The effect of tilmicosin administered to ewes prior to lambing on incidence of clinical mastitis and subsequent lamb performance.

The effect of tilmicosin on the incidence of clinical mastitis and subsequent lamb performance was studied in 9 sheep flocks in Ontario. Ewes were treated randomly with either tilmicosin or placebo approximately one month prior to lambing. Outcome was assessed by comparing rates of clinical mastitis, palpable udder abnormalities, and preweaning (50-day) lamb weights between the 2 treatment groups, while controlling for other important variables. Lambs raised by multiparous ewes treated with tilmicosin were significantly heavier than lambs from placebo-treated multiparous ewes at 50 days. Lambs from tilmicosin-treated ewes were on average 0.52 kg heavier than lambs in the placebo group. There was no difference between treatment groups in the weight of lambs from first parity ewes. Tilmicosin treatment resulted in a 43% decrease in palpable udder abnormalities. Incidence of clinical mastitis did not differ between experimental groups. The administration of tilmicosin prelambing, at the time of routine clostridial disease vaccination, may be a beneficial and convenient way to reduce mastitis infection and improve the preweaning gain of lambs.

Maternal smoking and preeclampsia.

OBJECTIVE: To study the relationship between maternal smoking and preeclampsia and whether this association differs between primiparous and multiparous women. STUDY DESIGN: We conducted a population-based, retrospective, cohort study of 58,216 singleton pregnancies from northern and central Alberta, Canada, between 1995 and 1997. Multivariate logistic regression was used to control for maternal alcohol consumption, drug dependence, maternal age, maternal weight, prior intrauterine growth restriction and other confounders. RESULTS: Maternal smoking was associated with a significantly reduced overall risk of preeclampsia (adjusted odds ratio [aOR]: .61; 95% confidence interval [CI]: .50-.75; P < .01). Stratified analyses showed that in primiparous pregnancies, maternal smoking was associated with a significantly decreased risk (aOR: .63; 95% CI: .50-.80; P < .01); in multiparous women, maternal smoking was not associated with a statistically significant decreased risk of preeclampsia (aOR: 0.72; 95% CI: .51-1.02; P > .05). CONCLUSION: Maternal smoking is protective against preeclampsia. Understanding the underlying biologic mechanisms of this protective effect may advance our knowledge of the pathogenesis of preeclampsia.

Seasonal changes in bovine fertility: relation to developmental competence of oocytes, membrane properties and fatty acid composition of follicles.

Follicle dynamics and oocyte viability in Holstein primiparous and multiparous cows and the relationships between fertility and the biochemical and physical properties of oocyte membranes with season were examined. The conception rates of primiparous (n = 70 885) and multiparous (n = 143 490) cows differed, peaking in the winter and decreasing in the summer. The number of follicles 3-8 mm in diameter per ovary was higher in winter (19.6) compared with summer (12.0). However, in winter the percentage of ovaries with fewer than ten follicles per ovary was 16%, in contrast to 50% in summer. After aspiration of follicles, 7.5 oocytes per ovary were found in winter and 5.0 oocytes per ovary in summer. Cleavage to the two- to four-cell stage after chemical activation was greater in winter than in summer; this was enhanced at the morula stage and embryo development to the blastocyst stage was significantly higher in winter than in summer. Determination of the lipid phase transition in oocyte membranes revealed a shift of 6 degrees C between summer and winter. Fatty acid composition of phospholipids from follicular fluid, granulosa cells and oocytes indicated that there was a higher percentage of saturated fatty acids during the summer and that the percentages of mono-unsaturated and polyunsaturated fatty acids were higher in oocytes and granulosa cells during the winter. Oocytes and granulosa cells had similar fatty acid compositions, in contrast to follicular fluid. These results may explain the differences in the ability of oocytes to develop to the blastocyst stage at different seasons. Thus, temperature changes may lead to changes in membrane properties, which, in turn, can influence oocyte function and fertility.

[The timing of tubal ligation (Author's transl)].

In spite of medical or mixed medical and social indications, the sterilization by tubal ligation is frequently not carried out or done very late as in sterilization combined with a therapeutic abortion. This is borne out by the review of case histories and consultations of patients who had a sterilization operation other than post-partum, multiparous patients who had no sterilization, multiparous patients who had sterilization post-partum and patients who had an application for therapeutic abortion rejected and patients who had a therapeutic abortion with concomitant sterilization. The medical indications, medical social indications, the duration of the disease, the desire for child bearing, the social conditions and the family conditions were of such nature that in 29-80 percent of the cases under review sterilization could have been carried out at an earlier time. The relatively high incidence of women who presented with indications for sterilization but whose sterilization was delayed shows that more active individual counseling regarding sterilization by the attending physician is desirable. If necessary the physician should raise the question of sterilization and discuss the problem with the patient. This type of counseling would insure that the patient can make an informed decision and would minimize the psychological side effects and after-effects of sterilization.

[Risk factors for infant mortality from diseases in Japan 1995-98].

OBJECTIVE: Since 1995, additional information (i.e. birth weight, singleton/multiple births, gestational weeks, maternal age, maternal parity and stillbirth experience) has been required for certificates of infant (less than 1 year of age) death from diseases in Japan. The present study examined the effects of biological, demographic and social variables, as reported on birth and death certificates, on infant, neonatal and postneonatal mortality in Japan. METHODS: Using data from vital statistics between 1995 and 1998, more than 4,787,000 livebirths and 16,000 infant deaths from diseases were analyzed. Univariate and multivariate analyses with the Poisson regression model were employed to assess the effects of variables on infant, neonatal and postneonatal mortality by singleton and multiple livebirths separately. RESULTS: The infant mortality rates from diseases were 3.2/1000 for singleton livebirths and 17.7/1000 for multiple livebirths. In singleton livebirths, low birth weight, infant born in earlier years, being a male infant, employment status as "unemployed or unknown", short gestational weeks, late birth in multiparity and maternal stillbirth experience were all significantly related to increased risk of neonatal and postneonatal deaths. Teenage mother were also at high risk of postneonatal deaths. Regional differences were observed. Compared with singleton livebirths, birthweight-specific mortality rates in multiple livebirths were relatively low among infants weighing under 2500 g. In multiple livebirths, elevated risk of death was associated with low birth weight, infant born in earlier years, employment status as "unemployed or unknown" and short gestational weeks. However, late birth in multiparity was related to a reduced risk of death, and maternal stillbirth experience was not a significant variable. CONCLUSION: This study provided the first quantitative estimate of risk of infant mortality from diseases in Japan. Since a more detailed elucidation of actual conditions and risk factors of infant deaths by vital statistics has become possible, efficient measures for improvement of infant mortality are to be expected.

Risk factors for antepartum fetal death.

OBJECTIVE: To determine the demographic, maternal, pregnancy-related and fetal risk factors for antepartum fetal death (APFD). STUDY DESIGN: From our perinatal database between the years 1990 and 1997, 68,870 singleton birth files were analyzed. Fetuses weighing < 1,000 g at birth and those with structural malformations and/or known chromosomal anomalies were excluded from the study. In order to determine independent factors contributing to APFD, a multiple logistic regression model was constructed. RESULTS: During the study period there were 246 cases of APFD (3.6 per 1,000 births). The following obstetric factors significantly correlated with APFD in a multiple logistic regression model: preterm deliveries: small size for gestational age (SGA), multiparity (> 5 deliveries), oligohydramnios, placental abruption, umbilical cord complications (cord around the neck and true knot of cord), pathologic presentations (nonvertex) and meconium-stained amniotic fluid. APFD was not significantly associated with advanced maternal age. CONCLUSION: APFD was significantly associated with several risk factors. Placental and umbilical cord pathologies might be the direct cause of death. Grand multiparity, oligohydramnios, meconium-stained amniotic fluid, pathologic presentations and suspected SGA should be carefully evaluated during pregnancy in order to decrease the incidence of APFD.

[Uterine rupture in Senegal. Results of 2 surveys taken in 1992 and 1996].

OBJECTIVES: National prospective descriptive study on dystocia were conducted in Senegal in 1992 and 1996. We examined more closely the data on uterine rupture to determine trends between these two surveys. METHODS: Data were collected for all patients undergoing an obstetrical intervention between January 1st and December 31st in 1992 and in 1996 in one of the referral maternity-obstetrical surgery units in each of the 10 regions in Senegal. RESULTS: The rate of uterine rupture was 1 per 51 interventions in 1992 (1.87%) versus 1 per 53 interventions in 1996 (1.94%). This dramatic event was recurrent in 7 of the 10 regions. The patients involved were aged 25-35 years (68% in 1992 versus 70% in 1996), multiparous or grand multiparous (52% in 1992 versus 70% in 1996), illiterate (90%), with poor follow-up (less than 3 prenatal consultations in 56% of the cases), and were generally evacuated to the unit (80% in 1992 versus 97% in 1996) without medical care (55%). Maternal mortality was high (28% in 1992 versus 12% in 1996). Infant mortality was also very high (98% in 1992 versus 86.6% in 1996). Conservative treatment of uterine rupture was used increasingly: the rate of conservative suture rose from 3.3% in 1992 to 22% in 1996. CONCLUSION: Prevention is a challenge for healthcare workers, public authorities and the population in general. Health policy should be directed towards forming general practitioners in obstetrical surgery. Emergency surgery kits should be made available to improve the quality of care in all maternity units throughout the country.

Accidental out-of-hospital delivery as an independent risk factor for perinatal mortality.

OBJECTIVE: To determine maternal characteristics and perinatal outcomes of unattended out-of-hospital deliveries. STUDY DESIGN: A population-based study including all singleton deliveries between 1988 and 1999. Maternal characteristics and pregnancy outcomes of accidental out-of-hospital births were compared with those of women who delivered in the hospital. Multiple logistic regression analysis was performed to investigate independent risk factors for out-of-hospital deliveries. Another model was constructed to assess the independent risk of out-of-hospital delivery for perinatal mortality. RESULTS: The incidence of unattended, out-of-hospital deliveries was 2% (2,328/114,938). Multiparity, Bedouin ethnicity and lack of prenatal care were independently associated with out-of-hospital deliveries. Parturients who delivered out of hospital had a significantly lower rate of previous cesarean deliveries. Perinatal mortality was significantly higher among out-of-hospital deliveries, and those newborns were significantly more likely to be small for gestational age as compared to newborns with in-hospital births. In a multivariable model investigating risk factors for perinatal mortality, out-of-hospital delivery was an independent risk factor for perinatal mortality. Other significant risk factors were Bedouin ethnicity and lack of prenatal care. CONCLUSION: Accidental out-of-hospital birth, associated with multiparity, Bedouin ethnicity and lack of prenatal care, is an independent risk factor for perinatal mortality.

Matrilysin (matrix metalloproteinase-7) selects for apoptosis-resistant mammary cells in vivo.

Overexpression of the matrix metalloproteinase matrilysin (matrix metalloproteinase-7) in the mouse mammary gland promotes mammary hyperplasia and accelerates the onset of oncogene-induced mammary tumors. In cell culture models, acute exposure of cells coexpressing Fas and Fas ligand (FasL) to matrilysin induces apoptosis, whereas chronic exposure to matrilysin selects for apoptosis-resistant cells. We now demonstrate that matrilysin promotes resistance to apoptosis in vivo. Matrilysin expression increased apoptosis in the involuting mammary gland of mice that had undergone a single pregnancy and lactation cycle. Premature basement membrane disruption was detected in matrilysin-expressing mice, which could account for the increase in apoptosis. However, multiparous mice, in which the involuting mammary epithelial cells have been repeatedly exposed to matrilysin, show a significant decrease in apoptosis. Mammary tissue from multiparous matrilysin-expressing mice showed decreased FasL expression, suggesting that loss of FasL is at least one mechanism of matrilysin-induced resistance to apoptosis. We propose that matrilysin promotes mammary tumor formation by enhancing the selection of cells that are resistant to apoptosis.

A case-control study on risk factors for low birth weight in China.

OBJECTIVE: To identify the risk factors for low birth weight (LBW) in Chinese newborns with varied characteristics. METHODS: A 1:1 matched case-control study, with 999 babies of LBW was performed in 44 counties of 11 provinces in China during July to October, 1998 for identifying their risk factors using simple and multivariate logistic regression models. RESULTS: The determinants of LBW in China included factors, such as multiparity (OR = 106.9), preterm birth (OR = 18.7), abnormal maternal health status (OR = 2.61) and maternal malnutrition (OR = 3.42), maternal medical conditions during pregnancy (OR = 1.93), maternal schooling (OR = 1.43), et al. Distribution of the risk factors for LBW was significantly different between coastal, inland and remote areas. LBW was mainly attributed to intrauterine growth retardation (IUGR) (71.6%) in the rural areas, and to multiparity and preterm birth, in addition to IUGR, in the urban areas. There was different in the risk factors for LBW with preterm births and IUGR. CONCLUSIONS: It is an effective way to reduce incidence of low birth weight in China that all measures for prevention and control should be relevant to its risk factors.

Parity and Illiteracy as Risk Factors of Cervical Cancers in Viet Nam.

The aim of the present study was to examine the risk of cervical cancers with reference to multiparity and illiteracy in Ho Chi Minh City where this neoplasm is a very serious problem but no reports have documented its risk factors. The 5,034 cervical cancer cases treated from 1989-94 in the Central Oncology Clinic of Ho Chi Minh City were derived from published sources. The observed number of children born and the education level among cervical cancer cases were compared with those in the reference group, the general population based on the results of the national census in 1989 and the inter-census in 1994. Among cervical cancer patients, the number of children born was in a wide range from 1-20 children with the mean number being 6.6 children per patient. Most of the patients finished primary or some primary school (54.9%), followed by the illiteracy group (30.0%). Multiparity was found to increase the risk of cervical cancer (RR = 1.31, 95% CI = 1.30-1.32). Illiteracy was also found to be associated with an elevated risk (RR = 3.43, 95 % CI = 2.85-4.14). The significant increase in risk of cervical cancers linked to to multi parity and illiteracy is a very important finding in the south of Viet Nam, where cancer is a very serious problem and there is a lack of information on which to base primary and secondary prevention.efforts.

Prophylaxis of early preneoplastic lesions of the mammary gland.

Daily treatment (for 12 to 14 months) of 2-month-old nulliparous or 8-month-old multiparous C3H/HeJ mice with 0.1 mg of 2-bromo-alpha-ergocryptine (CB-154) or 6-methyl-8-beta-ergoline-acetonitrile, efficacious inhibitors of prolactin secretion, markedly reduced the incidence of spontaneous mammary hyperplastic nodules and mammary tumors. CB-154 appeared to be more effective than 6-methyl-8-beta-ergoline-acetonitrile in suppressing the incidence of mammary tumors; the ergot virtually prevented the appearance of mammary tumors in nulliparous mice. Daily treatment of 5-month-old estrogen-treated, ovariectomized-hysterectomized C3H/HeJ mice for 12 months with CB-154 also significantly reduced the incidence of hyperplastic nodules and mammary tumors when compared with ovariectomized-hysterectomized mice treated with steroid alone. Daily treatment of multiparous C3H/HeJ mammary tumor-bearing mice with CB-154 or 6-methyl-8-beta-ergoline-acetonitrile generally failed, however, to promote regression of the mammary tumors. Thus significant prophylaxis of early preneoplastic lesions by drug-induced hormone (prolactin) suppression, resulting in a marked reduction in mammary tumor incidence, has been demonstrated in this study.

Changing trends in indications for caesarean sections in a tertiary hospital.

OBJECTIVES: To study the caesarean section rate and the trends in indications for caesarean delivery at the Singapore General Hospital (SGH) during two study periods of 6 months each. MATERIALS AND METHODS: The percentages of caesarean sections attributable to specific indications were computed for the first 6 months of 1998 and the last 6 months of 2001. Subgroup analysis of "less common indications" was performed. RESULTS: In the first half of 1998, 170 caesarean sections were performed giving a rate of 16.77%. 54.12% of women were multiparous. The main indication for caesarean section was dystocia (4.24% of deliveries). Two hundred and sixty caesarean sections were performed in the later half of 2001 giving a caesarean section rate of 25.10%. 53.46% of women were multiparous. The main indication for caesarean section was dystocia (5.41% of deliveries). Increase in caesarean section rate in 2001 was attributed to statistically significant increase in caesarean section for previous caesarean section and placenta previa major. Other changes in practice included shorter operating time which may be related to decision not to perform peritoneal closure. CONCLUSION: Studying indications for caesarean section are useful for hospitals, clinicians and researchers in determining strategies to lower primary and repeat caesarean section rate.

[Verification of the usefulness of Estrophan inj. Spofa in the control of reproductive parameters in sows after weaning of piglets].

Cloprostenol, a prostaglandin F2-alpha analogue, used in the Czechoslovak preparation Oestrophan ing. Spofa was tested by a new nontraditional use. The effect of cloprostenol use on reproductive parameters of sows after piglet weaning was followed in a set of 456 experimental and 434 control sows. The effect of single intramuscular instillation of cloprostenol was tested in five partial observations: to multiparous sows at a dose of 175 micrograms on the day of weaning (experimental group no. 1), on day 1 after weaning (group no. 2), 500 micrograms on day 1 after weaning (group no.3), to primiparous sows at a dose of 500 microgram on day 1 after weaning (group no. 4). The treatment used in group no. 4 was also applied to 42 primiparous sows kept on a farm with regular occurrence of post-weaning anoestria of sows (experimental group no. 5). These parameters were evaluated: dynamics of oestrus onset within five, and/or ten, days after weaning, average length of the weaning--first insemination interval, conception rate after the first insemination in dependence on terminated deliveries and parameters of piglet litter. Cloprostenol application did not have a statistically significant effect on the evaluated reproductive parameters. The percentage of multiparous, and/or primiparous, sows in which the oestrus onset was not detected by the 10th day after weaning, did not show any large differences in the various experimental and control groups (17.09 vs. 16.55%, 23.53 vs. 32.56%, 13.66 vs. 18.13%, 29.54 vs. 30.30%; P > 0.05). On the farm with regular occurrence of post-weaning anoestria there were 73.81% of primiparous sows with oestrus onset after cloprostenol instillation and 71.43% primiparous sows without treatment.(ABSTRACT TRUNCATED AT 250 WORDS)

History of abortion and subsequent risk of preeclampsia.

OBJECTIVE: To examine the effect of abortion type, number and timing on risk of preeclampsia in subsequent pregnancies. STUDY DESIGN: We conducted a hospital-based, case-control study in Seattle and Tacoma, Washington, between 1998 and 2001. Preeclampsia cases (n = 199) and controls (n = 383) provided detailed information regarding their pregnancy histories and other covariates, such as prepregnancy weight and adult height. Odds ratios (ORs) and 95% confidence intervals (CIs) were estimated by logistic regression. RESULTS: Multiparous women, both with a history of abortion and without, experienced decreases of 60% (adjusted OR = 0.40, 95% CI .23-.71) and 71% (adjusted OR = .29, 95% CI .16-.53), respectively, in risk of preeclampsia when compared to nulliparous women with no history of abortion. Type (spontaneous and/or induced), number and timing of prior abortion did not appear to influence the risk of preeclampsia among nulliparous women. CONCLUSION: These results confirm the work of others that multiparous women, both with and without a history of abortion, have a reduced risk of preeclampsia. However, much work remains with respect to exploring mechanistic hypotheses offering biologic explanations and examining possible confounding factors of this association, such as change in paternity and interpregnancy interval.

Distinctive patterns of hyperplasia in transgenic mice with mouse mammary tumor virus transforming growth factor-alpha. Characterization of mammary gland and skin proliferations.

Eight lines of transgenic mice expressing a mouse mammary tumor virus (MMTV) human transforming growth factor-alpha (TGF alpha) fusion gene were established. Three lines with distinctive phenotypes are presented. All have proliferative changes of the mammary gland. One line has sebaceous gland hyperplasia of the skin. Five histologic patterns of mammary gland hyperplasia based on two of these lines were identified: cystic hyperplasia, solid hyperplasia, dysplasia, adenoma, and adenocarcinoma. Human TGF alpha mRNA and protein were produced in all patterns but appeared reduced in solid hyperplasia, dysplasia, and adenocarcinoma. TGF alpha immunoreactivity in the mammary tissue, cystic fluid, and serum did not show significant differences; hyperplasia developed in 65% of multiparous mice and 45% of virgin mice by 12 months of age. Adenocarcinoma developed in 40% of multiparous mice and 30% of virgin mice by 16 months of age. These transgenic lines may provide useful models of mammary and sebaceous gland hyperplasia analogous to human disease.

Anti-lymphocyte, anti-monocyte, and anti-endothelial cell antibodies in chronic haemodialysis patients.

Patients receiving chronic haemodialysis treatment are known to have a high prevalence of anti-panel antibodies (anti-lymphocyte, anti-monocyte, and anti-endothelial cell) originating from a number of different possible sensitizing events such as blood transfusions, multiparity, or renal transplantation. In this study the prevalence of these antibodies in 123 chronic haemodialysis patients was evaluated. Anti-lymphocyte antibodies and anti-monocyte antibodies were found in 27.6% and 21.9% of the patient group respectively. Following exclusion of twelve patients within the study population with a history of previous graft rejection, anti-monocyte antibodies were encountered more frequently in transfused patients than those never having been transfused (22.4% versus 0%, P less than 0.05), whilst the prevalence of anti-lymphocyte antibodies was similar in both groups (28.2% versus 19.2%). Multiparity, patient age, or duration of dialysis treatment could not be demonstrated to significantly alter the prevalence of either of these antibodies. Of 12 patients with an episode of previous graft rejection, eight were positive for anti-monocyte antibodies and five for anti-lymphocyte antibodies. Anti-endothelial cell antibodies were common in patients in this group, being present in seven of eight individuals investigated.

[Podalic version in multiparae. Vaginal or cesarean?].

A review of maternal and perinatal results of multiparous patients with breech presentation was done during two period; two years before and two years after establishing the rule of cesarean section for all multi-parous with breech presentation. Seventy cases were considered at the first period in which 47.1% were vaginal delivery, proportion that significantly decreased (15%) at the second period. No significant differences on maternal results by delivery form were found and the same was found in perinatal results at comparing vaginal delivery and cesarean section. A different situation was reported studying only elective cesarean section, in which a significant decrease of severe neonatal depression was found comparing to vaginal delivery (24.2% and 0%). Because of rapid recovery there were no differences at fifth minute. As a conclusion, it appears as an exaggeration the option of cesarean section for all multiparous with breech presentation. Secondly, the adequate selection of cases to resolve by vaginal delivery may not produce adverse maternal and perinatal results.
