Automated prozone effect detection in ferritin homogeneous immunoassays using neural network classifiers.

The application of turbidimetric homogeneous immunoassays made the determination of several plasma components widely available. The sensitivity and accuracy of these assays are appropriate enough for routine laboratory use; however, in the case of many pathologically high concentration samples, prozone effect (high dose hook effect) can be observed, that leads to false-negative determination. Up to the present there are no cost-effective algorithms available for the safe detection of the prozone effect. Pathological serum ferritin values can be elevated up to 5000 ng/ml, while the measuring range covers only the 0-440 ng/ml range by a commercial assay. The determination of samples with ferritin concentration higher than 1500 ng/ml results in false-negative values because of the overlapping measuring range and prozone effect range. The prozone effect can be recognised by analysis of reaction kinetics after measurement. We have developed a neural network classifier system to analyse reaction kinetics of the measurements and check the prozone effect. One thousand five hundred determinations and 77 patient samples were used for neural network training and test. Using the trained neural networks, false-negative results can be filtered immediately after the determination, without re-run; thus, the sensitivity of plasma ferritin determination may become reliable enough, even in the case of high concentration samples. Applying this new technology, false-negative serum ferritin determinations can be avoided, thus even a relatively high hook effect rate (5-12% in different patient groups) can be handled safely.

Multisensory activation of the intraparietal area when classifying grating orientation: a functional magnetic resonance imaging study.

Humans can judge grating orientation by touch. Previous studies indicate that the extrastriate cortex is involved in tactile orientation judgments, suggesting that this area is related to visual imagery. However, it has been unclear which neural mechanisms are crucial for the tactile processing of orientation, because visual imagery is not always required for tactile spatial tasks. We expect that such neural mechanisms involve multisensory areas, because our perception of space is highly integrated across modalities. The current study uses functional magnetic resonance imaging during the classification of grating orientations to evaluate the neural substrates responsible for the multisensory spatial processing of orientation. We hypothesized that a region within the intraparietal sulcus (IPS) would be engaged in orientation processing, regardless of the sensory modality. Sixteen human subjects classified the orientations of passively touched gratings and performed two control tasks with both the right and left hands. Tactile orientation classification activated regions around the right postcentral sulcus and IPS, regardless of the hand used, when contrasted with roughness classification of the same stimuli. Right-lateralized activation was confirmed in these regions by evaluating the hemispheric effects of tactile spatial processing with both hands. In contrast, visual orientation classification activated the left middle occipital gyrus when contrasted with color classification of the same stimuli. Furthermore, visual orientation classification activated a part of the right IPS that was also activated by the tactile orientation task. Thus, we suggest that a part of the right IPS is engaged in the multisensory spatial processing of grating orientation.

Classifying risky-drinking college students: another look at the two-week drinker-type categorization.

OBJECTIVE: The present study examined the effectiveness of the 2-week period currently used in the categorization of heavy episodic drinking among college students. Two-week drinker-type labels included the following: nonbinge drinker, binge drinker, and frequent binge drinker. METHOD: Three samples of college student drinkers (104 volunteers, 283 adjudicated students, and 238 freshmen male students) completed the 3-month Timeline Followback assessment of drinking. Drinking behavior during the last 2 weeks of the month before the study was compared with drinking behavior during the first 2 weeks of the same month to compare behavior and resulting labels during both 2- week periods. RESULTS: Inconsistencies existed in drinker-type labels during the first 2 weeks of the month and the last 2 weeks of the month for all three samples. Between 40% and 50% of participants in the three samples were classified as a different drinker type across the month. Nonbinge drinkers experienced a wide range of alcohol-related problems, and much variation existed among the frequent-binge-drinker label. CONCLUSIONS: The results suggest that the current definition needs to be modified to accurately identify risky-drinking college students. Expanding the assessment window past 2 weeks of behavior, as well as developing different classification schemes, might categorize risky drinkers more accurately.

Efficacy of chimeric molecules directed towards multiple somatostatin and dopamine receptors on inhibition of GH and prolactin secretion from GH-secreting pituitary adenomas classified as partially responsive to somatostatin analog therapy.

OBJECTIVE: This study compared the potency of a somatostatin receptor (sstr)2-sstr5 analog, BIM-23244, of an sstr2-dopamine D2 receptor (sstr2-DAD2) molecule, BIM-23A387 and of new somatostatin-dopamine chimeric molecules with differing, enhanced affinities for sstr2, sstr5 and DAD2, BIM-23A758, BIM-23A760 and BIM-23A761, to suppress GH and prolactin (PRL) from 18 human GH adenomas that are partially responsive to octreotide or lanreotide. MATERIALS AND METHODS: The sstr2, sstr5 and DAD2 mRNA levels were determined by RT-PCR. The effect of drugs was tested in cell cultures at various concentrations. RESULTS: In all tumors, the sstr2, sstr5 and DAD2 mRNA levels were coexpressed (mean levels+/-s.e.m. 0.4+/-0.1, 5.3+/-1.9 and 2.0+/-0.4 copy/copy beta-glucuronidase). In 13 tumors, the maximal suppression of GH secretion produced by BIM-23A387 (30+/-3%) and BIM-23244 (28+/-3%) was greater than that produced by octreotide (23+/-3%). In six out of 13 tumors, BIM-23A758, BIM-23A760 and BIM- 23A761 produced greater maximal suppression of GH secretion than octreotide (33+/-5, 38+/-2 and 41+/-2 vs 24+/-2%). Their EC(50) values were 10, 2 and 4 pmol/l. BIM-23A761 was more effective than BIM-23A387 in GH suppression (41+/-2 vs 32+/-4%). The new chimeric molecules produced maximal PRL suppression greater than octreotide (62+/-8 to 74+/-5 vs 46+/-11%). CONCLUSIONS: Novel dopamine-somatostatin chimeric molecules with differing, enhanced activity at sstr2, sstr5 and DAD2, consistently produced significatly greater suppression of GH and PRL than either octreotide or single-receptor-interacting ligands in tumors from patients classified as only partially responsive to octreotide therapy. The higher efficacy of the chimeric compounds was, at least partially, linked to their high affinity for sstr2 (IC50 1-10 pmol/l). The other mechanisms by which such molecules produce an enhanced inhibition of GH remain to be elucidated.

The influence of age on the number of non-atretic follicles classified based on follicular size in the rat ovary.

The age-related changes in the number of follicles classified according to size were investigated. The number of large follicles expected to ovulate spontaneously was not influenced by age, though the number of small follicles 250-549 microns in diameter, recovered by PMSG and hCG, was not constant. There was no relationship between the number of large follicles and small ones. This study suggests that the variation in ovum count after superovulation depending on age may be based on the variation in the number of small follicles.

[The risk of colorectal adenomatous polyp in relation to serum total cholesterol levels in Japanese men classified by age group].

In order to investigate the relationship between the risk of colorectal cancer and serum total cholesterol (TC) levels, we studied the relationship between the risk of colorectal adenomatous polyp and high TC levels in Japanese men aged from 40 to 59 at a manufacturing company. We first diagnosed 283 subjects as having adenomatous polyp (AD) or normal (C), identified by means of an immunological fecal occult blood detection test and total colonofiberscopic examination (TCF), and then selected 94 subjects after excluding the patients with diseases possibly influencing TC. We classified the subjects into two age groups (40-49 and 50-59) and conducted a statistical analysis of AD and C groups by means of a two tailed t-test. The mean values for TC (DTC), which had been obtained within 3 months before TCF, were significantly different (p < 0.001 95% CI 15.79-48.49) for those in their 40s, but were not different for those in their 50s. In order to check the stability of the data, we analyzed the mean values for DTC and BTC, which had been obtained within 15 months. The results showed the same trend for those in their 40s (p = 0.001 95% CI 10.76-40.87). The mean DTC and BTC values were again not significantly different for those in their 50s. We further divided the subjects into the lower (-181 mg/dl) and higher (209 mg/dl-) DTC values and conducted further statistical analysis with Fisher's exact test. The results were significant in the 40-49 age group (p = 0.004), in the odds ratio (13.75 95% CI 2.32-81.49), but showed no significant difference in the 50-59 age group. The mean LDL-cholesterol values within 3 months before TCF were significantly different in the 40-49 age group (p < 0.001 95% CI 15.22-47.70) and were not different in the 50-59 age group. There was no statistical difference in the mean values for HDL-cholesterol, triglyceride and body mass index in either age group. From these results, it can be concluded that the risk of colorectal adenomatous polyp was associated with a higher serum total cholesterol level in the 40s, and seemed to indicate a close relationship with the risk of colorectal cancer.

[The development of diagnosis-related groups--DRG's. Methodology for classifying hospital patients].

The history of Diagnosis Related Groups (DRG's), a system for classifying patients in acute care hospitals, developed by researchers at Yale University, USA, is reviewed. DRGs are an instrument for measuring the hospital product, primarily from a management viewpoint. Starting with a review of the definitions of hospital product, the article follows the course from the first DRGs through the most recent revision, providing a summary of potential and current applications of the system in several countries, which range from payment mechanism to uses in quality control.

Geographical information systems and bootstrap aggregation (bagging) of tree-based classifiers for Lyme disease risk prediction in Trentino, Italian Alps.

The risk of exposure to Lyme disease in the province of Trento, Italian Alps, was predicted through the analysis of the distribution of Ixodes ricinus (L.) nymphs infected with Borrelia burgdorferi s.l. with a model based on bootstrap aggregation (bagging) of tree-based classifiers within a geographical information system (GIS). Data on L ricinus density assessed by dragging the vegetation in 438 sites during 1996 were cross-correlated with the digital cartography of a GIS, which included the variables altitude, exposure and slope, substratum, vegetation type and roe deer density. Ticks were more abundant at altitudes below 1,300 m a.s.l., in the presence of limestone and vegetation cover with thermophile deciduous forests and high densities of roe deer. A bootstrap aggregation procedure (bagging) was used to produce a model for the prediction of tick occurrence, the accuracy of which was tested on actual tick counts assessed by a further dragging campaign carried out during 1997 to determine infection prevalence and resulted in average 77%. Other tests of the model were made on additional and independent data sets. The prevalence of infection with Borrelia burgdorferi s.l, determined by polymerase chain reaction on 2,208 nymphs collected by random dragging in 245 transects selected within eight areas where the model predicted the occurrence of I. ricinus during 1997, was 17.5% and was positively correlated to tick abundance and roe deer density. These findings were used to relate the output of the bagged model (probability of tick occurrence) to the density of infected nymphs through a stepwise model selection procedure and thus to produce a GIS digital map of the probability distribution of infected nymphs in the Province of Trento at high resolution scale (50 by 50-m cell resolution). The application of the bagging procedure increased the accuracy of the prediction made by a single classification tree, a well-known classification method for the analysis of epidemiological data.

A substrain mouse serologically classified in ddN strain and its behavioral characteristics.

A special substrain mouse in ddN strain was classified by cross agglutination reaction of the red blood cells with anti-ddN mice red blood cells rabbit immune serume. The agglutination reactivity in the full-grown substrain mouse was as high as in an infant ddN mouse. Incidence of the substrain mouse in ddN strain aged older than 9 weeks was about 10%. The full-grown substrain mouse revealed a form of excitation after long-term administration of methamphetamine while other mice in the same strain were depressed. The behavioral and serological properties of histocompatible strain mice such as C57Bl and C3/ were studied to compare with those of the ddN substrain mouse.

Neuropathotyping: a new system to classify Marek's disease virus.

A statistical approach was used to establish a new classification system of Marek's disease virus (MDV) on the basis of neurologic responses. To develop the system, neurologic response data from 15x7 chickens inoculated with 30 strains of serotype 1 MDV were statistically analyzed by a cluster analysis. The goal was to identify a statistical system that would verify if three neurovirulence groups correlated with the three pathotypes previously described. The system was also validated in two additional strains of specific-pathogen-free (SPF) chickens, SPAFAS and line SC (Hy-Vac). The proposed system is based on analysis of three variables: 1) frequency of birds showing transient paralysis between 9 and 11 days postinoculation (dpi), (2) mortality before 15 dpi, and (3) frequency of birds showing persistent neurologic disease between 21 and 23 dpi. By use of this system, a MDV may be classified in one of three groups, designated neuropathotypes A, B, and C, which roughly correspond to the virulent, very virulent, and very virulent plus pathotypes, respectively. However, correlation between neuropathotype and pathotype was not absolute, and neuropathotyping is more a complement to the current pathotyping system than a replacement for it. Our results showed that neuropathotyping studies can be conducted in two types of commercial SPF chickens by the use of the same variables, although the system would first have to be standardized by the use of prototype viruses. Neuropathotypes can also be estimated with our statistical analysis with reasonable accuracy. By use of this analysis, we established that MDV strains within the very virulent pathotype may be subdivided into neuropathotypes B and C, thus establishing a previously unrecognized pathotypic classification. This finding illustrates how neuropathotyping may extend important information not identified by conventional pathotyping.

Interproximal contact and marginal ridge relationships in periodontally healthy young males classified as to orthodontic status.

Three groups of periodontally healthy young males, classified as to orthodontic status, were evaluated for proximal contact and marginal ridge relationships in the maxillary left quadrant. The majority of anterior teeth did not have satisfactory proximal contacts on both the mesial and distal aspects. Open or defective proximal contact relationships were common in all three orthodontic groupings. In posterior areas, the orthodontically treated group had the highest percentage of open contacts. The orthodontically treated group had a significantly higher percentage of even marginal ridges in the teeth that are customarily banded.

Occlusal characteristics and tooth mobility in periodontally healthy young males classified orthodontically.

Three groups of periodontally healthy young males classified as to orthodontic status by the Handicapping Labio-Lingual Deviations Index were evaluated for various occlusal characteristics. A much larger proportion (95%) of the orthodontically normal subjects (Group I) had an Angle Class I type of occlusion than the Group II subjects requiring orthodontic care (60.5%), or the Group III subjects who had received orthodontic care (63.4%). The percentages of subjects with a "cuspid-protected" type of occlusion were larger in the orthodontically normal and orthodontically treated groups than in the group requiring orthodontic care. Anterior displacement of the mandible in closing from centric relation to the intercuspal position was found in the majority of subjects in each group. Displacement of more than 2 mm was most common in the subjects requiring orthodontic care. Only 1 of the 41 orthodontically treated subjects reported that his occlusion had been adjusted by grinding after tooth movement. There were no statistically significant differences in mean tooth mobility values between the orthodontically normal and orthodontically treated subjects. The lateral incisor tooth had a significantly lower mean mobility value in subjects whose orthodontic care included removal of the first premolar than in subjects treated without removal of the first premolar. Wear facets were common in all three groups. Based on the findings, one can conclude that orthodontic therapy was only partially successful in obtaining the objectives described as part of an ideal result. It should be noted, however, that the status of the dentition and occlusion prior to orthodontic treatment could not be determined for more than a few subjects.

Robust Selection of Predictive Genes via a Simple Classifier.

Identifying genes that direct the mechanism of a disease from expression data is extremely useful in understanding how that mechanism works. This in turn may lead to better diagnoses and potentially could lead to a cure for that disease. This task becomes extremely challenging when the data are characterised by only a small number of samples and a high number of dimensions, as is often the case with gene expression data. Motivated by this challenge, we present a general framework that focuses on simplicity and data perturbation. These are the keys for robust identification of the most predictive features in such data. Within this framework, we propose a simple selective naive Bayes classifier discovered using a global search technique, and combine it with data perturbation to increase its robustness for small sample sizes. An extensive validation of the method was carried out using two applied datasets from the field of microarrays and a simulated dataset, all confounded by small sample sizes and high dimensionality. The method has been shown to be capable of selecting genes known to be associated with prostate cancer and viral infections.

New non-invasive automatic cough counting program based on 6 types of classified cough sounds.

UNLABELLED: Cough consisting of an initial deep inspiration, glottal closure, and an explosive expiration accompanied by a sound is one of the most common symptoms of respiratory disease. Despite its clinical importance, standard methods for objective cough analysis have yet to be established. OBJECT: We investigated the characteristics of cough sounds acoustically, designed a program to discriminate cough sounds from other sounds, and finally developed a new objective method of non-invasive cough counting. In addition, we evaluated the clinical efficacy of that program. SUBJECTS AND METHODS: We recorded cough sounds using a memory stick IC recorder in free-field from 2 patients and analyzed the intensity of 534 recorded coughs acoustically according to time domain. First we squared the sound waveform of recorded cough sounds, which was then smoothed out over a 20 ms window. The 5 parameters and some definitions to discriminate the cough sounds from other noise were identified and the cough sounds were classified into 6 groups. Next, we applied this method to develop a new automatic cough count program. Finally, to evaluate the accuracy and clinical usefulness of this program, we counted cough sounds collected from another 10 patients using our program and conventional manual counting. And the sensitivity, specificity and discriminative rate of the program were analyzed. RESULTS: This program successfully discriminated recorded cough sounds out of 1902 sound events collected from 10 patients at a rate of 93.1%. The sensitivity was 90.2% and the specificity was 96.5%. CONCLUSION: Our new cough counting program can be sufficiently useful for clinical studies.

A fuzzy-classifier system to distinguish respiratory patterns evolving after diaphragm paralysis in the cat.

We applied the fuzzy "k-nearest neighbor" (k-NN) classifier of the pattern recognition theory to fathom the abnormal way of breathing resulting from diaphragm paralysis and to distinguish the dominant component, tidal or frequency, of the breathing pattern on which ventilatory compensation relies in such a pathological state. We addressed this issue in the experimental model of diaphragm paralysis as a result of bilateral phrenicotomy in anesthetized, spontaneously breathing cats. Of several variables recorded, we selected two features, minute ventilation and arterial CO(2) tension, that were used for the k-NN analysis. The results demonstrate that the ability to maintain ventilation critically depended on the increase in frequency of breathing. Other breathing pattern strategies were ineffective. The k-NN evaluation with the two selected features discerned the prevailing pattern of breathing with sufficient probability. Such an evaluation may be a useful tool in predicting the development of compensatory strategies in disordered patterns of breathing.

Histologically classified venous angiomas of the brain: a controversy.

The term "venous angioma" (VA) usually refers to a developmental venous anomaly (DVA). However, a group of vascular malformations called VAs shows no venous abnormalities on angiography. The clinical and histological features of histologically classified VAs were studied in eight patients who presented with hemorrhage or seizures to reevaluate these venous anomalies. Angiography showed no venous abnormalities in six patients. Histological study included immunostaining for smooth muscle actin and glial fibrillary acidic protein. Surgical specimens of 10 cases of cavernous angiomas, 10 cases of arteriovenous malformations, and two cases of capillary telangiectasias were studied to compare these types of VAs. Angiographically occult VAs were surgically removed safely, whereas removal of DVAs was complicated by brain swelling and hemorrhagic infarction of the brain. Histological examination found angiographically occult VAs contained malformed and compactly arranged vessels with partly degenerated walls, whereas DVAs had dilated thin-walled vessels that were diffusely distributed in the normal white matter. This study of our cases and a review of the reported cases of VAs suggests that two different clinical and pathological entities are commonly categorized as "VA," angiographically occult VAs and DVAs. These two entities should be carefully distinguished.

Rorschach indices of children classified as hyperactive.

The Rorschach protocols of 20 boys (6 to 11 yr.) rated by their teachers as hyperactive and those of 20 nonhyperactive boys were compared along 16 indices, including R, P, M, FM, C, CF, FC, Sum C, A%, H%, F+%, Sum Shading, and the reaction times. The results suggested that children classified as hyperactive produce fewer movement (M) and Human (H) responses but a far greater percentage of Animal (A) responses than the matched clinic control group. Age-related effects occurred within the hyperactive group such that younger hyperactives produced a higher Sum C, a lower H% and F+%, and had shorter reaction times than the older hyperactives. Findings were discussed in terms of the assessment of hyperactivity within a conceptual framework which regards impulsivity as a major underlying dimension.
