[Evaluation of the effect of Snyder-Thompson scleroplasty based on the behavior of the axial dimension of the eye ball].

A trial of evaluation of the effectiveness and durability of the effects of the surgical technique performed on the basis of the behaviour of the antero-posterior axial dimensions of the eye. Observations concerned 75 persons (129 eyes), the control group consisted of 20 persons (40 eyes) treated conservatively. Evaluating the dynamics of the pathological process in persons treated conservatively one found a considerable increase of the eye ball. In a similar time 12 months examinations carried out on the operated eyes showed statistically essential shortening of the axis of the eyes and in the time of subsequent controls (1-1.5 years after surgery) their slight elongation. The last control (after 5 years) showed a still persisting, statistically essential shortening of the eye, on the average 0.22 mm, in relation to the measurements taken before the scleroplasty.

[Current views on the pathological changes in the nervous system of persons chronically exposed to organic solvents].

On the basis of a review of the pertinent literature the author discusses pathological changes present in subjects with chronic exposure to organic solvents. The author describes the most frequent symptoms and signs constituting clinical syndromes--neurosis-like, encephalopathic and polyneuropathic. A number of observations of subjects with long-term exposure to benzine and benzene homologues are quoted. Diagnostic investigations of the nervous system are described which are used in cases of suspected chronic poisoning with organic solvents.

[Assessment of recognition of emotions by healthy persons and patients with focal brain pathology].

To assess the involvement of different structures of the human brain into successive stages of the recognition of the principal emotions by facial expression, we examined 48 patients with local brain lesions and 18 healthy adult subjects. It was shown that at the first (intuitive) stage of the recognition, premotor areas of the right hemisphere and temporal areas of the left hemisphere are of considerable importance in the recognition of both positive and negative emotions. In this process, the left temporal areas are substantially involved into the recognition of anger, and the right premotor areas predominantly participate in the recognition of fear. In patients with lesions of the right and left brain hemispheres, at the second (conscious) stage of recognition, the critical attitude to the assessment of emotions drops depending on the sign of the detected emotion. We have confirmed the hypothesis about a correlation between the personality features of the recognition of facial expressions and the dominant emotional state of a given subject.

[Use of personal computers for early detection of cardiovascular pathology among industrial workers].

The results are provided on the development and introduction of automated working places on the basis of personal computers for the prevention, follow-up and treatment of patients with arterial hypertension (AH) and ischemic heart disease (IHD) at an industrial enterprise. The process of identifying persons with AH and IHD is automatized. Automated working places on ECG processing, assessment of central and cerebral hemodynamic parameters are introduced. Information load has been developed in the form of a computer game for identifying persons with early stages of AH. A base has been created for storage of the results of regular medical check-ups. The complex has been tested during 2 years in the settings of a health unit at the Lenin tractor plant in Minsk. As a result of comprehensive measures, provisional functional diagnosis expenditures were reduced by 30-70%. Losses due to temporary disability as a result of circulatory diseases were reduced by 26%.

Attachment pathology and low social skills in avoidant personality disorder: an exploratory study.

The criterion of desire for affection and acceptance was eliminated from the definition of avoidant personality disorder in the DSM-III-R. But clinical experience in our setting suggests that attachment pathology related to such desires is an important diagnostic feature and a frequent focus of clinical intervention with avoidant individuals. Forty-seven patients receiving a clinical diagnosis of avoidant personality disorder completed a brief questionnaire which rated desire for an attachment relationship, fear of an attachment relationship and level of social skills. The results support our hypotheses that attachment security is distinct from general social skills and that desire for but fear of an attachment relationship is more characteristic of those diagnosed with avoidant personality disorder than a low level of social skills.

Sequeale of acute viral hepatitis type B.

In 1976 we undertook to evaluate the incidence of chronic liver lesions in 161 patients treated in hospital during the years 1970-1975 for their serologically established acute viral hepatitis type B (AVH-B). At systematic control examination mode in 1976, after a period from 1-5 years since the acute onset of disease, it was established that in 133 individuals (82.6%) the antigen HBs had disappeared from blood and the BLT had become normal. Persistent HBs antigenemia was established in 20 (12.4%) individuals. In 15 (9.2%) patients persistent HBs antigenemia was accompanied by pathologic BLT, in 5 (3.1%) cases liver function became returned to normal yet with the persistent HBs antigenemia after their recovery from A VH-B. In 8 (4.9%) patients pathologic BLT persisted although HBsAg had disappeared from blood. Among 28 persons with persistent pathological BLT or with persistent HBs antigenemia out of a total of 161 patients who had had A VH-B, there were 11 (6.8%) cases with the bioptically proved CPHf, 8 (5.0%) cases with CPH, 5 (3.1%) cases with CAH, while 4 (2.5%) patients showed fatty liver metamorphosis or had by light microskopy completely normal liver. CAH was established only in cases with persistent BHs antigenemia and pathological BLT. The incidence of the chronic liver lesion and of the persistent antigenemia was among our patients who had had A VH-B in inverse ratio to the intensity of their initial infection. Our study suggests that no prodisposition for persistent HBs antigenemia is created by the prednisolone therapy.

High prevalence of personality disorders among healthy volunteers for research: implications for control group bias.

Individuals who volunteer as control subjects for clinical studies are regularly screened for Axis I diagnoses, but seldom screened for Axis II disorders. This study examined the relative rates of Axis II diagnoses among 341 volunteers passing an initial telephone screen for entry into biological research studies. Axis I and II diagnoses by DSM-IV were assigned by best estimate after structured clinical interview, and subjects were categorized into one of three groups based on their diagnostic profiles: (1) volunteers without lifetime Axis I or II diagnoses ("healthy controls"), (2) personality-disordered volunteers without any history of Axis I pathology, and (3) personality-disordered volunteers with past (but not current) Axis I pathology. The results revealed a high prevalence of personality disorders (44.4%) among these volunteers. Several clinically relevant self-report inventories were used to demonstrate important characterological differences between the three comparison groups. Although inventory results demonstrated multiple differences between all three groups, most scales revealed differences between healthy controls and the two personality-disordered groups (with or without lifetime Axis I diagnoses), suggesting that most of the variance was accounted for by the presence or absence of an Axis II disorder, not a past Axis I disorder. These results suggest that personality-disordered volunteers may bias a control group due to the infrequent screening for Axis II disorders among volunteers for medical and psychiatric research. Implications are discussed for routine Axis II screening of volunteers for research with specific diagnostic instruments.
