacopeia for light protections.

[Pharmacokinetics and pharmacodynamics of local anesthetics].

In clinical practice the efficacy of local anaesthesia is judged by the time of onset, duration and the quality of sensory and motor blockade. Apart from the physicochemical properties of local anaesthetics, a sufficient blockade depends on the volume and the concentration which are applied. Generally increasing the dose leads to a better quality of blockade as well as a higher risk of toxicity. This dilemma is evident in procedures, where, due to continuous or repetitive application within one day, the applied total dosage exceeds the recommended maximum limit. The values regarding maximum doses published in the German Pharmacopeia ("Rote Liste") can be defined as being more or less the product of the volume of distribution and the toxic concentration in the plasma. According to that formula the maintainance doses for continuous techniques in regional anaesthesia are derived from the product of the elimination half-life and the toxic plasma concentration. The values for toxic plasma concentrations are difficult to define since only the free protein-unbound fraction of a local anaesthetic is responsible for undesired side-effects. This fraction can be influenced by acidosis, body temperature and shortage of specific binding protein. Some well documented case reports show that another major cause of acute toxicity is nearly always due to inadvertent intravascular injections. This event can occur nearly unnoticed and leads to life-threatening complications even with lower doses than the recommended maximum doses. Only the application of high concentrations into well vascularised regions is followed by a similarly quick development of high plasma levels. Typical kinetics of local anaesthetics are presented for various methods of regional anaesthesia informing the anaesthetist on corresponding plasma concentrations if the recommended maximum doses are exceeded and thus he gets useful information for his daily work.

Comparative bioavailability of two tablet formulations of diclofenac sodium in normal subjects.

A comparative bioavailability study and in vitro characterization were conducted on two commercial products of diclofenac sodium (Voltaren "A" and Inflaban "B") in the form of enteric-coated tablets (25 and 50 mg). The two products were found similar in weight variation and content uniformity and both met the British Pharmacopeia requirements of disintegration for enteric-coated tablets. The dissolution in vitro revealed that product B was characterized by a faster rate compared to product A. The bioavailability of single doses, 2 x 50 and 2 x 25 mg, of each product was carried out respectively, on 14 and 6 normal male volunteers, according to a randomized complete block design. Blood samples were obtained over a 12-hour interval and serum concentrations of the drug were determined using an HPLC assay. The two products were found bioequivalent as assessed by AUC. However, the tmax value was found to be significantly smaller for product B compared to product A indicating that product B is apparently absorbed at a faster rate. Further, product B was characterized by higher Cmax values, although these were not statistically different from the corresponding values of product A. These findings are consistent with the in vitro dissolution pattern of the two products, and the differences in the rate of absorption may have therapeutic implications.

[Pharmacological action of various processed Mylabris phalerata Pallas].

We have compared the acute toxicity, sub-acute toxicity and in vitro and in vivo anti-tumor action of the rude drug, stir-fried with rice, and alkali-processed drugs of Mylabris phalerata before and after removing the head, feet and wings. The results indicate that the toxicity of the processed drugs is lower than that of the rude ones; the toxicity becomes higher without head, feet, and wings; and the alkali-processing method is better than stir-frying with rice recorded in the pharmacopeia.

[Use of a polarographic method for determining trichothecin].

The polarographic behaviour of trichothecin was studied. It was shown that the antibiotic could be detected in solutions at concentrations of 7.10(-7) moles with the help of the polarographic method. Conditions for the polarographic determination of trichothecin in fermentation broth were developed. The error was not more than 3 per cent. The reliability of the results was shown by statistical treatment of data performed in accordance with the requirement of the USSR State Pharmacopeia, X ed., prescribing that the precision of the assay is such that the fiducial limits at p = 95 per cent deviate from the average value by not more than 5 per cent. Comparison of the results of trichothecin determination in the fermentation broth with the polarographic and biological methods showed no significant difference. Therefore, the polarographic method may be recommended for trichothecin determination in the fermentation broth.

[Health information of intercontinental Spanish travelers].

To investigate the level of health information and degree of protection of Spanish travelers to tropical and temperate zones, 910 individuals were interviewed in the Prat del Llobregat airport before their departure. Very low levels of protection were detected, regarding both compulsory and optional vaccines as well as antimalarial protection. It was found out that the information sources that had been consulted were not the optimal ones and that they had delivered remarkably inaccurate advice. Only 44% had received some type of vaccination. Important geographic errors were recorded regarding vaccination against yellow fever. Few travelers received optional vaccines, although these provide protection against highly prevalent diseases in tropical areas. Errors in antimalarial chemoprophylaxis were related both to drug dosage and their adequacy to the corresponding areas. Travelers prematurely interrupted prophylaxis in a progressive fashion depending on the number of elapsed weeks. Only 33% of the 39% who started prophylaxis before leaving completed it correctly. Travelers from health care professions had a higher rate of early abandonment of antimalarial prophylaxis and a worse immune protection against yellow fever, with a 5% signification level (p less than 0.05) when compared with the rest of travelers. Suggestions to improve this situations are made, such as an adequate education of health professionals and the general population, the reshaping of official vaccination centers and the introduction of adequate antimalarial drugs in the Spanish pharmacopeia.

[The problems of the estimation of bacterial contamination in viral drinking water vaccines for poultry].

The definition of saprophytic and non pathogenic germs according to the DAB 9 (German Pharmacopeia) will be explained as follows. Germs should be seen as saprophytic and non pathogenic when they are either classified as a pathogen by biochemical methods or when the safety test in chickens shows no adverse local or clinical reactions. The reason for the different test results in testing the species and the amount of contaminating germs at the laboratories of vaccine producers and state laboratories are explained by the unequal dissemination of germs in each batch and by the discontinuity in cooling the vaccine samples.

[The solubility of drugs in molten suppository bases].

Solubility of 18 drugs used in rectal therapy in molten suppository base, (Massa suppositoriorum 15, Pharmacopeia of the G.D.R. Rosupol U, 37 degrees C) is determined. Separation of drug-saturated base from drug cristals is carried out in an air-heated centrifuge followed by a partition step between petroleum ether and an aqueous medium and photometrical determination of drug concentration. The procedure leads to results with relative standard deviation of 2% (medium value of 18 drugs). The investigation gave solubility values in the range between 0.002% (theobromine) and 26% (lidocaine), whereas benzoate, phenobarbital sodium and procaine hydrochloride are completely insoluble. Sodium salicylate has an unexpected high solubility (0.48%). The solubility is temperature depended and thus the solubility enthalpy can be determined. Relations between structure and solubility are discussed for pyrazolin-5-ones and purines. A correlation between solubilities in the base and in aqueous buffer (phosphate, pH 7.4) does not exist in the series studied.
