[Mycoses as opportunistic infections in AIDS patients].

Since January 1985 more than 100 patients with deep fungal infections have been treated with itraconazole (200 to 400 mg/day) in Northern Italy. Evaluation of the drug efficacy and tolerance was possible in one patient with sporotrichosis, in 34 with aspergillosis, and in 36 with cryptococcosis (mainly patients positive for human immunodeficiency virus). Response to itraconazole alone was obtained in the case of sporotrichosis and in 24 of 34 patients with different forms of aspergillosis (of the 18 patients with invasive pulmonary aspergillosis, 15 were cured). Patients with cryptococcosis received itraconazole for active infection and/or for prevention of relapse. Active infection was treated successfully with itraconazole alone in nine of twelve patients and with itraconazole plus flucytosine in eight of ten patients. Of the 31 patients who received itraconazole maintenance therapy for up to 27 months, four (13%) had relapses; 14 (45%) did not have relapses, and decline of serum antigen was detected in twelve of them; and 13 (42%) were completely cured (serum antigen titer dropped to zero). With the exception of hypokalemia in one patient, itraconazole was well tolerated even in patients who received the drug for several months or years.

[Multifocal mycoses in women: prevalence, species characteristics, and some intraspecific features].

The aim of this study was to detect fungi in three ontocenoses of 947 women (aged 20-45). The prevalence of multifocal fungal infections in women varied from 62.2 +/- 2.99% to 81.0 +/- 3.92% in different ontocenoses. They assumed the following mean values: 38.4 +/- 1.77% for the vagina, 42.2 +/- 1.80% for the oral cavity, and 25.0 +/- 1.58% for the anus. The clinical manifestations of sexual organs inflammations were statistically significantly (p < 0.01) more frequent in women infected with fungi than in those without fungi in the vaginal ontocenosis. The highest convergence of the fungi presence with subjective and objective symptoms (assessed using the Yule Q index value) concerned: pruritus of the vulva or/and vaginal walls, burning sensation or painful urethra, as well as oedema of the external opening of the urethra, vulva or/and the vagina (Q from +0.808 to +0.970). However, there was no statistically significant convergence between occurrence of the fungi and vaginal discharge and painful hypogastrium. Based on more than 40 phenotypical features (morphological, biochemical) of the axenic strains originating from vaginal, oral, and anal ontocenoses, a total of 12 species of fungi were identified: Candida albicans, C. guilliermondii, C. krusei, C. tropicalis, C. kefyr, C. utilis, C. humicola, C. viswanathii, C. glabrata, C. parapsilosis, Rhodotorula mucilaginosa, and Geotrichum candidum. For intraspecific features, depending on the profile of the compounds used for auxanogram, 9 codes were read for Candida albicans and only one code for each of other species was detected. In API ZYM test based on assessment of 19 hydrolase activities the enzymograms of species included from 6 to 11 enzymes.

[Deep mycoses observed in New Caledonia. Evaluation of observations from the laboratory of pathologic anatomy of the Pasteur Institute of New Caledonia 1975-1990].

All cases of deep mycotic diseases observed between 1975 and 1989 in the Laboratory of Anatomical Pathology at Nouméa's Pasteur Institute have been studied retrospectively together with all available data concerning this pathology both in New Caledonia and the Pacific area. During the period under review, two cases of histoplasmosis, 4 of actinomycosis, 3 sporotrichosis, 5 mycetoma and 5 chromoblastomycosis were recorded in New Caledonia. Clinical and histological data appear to be identical to those observed in other Pacific Islands. Diagnosis is most usually supported only by histological examination and, thus, for future studies it is vital that specific mycologic culture of all cases be carried out.

[Clinical trials in dermatology. Evaluation of the tolerability and efficacy of a topical antifungal agent in the treatment of superficial mycoses].

There is no single method for evaluating topical antifungal drugs. The localisation and the type of fungal, determine the treatment duration (from few days to several months). The main methodological characteristics of clinical trials in tinea pedis treatment (athlete's foot type) are reported. Aspects related to other clinical forms such as tinea versicolor and onychomycosis are also described. In any case, the main criteria of activity remains the mycological examination based on KOH microbiology and culture performed at the end of the treatment and again afterwards.

[Mycoses of the central nervous system].

The incidence of mycotic infections in the CNS has increased even outside those groups of patients with increased risk (AIDS, drug-addicts, etc.). These cases are secondary complications of other basic diseases favored by the influence of modern drug treatment and misinterpretation of modern diagnostic findings (radiology). A more frequent control of mycotic infections even in routine controls of chronic inflammations and a cautious individual handling of drugs (immunosuppressives, antibiotics, irradiation) should help to avoid these mainly lethal diseases.

[Effects of polymer materials in a furniture factory on the development of mycoses in workers].

Workers engaged in furniture making suffer from foot mycosis 2.5 times more often than those having no contact with polymers; patients with occupational skin diseases suffer from this disease 2.4 times more often than other workers. The analysis of skin samples and specific allergic diagnosis show that sensitization to fungi and occupational allergens is significantly higher among those contacting with polymers used in furniture making. Mycotic infection greatly raises sensitivity to occupational allergens. Exposure to polymers significantly increases sensitizing effect of foot mycosis pathogens on workers' organisms. This factor can be regarded as a predisposition to the development of mycotic infection.
