Perioperative nursing in the college curriculum. A custom fit.

Although this course is in its infancy, it has become popular among baccalaureate students. Incoming junior students are enthusiastic about an elective course that provides them an opportunity to focus on an aspect of nursing that is no longer available to them in most nursing programs. The morale of the operating room nursing staff, particularly among the preceptors, has increased tenfold. In an era of nursing shortages, creative and innovative methods of recruiting students and retaining experienced nurses in all fields are vital. This unique course may be one of those methods. It is widely known that high morale yields a more productive work environment, lower turnover, and a cohesive work force that provides high quality nursing care in a most cost-effective manner.

The impact of surgery on body image.

Surgery has an impact on the patient's view of himself and his body. It is within the professional nurse's realm to skillfully assess, make a nursing diagnosis, plan, and follow through on findings of body image assessment. Operating room nurses can expand their roles by contributing to this dimension of care. Because of their close contact with the patient, they are in an ideal situation to attend to this patient need. O.R. nurses need not be "little psychiatrists" in order to do this--only caring, intelligent, resourceful persons who use the nursing process and truly place the patient's welfare above all else.

Antimicrobial therapy for surgical patients.

The operating room nurse plays a critical role in the administration of antimicrobial therapy for prophylaxis and for treatment. When aminoglycosides are given, the fine line between a therapeutic and toxic dose presents an additional challenge. Timing is equally critical in administering a prophylactic dose of a cephalosporin; too early and it will be ineffective and too late, it may necessitate a full course of treatment rather than the one dose. Armed with the proper knowledge and skills, the operating room nurse will be able to provide the safe competent care that patients receiving antimicrobials should expect.

Surgeon--assistant communication.

Although conflict is often viewed negatively, in reality, patient care can be enhanced by a combination of conflict and collaborative techniques. Strategic timing, along with an appropriate location for the interaction, is important in resolving a disagreement. A defensive approach will only cause an interaction to escalate--use a calm, objective approach instead.
