Increases in thyroid gland blood flow after hemithyroidectomy in the rat.

After subtotal thyroidectomy, the thyroid gland remnant undergoes compensatory alterations in function and morphology. Under the trophic stimulation of elevated plasma TSH concentrations, the thyroid remnant responds with an increase in hormone synthesis and secretion and, in addition, increases in mass. We have examined the alterations in thyroid blood flow which accompany increased secretion and growth after hemithyroidectomy (HTX) in male Sprague-Dawley rats (200-220 g). At various times after surgical HTX (1, 2, and 3 weeks), blood samples for the determination of plasma hormone concentrations were obtained and tissue blood flows were determined using 15 +/- 5 microns diameter 141Ce-labeled microspheres in a modification of the reference sample microsphere technique. The microspheres were injected directly into the left cardiac ventricle via a 23-gauge needle passed through the chest wall while a reference blood sample was collected. After the animals were killed, tissues were cleaned and weighed, then tissue and reference blood sample radioactivities were determined. In addition, thyroidal immunoreactive vasoactive intestinal peptide was measured after acetic acid (0.67 N) extraction. After HTX, plasma TSH concentrations were significantly elevated. The plasma concentrations of T4 and T3 fell, but by less than the expected 50%. The mass of the remaining thyroid lobe increased progressively over the 3 weeks post thyroidectomy, reaching approximately 70% of the total thyroid gland weight of sham-operated controls. Thyroid blood flow per gram of tissue was significantly elevated at all times post HTX. HTX did not induce any alterations in thyroidal immunoreactive vasoactive intestinal peptide concentration. Thus, after HTX, the well documented compensatory alterations in thyroid remnant growth and secretion were accompanied by a prompt and striking increase in thyroid blood flow.

D-asparatate oxidase in the thyroid gland.

D-Aspartate oxidase was isolated from the pig thyroid gland and purified over 600 times. The enzyme was obtained in an inactive form of apoenzyme and was activated by FAD. It was specific towards the D-form of aspartic acid, had no effect on the L-form, and was also inactive towards other monocarboxlyic D-amino acids. The enzyme was only slightly active towards D-glutamate. The Michaelis constant based on the Lineaweaver-Burk plot was 5 mmol/l. The optimum pH was 8.7. D-Aspartate oxidase was inhibited by KCN in concentrations varying from 0.05 to 1 mmol/l. The biological role of this enzyme in the thyroid gland is discussed.

A prospective study on histochemical observation of thyroid gland at prepubertal Black Bengal goat.

In the present study, Period Acid Schiff (PAS), Verhoeff's and Van Gieson stains were used in the Department of Anatomy and Histology to observe the histochemical architecture of thyroid gland at prepubertal Black Bengal goat. Thyroid follicular cells were cuboidal in prepubertal goats. These cells showed moderate PAS-positive reaction in prepubertal group. PAS activity in the basement membrane of the thyroid follicles was mild in prepubertal Black Bengal goats. The lumen of the thyroid follicles contained homogenous intense PAS positive colloid materials in prepubertal goats. Parafollicular cells showed intense PAS positive reaction for glycogen in prepubertal goats. Capsule, connective tissue septa, interfollicular connective tissue of the thyroid gland mainly composed of collagen fiber. The wall of blood vessels in the capsule and interfollicular blood capillaries was composed of collagen fiber and elastic fiber. Elastic fiber formed the inner layer of the blood vessels showed blue black to black color. Inner layer of the capsule penetrated the gland parenchyma divided the gland into indistinct lobe and lobules. Collagen fiber distributed around the thyroid follicle formed the interfollicular stroma of the thyroid gland. The results of the present study can also be used to compare with that of human specially in AutoImmune Thyroiditis (AIT).

Action of different doses of I-131 on thyroid gland, its tissue distribution and elimination in adult male rats.

Adult male Wistar rats were administered 0.1, 1, 2 or 5 mCi/kg I-131 i.p. Activities were measured in the thyroid gland or region, blood plasma (total and protein bound), salivary gland, cerebral cortex (= brain), skin, liver, spleen, small intestine, kidney, heart and skeletal muscle after 1-30 (36) days. Beginning 8 days after radiothyroidectomy the remaining or possibly regenerated thyroid function was tested with a test dose of 5 microCi/kg i.p. After administration of 0.1 mCi/kg I-131 the thyroid gland shows a regeneration within 30 days, but not after treatment with the higher doses. The time course of tissue distribution of activity is dose dependent: after 0.1 mCi/kg I-131 increasing activities after the 3rd-8th day are observed in skin, brain, salivary gland, intestine, spleen, kidney, heart and skeletal muscle. After 1 mCi/kg I-131 only in skin, brain, liver and intestine increasing activities between the 8th and 30th day have been observed. After 5 mCi/kg I-131 only in skin, spleen and intestine increasing activities after the 15th day were measured. Thyroid gland activity 24 hs after administration of 5 microCi/kg I-131 showed during the experimental period of 50 days considerable variation with a factor of 4 for thyroid gland, 8 for protein bound and about 10 for total plasma I-131 activity.

[Diagnostic procedure in suspected functional disorders of the thyroid gland].

In an attempt to derive diagnostic concepts for thyroid diseases we present pathophysiological models for the prevalent thyroid disorders. 'Euthyroid goiter' is a disease mainly caused by iodine deficiency but an additional immunopathogenesis was recently proposed. The 'immunthyropathy' is the thyroid disease with orbitopathy and other extraglandular immunological manifestations. A complete model of the immunological phenomena which begin with a tolerance defect is given, and both the T-cell, and B-cell-mediated pathways are detailed. The complex interaction of immunoglobulins at the thyroid-stimulating hormone receptor and their dependency on human leukocyte antigen loci are presented. The peripheral metabolism depends ultimately upon a prevalence of thyroid gland stimulation (thyrotoxicosis) or glandular destruction (hypothyroidism) and this is true for overt thyroid disease under antithyroid drug therapy or any other therapy. Euthyroidism during 'immunethyropathy' is presented as an equilibrium between thyroid stimulation and destruction. This concept allows an exact description of the thyroid disease and the resulting clinical situation provided that established laboratory tests are used as suggested by the model. 'Disseminated thyroid autonomy and autononous thyroid adenoma' develops during goitrous thyroid disease as a consequence of uncoupling of thyroid cellular growth stimulation, iodine utilization, and thyroid hormone synthesis. The polyclonal origin seems more frequent than monoclonal foci. The size of autononous tissue and individual iodine supply determines the endocrine function in this disease. The TRH test monitors with great sensitivity subtle increases in T4 or T3 production and indicates critical clinical situations earlier than the scintiscan.(ABSTRACT TRUNCATED AT 250 WORDS)

Identity of calcitonin extracted from normal human thyroid glands with synthetic human calcitonin-(1-32).

Calcitonin in human thyroid glands obtained at autopsy from normal subjects were extracted with 2 M acetic acid. The extracts were additionally purified by adsorption to Sep-Pak C18 cartridges, and calcitonin was identified after gel filtration analysis, reverse-phase high-performance liquid chromatography (HPLC), thin-layer chromatography and isoelectric focusing. The purification steps were monitored by radioimmunoassay, and partially purified calcitonin was used for biological and physicochemical comparison with synthetic human calcitonin-(1-32) and its Met8-sulfoxide form. On gel filtration analysis a predominant peak coeluted with the synthetic hormone, and on HPLC two discrete peaks with the retention times of monomeric and dimeric human calcitonin were found. Thin-layer chromatography allowed the detection of two peaks with the Rf of human calcitonin-(1-32) and of its sulfoxide, respectively. The pI (7.9) of the predominant peaks of synthetic calcitonin were identical. Our findings provide strong evidence that the predominant forms of human calcitonin extracted from normal thyroid glands correspond to synthetic calcitonin-(1-32) and to dimeric calcitonin.

[A comparative study of cytological and histological studies of the thyroid gland].

We compare the results of fine-needle aspiration cytology of the thyroid gland with postoperative histological findings in 533 patients with thyroidectomy operated on between 1987 and 1994. The classification of the cytological preparations followed the proposal of Weiss and Pilz [35] with the groups 0-IV. In group 0 the most frequent findings were cystic changes in multinodular goitre or malpuncture, respectively. The groups I and II contained cases with multinodular goitre, thyroiditis and follicular adenomas, but 4 carcinomas too. The group III comprised particularly follicular adenomas and carcinomas (8 cases). Both cases in group IV were carcinomas. In group III (cytologic group with suspicious findings) there were 65 cases with false positive results of the cytologic investigation resulting in a specificity of 86.4%. 4 out of 14 thyroid carcinomas could not be detected by cytologic preparations corresponding to a sensitivity of 71.4% for carcinomas. The false negative rate for thyroid carcinomas in group II was caused by regression areas in the center of the tumors (3 cases) as well as one microcarcinoma. Preoperative fine-needle aspiration cytology of the thyroid gland reduces the number of patients operated on for multinodular goitre or cold nodules especially in cases suspicious of carcinoma.

Effects of toxic doses of a novel histamine (H2) antagonist on the rat thyroid gland.

The oral administration of high doses of a histamine H2 antagonist SK&F 93479 (up to 1000 mg/kg/day) to male rats for up to 21 days resulted in alterations in thyroid morphology indicative of increased activity of the thyroid gland. Measurement of thyroidal 125I incorporation substantiated these findings. Treatment with SK&F 93479 resulted in a dose-dependent increase in thyroidal iodide incorporation. This was apparent after a single dose of the compound and was reversible after dosing for 7 days. The increased incorporation of 125I into the thyroid gland was apparently dependent on thyroid-stimulating hormone (TSH) since both hypophysectomy and pretreatment with thyroxine (T4) markedly reduced thyroidal 125I uptake. Hypothalamic thyrotropin-releasing hormone (TRH) and pituitary TSH concentrations were not altered by SK&F 93479 treatment, and in TRH challenge experiments circulating TSH concentrations showed no change from control levels. These data suggest that hypothalamic-pituitary sensitivity was unaltered by treatment with SK&F 93479. Pharmacological ablation of thyroidal mast-cell function did not alter the thyroid response to 125I accumulation after SK&F 93479 dosing, indicating that the action of the compound is probably not dependent on changes in thyroid mast-cell histamine. Circulating T4 and TSH levels were altered in SK&F 93479-treated rats. Generally, T4 levels were reduced 6 hr after dosing and TSH levels were elevated 24 hr after dosing. Triiodothyronine (T3) levels were unaffected by SK&F 93479 treatment. The effect of SK&F 93479 treatment on T4 clearance was measured by examining the elimination of radioactivity from the circulation of rats previously injected with 125I-labelled T4. One oral dose of 1000 mg SK&F 93479/kg markedly increased T4 clearance. These results suggest that SK&F 93479 affects thyroid activity indirectly by a primary effect on T4 clearance. Reductions in circulating T4 lead to increased TSH levels and subsequent stimulation of thyroid activity.

[Anaplastic carcinoma of the thyroid gland with chondroid, osteoid, and marocellular metaplasia of the stroma, associated with Hashimoto's lymphocytic thyroiditis].

Anaplastic, mostly spindle-cellular, carcinoma of the thyroid gland was diagnosed bioptically in a 65-year-old woman. Necroptically it was found that the carcinoma was associated with Hashimoto's lymphocytic thyroiditis. In the hitherto-differentiated carcinomatous parts the papillary component prevailed and the original lymphocytic stromal infiltration was in evidence here. In the anaplastic parts isolated macrocellular elements occurred and, in some places, also chondroid and osteoid structures, especially the structures of macrocellular bone tumor character. These structures were entirely missing in the distant metastases and were, therefore, evaluated as a stromal nontumorous metaplastic process.

[Adenolipomas, lipomatosis and amyloidosis of the thyroid gland].

Adipose tissue in the thyroid gland was observed in 7 biopsies and 2 autopsies: in 3 cases of solitary adenolipoma, 3 cases of multiple adenolipoma, 1 case of pure lipomatosis and 2 cases of lipomatosis with multiple adenolipomas combined with amyloidosis of lobules as well of adenomas in one bioptic case. Pathogenesis of this unusual phenomenon was discussed.

Association of the risk of development of hypothyroidism after iodine 131 treatment with the pretreatment pattern of sodium pertechnetate Tc 99m uptake in the thyroid gland in cats with hyperthyroidism: 165 cases (1990-2002).

OBJECTIVE: To assess whether the risk of development of hypothyroidism after treatment with iodine 131 (131I) was associated with the pattern of sodium pertechnetate Tc 99m activity in the thyroid gland detected via scintigraphy before treatment in cats with hyperthyroidism. DESIGN: Retrospective study. ANIMALS: 165 cats. PROCEDURE: Medical records of cats with hyperthyroidism that had been treated with 131I (from 1990 to 2002) and had undergone scintigraphy of the thyroid gland before treatment were reviewed; data regarding signalment, scintigraphic findings (classified as unilateral, bilateral-asymmetric, bilateral-symmetric, or multifocal patterns), serum total thyroxine (T4) concentrations before treatment and prior to hospital discharge, and 131I treatment were collected. A questionnaire was sent to each referring veterinarian to obtain additional data including whether the cats subsequently developed hypothyroidism (defined as serum total T4 concentration less than the lower reference limit > or = 3 months after treatment). RESULTS: 50 of 165 (30.3%) 131I-treated cats developed hypothyroidism. Hypothyroidism developed in 39 of 109 cats with bilateral, 10 of 50 cats with unilateral, and 1 of 6 cats with multifocal scintigraphic patterns of their thyroid glands. Cats with a bilateral scintigraphic pattern were approximately 2 times as likely to develop hypothyroidism after 131I treatment than were cats with a unilateral scintigraphic pattern (hazard ratio, 2.1; 95% confidence interval, 1.04 to 4.2). CONCLUSIONS AND CLINICAL RELEVANCE: Cats with hyperthyroidism that have a bilateral scintigraphic pattern in the thyroid gland before 131I treatment appear to have a significantly higher risk of subsequently developing hypothyroidism, compared with cats with a unilateral scintigraphic pattern.

[Thyroid gland reaction to experimental resection and complete removal of the pancreas].

The end or the central part of the pancreas was resected or pancreatomy was performed in dogs. The morphological and functional state of the thyroid gland was investigated by the histological, histochemical, and morphometrical methods at different postoperative periods. The changes revealed showed structural and functional alterations of the thyroid gland to be dependent primarily on the type of surgical interventions on the pancreas. Resection of the terminal sections caused temporary and insignificant changes in the thyroid gland, but complete removal of the pancreas was followed by acute dystrophy and atrophic changes in the thyroid gland, this leading to the functional exhaustion.

[Folliculoid differentiation in medullary cancer of the thyroid gland].

Apart from production of argirophilic substance, tumor cells of medullary carcinomas of the thyroid gland form colloids due to differentiation of tumor elements into both argirophilic and colloid-producing cells. Accordingly, solid-follicular structures appear in them rather frequently. This requires the use of Grimelius test for differential diagnosis of medullar and follicular carcinoma of the thyroid gland.

Radiation dose to the lens of eye and thyroid gland in paranasal sinus multislice CT.

CT has become an established examination in the evaluation of the paranasal sinuses. Until recently this was achieved by the direct coronal technique on conventional and single slice helical scanners. With the advent of multislice technology, thin slice axial CT with excellent coronal and sagittal reconstructions is now the norm. We describe a study designed to evaluate the radiation dose to the lens of the eye and thyroid gland in the axial and coronal planes on a Siemens Volume Zoom quad slice scanner at 140 kV and effective mAs of 100 using 1 mm collimation. Thermoluminescent dosimeters were placed on the eyelid and thyroid gland of 29 patients scanned axially in the supine position and a further 28 patients scanned coronally in the prone position with gantry tilt. The results show mean doses of 35.1 mGy (lens) and 2.9 mGy (thyroid gland) in the coronal plane compared with 24.5 mGy (lens) and 1.4 mGy (thyroid gland) in the axial plane. Results obtained from a head phantom and from using the ImPACT CT dose calculator were comparable. The kV and mAs were then reduced to 120 and 40, respectively, and the axial study repeated using the head phantom and predicted doses using the ImPACT CT dose calculator. The low dose scanning technique revealed a lens dose of 9.2 mGy and thyroid dose of 0.4 mGy. The eye dose on a multislice scanner is still substantially less than the threshold dose of 0.5-2 Gy for detectable lens opacities. These results indicate that, in addition to the established perceived advantages of multislice axial sinus CT, i.e. patient comfort, no artefact from dental amalgam and reproducible true coronal images, should be included a decreased radiation dose to both the eye lens and thyroid gland compared with direct coronal scanning.

[Radiotherapy in malignant tumors of the thyroid gland].

The primary treatment of thyroid gland malignoma is surgery. Success of radiotherapy depends of extent of the previous surgery treatment. The types of radiotherapy are: curative (prophylactic and postoperative) and palliative. Tumor dose and radiotherapy tehnique depend on histologica type, extent of the previous surgery treatment, curative or paliative intent and general condition of patient, and they are from 40 Gy to 65 Gy by conventional fractionation. The basis of radiotherapy treatment planning and choice of radiotherapy treatment technique in survey of the region of interest by imaging procedure. On the basis of those data we determine therapeutic volume and structure of risk (spinal cord, lung) and protection of the risky structures is planned.

Neoplasms metastatic to the thyroid gland.

In this report, the incidence and pathologic factors of neoplasms metastatic to the thyroid gland in patients seen at the Mayo Clinic are examined. The most frequent primary tumors were renal cell carcinoma and adenocarcinoma of the breast. A review of recent studies of this topic at other institutions is also offered. It is suggested that fine needle aspiration of the thyroid gland be used diagnostically in all patients with a prior malignant tumor at a distant site who have a thyroid nodule. A therapeutic approach is suggested which limits thyroidectomy largely to instances of slow growing tumors without evident metastasis elsewhere.

[Dilated cardiomyopathy associated with autoimmune disease of thyroid gland].

Congestive heart failure is the main cause of cardiovascular morbidity and mortality with more than one million hospitalizations per year. Dilated cardiomyopathy is one of the main causes of congestive heart failure characterized by diminished function of the left, right or both ventricles and ejection fraction less than 40%. Numerous studies reported that the diseases of thyroid gland among the patients with dilated cardiomiopathy were very frequent: with prevalence from 18% to 49%. The impaired thyroid gland function was found as primary cause of cardiomyopathy in 1% of patients. At the same time, among patients with terminal dilated cardiomyopathy, euthyroid sick syndrome was very frequent with low values of T3. According to our results, 29.4% of patients with dilated cardiomyopathy also had autoimmunity disorder of thyroid gland with present antithyroglobin and antimicrosomal antibodies. These patients had lower ejection fraction and worse prognosis. In American College of Cardiology/American Heart Association guidelines for the evaluation and treatment of the congestive heart failure, evaluation of thyroid gland function was recommended; but the evaluation of antitireoglobulin antibodies and free T3 and free T4 was also essential, as was calculation of their ratio. This is the way to discover the autoimmunity disorder of thyroid gland and/ or "euthyroid sick syndrome". In patients with dilated cardiomyopathy and low-T3 syndrome, short-term administration of thyroid hormones resulted in improvement of ejection fraction, diminishing of symptoms and better survival.

Aspiration cytology of the oncocytic variant of papillary adenocarcinoma of the thyroid gland.

OBJECTIVE: To study the cytologic features of the oncocytic variant of papillary adenocarcinoma of the thyroid gland to distinguish this subtype from other oncocytic lesions of the thyroid. STUDY DESIGN: We reviewed the smears from aspiration biopsies of 6 proven cases of oncocytic variant of papillary adenocarcinoma and compared their cytologic features with smears from 19 oncocytic follicular neoplasms (11 adenocarcinomas and 8 adenomas). Smears were stained with a modified Giemsa stain (Diff-Quik). RESULTS: All smears were cellular. Colloid was variable but more abundant in cases of the oncocytic variant of papillary adenocarcinoma. The cells in papillary adenocarcinoma had round to ovoid, overlapped nuclei; prominent intranuclear inclusions; and "grooves." Nucleoli were generally absent. In oncocytic follicular neoplasms, the cells had round nuclei and prominent nucleoli. Nuclear inclusions and grooves were seen but were not as prevalent as in papillary adenocarcinomas. CONCLUSION: The oncocytic variant of papillary adenocarcinoma of the thyroid gland can be distinguished from other oncocytic lesions by fine needle aspiration biopsy, whereas the absence of prominent nucleoli in oncocytes favors the diagnosis of an oncocytic papillary adenocarcinoma.
