[The dynamics of the functional indices of the cardiovascular system in patients with a history of progressing stenocardia].

The objective of this study was an assessment of time-related course of functional indices for the cardiovascular system in patients with prior advancing angina pectoris (AAP). A close inverse relation was found out between the time-related course of threshold load and work done in load testing and changes in the number of episodes of silent myocardial ischemia, overall duration of myocardial infarction during the 24-hour period, and depth of ischemic depression of ST segment in ECG. Higher/lower tolerance to physical loads in those patients in the post-AAP period is largely related to changes in indices for intracardiac hemodynamics, such as ejection fraction and left ventricular volume and degree of signs of ventricular electrical instability.

American Heart Association-Bugher Foundation Centers for Molecular Biology in the Cardiovascular System.

BACKGROUND: The American Heart Association (AHA) and the Henrietta B. and Frederick H. Bugher Foundation in 1985 entered into a partnership to establish a group of Centers for Molecular Biology in the Cardiovascular System. The goal was to recruit and train young scientists with medical training to apply molecular and cellular biology knowledge and techniques to cardiovascular problems. METHODS AND RESULTS: Six Centers have been awarded (three in 1986 and three in 1991), and a total of 110 trainees have been involved as of June 30, 1994. Of these trainees, 77 were recruited and trained by the 1986 Centers. As of June 1994, 88% of these trainees remained in academic medicine and 54% progressed to higher academic ranks; 79% published papers in science and 66% in molecular biology; and 36% obtained extramural funding for their work. On this basis, the 1986 trainees appear to be well on their way to becoming successful academic cardiologists. CONCLUSIONS: The AHA-Bugher Foundation Center program has produced a cadre of cardiovascular scientists who are applying molecular biology knowledge to both basic and clinical problems.

[Functional state of the cardiovascular system in patients with chronic coronary insufficiency 12 months after direct myocardial revascularization depending on the bypass patency].

A comparative study of the functional state of the cardiovascular system (CVS) in the presence of chronic heart failure was performed 12 months following direct myocardial revascularization. It involved 49 patients with open aortal coronary bypasses and 20 patients with bypass occlusion. The CVS functional state was assessed by both invasive and noninvasive methods of investigation. The bypass patency was associated with an improvement in the patients' subjective conditions, an increased exercise tolerance and better indices of the left ventricle function. It is inferred that successful revascularization of the myocardium by an aortal coronary bypass produces a considerable therapeutic effect on the CVS functional state in patients with chronic heart failure.

Molecular cloning of human prostacyclin receptor cDNA and its gene expression in the cardiovascular system.

BACKGROUND: Prostacyclin elicits a potent vasodilation and inhibition of platelet aggregation through binding to its membrane receptor. The impairment of prostacyclin receptor activity is implicated in various human cardiovascular diseases. In the present study, we succeeded in the isolation and characterization of human prostacyclin receptor cDNA and elucidated its gene expression in human tissues. METHODS AND RESULTS: We isolated a cDNA clone encoding the human prostacyclin receptor from a human lung cDNA library. The isolated cDNA clone encodes a 386-amino acid protein with seven putative transmembrane domains, which belongs to the G protein-coupled receptor superfamily. [3H]iloprost, a prostacyclin receptor agonist, specifically bound to the receptor transiently expressed in COS-7 cells. The binding was inhibited in the rank order of iloprost = cicaprost, another prostacyclin receptor agonist, > prostaglandin E1 (PGE1) >> PGE2, PGF2 alpha, PGD2, STA2. In addition, iloprost dose-dependently stimulated cAMP generation in these COS-7 cells. These results are consistent with the characteristics of the human prostacyclin receptor. Northern blotting analysis on human tissues revealed that prostacyclin receptor mRNA is abundantly expressed in the aorta, lung, atrium, ventricle, and kidney. CONCLUSIONS: We cloned human prostacyclin receptor cDNA and elucidated its abundant gene expression in the human cardiovascular system. The present study will lead to better understanding of the significance of prostacyclin in humans and further facilitate the clinical application of prostacyclin.

Adrenomedullin: its role in the cardiovascular system.

Adrenomedullin is a 52-amino acid peptide that was first isolated from human pheochromocytoma. Subsequently, it was found to be distributed widely in the body, including throughout the cardiovascular system. It belongs to a family of peptides that include calcitonin gene-related peptide and amylin. Adrenomedullin causes vasorelaxation and influences vascular proliferation and interacts closely with nitric oxide, and it may have a role in the pathophysiology of hypertension, ischemic heart disease, and cardiac and renal failure. Nonpeptide agonists or antagonists of adrenomedullin may have potential therapeutic application. The role of adrenomedullin in septicemic shock also merits further investigation.

Quantitative coronary angiographic and intravascular ultrasound assessment of a new nonarticulated stent: report from the Advanced Cardiovascular Systems MultiLink stent pilot study.

OBJECTIVES: The purpose of this study was to evaluate the safety, feasibility, optimal deployment technique and 1-year clinical outcome for the Advanced Cardiovascular Systems (ACS) MultiLink stent. BACKGROUND: Optimal stent deployment assessed by quantitative coronary angiography and intravascular ultrasound (IVUS) is associated with improved clinical outcome. METHODS: Forty-nine consecutive patients with a discrete stenosis in a native coronary artery 3 to 4 mm in diameter were treated with the new, balloon-expandable ACS MultiLink stent. Stent expansion was assessed in all patients using quantitative coronary angiography and serial IVUS imaging after 8-, 12- and 16-atm inflations. Clinical follow-up was obtained at 30 days and 1 year. RESULTS: All 49 patients had successful placement of a MultiLink stent without death, emergency coronary artery bypass graft surgery or Q wave myocardial infarction. After placement of the MultiLink stent, the minimal lumen diameter increased from 1.24 to 2.98 mm (p < 0.001), and diameter stenosis decreased from 61% to 7% (p = 0.001). Minimal lumen cross-sectional area by IVUS increased progressively after 8, 12 and 16 atm (5.6 to 6.8 to 7.4 mm2, respectively, p < 0.001). However, only 64% of stents achieved a lumen/reference area ratio > or = 70%. No adverse clinical events occurred by 30 days, and by 1 year only one patient (2.0%) required revascularization of the stented artery. CONCLUSIONS: Treatment of stenoses in native coronary arteries with the MultiLink stent is associated with a high success rate and a low incidence of adverse events by 1 year, despite the fact that the majority of stents did not meet IVUS-defined criteria for "optimal stenting" derived from first-generation devices.

Evaluation of the alpha-1 and alpha-2 adrenoceptor-mediated effects of a series of dimethoxy-substituted tolazoline derivatives in the cardiovascular system of the pithed rat.

The alpha-1 and alpha-2 adrenoceptor-mediated effects of a series of dimethoxy-substituted tolazoline derivatives were investigated in the cardiovascular system of the pithed rat. The 2,5- and 3,5-dimethoxy-substituted tolazoline derivatives produced vasopressor responses that were inhibited by the alpha-1 adrenoceptor antagonist, prazosin (0.1 mg/kg i.v.), and were not affected by the alpha-2 adrenoceptor antagonist, yohimbine (1 mg/kg i.v.), suggesting that these derivatives selectively activate postsynaptic vascular alpha-1 adrenoceptors. The 2,5- and 3,5-dimethoxy-substituted derivatives of tolazoline did not produce an alpha-2 adrenoceptor-mediated inhibition of neurogenic tachycardia in cord-stimulated pithed rats and were therefore presumed to be devoid of alpha-2 adrenoceptor agonist activity. In contrast, 2,3-dimethoxytolazoline produced a vasopressor effect that was inhibited by yohimbine but not by prazosin, suggesting selective activation of postsynaptic vascular alpha-2 adrenoceptors. Consistent with this observation is the fact that 2,3-dimethoxytolazoline elicited a dose-dependent, alpha-2 adrenoceptor-mediated inhibition of neurogenic tachycardia in cord-stimulated pithed rat. 3,4-Dimethoxytolazoline was a weak alpha-1 adrenoceptor agonist in the vasculature of the pithed rat and was devoid of agonist activity at alpha-2 adrenoceptors. However, 3,4-dimethoxytolazoline was found to be an alpha-2 adrenoceptor antagonist of similar potency as yohimbine. The results of the present study indicate that dimethoxy-substituted derivatives of tolazoline possess different activities and selectivities at alpha-1 and alpha-2 adrenoceptors depending upon the positions of substitution.(ABSTRACT TRUNCATED AT 250 WORDS)

Clinical chronopharmacology of the cardiovascular system: hypertension and coronary heart disease.

There is sound evidence that the cardiovascular and the renal systems are well organized in time. Mechanisms of regulation and pathophysiological events are not evenly distributed over the 24-h scale. Moreover, certain diseases may even alter the physiological circadian pattern both in the cardiovascular system and in the kidneys. This observation bares implications for drug treatment, e.g., regarding drug formulations and dosing time intervals. Pitfalls may arise from neglecting circadian phase-dependency in pharmacokinetics and in the concentration-effect relationship. Moreover, different types of drugs may be superior to others when circadian time-related symptoms are concerned. No doubt, "time-of-day" has to be included in our diagnostic and therapeutic strategies.

Evaluation of the cardiovascular system in children with simple obesity. Part I. Clinical, electrocardiographic and functional studies.

In 45 children with simple obesity (overweight 70 +/- 16%) clinical, electrocardiographic and functional assessment of the cardiovascular system was carried out. The results were compared with those in a control group of 20 healthy children without overweight. Raised arterial blood pressure was found in 14 children (31%) in the studied group. In the control group the blood pressure was normal. Resting electrocardiogram was abnormal in 5 obese (11%) and 2 thin (10%) children. Exercise tolerance was similar in both groups, while the values of systolic pressure and double product were higher in obese children. The value of the tolerated exercise was not correlated with the degree of overweight.

A major involvement of the cardiovascular system in patients affected by Marfan syndrome: novel mutations in fibrillin 1 gene.

The aim of our study was to characterize the molecular defect in Italian Marfan patients, thus contributing to the effort of correlating the genotype with the phenotype. In particular, our ultimate goal was to identify the region(s) of the fibrillin 1 (FBN1) gene mainly involved in the health of the heart and of the aorta in terms of the cardiovascular system. We searched for a molecular defect in three patients with classic Marfan syndrome (MFS). The mutations were detected applying heteroduplex analysis to each of the 65 exons of the FBN1 gene amplified by polymerase chain reaction (PCR). Exons containing heteroduplex bands were sequenced directly from PCR products. This study reports the detection of three unique missense mutations in the FBN1 gene in three Italian patients: a 44-year-old adult male and 36-year-old female affected by classic MFS (with all the cardinal manifestations in the cardiovascular, ocular and skeletal systems), and an 11-year-old male affected by infantile (earlier onset) classic MFS. The first two are sporadic cases and present a Cys-->Arg amino acid substitution (T-->C substitution at nucleotide 7729) in exon 62 and a Cys-->Tyr amino acid substitution (G-->A substitution at nucleotide 6695) in exon 54. The third is a familial case which presents a Cys-->Trp aminoacidic substitution (C-->G substitution at nucleotide 3546) in exon 28. Our data confirm that cysteine substitutions in calcium binding epidermal growth factor (cbEGF)-like domains cause severe Marfan phenotype. Exon 24-32 cluster seems to produce an even more severe phenotype. The early characterization may be of clinical relevance for prevention and early surgical treatment of aortic aneurysm or dissection.

Limitations of the OPCS congenital malformation notification system illustrated by examination of congenital malformations of the cardiovascular system in districts within the Trent Region.

Data are presented from OPCS notification statistics and other sources on rates of congenital cardiovascular malformations in the Trent Health Region and in its constituent Districts. While Trent in general, and some of its districts in particular, show high notification rates, other data sources suggest that the underlying malformation rate may not be raised compared with that seen nationally. Reasons for this discrepancy are discussed and the OPCS notification system is compared with that in Scotland.

Interaction of the enantiomers of 3-0-methyldobutamine, a metabolite of dobutamine, with alpha- and beta-adrenoreceptors in the cardiovascular system of the pithed rat.

The pharmacological activities of the enantiomers of 3-0-methyldobutamine, a metabolite of dobutamine, were investigated at alpha- and beta-adrenoreceptors in the cardiovascular system of pithed rats. Neither enantiomer of 3-0-methyldobutamine produced vasoconstriction in pithed rats, indicating a lack of alpha 1- or alpha 2-adrenoreceptor agonist activity. However, both enantiomers of 3-0-methyldobutamine were found to be alpha 1-adrenoreceptor antagonists. The alpha 1-adrenoreceptor antagonist activity of the enantiomers of 3-0-methyldobutamine was selective, since neither enantiomer antagonized alpha 2-adrenoreceptors at doses up to 10 mg/kg, i.v. The alpha 1-adrenoreceptor antagonist activity of (+)- and (-)-3-0-methyldobutamine was studied in detail in pithed rats. Based on a Schild analysis of the antagonism of the alpha 1-adrenoreceptor mediated pressor effects of cirazoline, (+)-3-0-methyldobutamine had a DR2 value (i.e., dose required to produce a two-fold rightward shift in the dose-response curve of cirazoline) of 0.16 mg/kg, i.v., whereas the DR2 value for (-)-3-0-methyldobutamine was 10.3 mg/kg, i.v. Thus, the potent alpha 1-adrenoreceptor antagonist activity of 3-0-methyldobutamine resides predominantly in the (+)-enantiomer, the latter being approximately 64-fold more potent than the (-)-enantiomer. Both enantiomers of 3-0-methyldobutamine were weak beta 1-adrenoreceptor antagonists that, at high doses, inhibited the chronotropic effect of isoprenaline. In addition, both enantiomers were weak beta 2-adrenoreceptor agonists that produced modest decreases in diastolic blood pressure at high doses. The beta 1-adrenoreceptor antagonist and the beta 2-adrenoreceptor agonist effects of the enantiomers of 3-0-methyldobutamine occurred at doses in excess of 3 mg/kg, i.v. attesting to their weak activity, and neither effect was stereoselective. It is hypothesized that of the potent alpha 1-adrenoreceptor blocking activity of 3-0-methyldobutamine, which resides predominantly in the (+)-enantiomer, may contribute, in part, to the reduction in total peripheral vascular resistance observed following infusion of dobutamine.
