Nursing interventions for persons receiving immunosuppressive therapies for demyelinating pathology.

The client receiving immunosuppressive therapy for demyelinating disorders faces many challenges. The nurse should be involved in the constant assessment of the client for potential complications of the therapy as well as changes in the underlying disease process. Many of the nursing interventions focus on comfort and symptom management as well as providing information for the client and family to use in understanding the therapy and minimizing the effects of immunosuppression. Involving the family in client management will only strengthen the overall plan and outcomes for client care.

[Therapeutic effect of intravaginal application of estriol tablets in the treatment of pathological cervical factor as a cause of sterility].

81 women were included in a prospective randomised 12-year personal study. The "pathological cervical factor" was confirmed by periovulatory daily examination of the quality of the cervical mucus, using a new modified scheme of cervical score by examining the oestrogenic activity in the vaginal smear and results of sperm-penetration tests after Sims- Huhner , partly also by periovulatory determination of the concentration of luteinising hormone in the serum. The presence of ovulatory cycles could also be determined by regular registration of the cycle length, measurement of the basal body temperature and partly by premenstrual endometrium biopsies; these determinations were of course effected during cycles without therapy. All 81 cases with abnormal preovulatory production of cervical mucus were treated in a total of 234 cycles by intravaginal application of oestriol tablets 1 to maximal 3 mg daily for 1 or more subsequent cycles. On the basis of these criteria, the quality of cervical mucus and postcoital tests improved with a satisfying intravaginal resorption of the oestriol tablets; no side effects were seen. After this therapy, very good results were obtained with intravaginally administered oestriol tablets in 59.3% of the patients with a pregnancy rate of 27.2%; 24.7% of the cases only showed a moderate improvement of the mucus; 16.0% did not respond to oestriol treatment and, therefore, had to be considered as therapeutic failures. The present study again confirms the results of other investigators on the local effectiveness of intravaginal application of oestriol tablets.(ABSTRACT TRUNCATED AT 250 WORDS)

Prevalence and predictors of pathological gambling: results from the St. Louis personality, health and lifestyle (SLPHL) study.

OBJECTIVES: We report the prevalence of and risk and protective factors for DSM-IV sub-threshold gambling (1-4 criteria) and pathological gambling disorder (PGD; 5-10 criteria) in a non-clinical household sample of St. Louis area gamblers. METHODS: Of the 7689 individuals contacted via Random Digit Dialing, 3292 were screened eligible. Of these, 1142 from households in 6 contiguous regions in Missouri and Illinois consented to participate and were mailed a St. Louis Area Personality, Health, and Lifestyle (SLPHL) Survey. RESULTS: Post-stratification weighted data (n=913) indicate lifetime prevalence rates of 12.4% sub-threshold and 2.5% PGD (conditional prevalence=21.5% and 4.3% respectively). Risk and protective factors for gambling severity varied in the sample. CONCLUSIONS: Targeted prevention messages are warranted specifically for gamblers of varying risk for PGD.

[Psychopathology-like states after craniocerebral trauma].

The presence of stable personal disharmony in the form of a reduced organic mental syndrome served as a criterion for diagnosis of personality changes after traumatic head injuries. The patients were divided into a group with prevalence of personality changes (81 patients) and a group with predominance of pronounced organic mental syndrome (141 patients). Personality disorders prevailed in the patients who had got mild and moderate traumas at the age of 13-25 years, with less number of additional pathogenic factors of exogenic organic spectrum in anamnesis. 3 variants of the course of personality disorders were recognized: with a tendency toward reduction of emotional and drive pathology; with a tendency to intensification of intellectual and mnestic disorders and transformation into large-scale organic mental syndrome; with a tendency toward an increase of pathological traits of personality in conditions of growing influence of the environmental and psychogenic factors.

Liver histology in a 'normal' population--examinations of 503 consecutive fatal traffic casualties.

The liver histology of 503 consecutive victims of fatal (within 24 hours) traffic accidents submitted to medico-legal autopsy are used as a standard of reference. In 370 persons (74%) no pathological changes in the liver biopsies were observed. Fatty liver was found in 120 persons (24%), non-specific portal inflammation in 7 persons, alcoholic hepatitis in 6, and portal fibrosis in 5. No cases of cirrhosis, chronic aggressive hepatitis, changes compatible with chronic persistent hepatitis, viral hepatitis, or other internationally accepted morphological diagnoses were found. A significant positive correlation between the frequency of steatosis and age groups was demonstrable. Fatty liver was found in 1% of persons below 20 years, in 18% between 20--40 years, and in 39% of persons more than 60 years in this normal material. The persons with fatty liver had a higher body weight, but the overweight was not correlated to age. It is concluded that fatty infiltration in the liver is a normal observation in aged persons.

[Correlations of the Personal Sphere Model and Rorschach Projection tests. (A factorial examination of the validity of the Personal Sphere Model using the Rorschach Test)].

The most important results of the factor analysis of 11 variables of the Personal Sphere Model and 14 variables of the Rorschach-Test have shown in both tests common radicals appearing in the evaluation as well of the structure of personality as of the behaviour of man. Both tests emphasize the possibility of objectivating pathological traits of personality: the difference consisting in the fact, that this traits are coming out more distinctly in the Rorschach-Test, while in the Personal Sphere Model the psychodynamic factors of the ability of building object relationships of overcoming seperations are appearing more clearly. The most relevant conformity of both tests is the possibility of distinctly projecting the defense mechanism of aggression.

[Blood methanol as a marker of alcoholism. A diagnostic component within the scope of expert assessment of driving competence in alcoholic intoxication].

A blood methanol test which was done in addition to routine diagnosis of the blood methanol level of persons with alcohol driving delicts in the past who were alcoholized during the medical-psychological investigation (MPU) produced differently significant cues for (pre) alcoholism in nearly all persons. As a result 15 out of 40 persons reached a blood methanol level of 10 mg% and more (24 more persons a level between 1.5 and 9.9 mg%). This is a clear indicator of pathological alcohol drinking. Persons with a breath alcohol concentration from 0.2 to 0.99/1000 showed an average blood ethanol concentration of less than 10 mg%, but above 1.5 mg%, where as those persons with an alcohol level above this limit had a blood methanol concentration significantly above 10 mg%. The arithmetical mean and the standard deviation were significantly elevated compared to the range of reference, an effect that didn't show as clearly with the median of the Gamma-glutamyltransferase (GGT). The mean corpuscular volume of erythrocytes (MCV) reacted less sensitively. The correlation between the five parametres (blood ethanol at the last alcohol driving delict, breath alcohol concentration, GGT, MCV, blood methanol) were all significantly positive with one exception: the correlation between the breath alcohol and the blood methanol level. Altogether the results support the hypotheses that the protective assertions of the subjects concerned are hard to believe, even those of persons with a lower blood alcohol level. An additional test of the blood methanol level should be done, however, with persons reaching an alcoholization between 0.2 and 1.0/1000 during the MPU.(ABSTRACT TRUNCATED AT 250 WORDS)

[Various personality traits of patients with psoriatic arthropathy].

It is well known that patients affected by rheumatic diseases may present specific pathological trends in personality structure, as has been extensively reported in literature. Our study was aimed at investigating several aspects of the personality traits of 20 patients with psoriatic arthropathy, compared with a group of 20 patients with rheumatoid arthritis. All patients were evaluated with appropriate rating scales assigned in auto and hetero-administration. The study results points to a personality trait disturbance in psoriatic arthritis patients, which can be clearly differentiated from the anxious habitus and/or reactive-depressive state observed in patients with rheumatoid arthritis.

Education modifies the relation of AD pathology to level of cognitive function in older persons.

OBJECTIVE: To test the hypothesis that years of formal education modifies the relation of AD pathology to level of cognitive function. METHODS: A total of 130 older Catholic clergy participating in the Religious Orders Study underwent annual cognitive function testing and brain autopsy at the time of death. Individual cognitive function tests were z-scored and averaged to yield a global measure of cognitive function and summary measures of five different cognitive abilities. Neuritic and diffuse plaques and neurofibrillary tangles were counted in separate 1 mm(2) areas of maximal density. Counts were converted to standard scores by dividing by their SD, and combined to yield a global AD pathology score and summary scores of each postmortem index. Linear regression was used to examine the relation of education and AD pathology scores to level of cognitive function proximate to death, controlling for age and sex. Subsequent analyses tested the interaction between education and each AD pathology score to determine whether education modified the relation of AD pathology to level of cognitive function. Additional analyses examined these associations on five specific cognitive abilities. RESULTS: Both years of formal education (regression coefficient = 0.073, p = 0.0001) and the global AD pathology score (regression coefficient = -0.689, p < 0.0001) were related to level of cognitive function. When an interaction term between education and AD pathology was added to the model, the association between a unit of AD pathology and level of cognitive function was 0.088 (p = 0.0078) standard unit less for each year of education than the level predicted from the model without the interaction term. Whereas neuritic plaques, diffuse plaques, and neurofibrillary tangles were all strongly related to cognitive function, education only modified the relation of neuritic plaques (p = 0.002) and diffuse plaques (p = 0.03) to cognition, but not neurofibrillary tangles. In analyses examining five different cognitive abilities, the interaction between education and the neuritic plaque score was strongest for perceptual speed and weakest for episodic memory. CONCLUSIONS: These data provide strong evidence that the relation between senile plaques and level of cognitive function differs by years of formal education.

Transference regression and psychoanalytic technique with infantile personalities.

This paper describes a particular form of 'silent' regression during the psychoanalytic treatment of infantile personalities (a type of character pathology related to the hysterical personality). This regression is characterized by the development of rapid interchange in the roles enacted in the transference and projected on to the analyst, an apparent 'disconnexion' of the transference material from that dominant when the patient is in non-regressed states, and the intensification of the analyst's counter-transference reactions. The diagnosis and management of these regressive transferences requires a particular interpretive style and strategy outlined in this paper and illustrated with two case vignettes.

[Cranial fractures during growth. An analysis of the authors' personal cases and therapeutic management of such childhood pathology].

Growing skull fractures are a rare complication of head injury with a dome fracture. The cases described in literature highlight a higher frequency of this pathology in children under than over three years of age. The authors describe 4 personal cases, underlying the clinical, diagnostic and therapeutic aspects. The aetiological hypothesis of a dural tear and its anatomic-pathological evolution are investigated. The surgical technique of dural and bony repair is also discussed. The authors conclude that this complication must be recognized early in order to obtain very good results after surgical therapy.

[New developments in mass roentgen studies].

Since 1984 in the GDR the persons older than 40 years are summoned to participate in the mass X-ray examinations in a two-year sequence. The risk groups of tuberculosis are called up at shorter intervals, this also concerns younger people. To this belong patients with silicosis, so-called "healthy carriers of lesions", persons with contacts to patients suffering from tuberculosis, diabetics and the staff of tuberculosis hospitals and pathological institutes. Furthermore, persons working in national education, kindergartens and cattle-breeding are particularly controlled. Patients with chronic diseases of the respiratory tract need our increased attention.

[Critical views on narcissism theories].

After having given an overview about the different definitions of the Self, I have defined it as the seat of narcissism which furnishes the instances of Ego, Id and Super-Ego as well as to the body this narcissistic investment or information which gives the individual the feeling that the different instances and his body are belonging to his own and giving him throughout his life a feeling of continuity. Otto F. Kernberg describes, contrary to Heinz Kohut, the primarily pathological nature of narcissism of individuals having an Ego prone to fragmentation and very rigid defense mechanisms, e.g. splitting, projective identification, blind rage, hostility etc. The pathological narcissism results out of an early pathological object-relationship and of an Ego prone to fragmentation with a consecutive disturbance of the development of the Self. From these borderline personalities with their pathology of narcissism I differentiate the narcissistic neuroses--called by Kohut narcissistic personality disorders or behavior disorders--in individuals with a strong Ego, which are due to a lack--or more rarely to a surplus--of warmth-, stimulation- and cognition-experiences in early childhood. They have a tendency of undergoing more than usual a fusion with a Self-object, mirror transferences and developing a grandiose Self in their fantasy as compensations or defense mechanisms. The schizoid personalities are intermediate forms, which show a rigid defense, however do not have like the borderline-personalities the tendency to occasional break-throughs of the primary process. Whereas in the treatment of narcissistically disturbed borderline conditions early interpretations of the rigid archaic defense mechanisms and furthering reality testing are necessary, in narcissistic neuroses primarily the building up of a consistent Self and then a working through of the named narcissistic compensations and defense mechanisms are necessary.

[Interrelations between personality characteristics and the autonomic pattern in mild strokes developed in various forms of arterial hypertension].

The data on the characterological personality traits and the vegetative pattern in persons with cerebrovascular pathology (minor brain stroke-MBS) associated with different forms of arterial hypertension (AH) (stable, malignant, labile) make it possible to delineate two psychovegetative personality types. The first one is the hysterical one with an initial sympathetic tone and excess vegetative supply, the second one is the ++anxio-hypochondriac or depressive (more rarely) with an initial parasympathetic or combined vegetative tone with an insufficient level of vegetative supply, and vegetative reactivity. The labile (mild, episodic) forms of AH aggravated by cerebrovascular pathology (MBS) are likely to be the constituent part of the psychovegetative syndrome (the first psychovegetative personality type). The stable forms of AH (including malignant ones) aggravated by episodes of vascular dyscirculatory disorders (MBS) are similar to the second psychovegetative personality type as regards their characterological and vegetative traits. The data obtained can be used not only in subsequent studies of the etiopathogenesis of MBS associated with different forms of AH but also in terms of practical and preventive aspects.
