First metatarsophalangeal joint implant as a salvage procedure.

First metatarsophalangeal joint implants are used to correct deformity, to restore or improve motion, and to relieve pain. The authors report on a patient with pain in the first metatarsophalangeal joint that was not relieved by multiple first metatarsophalangeal joint surgical procedures. A two-component first metatarsophalangeal joint titanium implant was successfully used as a salvage procedure to relieve pain and restore foot function following a failed first metatarsophalangeal joint fusion.

The use of clean-room technology in medical device manufacturing: a tool in the service of quality. Part II.

Part 1 of this two-part article outlined the contamination-control requirements for medical device manufacturing, and the basic technical concepts for effectively and economically achieving them. Part II provides four case studies that illustrate how the basic principles of clean-room technology can be translated into industrial practice. It features examples of controlled clean-air manufacturing environments used in the production of injection cannulae, orthopaedic endoprostheses, heart pacemakers, and porcine aortic bioprostheses.

Wound cleansing by irrigation for implant surgery.

Wound irrigation is an adjunct to all implant surgeries involving surgical incisions or arthroplasties. An understanding of the physics of wound irrigation provides a scientific basis for recommending wound irrigation systems for implant surgery. When inserting implants through surgical incisions, a low-pressure irrigation system is recommended for removing blood from the wound surface. In contrast, high-pressure irrigation systems are indicated to cleanse the intramedullary canal before cemented arthroplasty. With the advent of static and dynamic pressure transducers, irrigation systems can now be specifically designed to achieve these goals.

Early experience with total knee replacement.

This paper presents a review of two years' experience with the geometric total knee replacement. The results of 23 arthroplasties in 22 patients are discussed. Relief of pain was consistent and dramatic, movement was increased postoperatively in only three patients, but imporvement in overall function occurred in 20 of the 22 patients. There was one failure requiring arthrodesis. In this patient, six months after surgery the medial tibial condyle collapsed and the polyethylene tibial component fractured. This complication has not been reported before. Early results are encouraging. Total knee joint replacement is a useful procedure in advanced arthritis when arthrodesis is the only alternative.
